Note from the Editor The fall season has arrived and what a way to celebrate with our biggest issue ever. September is “Healthy Aging Month” Women Encouraged To Keep An “Eye” On Vision-related Concerns By Cheryl L. Dejewski September is “Healthy Aging Month,” and the American Academy of Ophthalmology reminds Americans with no signs or risk factors for eye disease of the importance of getting a baseline screening at age 40—the time when early signs of disease and changes in vision may start to occur. By 2020, it is estimated that 43 million Americans will face significant vision loss or blindness from age-related eye diseases, such as cataracts, diabetic retinopathy, glaucoma and macular degeneration. The doctors at Eye Care Specialists, one of Wisconsin’s leading ophthalmology practices, offer insights into how some of these eye conditions are more likely to impact women and their overall health, safety, and freedom to see life to the fullest. Decreased vision in women has been linked to an increased risk of falling, which often causes more serious concerns, including hip fractures, the need for canes, walkers and/or nursing home care, and complication-related death. Decreased vision also increases the risk of car accidents and can have a devastating effect on quality of life in terms of limiting daily task performance, independence, social interaction, and other factors. “Poor vision, however, is not a fact of life as you grow older. If your vision is getting worse, and new glasses don’t seem to do the trick, you need to schedule a comprehensive, dilated eye exam to rule out possible causes,” advises Brett Rhode, MD, Head of Ophthalmology at Aurora Sinai Medical Center and private practitioner at Eye Care Specialists’ downtown Milwaukee and West Allis locations. “Upon examination, we often find that cataracts (age-related clouding of the natural lens inside the eye) are the cause of the problem and, in most cases, surgical removal and replacement with a customized lens implant is a relatively easy and painless solution.” Rhode stresses, “Since cataracts are a primary but reversible cause of diminished vision, early detection and removal can prevent needless pain, suffering and cost. I can’t tell you how many cataract patients say that the only issue they have with the surgery (which only takes about 10-20 minutes at our outpatient surgery center), is that they wish they had done it sooner.” Age-related macular degeneration (AMD) is another leading cause of vision impairment in Americans over age 50, and although the exact cause is unknown, it affects more women than men. AMD is a condition in which the macula, a highly sensitive area of the retina responsible for central and detail vision (about the size of this “O”), is damaged. There are two forms of AMD—wet and dry. Both cause loss of central or straight-ahead vision, impacting the ability to drive, read, see faces, etc. Mark Freedman, MD, an ophthalmologist with 27 years of experience and a continuing education lecturer for southeastern Wisconsin eye and health care professionals, explains, “Dry AMD usually does not result in rapid vision loss, and many patients retain good sight throughout their lives. Wet AMD, however, tends to cause a rapid and profound loss of sharp central vision, which may result in legal (less than 20/200 vision), but not total (because some side vision remains) blindness. Without treatment, about 70 percent of wet AMD patients become legally blind within two years. If dry AMD is diagnosed, we may recommend vitamin supplements, healthy omega fatty acid intake, sun protection, and avoidance of smoking, as measures to prevent or slow progression. When needed, we also educate patients about low vision aids (like magnifiers) and support/transport services to help them stay independent and productive. If wet AMD is diagnosed, we review the risks, benefits and candidacy for Avastin or Eylea medication injections, which inhibit the growth of the abnormal EYE>>page 59

“As baby boomers age and people live longer, more Americans are hitting the age bracket where risk levels for debilitating eye conditions increase,” warns Mark Freedman, MD, senior partner at Eye Care Specialists, a nationally recognized ophthalmology practice. How can you protect your vision? Brett Rhode, MD, Head of Ophthalmology at Aurora Sinai Medical Center and partner at Eye Care Specialists, advises, “Your best protection is to schedule regular, comprehensive eye exams. Our practice strives to offer the latest technology to detect, track and treat nearly every cause of vision loss. But the most advanced equipment and services in the state are of no use if people don’t utilize them.” The following are just a few of the cutting-edge devices and procedures his practice has available.

Customized Lens Implants
Cataract surgery requires making an opening in the eye to remove the cloudy lens (cataract) and replace it with an artificial lens (to once again focus light rays onto the retina and restore vision). Standard lens implants (IOLs) are “monofocal” or single-focus. They are like miniature internal contact lenses set for a single prescription power—usually to provide clear vision in the distance (for driving, watching TV, etc.). Most patients still need to wear reading glasses or bifocals after surgery. “When appropriate, however, we offer the option of ‘advanced technology’ IOLs, which are ‘multi-focal’ (provide multiple focusing zones for near and far) or toric (provide correction for astigmatism),” explains Daniel Paskowitz, MD, PhD, an eye surgeon with credentials from Harvard and Johns Hopkins. “At least 70% of advanced technology IOL patients can see to read, drive, do computer work, or perform most activities—with never or only occasionally needing glasses. At Eye Care Specialists, we customize each implant to fit each patient’s prescription, health, lifestyle, expectations, budget, and other concerns.”

Glaucoma Laser Treatment
Glaucoma is a condition in which the fluid pressure in the eye is too high for the health of the optic nerve causing loss of side vision and eventually all sight. It is typically treated with prescription eye drops to lower the pressure (by either
decreasing fluid production or increasing outflow). “For some patients, however, laser treatment may be an alternative. Our surgeons utilize highly specialized SLT and ECP laser procedures for qualified candidates. If successful, these 10-minute outpatient procedures can control glaucoma and help to reduce the need, expense and hassle of using daily drops,” explains eye surgeon Daniel Ferguson, MD, who is also trained to implant a specialized drainage device.

Macular degeneration (AMD) is the leading cause of central vision impairment in Americans over age 50—robbing people of their ability to read, see faces, drive, etc. “A few years ago, there wasn't much we could do. But, now, we can offer revolutionary new medications, like Avastin and Eylea, that inhibit the growth of the abnormal blood vessels that cause the ‘wet’ form of AMD as well as diabetes-related eye damage,” reports ophthalmologist Michael Raciti, MD, who performs sight-saving injection treatments on a daily basis. “Although there are NO guarantees, our practice has seen remarkable results with regular (about monthly) in-office injections, including stopping the progression of wet AMD in 90 percent of patients, and even having up to 30 percent gain improvement in vision. This advanced technology can detect, track and treat signs of glaucoma, diabetic retinopathy, AMD, and other sight-threatening diseases—often before damage occurs. Patients simply focus on a light while a safe, invisible laser scans inside the eye to create detailed cross-sectional images (similar to an ultrasound) that provide unparalleled accuracy in visualizing and measuring any changes to the retina and optic nerve.

Going Gluten-Free— the Latest Fad or a Healthy New Way to Eat?
Answers by Dena McDowell, MS, RD Some call it the latest craze, while others find it a detrimental health condition. There is a lot of confusing information about what gluten is and how it affects our bodies. Let me try to clear up some misperceptions about gluten by answering some frequently asked questions: Q Just what is gluten anyway? A Gluten by definition is a protein. It’s commonly found in wheat, barley and rye. The basic function of gluten is to give dough an elastic quality that helps shape the final product before cooking or baking. Gluten can also give texture to food, usually in the form of a chewy consistency. Gluten is prevalent as a natural protein source in grain foods and as an additive to foods to increase the protein content or change the consistency of the food product. Gluten is found in many foods that you may expect and in some foods that may surprise you. Common foods that contain gluten include: breads and bakery products that contain wheat, rye, barley, wheat germ, gluten flour, durum flour, wheat starch and semolina. Other foods that should be avoided are oat bran, spelt, faro and pasta made from wheat or wheat starch. Gluten can also be used as an additive and may be lurking in foods that you would not expect like ice cream and even ketchup. Q What is a gluten-free intolerance versus celiac disease? A There are two different medical conditions related to gluten that may adversely affect a person. The first disorder is known as gluten intolerance (or sensitivity). People with an intolerance react to gluten-containing foods and after eating them, may experience gas, bloating and changes in bowel pattern (which can be confused with irritable bowel syndrome). Other symptoms might include headaches, joint pain, increased fatigue, mood swings and an increased susceptibility to viruses. The second disorder is an allergy to gluten-containing foods. This condition is known as celiac disease. It is estimated to affect one in one hundred people worldwide, according to the Celiac Disease Foundation. Celiac disease is an autoimmune disorder that causes damage to the small intestine, which then reduces absorption of needed nutrients. Over time, this causes deficiencies in certain nutrients such as iron and calcium.
LASIK
Marc Hirsch,M.D In many instances, the laser can eliminate the need to wear glasses. The use of lasers for this treatment has been in existence since the 1980’s. Initially, the only procedure available was a called PRK(Photorefractive Keratectomy). In this procedure, the surface of the
outermost layer of the eye is removed and then the laser treatment is applied. Over the next few days, the surface would heal. Once the surface healed the effects of the laser could be appreciated. PRK is an excellent procedure and is still used today in certain situations. However, it has largely been replace by LASIK (laser-in-situ keratomileusis). What is the difference? In LASIK, the surface layer is not removed. Instead a flap is created and reflected prior to the application of the laser. Once the laser is applied the flap is repositioned. This allows for faster healing with significantly less discomfort compared to PRK. In addition, most patients can undergo the procedure on both eyes in the same day. Many people have heard of LASIK and are aware that a laser is involved in the procedure. However, most people did not realize that until recently the only option for creating the initial flap was by using a rapid-oscillating metal blade. The use of a metal blade to create the initial flap is a tested and safe method for flap creation. However, the few complications that can occur during the procedure are usually related to a

