Lights, Camera, Action! Making the Most of Your Private Duty Agency

Renee Bush, Home Care Association of Florida

Florida's private duty home care market continues to expand. In part, this growth can be explained by the increasing number of Baby Boomers entering the home care market every day, who are in need of quality care in the comfort of their own homes. Growing pains are associated with this expansion, including increased competition, staffing shortages, salary demands, programmatic shifts, and regulatory requirements. Despite these growing pains, this workshop offers a unique opportunity to gain insights into the private duty home care industry, with the potential to make an informed decision on entering the market or expanding current services. So, cue the lights, roll the camera, and get an action plan in place with HCAF's Director of Private Duty Services Renee Bush.

Do you and your staff know the ins and outs of the OASIS (Outcome and Assessment Information Set)? Do you know how this information is used to determine your outcomes or how patient revenue will be calculated? This full-day workshop will explore all the details of OASIS-E. Increasing Ticket Sales: Half-Day Sales & Marketing Workshop

Topics to be explored include the importance of efficiently and adequately onboarding new employees, leadership styles, incentives, and other ways to empower your staff and create loyalty. Various staffing models will be delved into including utilizing a Fractional Sales Manager, where a company could get the benefit of having a sales leader without having to hire a full-time manager. This individual would temporarily be available to train, ride-alongs, run sales meetings, and help with other activities. In addition, this workshop will include proven sales training to give managers and employees the tools necessary to train and implement a sales team and focus on etiquette when interacting with employees and management. Lastly, if the rules change regarding non-compete agreements, between now and the conference, we will discuss this as well.

A Deep Dive Into the VBP Era: How to Succeed if You Fail to Qualify for the Bonus

Arnie Cisneros & Kimberly McCormick, Home Health Strategic Management

Ongoing reforms triggered by the federal IMPACT Act have effectively rewired how Medicare providers develop, deliver, and are rewarded for quality care programming. Providers have had difficulty integrating the operational changes required by the reforms, and their outcomes have suffered. Future IMPACT Act changes will further challenge traditional home health operational structures. Home health reform challenges are rooted in the volume-to-value (V2V) shift, which is the basis of the IMPACT Act. Many providers struggle to adapt to V2V reform's rewired processes because of legacy practices. Many volumebased operations will prevent success in the Expanded Home Health Value-Based Purchasing (HHVBP) Model, including the inefficient start of care (SOC), inaccurate Outcome and Assessment Information Set (OASIS), disconnected plan of care (POC), functional impairment level (FIL)-based rehab, case conferences, and clinicianled care. Several providers have already rewired their agencies for success in these areas. This workshop will teach participants how to do it in their agencies.

How does the future look for providers who don't succeed in the HHVBP? A provider interested in qualifying for the HHVBP bonus should already be tracking benchmark metrics and seeking input on where they should make changes. However, a cursory assessment of the HHVBP structure shows that more than half of providers will not qualify. What will it take for those agencies to succeed? During this workshop, the speakers will demonstrate operational processes, management models, and supervisory structures to achieve efficiencies that will enhance your bottom line and results.

John Dalton, Optimum RTS

10,000 Baby Boomers are enrolling in Medicare every day with home care being the nation’s fastest-growing sector. Quite often this means your leadership pipeline is non-existent as if it had been wiped out by the empire’s death star. No Jedi mind tricks here! The force is strong in your organization and this session will help you focus on not only understanding and retaining your Millennial and Generation Z staff, but it will provide you with the tools you need to develop succession leadership within your agency.

The Real Caregiver World: A Glimpse Into the Lives of Those on the Frontlines

KristenWheeler,NationalAssociationforHomeCare&Hospice&BrandiKurtyka,MissionCareCollective

It is no secret that the workforce shortage in home-based care has reached crisis-level proportions. Despite industry leadership's best efforts, the gap between the number of people needing assistance and the number of workers available to deliver that care is accelerating at an unsustainable pace. In an effort to identify the overarching issues complicating recruitment and retention efforts, the National Association for Home Care & Hospice partnered with Mission Care Collective to produce The State of the Direct Care Workforce, a national study that included data from over 67,000 caregivers, certified nursing assistants, and home health aides. In this groundbreaking study, the first of its kind, seven (7) distinct types of caregiver were identified. This session will teach participants about each of these personas, how to recruit them, and, most importantly, how to tweak your organization's culture to retain them. The final report will be available for download.

Medicare Advantage: Show Me the Money!

Melinda Gaboury, Healthcare Provider Solutions, Inc. & Regina Wild, Managed Care Consultants of America

With enrollment doubling in the past few years, Medicare Advantage is becoming the coverage of choice for Medicareeligible people. Because of this growth, Medicare Advantage has become an integral part of home health care. This session aims to provide participants with strategies for overcoming common problems agencies face when working with Medicare Advantage plans, such as missing authorization deadlines, clean claims denials, billing and payment requirements, and reading, negotiating, and enforcing contractual terms with payers. This session will teach participants how to measure their agency's success or failure in serving Medicare Advantage patients.

