We’ll tell you about your Health Alliance Connect plan as we go, but here are the basics to get you started. At Health Alliance Connect, we help you and your doctor manage your health. That means you’ll get a primary care physician (main doctor, also called PCP) to focus on your total health care, community resources, rides to appointments and other support. Plus, all your health records stay in one place.

Help Your Little

Grow

Finding out you’re having a baby can make you feel excited and scared. You might be worried about all you need to do to take care of yourself and the baby. We can help. Join Sprout, our free prenatal (before birth) program. When you sign up, a nurse care manager will call you to talk about what you need. He or she will be there for you during your pregnancy to answer your questions and help make sure you and your baby get the care you need.

You can get rewards for going to your prenatal doctor visits. We will give you cards to take to each visit. Have your doctor sign the card, and then send it to us to get:

A pack of diapers after each office visit

A gift card, which you can use at many major online stores, after six office visits

A stroller or swing if you go to all your visits

Why do I need prenatal care? Prenatal care can save your baby’s life. It’s that important. Babies whose moms do not get prenatal care are five times more likely to die and three times more likely to have a low birth weight than babies born to mothers who get care, according to womenshealth.gov. Don’t risk it. Get the prenatal care you and your baby need.

Ways to track your exercise

Don’t use tobacco or drink alcohol. It might be hard to quit, but everything that goes into your body also goes into your baby’s body. Alcohol, tobacco and other drugs can hurt or even kill the baby. If you need help quitting, call us at 1-877-633-2526.

Schedule an appointment to see your doctor as soon as you can. If you have questions about your pregnancy, call Health Alliance Connect at 1-877-633-2526. 4


Take These Steps to Stay with Health Alliance Connect Each year, the State looks at every Medicaid member’s case to see if he or she still meets the rules for getting Medicaid. This is part of the Illinois Medicaid Redetermination Project (IMRP).

What You Need to Know

You’ll get a letter to let you know a form is coming soon. About two weeks later, you’ll get a form with the latest info the State has about you.

Watch for mail from IMRP, the Illinois Department of Human Services (DHS) or the Illinois Department of Healthcare and Family Services (HFS).

Follow the directions on the form. Fix any wrong info, and send proof for any new info.

Return the form by the date listed even if you don’t change anything. If you need more time or can’t find your form, call 1-855-458-4945 (TTY: 1-855-694-5458). If you get All Kids assistance, you’re still a Medicaid member. You need to keep paying your premium if you have one and fill out your redetermination form when you get it. If you get food stamps or cash assistance, IMRP might not send you a letter. DHS will let you know if you need to do anything.

Update Your Address If the State doesn’t have the right address for you, you won’t get the letter or form, and you will stop getting your Medicaid benefits soon. Here are some ways you can let the State know you’ve moved.

Update your address at a U.S. Post Office.

Go to dhs.state.il.us. Click the More for Customers link. Then choose the Change of Address, Income or Assets link under “Most Requested Services.”

Let’s Talk Teeth Taking care of your teeth is important to your overall health. Disease in your mouth can put the rest of your body at risk. As a Health Alliance Connect member, you can go to the dentist for a cleaning each year. And for members under 21, we have extra dental services, like orthodontics. Help your kids build good habits to protect their teeth now and in the future. Here are some things your family can do at home. • Brush your teeth twice a day. • Floss each day. It prevents cavities and gum disease. • Drink more tap water. There’s fluoride in it, which prevents cavities. • Cut out soda. It has sugar and acids that eat at your teeth.

Talk to your dentist to learn more, get information on supplies, and make healthy choices.