A recent improvement in safety has been the development of a laser to create the initial flap. The laser is an infrared beam that separates the tissue by using a process called photodisruption. Light pulses as short as one-quadrillionth of a second are used to divide material at the molecular level without any impact to the surrounding tissue. This laser is called a femtosecond laser and is made by a company called Intralase. This is a completely different laser than the laser used for vision correction, which is called an excimer laser. The major advancement in the femtosecond laser over the metal blade is safety. During a flap creation with a metal blade, the blade runs across the surface of the eye. This leads to the potential of inadvertently causing a corneal abrasion. In addition, there is possibility that the metal blade may cause an uneven cut across the cornea, the outer layer of the eye. The femtosecond laser addresses these potential safety issues by creating the flap in a different manner. Rather than moving across the surface of the eye, the femtosecond laser is applied from directly
above the eye. This method removes the potential of creating a corneal abrasion during flap creation. As mentioned this is a major safety improvement over the conventional oscillating metal blades. A second improvement is the accuracy of the depth of the flap. The laser is programmed to apply the laser spots at a specific depth. This fixed depth aids in calculating the amount of treatment that is safe for a particular eye. The conventional metal blades are not quite as accurate as the femtosecond laser. This means that often the actual depth of the flap made by a mechanical blade may be different than the intended depth. While this translates into small micrometer changes in the flap thickness, it may be important to a patient who has a cornea that may be too thin for their necessary treatment. In addition, it becomes quite important for the occasional retreatment that may need to be performed. The Intralase removes the variability in the flap thickness and improves the ability for delivering a safer treatment. Some of the most devastating, but rare, complications occur when the metal blade does not perform a smooth flap LASIK>>page 27

Mindfulness Meditation May Benefit People With Chronic Insomnia
Mindfulness meditation—and particularly a form designed for insomnia—may help people with chronic insomnia and could be a viable treatment option, according to a recent small study. Results of this NCCIH-funded research were published in the journal Sleep. Researchers randomly assigned 54 adults with chronic insomnia to participate in one of three groups: 1) mindfulness-based stress reduction (MBSR), 2) an adaptation of MBSR called mindfulness-based therapy for insomnia (MBTI), or 3) an 8-week self-monitoring program using sleep diaries. The MBSR group met for 2.5 hours once weekly for 8 weeks, and participated in a 6-hour meditation retreat. The weekly meetings included meditation practice, general discussion about at-home meditation, and education on the daily applications of meditation. The MBTI group had the same amount of meditation practice as the MBSR group, but instead of a discussion on general health and education on meditation, the MBTI group learned specific behavioral strategies for insomnia (e.g., sleep hygiene, sleep restriction therapy). Participants in both meditation groups were asked to meditate 30–45 minutes at least 6 days a week. All three groups completed sleep diaries at the start of the study, at each treatment/monitoring week, at the end of the study, and at the 3-month and 6-month followup. The researchers also measured sleep using wrist monitors and overnight laboratory sleep tests at the start and end of the study and at the 6-month followup. Findings from the study showed that the meditation-based treatments (MBSR or MBTI) provided improvement by significantly reducing total wake time and presleep arousal. Both meditation-based therapies were better than the self-monitoring on each of the patient-reported measures. Only one significant difference was found between them across the study: the MBTI group had a significantly greater reduction in the severity of insomnia compared with the MBSR group, and this difference was largest at the 3-month followup. The researchers noted that the small number of participants, as well as the participants’ lack of diversity, should be taken into consideration when interpreting the findings. They also noted that future research, with a larger sample size, comparing meditation-based therapies to standard treatment, such as cognitive-behavior therapy for insomnia, would help determine the position of meditation-based therapies among treatment options.

According to the Institute of Medicine of The National Academies over 100 million Americans suffer from chronic pain. That is more than diabetes, coronary heart disease, stroke and cancer combined. According to The American Academy of Pain Medicine, chronic pain is different because the pain signals keep firing in the nervous system for weeks, months, even years past the initial mishap. Such mishaps can include things such as a sprained back, serious infection or past injury. Other causes of ongoing pain include arthritis, fibromyalgia, headaches, cancer, etc. When asked about four common types of pain, respondents of a National Institute of Health Statistics survey indicated that low back pain was the most common (27%), followed by severe headache or migraine pain (15%), neck pain (15%) and facial ache or pain (4%). How many people do you know that suffer from one of the chronic pain issues? Probably more than one because back pain is the leading cause of disability in Americans under 45 years old. Low back pain puts a strain on the body as a whole. Adults reporting low back pain were three time as likely to be in fair or poor health and more than four times as likely to experience serious psychological distress as people without low back pain. Treatment for pain is broad spectrum including, but not limited to, a variety of injections, acupuncture, chiropractic care, physical therapy and medication management. Medication management can be done by your primary care doctor however, many doctors prefer that the patient see a physician that focuses specifically on pain management. Latest treatment options and up to date information are crucial in effectively managing chronic pain. Patients need to be monitored closely and often to re-assess goals, address side effects and document effectiveness of treatment. This can often be too time consuming for the busy schedule of many primary care physicians. Chronic pain needs to be managed with a multidisciplinary approach, with empathy, and in a non-judgmental environment. We welcome all new students, regardless of fitness or experience level, to Milwaukee Yoga Center (MYC). The Yoga Center is convenently located on the east side of Milwaukee in Wisconsin. MYC is run by certified Iyengar instructors with two fully equipped studios, and classes range from Introductory to Advanced, with a variety of specialty classes. Students new to Yoga or Iyengar Yoga must enroll in an Introduction to Iyengar Yoga series. Each series will run 46 weeks and teach the fundamental poses and principles of alignment. Even if you’ve taken yoga before, this series will be highly useful as you learn Iyengar principles. The series will cover 15 of the most important standing and seated poses. Students should come to the Milwaukee Yoga Center fifteen minutes before class. Wear T-shirt and shorts or leggings. Be prepared to work in bare feet. Do not eat two hours before class. A CLOSER LOOK INTO HOT STONE MASSAGE Hot stones were used as healing therapy for thousands of years. In 1993, the technique was formally introduced as LaStone® therapy by Mary Nelson, a massage therapist from Tucson, Arizona. The concept of using hot stones was to bring the body to a deep relaxed state, therefore allowing the body to self-heal. When the treatment is applied in the most effective way, this style of massage can be both relaxing and therapeutic at the same time. The stones are smooth black basalt, which retain heat very well. They are placed in water and heated to approximately 120 to 130 degrees. The therapist can adjust the temperature of the stones to meet the needs of each client to assure they are comfortable. A sheet or towel is placed between the stones lying directly on the client as a protective cover. The direct heat penetrates into the muscle tissue causing it to soften and relax to a lengthened state, while bringing oxygen and blood to the area. As a result, the pain starts to decrease. The therapist can apply deeper pressure if necessary to easily melt away the knots with minimum discomfort to the client. The therapist will also be massaging with the stones using Swedish techniques to introduce the heat to the body. This increases blood and lymph flow, which promotes the detoxification process. A typical hot stone massage will last for 90 minutes, giving the therapist enough time to do a full-body treatment. A wide range of stone sizes are used depending on the area of the body being worked on. The client will be lying on their back to start, with larger stones placed on the abdominal area. This is helpful to relieve any digestive discomfort or menstrual pain for women. Stones are also placed between the toes and under the palms of the hands. Both of these areas have reflex points that affect the entire body, and when exposed to heat, the body begins relax. A large oval stone that contours to the curve of the neck is placed under the head for the client to rest on. This will help to alleviate any tension caused from headaches and stiff neck muscles. Once the stationary stones are in place, the massage begins with warm stones applied to the face to loosen the jaw, temples and forehead. Cold marble stones are then placed above the brow to relieve sinus pressure and below the eyes to reduce puffiness. The stones on the face and under the head are removed after a few minutes. The therapist begins massaging with the stones, using a series of applications to loosen and soothe the muscles of the neck, upper shoulders, chest, arms, hands, legs and feet. Trigger point stones are longer, shaped more like a thumb and are designed to treat common problem areas, such as the neck, upper shoulders, along the spine and the arch of the foot. Stones starting to cool are always replaced with hot ones to ensure a constant flow of heat throughout the treatment. The client will now turn over, lying face down as the therapist places stationary stones along the spine and a large stone over the sacrum or tailbone. The backs of the legs and arms are massaged while heat penetrates the back. Finally, the back is thoroughly worked using Swedish and deep tissues techniques with a constant flow of heat to finish the massage. The client is encouraged to rest for a few minutes and take their time getting off the table so the body can adjust to all the changes that just occurred. It’s strongly recommended to drink plenty of water to rehydrate and flush the body of toxins. By Kalpana (Rose) M. Kumar, M.D. As an internist who sees hundreds of women in the second half of life, I am all too familiar with women’s waning libidos. Having gone through menopause myself, I have personally experienced the power that changing hormones have on many psychological and bodily functions that the younger me took for granted. As an integrative physician, I am always searching for causes of symptoms and for natural and integrative ways to treat and heal them. Our society sees a waning libido as a pathological symptom. Viewing our aging bodies as pathological is where our problem truly lies. Our collective maturity level in America can truly be seen as ‘adolescent’. We are a society with many compulsions. We seek fast ways to manipulate the body, to manage symptoms, and to regain youth. We want pills as substitutes for inner work, diets for rapid weight loss, and shots to stop menstruation considered inconvenient by many. We are addicted to an ideology that has proven not to work over the past three decades. Somehow, we are unable to make the shift to a more mature way of thinking. Industries that support these compulsions make a lot of money. The money itself becomes an incentive to keep the compulsions in place. We have promoted the ‘anti-aging’ ideology for nearly 3 decades. We have yet to find a way to turn back the clock. We are fearful of aging because we lack respect for process, and have not created a way to mark and consecrate our life stages from a place of reverence. Our society normalizes this. Mother Nature, in all of Her wisdom, marches on in time. It is about time we find a way to gracefully honor the rhythms of the natural order of life and learn how not to resist aging due to our adolescent fear. A woman’s libido has many facets that influence it – hormonal, psychological, emotional, and spiritual. Hormones have powerful effects on the biological nature of sexual desire. When we are young and fertile, our hormone surges are aligned with the zeal to procreate. In other words, the desire for sex is aligned with Nature. This is a powerful force that underlies our libidos when we are young. As we age our hormones change, and we are less able to procreate - and Nature is less supportive of our libidos. Women in their mid-40s complain about their waning libido, their changing body, and mostly their newly discovered need for balance. They feel depleted after half a life of caring for others. Energetic depletion, alongside changing biology creates less desire for sex. There is a greater desire for more intimacy, spiritual and emotional fulfillment, and the restoration of balance both a woman’s body and life. By our 50’s, most of us have experienced enough pain and difficulties to last us a lifetime. These experiences are meant to serve as catalysts for inner growth and self-discovery. In midlife we must move out of the compulsion to ‘please the world’ and into an inner place of authentic connection. We must learn how to be true to ourselves. For us to find meaning and not feel victimized by our past requires hard work and a desire to seek. The process of seeking itself deepens our ability to be intimate and fulfilled. In my medical practice, I find that women, who are committed to ‘seeking’ as they age, feel more fulfilled with intimacy, rather than the sex act alone. They are able to more readily accept their changing bodies. Sexual pleasure to them feels different, in some ways more fulfilling, even though their libidos may not be as heightened as in youth. The irony is that they are the ones who feel more sexual fulfillment than those who continue on the treadmill defined by the collective, with expectations of eternal youth. Because our society has glorified youth and the sex act in favor of aging and intimacy, as we age, society expects us to perform both physically and sexually similar LIBIDO>>page 27

