ello, “Millennials”! Are the skin changes related to stress, pollution, smoking, and prior sun damage and tanning starting to show on your facial, neck, and chest skin? Care tips for a beautiful, lasting bouquet alentine’s Day is about showing that special someone in your life how much you care, and what better way to express those heartfelt sentiments than a big bouquet of flowers?

The only problem is, cut flowers always seem to fade too quickly, and that doesn’t seem like the right message to send to the one you love.

After all, true love is meant to last forever, right? Fortunately, there are some simple steps you can take to keep cut flowers fresh and fragrant longer—maybe not forever, but at least as long as it takes to devour a box of cherry cordials.

Tips for the bouquet giver

When buying a bouquet from your local flower shop, look for flowers with firm petals or buds that show a little color so you know they’ll open properly. Make sure the flowers are wrapped and the stems kept moist, either with a damp paper towel or a plastic water “pod.” Most florists will provide a free package of cut flower food when you buy, but if your florist forgets, be sure to ask for one. These little packs contain all the necessary nutrients to keep your flowers looking their best for as long as possible. Also, be sure to avoid exposing the delicate blooms to extreme temperatures when leaving the store or your home with the bouquet.

If creating your own mixed bouquet, never combine daffodils (narcissi) with other cut flowers. When freshly cut, daffodils ooze a type of latex from their stems, which will cause the other flowers in the arrangement to wilt and die. In florist parlance, this substance is known as “daffodil slime.” If you’re really determined to present your sweetheart with a mixed arrangement that includes daffodils, you could try one of the cut flower foods on the market that neutralize daffodil slime.

this Valentine’s Day, there are steps you can take to prolong its beauty, as well: • Cut the stems at an angle at least one inch from the bottom with a sharp knife or scissors.

Angled cuts provide more surface area to take up water.

Do not smash the stems. This practice damages the vessels that conduct water, promotes bacterial growth, and stresses the flowers.

• Strip off the leaves below the water level in the vase.

• Always use a clean vase to discourage the growth of harmful bacteria.

• Use lukewarm water in the vase. Warm water not only encourages the blossoms to open, but it also holds less dissolved oxygen. That means it’s less likely that air bubbles will form in the stem and block water uptake.

• Never place cut flowers near ripening fruit, such as bananas.

Fruit releases small amounts of ethylene gas as it ripens, and the gas causes flowers to wilt prematurely. Aging flowers do the same thing and should be removed from the bouquet promptly.

• Avoid putting copper coins, aspirin, or other homemade formulas in the vase water. They generally do more harm than good.

• Avoid placing the vase in direct sunlight or a drafty area. Both environments will shorten the life of your bouquet.

• Top off the water frequently, adding the correct proportion of cut flower food. ❦

inter in Northwest Ohio typically brings bone-chilling temperatures, slippery walkways, treacherous driving conditions, and the strenuous task of snow and ice removal. For many area seniors, winter’s arrival also raises the question of whether it might be time to consider making the move to a retirement community and leaving behind the chore of snow shoveling along with all the other burdens of homeownership.

As they contemplate this decision, some may hesitate out of concern that moving from their home into a retirement community will mean compromising their independence or abandoning many of their favorite activities. However, for seniors who choose the maintenance-free lifestyle at Fieldstone Villas at Sunset Village, nothing could be further from the truth.

The original six villas (12 units) that were completed at Fieldstone Villas have already been filled, so construction is now underway on five new villas. That means 10 new homes in total will soon be available in this Life Plan Community (formerly called a CCRC) that redefines the traditional image of senior living.

“When we finished construction on the first phase of villas in the fall of 2016, we knew interest would be high, and that proved true beyond our expectations. Those original homes filled up quickly, so we’re building additional villas to ensure that more area seniors who want to experience the Fieldstone Villas community and enjoy all its amenities have the opportunity to do so,” explains Gayle Young, Director of Marketing, Communications and Public Relations for Sunset Retirement Communities.

Several different villa styles will be represented in the new phase of construction. Ranging in size from

just over 1,200 square feet to 1,800 square feet, each barrier-free home will include a full kitchen with all new appliances, two bedrooms, two baths, and a gas fireplace. All of the homes will also feature a two-car garage, with the exception of the smallest floorplan, which has a onecar garage.

Young emphasizes that in addition to enjoying all the amenities the Fieldstone Villas community has to offer, residents can expect their monthly fee to cover all aspects of home maintenance, both inside and outside, including lawn care and snow removal, preventive maintenance, appliance and plumbing repairs, and even bi-monthly housekeeping. “Our residents are thrilled to leave behind all those arduous chores, which at this time of year can mean a lot of snow shoveling, allowing them more free time to pursue the activities and pastimes that bring joy and fulfillment to their lives,” she adds.

Residents of Fieldstone Villas at Sunset Village also enjoy access to the community’s brand-new clubhouse, which opened in mid-December of last year. The clubhouse features an indoor swimming pool, an area for exercising, chef-prepared dinners served to residents Monday through Friday, and ample opportunities to gather and socialize with friends and family.

Which I thought was close enough because our daughter Sarah lives in Sylvania. I’m still putting this down as a twofer on my scorecard.

At Rainbow falls in the Smokies, we met a TV newscaster on his way to his new job in Toledo.

Sometimes the connection to Toledo is in the future tense. Shirley and I were at Rainbow Falls in the Smokies where I was taking photos of Shirley posed fetchingly. Another couple came up for the same purpose.

No, they weren’t from Toledo. But they were on their way there. He had just been hired by a local TV station to read the news. A month later we would have recognized him without the introductions.

On the subject of media personalities, Shirley does a lot of our travel research by consulting the experience of other Roadtrek owners. In addition to the regular Roadtrek Facebook page, there is one devoted to vintage models, which ours has recently become. (“Vintage” sounds so much better than “old,” don’t you think? And more flattering than “senior citizen” or even my usual preference, “geezer.”) But, as I started to say, two of Shirley’s favorite Roadtrek Facebook personalities are Janet and Mary because they focus on out-of-the-way places that your average traveler probably overlooks.

One afternoon at Twin Peaks Campground in Arizona, a nice woman stopped by to chat about where we had been and where we were going.

She recommended Whitewater Draw to see 30,000 sandhill cranes that spend winters there. (Shirley and I are not serious birders, but we are observers of all kinds of wildlife.)

Said her name was Janet and she was waiting for her friend Mary to arrive so they could head to some mysterious destination together.

A few minutes later, Mary herself showed up.

This was Shirley’s cue to go all fan-girl gushy about meeting two of

her favorite celebrities. I thought she was getting a little carried away but was in no position to object because readers of this publication do the same thing whenever they recognize me.