Cash in at the Box Office With ICD-10 Diagnosis and OASIS Assessment Coding Accuracy

Nanette Minton & Lisa McClammy, MAC Legacy

According to the Patient-Driven Groupings Model (PDGM) regulations, a home health agency generates 75% of its revenue from ICD-10 diagnoses and Outcome and Assessment Information Set (OASIS) coding. To maximize revenue, all coding elements must be accurate. In this session, participants will gain a better understanding of the significance of meticulous coding for primary and co-morbid diagnoses, as well as OASIS assessment coding. Don't leave money on the table because of inaccurate coding or assessments.

GENERAL TRACK • HARMONY Compliance: The Companion You Never Realized You Had

In this post-pandemic world, we can assume that regulatory agencies will continue, and indeed increase, the execution of their broad and ambitious agendas. We can expect increases in the frequency, intensity, and scope of supervision, enforcement, and investigations under both old and new regulations. At the same time, as our organizations struggle to grow and thrive in this environment, the need to be creative and innovative grows more urgent every day. While creativity and innovation are admirable qualities in any organization, they can be risky. Managing that risk is much easier with guidance from and collaboration with a well-appointed and active compliance department.

Compliance is not just about regulatory and policy enforcement or stifling creativity and innovation. It is about connecting positively with our employees to stabilize their complex and changeable environment and help them move forward with positivity and optimism. It matches creativity and innovation with responsibility and accountability. Achieving this balance requires developing a strong ethics and compliance culture based on trust, transparency, and consistency. This presentation relates how one compliance team attempted to achieve this balance and offers ideas on how compliance is the companion you never realized you had.

Straight Talk About Value-Based Purchasing: How to Make a Comeback if Your Debut is a Flop

Arnie Cisneros & Kimberly McCormick, Home Health Strategic

Management

The Expanded Home Health Value-Based Purchasing (HHVBP) Model represents the most significant change to the home health sector since the introduction of the Prospective Payment System (PPS) over 20 years ago. By rewiring the home model in value-based terms, Medicare has linked payment to outcomes, rewarding providers who deliver efficient care programs. Further complicating the Expanded HHVBP Model is the introduction of the Outcome and Assessment Information Set

Version E (OASIS-E) as the data collection tool for this value-based reform. Through extensive education focused on both OASIS-E and the HHVBP structure, most providers know the changes they produce. But the challenge is rewiring operations to maximize your chances of success, and most of us don't know exactly where or how to start.

Considering that six out of 10 agencies will fail to qualify for the HHVBP bonus, it will be imperative for providers to address the payment loss they will experience in the coming years. This progressive, interactive presentation addresses these and other essential topics for success under the HHVBP.

UPIC Audits Don't Have to Be a Scary Movie: How to Handle an Unwelcome Event

Joe Osentoski, Gateway Home Health Coding & Consulting

The Centers for Medicare & Medicaid Services (CMS) has fully resumed all medical review activities. Getting selected for an audit has significant ramifications, including the need to devote resources to responding to and dealing with the results. Post-payment reviews, such as Unified Program Integrity Contractor (UPIC) audits, can put $100,000 on the line, potentially reaching into the millions. Providers must be aware of risk factors and trends in agency documentation that may prompt review or denial. It is crucial to reduce risk continuously and respond assertively to reviews.

The Medicare Review Choice Demonstration (RCD) does not protect agencies from UPIC activity, and audits clearly show the provisional nature of pre-claim review affirmations. With numerous reviews of medical records and multiple contractor audits, prevention is the best course of action. In the face of financial disruptions resulting from a review, prompt positive actions and knowing your options are key to keeping your agency solvent. This presentation is based on 25 years of experience assisting agencies with medical reviews and current UPIC audit trends.

The Power of the PEPPER Data That is Driving Medicare Audits

Melinda Gaboury, Healthcare Provider Solutions, Inc.

Is your agency a box office hit or a flop? The Program for Evaluating Payment Patterns Electronic Report (PEPPER) summarizes provider-specific Medicare data statistics for target areas often associated with Medicare improper payments due to billing, coding, and admission necessity issues. To improve PEPPER scores, providers must understand how the data is analyzed and consider best practices to avoid medical review audits. This session examines each PEPPER target and discusses how to evaluate your agency's success using data.

Avoiding the Blooper Reel: How to Produce Award-Winning Therapy Documentation

Beulah Scott, Healthcare Provider Solutions, Inc.

Medical necessity audits can be time-consuming, expensive, and frustrating for providers. This session will examine the expectations of the Centers for Medicare & Medicaid Services (CMS) regarding effective documentation as well as the most common reasons for claim denials. Skilled services are frequently denied due to insufficient documentation supporting medical necessity. This session teaches participants how to document effectively and accurately. It also teaches them to identify reasonable criteria for skilled therapy services and develop and write functional-based goals. Additionally, the speaker will review the regulatory requirements for medical necessity, common mistakes, and how to skillfully paint a picture of their current functional abilities. This session is intended for occupational, physical, and speech therapists, quality improvement personnel, and administrators/managers responsible for agency success.

This session introduces participants to steps for evaluating your business' bandwidth and tolerance for risk when negotiating contracts.