Certain anatomic features of the eye are felt to increase the risk of these complications, but all eyes are at least a very small risk of these occurrences. Our aging bodies have different needs: the need for a pure diet, exercise, work/life balance, attention to inner process, and - one that we have not yet normalized in our society - a search for meaning. The spiritual aspects of ourselves call to us more powerfully and with greater zeal as we age. Our physical bodies also require more attention. Continuing the unhealthy diet of our younger years, does not nourish our changing bodies, and many of the lifestyle choices that have gained traction do not support our emotional or physical bodies. This level of self-awareness needs to be paramount as we age if we desire to achieve a state of true health, which should no longer be defined by a ‘hard body’ or a ‘boundless sex life’. For men, Viagra and Cialis do not fulfill these needs. The ability to hold an erection for a longer time is not an adequate substitute for health, meaning, or intimacy. Our society is struggling with the choices it has collectively made. Frankly, our collective ideology is in contrast to the laws of Nature. In other words, it goes against the laws of Nature. Due to the decades of traction behind our unnatural ways of life, we continue to pump out pills as substitutes for what is missing at deeper levels of our being. The woman’s libido pill Addyi will not be able to offer what women consider as sexual fulfillment – namely: intimacy, meaning, and a renewed relationship with themselves and their partners. The risk of life-threatening side effects alone is not worth taking a libido pill. As we age and seek deeper levels of meaning, biology (not surprisingly) follows suit. Not necessarily with the same intensity as it did during youth, but our aging physical bodies respond to emotional and spiritual depth differently than when we were young. The zeal to live from depth is what Nature supports as we age, in favor of the zeal to procreate as in youth. At this time we do not need yet another pill with more side effect casualties left at the altar of our flawed ideology. We need to reframe our collective consciousness and normalize the necessity for healthy aging as defined by physical, emotional, and spiritual health. The Best Alternative Method For Pain Relief
Anthony S Blair and Kimberly Stueber My body was slightly tense, waiting on the needles to prick my skin. I’d heard of acupuncture and the benefits of acupuncture treatment. This was my first try and didn’t know what to expect. His method is diversion. As I lay on the table, Dr. Zhou talks to me and touches different areas of my leg. Where I expected to feel the prick of the needles and where the needles actually went in were different which minimized the pain. Dr. Zhou is skillful in his insertion of the needles. I believe most people’s aversion to needles makes acupuncture seem more painful than it is. Injuring my Achilles tendon playing basketball I sought to heal myself. Traditional doctors told me I needed surgery. I never had surgery and was resistant to being cut, scarring,
anesthesia, and the cost. A friend recommended acupuncture and after doing some research, settled on Dr. Zhou. 30 minutes, 15 needles, 3 glass suction cups, and a heat lamp later I had noticeably greater mobility & flexibility I came in on crutches and left walking in a brace. After further treatments my injury healed in the same amount of time it would have taken with invasive surgery. And I don’t have the scar or the $5,000 bill, amazing. Stories like these are very common after people visit Dr. XiPing Zhou. Zhou, a doctor of oriental medicine, licensed acupuncturist, and the founder and president of Dr. Zhou’s Acupuncture and Pain Management Clinic in Madison, WI, says “Acupuncture is perfect for naturally treating pain and many other conditions.” The name given to one of the points in acupuncture work - Gushing Spring – is symbolic of the roots of this ancient Chinese folk medicine that aims to balance the energy system of qi (pronounced chee) and treat common disorders and pain all at the same time. Acu comes from the Latin word Acus meaning “needle” and puncture comes from Late Latin word punctura meaning “a pricking”. Thus the technique consists of pricking an acupuncture point with a needle. Acupuncture developed out of the need to apply pressure to the body, treat wounds, and lance infectious lesions. The earliest evidence of practices similar to acupuncture date back to the New Stone Age (8,000-2,000 B.C, the first acupuncture needles were crudely shaped & made from stones known as Bian Stones. Crude Bian Stones gave way to more skillfully shaped needles made of pottery. With the development of metallurgical techniques, the metal needle superceded the pottery needles and Bian Stones. The term acupuncture didn’t surface until shortly after the Ming Dynasty in 1684. According to Zhou, acupuncture is an element of traditional Chinese medicine (TCM). Beyond the benefits of pain relief and treatment of common disorders, acupuncture is holistic - keeping the mind, body and spirit balanced and in harmony. “It is a complete approach,” Zhou says. By working treats the source of the disorder not just the symptoms. It would be accurate to say that acupuncture treats disorders of QI, Blood and disturbances of the Organ Networks – but this doesn’t correspond to the Western vocabulary of named diseases and conditions. Acupuncture can be helpful for: pain management, withdrawal from addictions, post-surgical recovery, chronic fatigue and signs of aging and decreased immunity. Some of the many conditions for which acupuncture is considered appropriate are listed by the World Health Organization of the United Nations. Mindfulness Practice HELPS CHILDREN, TOO
“Mindfulness is the awareness that arises through paying attention in a particular way: on purpose, in the present moment, and non-judgmentally.” -- Jon Kabat-Zinn
Mindfulness meditation, inspired by Buddhist traditions, really took root in the United States after 1979, when Jon Kabat-Zinn founded the Mindfulness-Based Stress Reduction program at the University of Massachusetts. The practices, which emphasize attention to breathing and focusing on the present moment, offer many benefits, according to the American Psychological Association. Those include stress and rumination reduction, increased focus and less emotional reactivity. While it’s easy to understand how that would benefit the typically harried adult in this country, it’s a little more difficult to picture a child sitting quietly meditating. Yet mindfulness practices are being introduced to children in school settings, and with good reason. “I think schools are really stressful for kids,” said Tracey Sparrow, Ed.D., vice president of Children’s Programs at the Milwaukee Center for Independence. “So are social situations and home situ-
ations.” Mindfulness practice, she said, “gives them a tool to deal with that stress.” At MCFI’s School for Early Development and Achievement, Sparrow said mindfulness practices are taught using the MindUp™ Curriculum, 15 lessons with activities designed to help children concentrate, manage their emotions, better handle conflicts and have less stress. MindUp™ is an evidenced-based, CASEL-accredited social and emotional literacy program of The Hawn Foundation, founded by Goldie Hawn. “They go through structured lessons,” Sparrow said. “They do a lot of breathing exercises.” But if you’re imagining a room filled with children sitting cross-legged with eyes closed mouthing “Om” in unison, you’re picturing it wrong. Sparrow said students will place a teddy bear on their stomach and watch as it goes up and down with each breath. “It’s a good way for them to get focused.” She said mindfulness practices have even greater implications for children with special needs, who can learn to self-regulate t heir behaviors. For instance, she said an active child with a sensory disorder can be taught to use breathing techniques to calm down. And once the practice is learned, that child can, on his or her own, practice it anytime, anywhere. “Our kids have started, when something upsets them, to step back and take a breath,” said Sparrow. Research indicates mindfulness practice also enhances executive function – the ability to plan, organize and manage time. Academically, that will benefit all children, said Sparrow. MAKING THE GRADE?
As biomechanical, postural and anatomy specialists, Physical Therapists are knowledgeable to address the growing needs of students, assessing the benefits and dangers of specific types of back packs. While the backpack is still one of the best ways to bear/carry a load, a too heavy or improperly adjusted backpack may be harmful to muscles and joints, especially in the younger students. In time for the return to school season, physical therapists offer some specific advice on how to properly wear a back pack and avoid injury. In the hierarchy of bags, a backpack is a better way to carry the necessities than a briefcase or shoulder bag, especially for longer periods of time. Properly worn, a backpack is supported by the strongest muscles in the body; the back and abdominal muscles which work together to stabilize the trunk and hold he body in postural alignment. An improperly worn back pack can also present some real dangers, especially the young, still growing joints and muscles .
Here are some guidelines to follow:
Wear both straps: slinging a backpack over one shoulder causes a person to lean to one side and compensate for the uneven load thereby curving the spine. Overtime, this may cause functional scoliosis, curvature of the spine. Teenaged girls are especially susceptible to scoliosis. Both straps should be shortened enough so the pack rests centered between the shoulder blades, not hanging down the back to the waist, therefore it won’t add an undue backward force to the shoulders and lower spine Make certain the backpack is not too heavy. Students of all ages seem to carry heavier books and loads, often toting a full day’s worth of books and a change of clothes for after school events. A backpack can range anywhere from 20-50 pounds of extra added weight to the body’s own structure, which is greater than the amount typically gained during a female’s pregnancy. Even when properly worn, utilizing both straps, the compensatory leaning forward then affects the natural curve of the spine and the disc (cushion) structures between t he bones. This leaning due to the extra weight will eventually cause rounding of the shoulders and an exaggeration of the forward curve on the mid and upper back regions. As a result the student may experience back pain, shoulder or neck pain. A general rule to follow: the back pack should be no more than 15-20% of the child’s body weight . Pay attention to the type of backpack. Look for backpacks that have wider shoulders straps, narrow strap may cause painful shoulders by “digging into the shoulders affecting the nerves close to the surface of our clavicle (collarbone). Narrow straps can also hinder circulation, causing other numbness and/or tingling in the arms, which over time may cause weakness in the hands. Also look for a backpack with a waist strap and utilize the strap to off set the shoulder forces. It is also wise to consider the weight of the backpack while it is empty, comparing canvas verses a leather type backpack. To remain injury free, have your kids use both straps and make frequent stops at their locker throughout the day to avoid carrying unnecessary items. Leave non essential items at home or school and when possible, get a duplicate set of books or studying materials to be left at home. Above all, urge your child to tell you when they are in pain or have discomfort so the concerns may be addressed early. In addition to these backpack concerns, with the advent of more computer technology and resources, observe your child’s static posture for good alignment. Note if they are utilizing the chair correctly; in a properly fitted chair, the back supported up to the mid shoulder region, with both feet on floor allowing hips and knees at 90 degree bends. If concerns are noted in these areas, work with a postural specialist and consult your child’s physician and seek a referral for physical therapy to assess their posture and learn the stretches and strengthening exercises to address the noted deficits. THE ABCS of kid’s physicals Shenell Miller, MD Working your way through your kids’ back-to-school checklists? Make sure their annual summertime checkup or sports physical is at the top of the list. The American Academy of Pediatrics recommends annual exams for children and teens ages 5-18 years. Routine health screenings are one of the most important steps you can take for your school-age children. Making sure your kids are in good health can even impact how well they will adjust to the new school year and perform in the classroom. The importance of annual check-ups Many school districts require a signed form from your child’s pediatrician to start the school year. Kids who join WIAA organized sports will need clearance from their doctor before they can play. During their school-age years, children go through a time of rapid growth, learning, exploration and emotional and physical development. That’s a key reason to make physicals a top priority. An annual exam can: • Keeps your child’s health record up-to-date: Find a trusted doctor who will develop a long-term relationship with you and your child and carefully monitor your child’s health year by year. The doctor will follow your child’s growth and development and medical history, including any illnesses or injuries since the last check-up. • Provides the peace of mind of knowing your child is healthy. During the exam, your child’s doctor will do a thorough medical examination and discuss any developmental or health issues your child may be facing. • Makes sure immunizations are up-to-date. Your doctor will discuss the timing for various immunizations and give your child any required shots or booster shots. • Helps your pediatrician detect health problems. An annual physical is when many doctors spot irregularities. Your child’s doctor might recommend tests, referral to specialists or a follow-up exam, if needed, to monitor a problem. Also, because childhood obesity is a national problem, your doctor may recommend dietary changes for your child, if warranted. • Helps active kids avoid injuries. Many kids spend hours a week practicing and competing in team sports. The repetitive motions in many sports can cause serious injuries to growing bones and muscles. An exam allows doctors to screen for problems and discuss ways to avoid injury. Your child doesn’t need to be an athlete to benefit from a sports exam – after all, most kids are active riding bikes, swimming and skateboarding. • Gives teens a chance to talk with their doctor. A yearly check-up is crucial for teens who are going through major developmental and hormonal changes. Because kids tend to trust their doctor, they feel comfortable answering questions about bullying, Internet safety, driving, drinking, smoking, drugs, sexual activity, even lifegoals. Your pediatrician may also ask questions to screen your teen for depression or thoughts of suicide. • Gives you time to talk with child’s doctor. Exams are the perfect time to ask questions about proper nutrition, sleep requirements, exercise and other important aspects of your child’s life. Your doctor will share important safety tips like age-appropriate use of booster seats, seat belts, bike helmets and sunscreen. Getting your kids in to see the doctor will make you feel good, too. You’ll know you’ve done all you can to keep them healthy and help them succeed in the coming school year. Acupuncture can be helpful for: pain management, withdrawal from addictions, post-surgical recovery, chronic fatigue and signs of aging and decreased immunity. Some of the many conditions for which acupuncture is considered appropriate are listed by the World Health Organization of the United Nations. A new study of acupuncture — the most rigorous and detailed analysis of the treatment to date — found that it can ease migraines and arthritis and other forms of chronic pain. The findings provide strong scientific support for an age-old therapy used by an estimated three million Americans each year. Though acupuncture has been studied for decades, the body of medical research on it has been mixed and mired to some extent by small and poor-quality studies. Financed by the National Institutes of Health and carried out over about half a decade, the new research was a detailed analysis of earlier research that involved data on nearly 18,000 patients. Acupuncture, which involves inserting needles at various places on the body to stimulate socalled acupoints, is among the most widely practiced forms of alternative medicine in the country and is offered by many hospitals. Most commonly the treatment is sought by adults looking for relief from chronic pain, though it is also used with growing frequency in children. According to government estimates, about 150,000 children in the United States underwent acupuncture in 2007. (http://well.blogs.nytimes.com/2012/09/11/acupuncture-provides-true-pain-relief-in-study/, O’Connor, Anahad) Additionally Time magazine stated that more than a quarter of all Americans suffer from pain daily. This costs the U.S. about $60 billion in lost productivity annually. Additionally, Americans spent $2.6 billion on over-the-counter pain medications and another nearly $14 billion on out patient analgesics. Recent findings in the Journal of the American Medical Association found that spending on spine treatments totaled nearly $86 billion in 2005 which is a 65% increase from 1997. However the proportion of people with impaired function because of spine problems increased during the same period even after controlling for an aging population. All of these facts point to the reality that modern pain treatments aren’t working. On the other hand people are finding great success with Acupuncture, a treatment that is over 5,000 years old. Are Baby Boomers Forgetting Something Essential When Planning Retirement? Many Don’t Realize It Until They Stop Working Most people have some kind of lifestyle vision for retirement. Unfortunately, without proper planning their dreams won’t always become a reality as they enter the encore time of their lives, says Michael Bivona, a certified public accountant who retired almost 20 years ago. “I had a simple plan: When I stopped working I planned on living on my 42-foot Chris Craft cruiser with my wife, Barbara, which was a very pleasant pastime during my busy working years,” he says. “But, after a few weeks, as we tried to make our dream a reality we found that we were bored out of our minds. “Pre-retirees and retirees are rightly concerned about whether they can afford retirement. But not having enough to do is another kind of deficit that is frequently overlooked until it’s too late,” says Bivona, author of the book “Retiring? Beware!! Don’t Run Out of Money and Don’t Become Bored” (www.michaelbivonabooks. com). He offers possible avenues to explore for a fulfilling retirement lifestyle. • Develop a social network with senior civic centers. Civic centers, which usually have a department dedicated to the betterment of the senior citizens who live in their areas, can be found in almost every municipality in the United States. These centers offer a wide range of activities. Additionally, the following online search, “Fun Activities for Senior Citizens,” offers a nice list of activities and associated details that can be explored prior to retiring. • Try on a pair of dancing shoes. For some, the thought of dancing may elicit a strong sense of aversion, but you may want to try it anyway. The benefits include exercise, coordination and possibly enjoying a romantic hobby with your spouse or others. Dancing is a beautiful art form that gives participants something with which to challenge themselves, Bivona says. A goal-oriented mindset is a healthy one especially when approaching the encore years. • Make the most of your travels. Traveling is a common bucket list item for most retirees. Of course, it’s going to cost you. That’s why this is a subject that perfectly combines the two great concerns for retirement: money and purpose, both of which can be maximized with “research, research, research, Bivona says. You might even parlay traveling with another interest, such as your family history. You may learn about your roots at www.Ancestory.com, and then visit areas based on your research. Or, you may be more interested in stretching your dollar. There are many cost effective deals to be had by researching your heart’s desire on the internet. • Stay sharp and keep learning. The mind is much like the body: If you don’t use it you will accelerate the process of losing it. Building bridges to new adventures is the key to maintaining your mental acuity and increasing your vitality. There are an abundance of educational courses developed for seniors to keep them exercising their mental prowess. Remember, if you started working in your twenties and retire in your sixties, there’s a good chance you’ll spend as many years in retirement as you did working. So building bridges to what you want to do in a rational manner for your encore years is imperative if the last phase of your life is to be enjoyable for you and your love ones. KNOWING WHEN TO ASK FOR HELP Age-Related Changes In Memory Forgetfulness can be a normal part of aging. As people get older, changes occur in all parts of the body, including the brain. As a result, some people may notice that it takes longer to learn new things, they don’t remember information as well as they did, or they lose things like their glasses. These usually are signs of mild forgetfulness, not serious memory problems. Some older adults also find that they don’t do as well as younger people on complex memory or learning tests. Scientists have found, though, that given enough time, healthy older people can do as well as younger people do on these tests. In fact, as they age, healthy adults usually improve in areas of mental ability such as vocabulary. Other Causes Of Memory Loss Some memory problems are related to health issues that may be treatable. For example, medication side effects, vitamin B12 deficiency, chronic alcoholism, tumors or infections in the brain, or blood clots in the brain can cause memory loss or possibly dementia (see more on dementia, below). Some thyroid, kidney, or liver disorders also can lead to memory loss. A doctor should treat serious medical conditions like these as soon as possible. Emotional problems, such as stress, anxiety, or depression, can make a person more forgetful and can be mistaken for dementia. For instance, someone who has recently retired or who is coping with the death of a spouse, relative, or friend may feel sad, lonely, worried, or bored. Trying to deal with these life changes leaves some people confused or forgetful. The confusion and forgetfulness caused by emotions usually are temporary and go away when the feelings fade. The emotional problems can be eased by supportive friends and family, but if these feelings last for a long time, it is important to get help from a doctor or counselor. Treatment may include counseling, medication, or both. More Serious Memory Problems For some older people, memory problems are a sign of a serious problem, such as mild cognitive impairment or dementia. People who are worried about memory problems should see a doctor. The doctor might conduct or order a thorough physical and mental health evaluation to reach a diagnosis. Often, these evaluations are conducted by a neurologist, a physician who specializes in problems related to the brain and central nervous system. A complete medical exam for memory loss should review the person’s medical history, including the use of prescription and over-the-counter medicines, diet, past medical problems, and general health. A correct diagnosis depends on accurate details, so in addition to talking with the patient, the doctor might ask a family member,