[Editor’s note: This sentence should be struck because it is seriously bad taste to begin a sentence with “I” and end it with “me.”]

A year later, Janet showed up again in the same campground. She leaned out the window of her Roadtrek and said, “Hey, I know you. Where ya been?” Like all of us celebrities, she tries really hard to remember the little people who make up our fan base.

On our way to Everglades National Park, we paused for a few days at Alexander Springs near Ocala.

Shirley had called ahead to make sure campsites were available. The volunteer host said they couldn’t reserve a site for us without three days advance notice. But they could “save” a site for a couple hours until we arrived.

The host couple at the entrance station noticed that we were from Ohio and…Well, surprise, surprise! They lived between Toledo and Bowling Green. And because we were obviously such nice people, he said there was probably a better site available.

Just come along and we’ll see what we can do. And he did really well by us. Honestly, though, we would have been quite satisfied with his original choice.

Hearing loss linked to dementia

ot too long ago, I had a gentleman come in to our office with his mother. After testing his mother’s hearing and recommendations, he stated, “My mother ’s doctor wants her to get hearing aids because we can’t figure out if the problem she is having with her memory is due to dementia or due to the fact that she doesn’t hear well.” We see many patients in our office with some sort of cognitive disability. If these patients would have worn hearing aids sooner, would it have slowed their cognitive decline?

In a 2011 study from John Hopkins University, Frank Lin, MD, PhD, looked at ways hearing loss affected cognitive brain function. He along with his colleagues used data from the Baltimore Longitudinal Study on Aging. The study focused on 639 people whose hearing and cognitive ability was tested over a 12-18 year period. About a quarter of those people had hearing loss, but none of them had dementia. After years of monitoring the patients, 58 of them developed dementia. The study showed that people with mild, moderate, or severe hearing loss were more likely to develop dementia.

The risk increased in conjunction with the severity of the hearing loss; the greater the loss, the higher the likelihood of developing dementia.

The next question was, how is hearing loss leading to dementia? Lin suggests four theories. One is that hearing loss and cognitive decline have common physiological pathways in the brain, but due to the way the study was done, Lin does not hold this theory very credible. Another thought is that hearing loss and straining to hear stresses the brain.

According to Arthur Wingfield, who heads the neuroscience program at Brandeis University, “If you put in a lot of effort just to comprehend what you're hearing, it takes resources that would otherwise be available for encoding [what you hear] in memory.”

Wingfield's lab has documented this effect on a short-term basis. The big question, he says, is whether years of drawing resources away

from brain functions like working memory eventually reduces the brain's resilience. A third theory suggested by Wingfield and Linn is that hearing loss may affect the brain structure. Looking at images of the brain, it was shown that older adults with hearing loss have less gray matter in the part of the brain that processes speech.

Wingfield stated that certain parts of the brain shrink when they don’t get enough stimulation. Getting a clear signal to the brain may allow the brain structures to grow back to their previous size and function.

Finally the last theory of how hearing loss could lead to dementia is the social isolation that is common in people with hearing loss. When hearing is difficult, people are less likely to socialize. Social isolation is known to be a risk factor in dementia development (AARP, August 8, 2013).

So, knowing all this information, does wearing hearing aids reduce the risk of dementia? Well, the answer is yes and no. Studies did not show a direct benefit of hearing aid use decreasing dementia risk, but wearing hearing aids can decrease other factors that indirectly increase the risk of dementia.

According to a study by the National Council on Aging, hearing-impaired adults over the age of 50 who do not wear amplification are more likely to develop depression, anxiety, and paranoia and less likely to participate in organized social activities when compared to the same population of people with hearing aids. As mentioned above, this social isolation is known to be a risk factor in the development of dementia. So, with the use of hearing aids, this risk factor is greatly reduced. Unfortunately only about 15% of people who could benefit from hearing aids are actually using them.

With our patients, we have seen amplification improve quality of life. Our patients report being able to participate in activities that were once deemed too difficult because of their inability to hear and understand

speech. Their family members have also reported noticing an increase in confidence and self esteem with their loved one after they received their hearing aids.

The use of hearing aids is one way to stay active and engaged in social activities, but what are some other ways to maintain your brain health? According to geriatric medicine specialist Ronan Factora, MD, keeping your brain active by reading or engaging in hobbies that are challenging, like dancing, learning a foreign language, or playing a musical instrument, is another way to maintain a healthy brain. He also stated that regular exercise and a healthy diet, like the Mediterranean diet, will also promote brain health.

We have seen firsthand how hearing aids can improve one’s quality of life. The ability to have meaningful conversations with our friends and family is so important for our well being. To have untreated hearing loss be the cause of one’s social isolation is very distressing. Yes, hearing aids are an investment and many people

cannot afford them, but some people do have coverage through their insurance, and for those with no coverage and no other means to afford hearing aids, we have a non-profit called HearCare Connection. Through this non-profit, we have the ability to fit people who qualify with hearing aids that were never able to afford them.

Having a family member or a loved one with dementia is hard enough, but adding hearing loss on top of it can make communicating with them even more difficult. Taking hearing loss out of the equation by using amplification could make living with dementia a little easier.

Harbor and The Central Community Health Board of Hamilton County form joinder agreement

H

arbor, Inc. of Toledo has formed a joinder agreement with The Central Community Health Board of Hamilton County (CCHB). The agreement was effective January 1, 2019. The joinder allows both agencies the opportunity to expand their programs and work together to grow and enhance the complimentary services each has to offer.

Harbor is a leading mental health provider in Ohio, serving over 24,000 individuals annually. The $47 million non-profit agency employs over 600 people in 24 locations across Lucas, Wood, and Defiance counties. Harbor has served the Toledo area for over 100 years and offers youth and adult counseling, psychiatry and psychological services, substance use disorder assessment and treatment, telehealth services, and developmental pediatrics as well as vocational rehabilitation services, an Employee Assistance Program, and multiple grant-funded programs.

The Central Community Health Board of Hamilton County is a regional diversity leader in behavioral health and chemical dependency treatment in the greater Cincinnati area. The $10 million non-profit, community-based organization employs 124 employees in Hamilton County. CCHB offers adult outpatient services, community psychiatric supportive treatment, crisis stabilization, day treatment, opioid treatment, HIV early prevention and intervention, and residential program services.

Dispelling the myths about mental illness

W

hen someone is diagnosed with a serious medical ailment, such as heart disease, diabetes, or cancer, the typical reaction from family, friends, and acquaintances is sympathy and support. However, a diagnosis of mental illness oftentimes elicits a very different type of response, ranging from wariness and suspicion to outright fear and even discrimination. The perception seems to be that mental illness is a flaw rooted in an individual’s character or personality rather than a neurobiological disease. As a result of this stigma, people with mental illness too often feel ashamed, suffer in silence, and avoid seeking vital treatment.

