Get Help Managing Multiple Medications Taking your medications as directed by your doctor is key to being as healthy as possible. But if you take a lot of different meds, making sure they’re all working well for you and staying on top of the various directions and dosages can get complicated. Members with more than one chronic condition who take at least eight covered Part D medications to treat those conditions qualify for a free program. This program, called Medication Therapy Management, helps you take your meds safely. We’ve partnered with OptumRx to offer this program to qualifying members. If you qualify, you’ll be automatically enrolled in Medication Therapy Management and a pharmacist will review all of your medications with you annually at no extra cost. You’ll get a welcome letter in the mail, and then an OptumRx team member will call you to schedule your medication review.

After the phone call, you’ll receive a package of information about your medications. The pharmacist from OptumRx will also talk to your primary care provider about any changes they recommend. Managing multiple medications is a big responsibility, and you don’t have to do it alone. If you qualify for this free program, we encourage you to take advantage of it to make sure you’re getting the most from your meds. If you have questions, call the Member Services number on the back of your ID card.

The medication review will take about 30 minutes. During the call, the pharmacist will: • Make sure you’re taking the safest and most effective medications • Discuss low-cost generic options for your meds • Address your medication management goals • Answer any questions you have

Talk to your doctor about each of these topics at your next visit. • Improving your physical and mental health • Monitoring your physical activity

It’s important to know how your Medicare Advantage plan works in the following areas. • Annual flu vaccine • Getting needed care and appointments quickly • Customer service • Healthcare quality • Rating of health plan • Care coordination

• Lowering your fall risk and osteoporosis

• Rating of drug plan

• Improving bladder control

• Getting needed prescription drugs