Remembering specific details from high school may come flooding back with ease but when your co-worker asks you what did last weekend, the details seem a little fuzzy. Forgetting those details don’t seem to bother us as much as when you can’t fit into your favorite pair of jeans but it should. You’re out of shape. Exercising your brain is just as important as exercising your body, especially as you get older. Numerous studies have been conducted to find out what effect mental exercise has on seniors specifically. They have found that mental exercise may help in keeping seniors living independently longer. Seniors who do regular exercises were found to be much faster at activities of daily living such as reaction time when driving, looking up a telephone number, and following medication instructions. The benefits of mental exercises have been shown to not only affect seniors’ daily activities but health as well. A study published in the January issue of Archives of Neurology provided evidence that keeping your brain active may help ward off Alzheimer’s. In the study, researchers compared the brain scans of healthy people who were on average 76 years old with scans from patients with Alzheimer’s and scans from healthy 25 year olds. They found that people who had been the most mentally active were less likely to develop brain plaques that are tied to Alzheimer’s disease. With the evidence for the importance of mental exercises growing, so has the prevalence of seniors participating in programs designed around mental exercises. “Every activity has a different difficulty level, so everyone in the class can continuously feel challenged,” says Athena Logothetes, who facilitates the Mind Aerobics Classes at Milwaukee Catholic Home. In a Mind Aerobics class, seniors work on crossword puzzles, brain teasers, word games, and trivia. Trivia of current events specifically is important because like the details of our weekend that we can’t remember, short-term memory is where seniors often have more difficulty. “It gets their brain juices flowing and keeps them actively thinking even if at some point their bodies aren’t as strong, their brain will always be strong.” says Athena. Don’t hesitate to bring up concerns, even if they don’t seem directly related to a medical condition. You and your doctor can make better decisions together if the doctor knows about your troubling non-medical concerns. If the doctor can’t help solve your non-medical problems, he or she may be able to refer you to other resources that can help. Tips: Advance Directives Advance directives allow you to say what you’d prefer if you are too ill to make your wishes known. There are two main kinds: Living wills — A living will describes your decisions for medical treatment at the end of life. Living wills typically refer to life-prolonging treatment when death is imminent. Healthcare proxies — Also known as a medical or health care power of attorney, a healthcare proxy is more detailed than a living will. It allows you to appoint the person or persons you trust to make health decisions for you if you cannot. It also allows for more detailed advance care planning, letting you state your wishes about resuscitation, feeding tubes, antibiotics, hospital transfers, respirators, and more. Because the healthcare proxy involves more decisions, you may want to talk to your doctor about various options for care. For instance, many people would be willing to try a feeding tube or a ventilator for a while, but then would want it to be stopped if their condition did not improve. Also talk to family members and the person you have appointed as your proxy to be sure they understand your wishes. Advance care directives are legally valid everywhere in the United States, but laws concerning them vary from State to State. Forms approved for the State you live in are available from many different health care organizations and institutions