Mental illness is no small challenge in our nation. According to the National Alliance on Mental Illness (NAMI), in a given year in the US, approximately one in five adults experiences mental illness and one in every 25 adults experiences a serious mental illness that substantially interferes with or limits one or more major life activities. With so many lives touched, why does our society still tend to stigmatize those who have a mental illness? The answer may lie in several persistent myths.

Among them:

that of the general public. This misconception is likely rooted less in objective reality than in unflattering or outrageous media portrayals of people with mental illness.

Mental illness will never affect me

Negative thought patterns can be improved dramatically with proper treatment; however, just as a person with diabetes or heart disease can’t “think away” his or her condition, individuals with mood disorders or other forms of mental illness cannot “snap out of it,” “cheer up,” or “chill out” simply by deciding to think or feel differently.

People with mental illness are violent and may hurt me or someone I love

Taken as a group, people with mental illness are no more violent than people without mental illness.

In fact, according to NAMI, people with mental illness are much more likely to be victims of violence than perpetrators of it—a rate four times

People with mental illness are weak and can’t handle the rigors of work or school

Mental illness stems from a variety of biological and environmental factors, not individual weakness. On the contrary, many people with mental illness are quite capable of holding up under the pressures of work, school, and day-to-day life because they’ve learned certain skills and techniques that help them cope with and adapt to stress before it reaches an unmanageable level.

People with mental illness are strange and unpredictable

Some forms of mental illness can manifest through odd or unpredictable behaviors, but many people with mental illness lead active, productive lives without anyone around them knowing what they’re going through.

Also, those individuals who do exhibit unusual behaviors as a result of their illness can often learn to recognize and manage them effectively through proper treatment.

Nearly 44 million Americans experience mental illness each year, so it’s the rare individual whose life will never be touched by some form of it. Those who think no one close to them has mental illness might be surprise to learn that a friend, family member, coworker, or acquaintance lives and functions with mental illness every day. Whether it is experienced firsthand or not, mental illness is an issue that affects everyone.

Arrowhead Behavioral Health offers specialized care to promote healing and recovery

B

ehavioral health issues and addiction are complex, often interrelated problems that demand a comprehensive, individualized approach to treatment. At Arrowhead Behavioral Health, a multidisciplinary team of professionals stands ready to help individuals who are struggling with these issues find a path to recovery and reclaim healthier, more productive lives.

According to Mary Beth Alberti, Director of Business Development for Arrowhead Behavioral Health, Arrowhead is a private, 48-bed facility poised to provide specialized, medically based behavioral health and addiction treatment to individuals age 18 and over. No-cost patient assessments are provided 24/7 and, while appointments are appreciated, walk-ins are always welcome.

The Arrowhead treatment team takes an inter-disciplinary approach and consists of a psychiatrist, nurse, therapist, mental health professional, recreation therapist, and the patient and his or her family, who are encouraged to be active participants in the treatment process. There are six physicians on staff, including board-certified psychiatrists and addictionologists.

These physicians are: Siva Yechoor, MD, who serves as medical director; Habeeb Arar, MD; Kenneth Adler, MD; Kul Gupta, MD; Randy Climo, MD; and Christine Ellis, MD. Programming is provided on both an inpatient and outpatient basis and can follow one of two pathways depending on the patient’s need and unique circumstances: a straight mental health track or a dual-diagnosis track for those with mental-health and substance-abuse issues. “We see a lot of dual-diagnosis patients because oftentimes an underlying mental-health issue is either caused by or a result of substance abuse,” Alberti says.

Arrowhead also provides, among many other services, a Partial Hospitalization Program (PHP), which meets five days per week for six hours each day, and an Intensive Outpatient Program (IOP), which meets three days a week and three hours per day.

Alberti notes that a new lodging component has been added to the PHP program to help facilitate

participation for patients who may be unable to drive to the facility for various reasons. “Our service area has been expanding, and more and more patients are coming from far away, so we are contracted with a local hotel to provide lodging for appropriate patients. We provide transportation between the hotel and Arrowhead as well. This new service is beneficial not just for patients traveling from remote areas, but also for some local patients for whom going home may not be a healthy option. In many cases, home is where their triggers are, so staying elsewhere during treatment gives them the opportunity to become healthier and farther along in recovery before going back to that environment,” she explains.

For patients suffering with addiction, Arrowhead provides multiple treatment interventions tailored to the individual. “Patients are carefully assessed for candidacy into our Medication-assisted Treatment programs.

One option is a medically supervised Suboxone treatment program for adults struggling with opiate dependence.

There is a strong education piece to our Suboxone program as well as a monthly group meeting. Patients in the program can attend every 28 days to get their prescription,” says Alberti.

Patients and their families also appreciate that Arrowhead has a very active Alumni Group, which meets weekly to coordinate sober social activities throughout the year, including an annual picnic in the summer, and provides peer support to current patients so they are more likely to remain on the path to recovery.

Furthermore, Arrowhead has a robust presence in the larger community. For instance, the facility offers quarterly lunch-and-learn events, which are open to the general public, and collaborates with the Thomas M. Wernert Center, a local organization dedicated to improving the quality of life for persons living with mental illness through education, peer support, and advocacy.

Members of the Arrowhead team also attend numerous community events throughout the year, setting up information tables and introducing the facility as a powerful resource for the community. ❦

Know your rights as a nursing home resident

T

he nursing home Bill of Rights states that a home must provide privacy for visits by a spouse or allow spouses to share a room if both residents are of the same home. It goes on to assure privacy for visits when a spouse, relative, or partner resides outside of the facility.

Often with age comes the stigma of growing “old.” However, an increase in age does not necessarily mean the loss of independence, vitality, and ability. As Long-Term-Care (LTC) Ombudsmen, it is often our job to assist consumers in understanding their rights and their ability to be self-determined in their life choices.

This includes the decisions to visit privately with a spouse or partner that we choose. This can become complicated and difficult to accommodate by a provider. The right remains clear that private visits must be allowed and accommodated.

For the sake of example, imagine for a moment that you have aged into needing care in a home. You have recently lost your spouse, and upon

moving into the home you begin enjoying spending time with another consenting adult. About a year into your relationship, you begin to have problems remembering things. Your family never really warmed to your new significant other and is now vehemently against any ongoing physical relationship. At this point, do you have the right to continue your relationship?

This is where clear advance directives need to be in place. These need to be comprehensive to include sexual consent and capacity. This situation is very complex and could unfold in a number of ways. As such, it is vital to have specific and sometimes uncomfortable conversations with the ones closest to you. In doing so, you can take the steps to remain independent and retain your ability to exercise choice over your circumstances.