No one ever plans to be sick or disabled. Yet, it’s just this kind of planning that can make all the difference in an emergency. Long before she fell, Louise had put all her important papers in one place and told her son where to find them. She gave him the name of her lawyer as well as a list of people he could contact at her bank, doctor’s office, insurance company, and investment firm. She made sure he had copies of her Medicare and other health insurance cards. She added her son’s name to her checking account, allowing him to write checks from that account. His name is on her safe deposit box at the bank as well. Louise made sure Medicare and her doctor had written permission to talk with her son about her health or any insurance claims. On the other hand, Ben always took care of family money matters, and he never talked about the details with Shirley. No one but Ben knew that his life insurance policy was in a box in the closet or that the car title and deed to the house were filed in his desk drawer. Ben never expected that his wife would have to take over. His lack of planning has made a tough job even tougher for Shirley. Steps for Getting Your Affairs in Order *Put your important papers and copies of legal documents in one place. You could set up a file, put everything in a desk or dresser drawer, or just list the information and location of papers in a notebook. If your papers are in a bank safe deposit box, keep copies in a file at home. Check each year to see if there’s anything new to add. *Tell a trusted family member or friend where you put all your important papers. You don’t need to tell this friend or family member about your personal affairs, but someone should know where you keep your papers in case of emergency. If you don’t have a relative or friend you trust, ask a lawyer to help. *Give consent in advance for your doctor or lawyer to talk with your caregiver as needed. There may be questions about your care, a bill, or a health insurance claim. Without your consent, your caregiver may not be able to get needed information. You can give your okay in advance to Medicare, a credit card company, your bank, or your doctor. You may need to sign and return a form.

The doctor also might do tests for memory loss and test the person’s problem-solving and language abilities. A computed tomography (CT) or magnetic resonance imaging (MRI) brain scan may help rule out some causes of the memory problems. Amnestic Mild Cognitive Impairment (MCI). Some people with memory problems have a condition called amnestic mild cognitive impairment, or amnestic MCI. People with this condition have more memory problems than normal for people their age, but their symptoms are not as severe as those of Alzheimer’s
disease, and they are able to carry out their normal daily activities. Signs of MCI include misplacing things often, forgetting to go to important events and appointments, and having trouble coming up with desired words. Family and friends may notice memory lapses, and the person with MCI may worry about losing his or her memory. These worries may prompt the person to see a doctor for diagnosis. Researchers have found that more people with MCI than those without it go on to develop Alzheimer’s within a certain timeframe. However, not everyone who has MCI develops AD. Studies are underway to learn why some people with MCI progress to AD and others do not. There currently is no standard treat-

ment for MCI. Typically, the doctor will regularly monitor and test a person diagnosed with MCI to detect any changes in memory and thinking skills over time. There are no medications approved for use for MCI. Dementia. Dementia is the loss of thinking, memory, and reasoning skills to such an extent that it seriously affects a person’s ability to carry out daily activities. Dementia is not a disease itself but a group of symptoms caused by certain diseases or conditions such as Alzheimer’s. People with dementia lose their mental abilities at different rates. Symptoms may include: Being unable to remember things Asking the same question or repeating the same story over and over Becoming lost in familiar places Being unable to follow directions Getting disoriented about time, people, and places Neglecting personal safety, hygiene, and nutrition Two of the most common forms of dementia in older people are Alzheimer’s disease and vascular dementia. These types of dementia cannot be cured at present. In Alzheimer’s disease, changes to nerve cells in certain parts of the brain result in the death of a large number of cells. Symptoms of Alzheimer’s begin slowly and worsen steadily as damage to nerve cells spreads throughout the brain. As time goes by, forgetfulness gives way to serious problems with thinking, judgment, recognizing family and friends, and the ability to perform daily activities like driving a car or handling money. Eventually, the person needs total care. In vascular dementia, a series of strokes or changes in the brain’s blood supply leads to the death of brain tissue. Symptoms of vascular dementia can vary but usually begin suddenly, depending on where in the brain the strokes occurred and how severe they were. The person’s memory, language, reasoning, and coordination may be affected. Mood and personality changes are common as well. It’s not possible to reverse damage already caused by a stroke, so it’s very important to get medical care right away if someone has signs of a stroke. It’s also important to take steps to prevent further strokes, which worsen vascular dementia symptoms. Some people have both Alzheimer’s and vascular dementia. Treatment For Dementia A person with dementia should be under a doctor’s care. The doctor might be a neurologist, family doctor, internist, geriatrician, or psychiatrist. He or she can treat the patient’s physical and behavioral problems (such as aggression, agitation, or wandering) and answer the many questions that the person or family may have. What You Can Do If you’re concerned that you or someone you know has a serious memory problem, talk with your doctor.
Understanding A Devastating Disorder ALS most commonly strikes between the ages of 40 and 60, but younger and older people also can develop the disorder. In the summer of 2014, social media was taken by storm with videos of people pouring ice water on themselves for the Ice Bucket Challenge. The worldwide phenomenon raised awareness—and millions of research dollars—for a fatal disease called ALS. ALS stands for amyotrophic lateral sclerosis. It’s also called Lou Gehrig’s disease. ALS attacks motor neurons, the nerve cells responsible for regulating “voluntary” muscles that we’re aware of controlling, such as those in our arms, legs, and face. As these motor neurons degenerate or die, they stop sending signals to muscles. Unable to function, the muscles gradually weaken and waste away. “At first only a single limb may be affected, such as some weakness in a hand or a leg, or a person may have problems speaking or swallowing,” explains Dr. Amelie

Gubitz, who oversees much of NIH’s ALS research. Other symptoms may include stiff muscles, cramps, or stumbling. It may become hard to turn a key, lift a coffee pot, or button a shirt. Eventually, all muscles under voluntary control are affected, and people lose the ability to move different parts of their body. When breathing-related muscles fail, ventilation support can help people breathe. Most people with ALS die from respiratory failure, usually within 3 to 5 years after symptoms first appeared. However, about 10% of people with ALS survive for 10 or more years. Nationwide, more than 12,000 people have ALS. It affects people of all races and ethnicities. ALS most commonly strikes between the ages of 40 and 60, but younger and older people also can develop the disorder. Men are more likely than women to be affected. In most ALS cases, the underlying causes are unknown. “The reasons why you get it are not really understood,” Gubitz says. “Even a healthy young person can get this disease out of the blue.” About 10%, are inherited. In 1993, scientists discovered that mutations, or alterations, in a certain gene were associated with some cases of ALS. Since then, mutations in over a dozen genes have been found to cause familial ALS. These discoveries provide new information about the disease that will help guide future research. Although ALS has no cure, it can be managed to some extent with medication and therapy. In 1995, the U.S. Food and Drug Administration approved a medication called riluzole (Rilutek) as the first drug treatment for ALS. The drug prolongs life by several months, and it can extend the time before someone needs ventilation support. Riluzole doesn’t reverse the damage already done, and patients must be closely watched for possible side effects. Other treatments for ALS aim to ease symptoms and improve quality of life. For example, an FDA-approved drug combination of dextromethorphan and quinidine (Nuedexta) helps manage intense emotional changes, such as uncontrollable laughing or crying, that are often seen with ALS. Because the mind remains relatively intact, people with ALS may be keenly aware of their continued loss of function. They may become anxious and depressed. Health care professionals can tailor plans for therapy and equipment to keep people as mobile and comfortable as possible. While much is still unknown, scientists are working to gain new insights into ALS. “I believe potentially promising approaches for treatment are in the pipeline,” Gubitz says. “Researchers are trying to attack the disease from many different angles, which brings much hope for progress.”