For those individuals lucky enough to be trusted in a decision-making capacity for a loved one, it is your duty and responsibility to respect the choices and preferences of your loved one and remove your own feelings from the equation. Both formally (through POA documents) and informally, many of us will be

asked to step in and be involved in important decision-making processes at one point or another.

I encourage all of you to have candid conversations about your choices and desires with your loved ones, regardless of their feelings about the choice itself. Age is just a number, but often increased age prompts others to dictate your life choices. Again, while most of our loved ones have our best interest in mind, it is important to make our choices known and involve in our care those who we can trust to respect our decision making.

So remember, you have the right to private visits with the people you choose. When you can no longer choose, make sure you entrust someone with those decisions who will make the decision you would have made.

NHA clinics provide quality, affordable dental care to patients of all ages

R

egular dental visits to promote good oral health should begin as soon as a child’s first tooth emerges and continue throughout his or her life. However, for individuals who are low-income, uninsured, or struggling to find a dentist who accepts Medicaid, access to quality dental services is far from guaranteed. As a result, too many children and adults in this population simply forego routine dental care and often end up requiring treatment for severe oral and dental conditions that could easily have been prevented.

To help eliminate these barriers for underserved members of our community, the Neighborhood Health Association (NHA) provides quality, affordable dental care at three area clinics—Nexus Health Care, located at 1415 Jefferson Avenue; the Cordelia Martin Community Health Center, at 615 Division Street; and the Mildred Bayer Clinic for the Homeless, also at 1415 Jefferson Avenue. Adults and children of all ages are accept-

ed at Nexus and Cordelia Martin, while Mildred Bayer primarily serves homeless adults.

“These clinics are a tremendous resource for the community,” states Kalpana Yadav, DDS, Chief of Dentistry for the NHA. “The health of the teeth and gums affects one’s overall health and wellness, so it’s critical to start seeing a dentist and practicing good oral hygiene at an early age.

While patients are here, we can also assess them for other health problems and refer them to appropriate physicians for treatment if necessary. So, by providing these clinics, we’re supporting our patients’ future health in a huge way.”

Dr. Yadav and her colleagues, Trang Nguyen, DDS, and Warren Marshall, DDS, are poised to provide cleanings, fillings, sealants, fluoride applications, x-rays, simple and surgical extractions, restorations, as well as scaling and root planing. The only procedures they don’t perform on site are root canals, crowns, and dentures.

What’s more, the clinics accept all

insurances, including Medicare and Medicaid, and assistance is available on a sliding fee scale for those who are uninsured or underinsured. Individuals who qualify for services at Mildred Bayer receive their entire treatment at no cost.

The 43,000-square-foot Nexus Health Care facility, which is the latest addition to the NHA network of healthcare providers, provides a broad range of healthcare services in addition to dental care, including adult primary care, pediatrics, cardiology, general surgery, gerontology, immunizations, laboratory and pharmacy services, OB/GYN, podiatry, and many others. Housing all these services under one roof allows patients to get all their healthcare needs met in one convenient location, which eliminates the necessity to arrange transportation to multiple locations around the community. “Also, we have strong partnerships with UTMC, ProMedica, and Mercy Health, so if patients need treatment that we don’t offer here, we can refer them to expert providers in any of these other organizations,” Dr. Yadav adds.

Nexus patients appreciate the

top-quality dental care they receive and are often surprised by the exceptional condition of the facility. “Most people think of a non-profit clinic as an old and outdated building, but Nexus really defies this expectation.

The facility is state-of-the-art, neat and clean with all new furnishings, and the atmosphere is bright, cheerful, and welcoming. Doctor-patient communication…Can we talk?

S

ee if this sounds familiar: You have a medical appointment. You check in at the front desk on time and take a seat. You wait for quite a long time, and then finally your name is called. You go to the exam room, and the nurse comes in, asks a few questions, and takes your blood pressure. You wait some more. The doctor comes in, asks a few more questions, writes a prescription, and is gone. You leave the office with more questions than answers. You aren’t quite satisfied.

Patient-physician communication is very important for successful healing. An analysis from The Joint Commission, a nonprofit that accredits US healthcare organizations, faulted “inadequate communication”—both between physicians and between patients and their doctors—in more than 70 percent of sentinel events, or adverse health outcomes not related to the natural course of a patient’s illness. This means that if we can improve the communication and effectiveness

of the visit to a physician, there is a greater chance for a positive outcome.

As a patient, you are not powerless when it comes to effective communication in the exam room and after you leave. There are things you can (and should) do to ensure your voice is heard and your concerns are addressed. They include:

1. Come prepared

Know what you intend to talk about before you arrive for your appointment. Write down your concerns and questions, and make them specific.

The more pointed your questions are, the more direct answers you’ll get.

2. Prioritize your concerns

Know that your doctor may not have time to answer all 50 of your questions, so ask them in order of importance. “When a patient prioritizes their concerns, it tells me they have very specific health interests and they understand we’re both working with limited resources,” says Dr. Nirmal Joshi, chief medical

“It also tells me you respect my time and allows us to focus on what concerns you most about your medical situation.”

3. Don’t be afraid to ask for another appointment

Ultimately, you want all of your concerns addressed with ample time and attention. So rather than trying to rush through your list of questions, if you don’t finish, ask for a follow-up appointment.

Dr. Joshi says it’s not uncommon for patients to remember something they wanted to talk about as the doctor is getting ready to leave the room. In that situation, the doctor has a choice—address the concern quickly, possibly sacrificing good clinical care, or set up another opportunity to discuss the issue. As a patient, you can take the reins by requesting a follow-up.

4. Be willing to communicate outside the exam room “Never underestimate the power

of communication that’s not necessarily face-to-face,” says Dr. Joshi, who recommends patients ask their doctors whether they are willing to email. “Many physicians love that. I personally love that. That freedom allows me to instantaneously communicate with patients when I have the time for them.”

t’s that time of year when the Christmas decorations are gone and the Valentine decorations and candy fill the shelves. When I was a kid in grade school, Valentine’s Day was an anxious one for me because I wasn’t sure if anyone would give me a Valentine. We didn’t bring treats, just silly cards which some of us made by ourselves. I usually brought some home, so I guess my classmates were nice to each other because none of us felt left out.

When I taught elementary students, I made sure we had a pretty inclusive Valentine party and the mothers made sufficient treats so that everyone went home with a sugar rush and a bag full of cards and candy. Is that what love looked like? I think I thought so back then.

Running tips for beginners

A

re you interested in running but afraid of the first steps? Here are a few tips to get you off on the right foot and on your way to enjoying the sport of running. Most important, consult your physician before starting any exercise program.

As a runner, the most important piece of equipment is your pair of running shoes. Before starting your running program, visit a shop such as Dave’s Performance Footgear, which specializes in running shoes, for an assessment of your needs and a personalized fit.

One of the most common complaints of new runners is, “I can’t breathe.” Focus your energy on exhaling rather than inhaling. As your need for air increases, think about forcing the air out of your lungs rather than sucking it in. This will make you less tired than when you are huffing and puffing and will also help prevent hyperventilation.

In the beginning, take frequent short runs after resting sufficiently.

Your runs should last long enough for your body to get a workout but still be able to recover in time for your next run. If you over exercise and your muscles do not recover to their original condition, you will not gain any benefit. Strength is gained during rest when your muscles repair the fibers that are broken down during exercise. But remember, every runner is different when it comes to the amount and frequency of exercise that is appropriate. Listen to your body.

As a novice runner, take quick steps rather than long ones. You will not tire as fast if you take short, fast steps rather than long, slow ones, especially when you are tackling hills. When running downhill, lean your body forward in order to try to keep your body perpendicular to the hill.

Novice runners need motivation and a schedule. Join a beginner run-

ning group or seek out a running partner. Some runners find that taking music along on their workouts helps make running fun. When using headphones, though, play the music at a reasonable level so you can hear traffic noise and remain alert to potential hazards. Speaking of hazards, if you are running alone, carry identification and always run against traffic, preferably on sidewalks or running paths.

One of the best ways to improve and

get more enjoyment out of running is to enter road races. The experience of finishing a running race is exhilarating.

Start with the goal of a 5K (3.1 mile) road race, “fun run,” or a relay race with your running friends. Races can provide a foundation for enjoying a long life of running. Happy running!

There are two types of macular degeneration, wet and dry. Both of these become more common with age, affecting as many as 30% of people over age 70. In wet macular degeneration, abnormal new blood vessels grow under the retina and cause bleeding or swelling in the macula. Cosmetic procedures under local anesthesia offer greater safety, comfort, control, and cost savings T raditionally, all surgical procedures were performed under general anesthesia, with the patient in a completely unconscious state and unable to interact with the surgical team. This applied not only to major medical procedures, but also to cosmetic procedures, including those considered minimally invasive. The technology and know-how to perform surgeries under local anesthesia simply didn’t exist. Unfortunately, many people who could have benefitted from cosmetic treatments avoided them due to anxiety over being unconscious and out of control during the procedure.

However, along with the tremendous growth of the cosmetic medicine industry came numerous advances in the techniques and technology used to perform cosmetic procedures—including the approach to anesthesia.

Dr. Wade Banker of Luxe Laser Vein & Body Center notes, “About ten years ago, people started performing certain cosmetic procedures under

local anesthesia, eliminating the risks associated with general anesthesia, saving patients money, and creating a more comfortable surgical environment.”

Dr. Banker further explains that the form of local anesthesia currently used in cosmetic medicine is sometimes referred to as “twilight” anesthesia. With twilight anesthesia, the patient is numb to everything that is going on, so he or she experiences no pain or discomfort, but remains awake throughout the procedure and able to interact with the staff and surgeon.

This last point is highly significant according to Dr. Banker.

“One of the most important considerations patients have when choosing between general and local anesthesia is the degree of control they have over their environment during the surgery.”

“With local anesthesia, they are conscious throughout and if they feel uncomfortable or need anything, they can communicate this to the staff or physician, who can then address the issue immediately.

General anesthesia, on the other hand, removes all control from the patient,” he says.

Furthermore, healing is easier following procedures performed under local anesthesia because post-procedure pain medications can be administered before the local wears off, so there’s no transition period as there is when a patient is waking up from general anesthesia.

Today, it is commonplace for cosmetic procedures—when the patient is typically paying out of pocket—to be performed under local anesthesia. This is the case not only because of the safety, comfort, and control factors, but also

because of the associated cost benefit.

An anesthesiologist does not have to be present during procedures performed under local anesthesia, so the overall cost of the procedure is significantly lower.

Dr. Banker states that nearly all cosmetic procedures can now be performed under local anesthesia, including liposuction, fat transfer, breast augmentation, and tummy tucks, so there’s no reason for anyone who is considering one of these procedures to avoid having it performed due to fears or anxieties related to the anesthetic.

EATING WELL The best healthy eating plan for 2019

y now, some of us have already set aside our 2019 resolutions while others may be just getting it together to start making some healthy changes. If the changes you have in mind involve weight loss, we need to talk about your eating plan (notice I avoided using the four-letter word: D-I-E-T).

You might find it surprising that I didn’t say we need to talk about exercise. After all, when it comes to weight loss, healthy eating and exercise go hand in hand, right? Well, it’s true that exercise is an important part of a healthy lifestyle, but losing weight by just working out is very difficult—and it’s very easy to negate any calorie loss achieved in the gym with a few bites of the wrong food.

In other words, rewarding yourself with ice cream after an hour at the gym is likely to undermine your progress on the bathroom scale.

There’s no getting around it; making changes in what you eat is a must if you want to lose weight.

With that in mind, let’s get back to eating plans. It seems a few plans in particular are all the rage nowadays.

You can hardly go anywhere without hearing someone talk about the Keto Diet or the Paleo Diet and all the fantastic weight loss they’ve achieved on it. With all the buzz these diets are generating, I’m reminded of the “Atkins Revolution” of 20 years ago!

Keto vs. Paleo

People often assume the Keto and Paleo diets are more or less interchangeable since both are low-carb.

But there are some key differences.

On Keto your calories come mainly from fat, whereas on Paleo your calories come from protein. Also, if you think Keto is a “cheese-forall,” you’re mistaken because cheese contains carbohydrate in the form of lactose. It’s more about full-fat foods like butter and olive oil as well as meats, poultry, eggs, and fish.

Paleo, on the other hand, is more liberal and allows more options like fruit and vegetables. This eating plan, which is about eating like our ancestors of the Paleolithic age, does not enforce super-strict rations of nutrients. You do, however, avoid dairy foods, processed foods, legumes, and whole and refined grains—essentially everything

a caveman could not eat. You can have grass-fed meats, poultry, seafood, eggs, nuts and seeds, fruits and vegetables, coconut oil, olive oil, and avocado.

You lose weight on both plans because you’re restricting foods and eliminating whole food groups. However, they’re often not sustainable, and once you start eating some of the restricted foods, the weight comes right back on (like the Atkins Diet of 20 years ago!). If you have to choose one of these, Paleo would be a better option as it is slightly healthier.