Senior Housing COMING A LONG WAY OVER THE YEARS By Jan Rupnick .Starting approximately 30 years ago, this industry saw new options develop to meet the demands of a growing senior population. Initially limited in its offerings, there is now a plethora of senior housing options available. With more and more baby boomers getting ready to retire, the senior housing market is growing with new senior housing developments. Some baby boomers are looking into senior housing options for themselves, while others are looking for communities that can help care for their aging parents. Finding the right option comes down to knowing what’s available and understanding the needs of the person who is going to be living in the community. Below are descriptions of the various senior housing options available and the segments of the senior population that they serve. Independent Senior Housing For seniors who are tired of maintaining their own home or looking to down-size, independent senior housing is ideal. Those looking to move into an independent senior living community have two choices. One is to purchase a senior condominium and the second is to rent a senior living apartment. Both provide independent, active seniors with the freedom they enjoy, but without all of the burdens of home ownership like mowing the lawn, shoveling the snow, or making home repairs. Some independent senior communities have also adapted a lifestyle approach, including such amenities as a salon and spa, golf course, on-site restaurants, on-site shopping, housekeeping and more. Assisted Living Assisted living provides a great alternative for seniors who need some additional help with medication, grooming, dressing, eating, etc. Assisted living has two options including; Residential Care Apartment Communities (RCAC), which allows seniors to live in their own apartment, but only offers 28 hours of care, and CommunityBased Residential Care Facility or CBRF. This type of community is an ideal living option for seniors who need more assistance that what can be provided by an RCAC, offering care 24 hours a day, seven days a week. Assisted living communities tend to offer more of a residential, home-like environment, as well as provide meals, social activities, housekeeping and transportation. Specialized Care Community There are some CBRFs that specialize in caring for one kind of illness. One of the most common is an Alzheimer’s or memory impairment care community. In specialized assisted living communities, the environment, staff and programming is specifically designed to meet the unique needs of those individuals who are living there. Like a non-specialized assisted living community, a specialized care community also provides 24/7 care, social programming, meals, housekeeping and transportation are generally provided. Nursing Home This is probably the most commonly known, but often confused type of senior living option. It’s also one of the only options that used to be available to seniors. Nursing homes provide 24-hour skilled nursing services for seniors who need rehabilitation after a surgery, or require care for issues like feeding tubes. The levels of hospitality services in nursing homes vary greatly depending on their targeted clientele. Whether you’re looking to down-size your home or a community to meet the needs of an aging parent, there is a senior living community to meet your needs. Bird watching is another outdoor activity that takes advantage of fall’s cool temperatures, which means you can take longer strolls, giving you even more opportunity to catch a glimpse of birds flying past. This activity satisfies the desires of animal lovers, science enthusiasts and exercise-minded seniors alike. Outdoor yoga Whether you’ve never done yoga or are already a practiced yogi, performing yoga outdoors can be a fun, fresh experience. Taking an outdoor yoga class in the fall means that as you engage your muscles, you’re also taking in all the beautiful colors of fall. Waiting until fall to head outdoors with your mat also means avoiding the extreme heat of summer. Still, fall can get cold and windy, so make sure you’re not underdressed when exercising outdoors. Yoga pants and long-sleeved, sweat-wicking clothes will help keep you dry and warm. To the farm Fall is known for apples and pumpkins, and there are plenty of opportunities in autumn to get closer to both. They may have stands selling locally made art or produce, which let you support the people around you while getting an insight into what they make and do. Quick Tips: Depending on your location, fall weather can be unpredictable. Be sure to check the forecast before you head out to make sure you’re prepared for temperature changes or rain. Layers are your best friend in fall, since they’re easy to take off or put on as you move from inside to outside. Scarves, shawls and sweaters will help let you control your personal temperature. All those leaves covering the ground may be beautiful, but they can also be very slippery. Take care when walking through or across leaves, particularly if it has rained recently. I have always been an active individual and after hitting 49 or so I just didn’t feel so energetic anymore. No matter how much more I exercised, improved my diet, changed sleeping patterns etc. I had blood tests to rule out any major issues and even a sleep test to rule out sleep apnea. Everything came back GOOD!!! All my physician friend would just tell me “Roberto you’re just getting older” . I didn’t agree with this statement so I just kept looking. Then about 11 months ago I met Mr. Tristen Dunn, the owner of the B12 store at Southridge Mall, we started talking about my concern and came to find out that I was B12 deficient. It even showed on my previous blood tests and nobody caught it. I was taking vitamin supplements but it was still deficient. Keep the Fork Rev. Joe Jagodensky, SDS It’s a beautiful display of all the food groups at once facing you, filling nostrils and mouths beginning to water. The table is set and everyone is seated except the oldest who seems to appreciate the bathroom more than the rest of us. I think to myself, it’s okay, it’s a party. We’re in no hurry for it to end. “When was the last time I had a linen napkin?” I wonder. If I had this party, I’d have to buy a bunch of them for a group this size. The dishes look as elegant as the napkins but I’m too shy to turn the plate over to see who’s responsible for creating this fine china. I resign myself to “go with the flow,” as they say and just enjoy the evening as it unfolds. I see lots of spoons in bowls and platters but I also see that I have only one fork. Ummmm. “I guess I better take care of this fork,” I think to myself. “Family style” they call it as you pass dishes to each other amid loud conversations and feel as though you’re reenacting a scene from “The Waltons.” I see one person holding the bowl making it easier for the older woman who just can’t seem to get that bean she wants onto her plate. Another courteously refrains from the portion he truly wanted so that the last person can enjoy some as well. A perfect meal with delicious tastes at each round. The dishes are carefully picked up when the host surprisingly alerts us to “keep your fork.” I’m dumbfounded. “That explains the expensive dishes and napkins,” I think to myself. “She couldn’t afford enough forks for us. Poor thing. Surely there must be another set of forks laying around that she could extend to us.” I stare at my one fork and am glad that it’s not as dirty as it could have been. I’m also wondering if I should take the fork home as a souvenir of my “one-forked” evening. I assume she’d miss it since we needed to keep the one already in our possession. I play with it during the lull while I see others moving theirs around as they talk and laugh. We are all waiting for our one fork’s final use. Something was mentioned about dessert but the youngest among us dismisses the notion. There are many courses during life. Some include lessons and others are academic that we work through to get to the next course. All of it includes food – for the body, mind and spirit. The most important part of any course or meal is the digestion, the time of simmering and letting rest what has been taken in. We digest a lot about our relationships and about ourselves. Hearing Loss Questions Answered Hearing aid technology has increased significantly over the last 15 years. Audiologists are the most highly skilled and trained professionals to manage and treat hearing loss. What are some signs that your hearing may be declining? Most people start to notice that they can hear other’s speaking, but that they cannot understand what they are saying. This is common complaint of people who slowly lose their hearing due to the aging process. Another strong sign is that family members and friends have noticed your hearing loss and are forced to repeat themselves so that you understand what they are saying. How has technology improved over the last 15 years regarding hearing aids? Hearing aid technology has increased significantly over the last 15 years. The overwhelming majority of hearing aids fit today are digitally programmable. Digitally programmable hearing aids allow audiologists to make more precise fittings to match individual hearing losses and sound quality desires from patients. Also, the hearing aids can be re-programmed to fit your hearing loss if you continue to lose more hearing in the future. Today’s hearing aids offer automatic volume control’s and features that allow better speech and noise management. Some hearing aids communicate with each other while the patient is wearing them to allow for automatic volume and noise management in each ear at the same time. Today’s technology now has some hearing aids that use rechargeable batteries, remote controls, and direct Bluetooth connections to telephones, TV’s and other electronic devices. Are all hearing aids sold by licensed audiologists? No. Audiologists are the most highly skilled and trained professionals to manage and treat hearing loss. New audiologists are now required to hold a Doctorate of Audiology degree to be licensed to practice. I strongly recommend that people seek an Audiologists’ opinion before purchasing a hearing aid. Audiologists are trained to identify certain auditory conditions and refer to physicians for medical intervention before hearing aids are fit. Hearing aids are also sold by hearing aid dispensers. The level of training for Audiologists vs hearing aid dispensers is significantly different. Many hearing aid franchise establishments employ hearing aid dispensers instead of Audiologists. What is the price range of a hearing aid package? Common price ranges are from $1000.00 to 3,000.00 per hearing aid. I caution people not to shop for hearing aids based solely on price. The most expensive hearing aid on the market, fit on the wrong hearing loss or fit incorrectly, will produce poor results. On the other hand, someone with an active lifestyle will need the correct hearing aid technology to match that lifestyle and hearing loss. If it is not met correctly, the hearing aid will not be able to keep up with the patients listening lifestyle and the results will be poor. It is important to match the level of hearing aid technology to the patient’s hearing loss AND lifestyle. My suggestion is to get a complete audiological evaluation and hearing aid consultation. A hearing screening is NOT the same as a complete audiological evaluation. Don’t be fooled by advertisements that offer a “free hearing screening”. How long do hearing aids last? The general life of most hearing instruments is commonly 5-7 years. I do see patients with 8 -10 year old hearing aids that are functioning very well. Why do some people do better with hearing aids than others? It depends on the person and the hearing loss that they have. When we test your hearing, we are commonly looking for how much hearing sensitivity you have left and how clearly you can hear speech. It’s the clarity of speech issue that really separates many people. The human brain is much more powerful than even the most sophisticated hearing aids. Our ears are just the way in. Once someone gets new hearing aids, the brain will take some time to adjust to the new sound it is now receiving. Full adjustment takes about 6months for the brain to “re-wire” itself to the new sound. If people use their hearing aids once in while, or just on one side, or even just a few hours a day, then the brain does not get a consistent auditory signal and results are poor. If you have hearing aids, you should be using them during all waking hours. Your brain needs to get a consistent signal and it needs to hear the sounds you are regularly exposed too. Even if you live alone or commonly live in a quiet environment. It is important that you hear the normal sounds in your house while using aids (ex. Air conditioner, refrigerator, ceiling fan etc…). You will eventually learn how to disregard the sounds of your home because you are so experienced at listening to them daily. When someone does come to your home or calls you on the phone, your brain is already trained to ignore the sounds of your home and concentrate on the speech you are trying to hear. By Donna Spars People are often surprised to learn that we have a great deal of control over whether we get dementia. Research has consistently shown that choices we make have as much to do with preventing dementia as do genetics. And better yet, changing some of our choices—at any point in our lives—rewards our brain. By the way, “dementia” does not mean sometimes forgetting names or appointments or occasionally forgetting words or sometimes making errors in your checkbook. Dementia is a word that describes symptoms all caused by damage to brain cells. The most common disease that causes dementia symptoms is Alzheimer’s. There is more information about dementia in the rack by the LifeStreams fitness room. So, what can you do to increase your chances of being able to use your mind as you’d wish? The recipe for success includes taking care of our bodies, minds and spirits. This month we’re going to discuss taking care of our bodies and in subsequent months our minds and spirits. (And by spirits I don’t mean cocktails!) Taking care of our bodies includes moving, eating smart and living with our brains in mind. So, the first word there is “moving”. Yes, that means exer-cise. Of everything you can do, exercise has the strongest research support for helping to prevent dementia. What does exercise have to do with a healthy brain? Exercise boosts the brain’s physical health, sharpens our intellectual abilities and may contribute to building a protective buffer in the brain against memory loss. So how much exercise is enough? The literature on that subject gives a variety of answers but, in general, about 30 minutes a day of exercise that increases your heart rate to the point that you’d have a difficult time carrying on a conversation is enough. Don’t want to exercise every day? That’s fine, but then do more minutes on the days you choose to exercise. Don’t get any or enough exercise? Perhaps in a conversation the two of you can come up with a plan you’ll find enjoyable, or at least palatable! The second part of taking care of your brain by taking care of your body is to eat healthy foods and maintain a healthy weight. You probably have a very good concept of what constitutes a “healthy diet”. However, the guidelines have changed re-cently so it’s worth covering them. According to the CDC website, a healthy daily diet consists of 1 1/2 cups of a variety of fruits and 2 cups of a variety of vegetables, 6 servings of mostly whole grains (one slice of bread is a serving), 5 servings of lean protein (1 egg or 1 tablespoon of peanut butter is a serving) and 3 cups of low-fat milk, yogurt or cheese. “Dry scratchy, irritated eyes are a common problem, affecting people of all ages, especially women over age 40. blood vessels that cause wet AMD.” Freedman, who has treated thousands of AMD patients, reports, “Although there are NO guarantees, we have seen remarkable results with these medications. With regular injections (about every month or so), we have been able to stop the progression of wet AMD in 90 percent of our patients, and even had up to 30 percent gain improvement in vision.” (Injection treatment is also successfully used to control diabetes-related vision damage.) “Dry scratchy, irritated eyes are a common problem, affecting people of all ages, especially women over age 40. The most frequent cause is the natural decrease in tear production that occurs with aging,” notes Daniel Paskowitz, MD, PhD, an ophthalmologist with credentials from both Harvard and Johns Hopkins. Dry eyes can also be related to rheumatoid arthritis, lupus, psoriasis, Parkinson’s disease, chemical burns, thyroid disease, eyelid position or closure problems, and prescription and over-the-counter drugs. Hormonal changes—as with pregnancy, menopause and hormone replacement therapy—may also cause dry eyes. For most people, dry eye disease is uncomfortable but not vision-threatening. Sometimes, however, a faulty or diminished tear film can affect the cornea’s focusing ability and increase the risk of eye infections. “Dry eyes are usually a life-long condition that can be controlled, but rarely cured. If frequent use of artificial tears does not halt discomfort, you may be a candidate for two other options: a five-minute office procedure that places tiny silicone plugs into the drain openings at the corner of the eye to eliminate tear loss and retain moisture; or a prescription eye drop called Restasis™ that attacks the underlying inflammation behind dry eye disease,” explains eye care specialist Michael Raciti, MD. He adds, “Don’t suffer in silence. Talk to your gynecologist, doctor, and/or eye care specialist if you think HRT or other medications or conditions are affecting your vision. They can evaluate and address your concerns so that you can
experience tears of joy rather than pain.” Daniel Ferguson, MD, a corneal specialist, surgeon and partner at Eye Care Specialists ophthalmology practice, has helped countless patients overcome eye infections, abrasions and injuries. “It’s rewarding to help someone resharpen their focus. What’s frustrating, however, is if the problem didn’t have to happen in the first place. For example, I’ll see men with minor to serious eye injuries because they did not wear proper eye protection when doing sports, hobbies or handyman projects. A simple pair of safety glasses or goggles could have saved them time, money and suffering. But, for women, I’ll sometimes see vision-threatening issues caused by something you may not expect . . . cosmetics. “Many eye makeup-related dangers (like infections and abrasions) can be avoided by following simple common sense precautions. One analogy I like to use is that you most likely wouldn’t want to shake someone’s hand after watching them sneeze into it, so why would you want to take their mascara wand or eye shadow and wipe the same bacteria around your eye?” In addition to the “no-sharing” rule, Ferguson recommends following safe storage, use and replacement precautions for eye makeup. This includes always washing your hands prior to application, insisting on the use of disposable applicators when trying makeup at a store or salon, never sleeping in makeup, never putting mascara on while driving, keeping makeup containers out of the sun and heat, carrying makeup in frequently replaced plastic zip lock bags, and discarding any product that causes an allergic reaction or infection. How can women protect their vision to maintain independence and quality of life? “Scheduling regular comprehensive dilated eye exams is the best way to protect your health, safety, and freedom to see, drive, cook, read, shop, visit, and enjoy life to the fullest,” advises Dr. David Scheidt, past president of the Wisconsin Optometric Association. “During your appointment, ask your eye care specialist to review the risks and benefits of any applicable prevention and/or treatment options, including vision-related nutritional supplements. Don’t Ignore an Uncomfortable Problem Most men probably don’t like thinking about their prostates, but it’s worth doing once in a while. Problems with the small gland are common in men past the age of 50. The good news is that most prostate conditions can be successfully treated. The prostate is about the size of a walnut. It wraps around the urethra, the tube that carries urine out of the bladder. During sexual climax, or ejaculation, the prostate adds fluid to sperm to create semen, which also leaves the body through the urethra. For men under 50, the most common prostate problem is prostatitis. It can cause a burning feeling when you urinate or an urge to urinate more often. You might have a fever or just feel tired. Prostatitis is caused by the prostate becoming inflamed or irritated. Some kinds of prostatitis are caused by bacteria. If you have bacterial prostatitis, your doctor can spot it by looking at your urine through a microscope. Bacterial prostatitis can usually be treated with an antibiotic. But most of the time, there’s no clear cause for prostatitis. Researchers have yet to identify a clearly effective treatment when the cause is unknown. You may have to work with your doctor to find a treatment that works for you. Changing your diet or taking warm baths may help. No single solution works for everyone. For men over 50, the most common prostate problem is prostate enlargement, or benign prostatic hyperplasia (BPH). The prostate naturally grows larger as you get older. As it grows, it squeezes the urethra. The pressure can affect bladder control. BPH can lead to more serious problems, such as urinary tract infections. In rare cases, the constant urination problems can lead to kidney damage. Several treatments are available for BPH. In recent years, scientists have developed medicines that can shrink or relax the prostate to keep it from blocking the bladder opening. Researchers have also developed devices that allow doctors to remove parts of the prostate without major surgery. The procedures can usually be done in a clinic or hospital without an overnight stay. More invasive surgery is also an option. The symptoms of prostate cancer, in which cancer cells form in the tissues of the prostate, can be similar to those of BPH. However, most of the time patients are diagnosed with prostate cancer after results from a blood test prompt a prostate biopsy. Prostate cancer is the most common cancer in American men after skin cancer. But most men with prostate cancer don’t die from it. Many prostate cancers never even cause symptoms or become a serious threat to health. That’s because prostate cancer tends to grow more slowly than many other cancers. A prostate tumor may grow for 30 years before it gets large enough to cause symptoms. Several treatment options are available. Certain risk factors have been linked to prostate cancer—for example, eating a high-fat diet. NIH-funded scientists are now looking at how prostate cancer can be prevented. NIH also has many research programs aimed at finding treatments for BPH and other prostate problems.