Even better choices

Better yet, choose the Mediterranean Diet, which is your best bet for a healthy eating plan. This plant-based diet—which has been around for at least 20 years (actually for centuries) and was named the “Best Diet of 2019”

by US News and World Report—is often recommended to diabetics for blood sugar control.

Many studies have been done on the Mediterranean Diet, and it has been found to help with weight loss, depression, dementia, diabetes, heart disease, breast cancer, stronger bones, and a longer life, to name a few benefits. More a lifestyle than a diet, this eating plan encourages eating with friends and family, socializing with meals, and mindful eating.

Processed foods are avoided, and the focus is on fruits and vegetables, beans, lentils, whole grains, seeds, and nuts. Animal-based choices revolve primarily around seafood and fish with some poultry and minimal intake of red meat. Oh, and let’s not

forget the glass of red wine per day (now white wine is okay too)!

If you happen to pick up that US News and World Report article, you will also find that the number-one diet for weight loss is WW (formally Weight Watchers ), on which you can eat over 200 zero-point foods every day—mostly fruits, vegetables, and lean protein.

Think the “Three S’s”

So much of weight loss is mental, and having the ability to eat the suggested foods, with an occasional indulgence, is what is really important with a healthy eating plan. To achieve that, it’s helpful to think the “Three S’s”—Simple, Satiety, and Sustainable.

Is the plan something you can do?

Is it going to make you feel satisfied?

If you’re starving, you’re not likely to stay on the plan. When you start to eat again—“re-feed”—the weight comes right back on and perhaps more. Is the plan sustainable? Can you eat this way not just now but for a lifetime?

Remember, diets that eliminate entire foods groups and are very restrictive are not substantiated by science. Learning how to make healthy food choices is key, and making those choices on most days is my mantra!

Whatever you do, be sure to educate yourself and choose a safe plan to improve your health.

Also, numerous clinical studies over decades have also shown that those who participate in cardiac rehab have better long-term outcomes including reductions in hospital readmissions, reduced risk of future cardiac events, and deaths related to cardiovascular disease.

In fact, patients who participate in a cardiovascular rehab program have a 50% lower risk of death and a 30% lower risk of another heart attack than patients who do not go through a rehab program.

Patients’ quality of life tends to improve significantly after cardiovascular rehab as well. Participants find that they’re able to do more daily physical activity, sleep better, and experience less shortness of breath, less depression and anxiety, and fewer episodes of angina. Moreover, Dr. Mathew observes that rehab participants have a greater sense of empowerment over their own health and are more motivated to

exercise and less anxious about having a cardiac event while exercising unsupervised.

Though cardiovascular rehab programs are offered all across the nation, Dr. Mathew points to several features of UTMC’s program that set it apart: “First, we have a 20-year history of providing cardiovascular rehab services to the community, and our program is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation.

Furthermore, all the members of our team are certified in their area of expertise such as exercise physiology.

Laurels rehab team helps couple keep the competition alive

R

uth and Jerry Cunning, 77, have been actively involved in sports since their teenage years. In fact, a shared passion for competition has strengthened the couple’s bond and provided a continuous source of joy and inspiration throughout their long marriage. However, after experiencing significant physical challenges in recent years, both are currently sidelined from organized athletics and unable to participate in their favorite sport—platform tennis. The good news is, with a little help from the rehab team at The Laurels of Toledo, both are beginning to make strides against the physical challenges that are keeping them out of the game.

For Ruth, the biggest challenge is Parkinson’s disease, which predominantly affects the right side of her body.

She recalls, “While playing platform, I found it very difficult to take that first step forward to get to the ball and would lose my balance while walking backward to take an overhead shot.

Instead of flinging my body at the ball like I could before, I was struggling just to keep my feet under me.”

With guidance from Laurels physical therapy assistant Matt Snyder, it wasn’t long before Ruth began to notice improvements in all of these areas.

Snyder works with Ruth using a variety of different techniques to strengthen her hand-eye coordination and reflexes, increase her confidence in maintaining balance, and help her recruit the appropriate amount of muscle force to execute the various movements that platform tennis demands. To help replicate the platform experience, Snyder incorporates a racquet into Ruth’s program, using a balloon in place of the ball.

Returning to platform tennis—a fast-paced sport that demands heavy exertion—may or may not be in the cards for them, but it’s likely they’ll find other ways to keep the competition alive after they’ve completed their outpatient therapy at The Laurels of Toledo.

She has been told this can happen due to the pregnancy. Is this true?

A

: Thank you for bringing this question to us this month, and Iâ&#x20AC;&#x2122;m happy to review the subject. As we all know, pregnancy is so special and exciting for a woman to go through, and along with pregnancy come physical, emotional, and other types of changes. These changes lead to exciting moments but at times can lead to complications. Hearing loss is one of the potential complications during this period. The hearing loss can also lead to symptoms such as tinnitus (ringing in the ear), pressure in the ear, otalgia (an ache or pain in the ear), secretion in the ear, otosclerosis (abnormal growth of bone in the ear), and sudden sensorineural hearing loss.

There are many reasons these hearing problems can develop during pregnancy, and among the key factors are the hormonal changes the body goes through. Otosclerosis often surfaces during pregnancy because of the hormonal modulation. This is one of the more common causes of hearing loss in pregnancy. Keep in mind that pregnancy does not cause otosclerosis; rather, it makes the symptoms get worse and progress more rapidly.

Otosclerosis is a common cause of hearing loss in young adults.

The cardiovascular system, endocrine system, and hematological system (blood flow) are a few other parts of the body that go through changes during a pregnancy. The bodyâ&#x20AC;&#x2122;s transformation affects the circulation of the fluid in the cochlea (inner ear), in turn resulting in hearing difficulties such as the sensorineural hearing loss, otalgia, and other impairments listed above.

Along with these common symptoms of hearing problems that may occur during the pregnancy, there are

other known symptoms, including: â&#x20AC;˘ Reduction in the sensation of hearing â&#x20AC;˘ Tinnitus, or ringing, in the ear â&#x20AC;˘ Drainage or secretion in the ear â&#x20AC;˘ Otalgia or pain in the ear.

But there can also be other symptoms such as: â&#x20AC;˘ Dizziness or vertigo â&#x20AC;˘ Nausea and vomiting â&#x20AC;˘ Balance difficulties â&#x20AC;˘ A feeling of fullness in the ear.

If any of these symptoms occur, it is crucial to discuss it with your physician who most likely will consult with an otolaryngologist and audiologist. After having the audiologist examine your hearing loss, the doctor will most likely recommend treating such hearing problems differently depending on the type and severity of the problem.

Expert advice on choosing a Medicare plan that meets your needs

P

eople approaching age 65 are inundated with pamphlets, booklets, and brochures on the various Medicare options available to them.

More often than not, this deluge of information leads to more questions than answers. Is Original Medicare sufficient? Would a Medicare Advantage Plan be a better choice? Do I need supplemental coverage? How do I cut through all the confusion to reach the best possible decision?

Dan Trombley, Senior Coordinator, Medicare Sales, for Paramount, explains that there is no one-size-fitsall Medicare plan and offers some advice on how to choose coverage that makes the most sense for your healthcare needs as well as your budget. He recommends:

Know when to enroll

There is a seven-month initial enrollment period for people aging into Medicare, which includes the three months before their birthday, their birthday month, and the three months after their birthday. However,

coverage will be effective no sooner than their birthday month. Eligible people can also elect or change their Medicare coverage during the annual election period which runs from October 15 to December 7 every year.

Know the main Medicare options

Original Medicare includes Part A and Part B (hospital and medical insurance respectively) and is essentially

an 80/20 plan with a deductible and no out-of-pocket maximum. If you’re covered under Original Medicare and want prescription drug coverage (Part D), you’ll need to choose a Medicare Prescription Drug Plan, which typically requires paying an additional monthly premium.

Medicare Advantage Plans (Part C), such as Paramount Elite, have low premiums, eliminate deductibles,

and include drug coverage. Copays are charged for services under these plans, but there are also out-of-pocket maximums. “This feature can be very important,” notes Trombley.

“An out-of-pocket maximum can protect you from racking up major costs if you have to undergo multiple procedures in the same year. Under our Enhanced Medicare Advantage Plan, the most you’ll spend per year is $3,400 in medical copays.”

There are also Medicare Supplements or Medigap plans people can choose from. According to Trombley, these plans can be expensive but offer considerable freedom for the premium—allowing you to go to any Medicare-approved provider in the country with little to no outof-pocket expense.

the plan’s network. Before signing on the proverbial dotted line, make sure any physicians you need to see and any hospitals you may need to go to participate in the plan you’re considering,” Trombley advises.

Check the plan’s drug coverage

Likewise, it’s essential to verify that any prescription medications you’re taking are covered under the plan’s formulary. “Most Medicare Advantage Plans include drug coverage, but be on the lookout to ensure any specific drugs you’re taking—for example, atorvastatin to lower cholesterol—are on the plan’s formulary. Otherwise, you could end up stuck in a plan that doesn’t meet your needs until the next open enrollment period,” says Trombley.

Check the plan’s service area

This point may seem obvious, but it’s sometimes overlooked. For example, if you decide you’ve had enough of Northwest Ohio winters and move to Florida full time, the Medicare Advantage Plan you were covered under here may not be valid down in the Sunshine State.

Research the plan’s quality rating

On the Medicare plan finder at medicare.gov, the agency rates the quality of different plans using a one- to five-star rating system. Also, Medicare continues to add metrics that have to be met in order to maintain that rating. If a plan’s rating gets too low, a caution symbol is added to the plan on the website. “Paramount Elite is a four-star plan, which is a solid rating, and has been holding steady at that rating.” Trombley adds.

Consider what’s important to you

Last but certainly not least, make sure any benefits that are of special importance to you are included in the plan, for example vision and dental coverage or a fitness benefit such as SilverSneakers, which allows participants to work out and socialize at different fitness facilities at no charge.

cupuncture has been shown to heal damaged nerves. A comprehensive study published in Neural Regeneration Research demonstrated that the addition of acupuncture treatments aided in the recovery from both spinal cord neuron damage and peripheral nerve damage and chronic pain. Objective positive results were seen in specific nerve testing using Electromyographic (EMG) and Nerve Conduction Velocity studies (NCV), in which the nerves are stimulated and the resultant nerve impulses measured.

To be sure, researchers found some acupuncture treatments less beneficial than others. The main points needed include the Du meridian, which is a pathway composed of 28

primary acupuncture points. The Du meridian acupuncture points are located on the midline of the back between the spinal vertebrae.

The Du meridian is considered the “governor vessel” of the body and is important in the treatment of many disorders, including spine and brain nerve damage, dizziness, numbness, tremors, inflammatory diseases, and infertility.

The researchers prescribed a program utilizing a unique type of advanced acupuncture in the treatment of their patients. The patients were seen intensively at a rate of five times per week for a total of six weeks.

The total number of acupuncture treatments was 30 sessions per patient.

several acupuncture needles, sending a micro pulse of energy directly into an acupuncture point.

In the theory of acupuncture energy, qi (pronounced “chi”) circulates throughout the body along specific channels or pathways termed meridians. Along these meridians are “points” that can regulate the flow of the qi in numerous ways. Inserting acupuncture needles at these points (or, indeed, stimulating them any number of different ways) can influence this flow of qi. With over 50,000 individual acupuncture points just

on the outside of the body, it takes a practitioner with a high skill level to pick out the set of points, termed a treatment prescription, that will give the best results for a given case.

In many types of nerve damage, chronic osteoarthritis, or prolonged illness a blockage of normal qi flow can occur. Many people with low back osteoarthritis have felt a “cold spot” where their chronic pain originates. The pain is like an unrelenting tooth ache.

Microstim acupuncture is a way to add energy locally that the body

seems able to utilize to assist in healing—like adding more gas to a car engine to get it running right again.

Mainstream medicine has even adopted this technique to heal fractures faster. Many times, a broken bone will refuse to heal, or heal slowly, due to the body’s lack of sufficient energy.

Medically this is termed non-union of a fracture. Surgeons can implant needles around the fracture site and hook them up to a bone stimulator.

Getting back to the study published in Neural Regeneration Research, they treated a broad spectrum of nerve problems using this Microstim acupuncture technique. Centers for Disease Control (CDC) statistics are astounding: in the United States (US), 6 in 10 adults have a chronic disease and 4 in 10 have two or more. Chronic disease is the leading cause of death and disability and the leading driver of the nation’s $3.3 trillion in annual healthcare costs.

Closer to home, the 2016/2017 Lucas County Community Health Assessment reported that 74% of Lucas County adults were either overweight (38%) or obese (36%) as defined by Body Mass Index (HLC, p.54).

All food and calories are not equal

he concept of “food as medicine” has a rich history in many ancient and contemporary healing traditions.

Hippocrates, the traditional ancient Greek “father of medicine,” is often quoted as saying “Let food be thy medicine, and medicine be thy food.”

The importance of this message—that food has the ability to help prevent

The key lifestyle risks for chronic disease are the usual suspects: tobacco use, lack of physical activity, excessive alcohol use, and poor nutrition (NCCDPHP). All these factors are changeable when we desire a healthy state and find the help needed to make that happen.