Keep In Touch With Your Social Security Benefits By Karyl Richson Social Security Public Affairs Specialist in Milwaukee, WI If you’ve recently moved to a new address, chances are some of your former neighbors and friends from the community asked you to be sure to keep in touch. If you receive Social Security benefits, we’d like you to keep in touch too. In fact, you need to let us know about your new address. Even if you receive your payments by direct deposit, Social Security needs your current mailing address so you can continue to receive important notices about changes in benefits. Don’t worry; changing your address with Social Security can be as convenient and easy as logging on to your computer. Just go to www.socialsecurity.gov and select the “What You Can Do Online” link in the top left corner. The fourth item down is “If you get benefits.” That’s where you’ll want to go. Then select “Change your address or telephone number.” Once there, the web page will tell you exactly what you need to do to change your address with Social Security. You’ll need to provide your new address, including your postal ZIP code. (If you don’t know your ZIP code, the page includes a directory where you can locate it easily by entering your street address.) If more than one person receives benefits at an address (such as a husband and wife), each person has to make the address change. If you have a password with Social Security, that is all the information you will need to provide. But remember: you don’t need a password to change your address or phone number. You just have to prove to Social Security who you are by providing certain identifying information. It’s that easy. Once your address is changed, you know you’ll continue to receive important notices from Social Security when needed, such as letters telling you about changes in benefit amounts. MIDLIFE AS A GATEWAY TO AUTHENTICITY
By Kalpana (Rose) M. Kumar, M.D. Are you a midlife woman who doesn’t understand what is happening to your body and mind? Have you told your doctor how you’re feeling and were you dismissed with medications or hormone replacement therapy? Do you feel overwhelmed by the volume of information available about managing midlife and peri-menopause? I am not only a practicing internist, but a midlife woman. I have been helping midlife women for over two decades with their menopausal symptoms and have gained significant personal insight about how little support is currently available for us. During my forties I noticed my body and energy level change. I began to feel tired and heavy. I also felt restless and had trouble sleeping through the night. I looked for answers to these symptoms in my medical books and journal articles. I even went to my gynecologist to understand why I was feeling this way. All she had for me were prescription drugs and hormone replacement therapy. She told me I entered the category in life known as peri-menopause. “It’s downhill from here” she said. My visit was a huge disappointment. As a scientist and a problem solver, I was determined to understand why I was having these symptoms and what I could do to restore my sense of health and well-being. I found a lot written on this subject. What I read was theoretical, not practical and it did not address the deeper aspects of what I was looking for. Why did my soul feel restless? Why did I feel emotional ‘heaviness’? Why was my body changing in this way and why were issues resurfacing that I thought were solved? I was fortytwo when I began to feel this way. I was an intelligent and dynamic woman who had lived a very healthy lifestyle. I had also created a successful medical practice and on the surface, appeared to be at the top of my game. Deep inside, I felt empty and disconnected. My marriage felt deeply unsupportive and I found myself less able to tolerate the disrespectful ways I was being treated. It was as if I could no longer compromise myself to adapt. My tolerance for the superficial was receding. I found myself being more reactive and angry. What I discovered was that hormonal shifts in my body, normal during this stage of life, brought with them emotional and soul shifts that were offering me an opportunity to take a look at my life from a deeper perspective. This time in a woman’s life holds immense transformational power. When a woman arrives at this gateway her hormones shift and she recalibrates her identity. She is meant to question who she has pretended to be and move deep into her authentic self. Our society deems this a ‘midlife crisis’. I like to think of it as an alchemical process of becoming real. Women need to learn how to take care of themselves through this gateway and beyond. They need to learn how to use the framework of transformation to connect with their intrinsic power and truth. At this juncture, women are in need of balance at all levels. Most of us live unbalanced lives up to this point, taking care of others, but neglecting to take care of ourselves. As our bodies change we are called to heal our self-neglect.