In terms of eating healthier to improve our well-being, a good first step in making a change is to understand how what we eat impacts our body and what good nutrition entails. When we eat, the calories we eat come from the fats, protein, and

carbohydrates that make up our food.

Vitamins, minerals, and other special micronutrients are present as well.

In the old way of thinking, the general opinion among healthcare providers was that maintaining a desirable weight was a simple matter of how many calories were eaten minus how many calories were burned off.

Likewise, when trying to lose weight, simply eating less while increasing exercise would lead to weight loss.

However, this view has been replaced with the knowledge that “food is information.” Not only does the food we eat provide the building blocks to help our cell and body functions work properly, but it also provides “information” to our genes.

The food we eat can work for us or against us. In response to the food we eat, genes are instructed to either turn on or turn off. If the food we eat sends the wrong information to our genes, we can become undernourished, overweight, and inflamed. This often sets the stage for the development of cancer, dementia, lung disease, heart disease, stroke, and diabetes. On the other hand, eating nutrient-rich foods helps brings out the best in our genes to assure that health is maintained.

For example, when trying to decide

whether to eat 100 calories from an apple or 100 calories from a doughnut, we know that our body will “read” this information very differently.

The average doughnut is filled with saturated fat and refined sugars and will have very little nutritional value except abundant energy from the calories it contains. Like a doughnut, a clean, healthy apple will provide energy, but in the healthy form of fructose, along with plant nutrients and dietary fiber. These nutrients and fiber send information that helps regulate and nourish cells as well as manage blood sugar and eliminate waste and toxins from the body. The information sent to the body from the doughnut promotes inflammation and fat storage.

The good news is that we are not destined at birth to develop chronic inflammatory diseases. Due to variations found in genes, the “volume” of genes can be fine-tuned up or down.

The food we eat has a big influence on our microbiome. Many microbes in our gut help pull out nutrients in food we would not otherwise be able to digest. Different microbes thrive on different types of food. In turn, the food we eat determines the health of our gut.

It is never too late to start eating healthy

The good news is that the impact of eating for the microbiome and body does not take days, weeks, or years.

Changes can be seen within hours, with a significant impact realized within three or four days of a big shift in what you eat.

Quick tips for eating healthy

• Stay hydrated! Every day, drink approximately half your body weight in ounces of water and other non-caffeinated beverages free of added sugars.

• Get most of your calories, including fat and protein, from plants. Aim for 9-13 servings of plant foods every day. A typical serving is about ½ cup of cooked vegetables, one cup of raw leafy vegetables, or a medium-size fruit.

• When eating whole plant foods, think about eating the colors of the rainbow. This has significant benefits.

• Eat plenty of high-fiber vegetables, which help maintain a healthy digestive system.

Foods containing dietary fiber are fruits, non-starchy vegetables, beans, nuts, seeds, and whole grains. These are the same foods known to fight inflammation due to their antioxidants, phytonutrients, and good fat content.

• When navigating the grocery aisles, an easy way to remember what foods you should eat is to memorize Dr. Joel Fuhrman’s G-BOMBS acronym. G-BOMBS stands for: G-Greens, B-Beans, O-Onions, M-Mushrooms, B-Berries, S-Seeds. These daily foods should make up a significant proportion of your diet. “So what drug are you taking now?” My friend knew that for the last 20 years I had been either injecting or swallowing a disease-modifying therapy (DMT) that the Food and Drug Administration (FDA) approved for slowing down the progression of my multiple sclerosis.

The problem I had with telling her which medication I was taking was that I did not know how to pronounce it.

I read books, took chemistry in high school and college, studied Latin and French—all of which should help a person sound out and pronounce words. However, for the first six months I took this pill, Gilenya, I had to call the drug hotline and listen to how the Gilenya staffers pronounced it. Soft G or hard G? Long I or long E? Sometimes I feel that those who work in pharmaceutical research are out to get us!

So, I asked Barbara Stallman, PhD in Organic Chemistry, and Professor of Chemistry at Lourdes University in Sylvania, what was with all these names. She commented, “There are generally three names for each drug.

When first discovered, a drug is given a chemical name. These names are pretty complex and cumbersome.

Often the pharmaceutical company shortens this version of the drug’s name or gives it a code name like RU-46, which is used for easy reference for researchers. Later, the pharmaceutical company can submit a brand name for the drug to the FDA for approval. The drug is also assigned a generic name, which is the official name of the drug.”

For example, my oral MS drug has the generic name of fingolimod. Its brand name is Gilenya. In the United States, the generic name is assigned by an official body—the United States Adopted Names (USAN) Council.

The brand name is developed by the company requesting approval for the drug and identifies it as an exclusive property of that company.

The Merck Corporation, a global healthcare corporation with a 125-

year history in the pharmaceutical business, explains how generic and brand drugs are marketed: When a drug is under patent protection, the company markets it under its brand name. When the drug is off-patent (no longer protected by patent), the company may market its product under either the generic name or the brand name. Other companies that file for approval to market the off-patent drug must use the same generic name but can create their own brand name. So, the same generic drug may be sold under either the generic name, for example ibuprofen, or one of many brand names, such as Advil or Motrin.

Often brand names are shorter, might be catchy, and can be related to what it is supposed to do in our bodies. The drug Lopressor lowers blood pressure, and Glucotrol controls high blood sugar (glucose). Most doctors, researchers, and healthcare professionals talk and write about a drug using its generic name because it refers to the drug itself. Often, doctors use the drug’s brand name because it is easier to remember and pronounce.

Many times consumers shy away from buying generic drugs. The word generic, when applied to food and household items, often describes a less expensive, sometimes less effective or lower-quality copycat version of a brand-name product. However, most generic drugs, although less expensive than the comparable brandname-drug, are as effective and of the same quality as the brand-name drug. (Bioequivalence and interchangeability of generic drugs is another article.) Sometimes generic drug makers manufacture the brandname drugs for the companies that control the brand names!

So when you buy a generic drug for Motrin, ibuprofen, the FDA requires its active ingredient to be identical to that of the brand name. Federal law requires that they are chemically equivalent. Some differences between the generic and brand name might include fillers, which hold the pill

together, and dyes.

Stallman adds, “So talk to your healthcare provider and ask if a generic drug would work for you.

It may offer a price break for the same help a brand-name drug offers.

Sometimes, there is no generic drug available. But, still ask.”

In his New Yorker magazine essay “Side Effects”, comedian Steve Martin addresses the reality of drugs causing side effects.

He begins, “Dosage: take two tablets every six hours for joint pain.

Side Effects: This drug might cause joint pain, nausea, headaches, or shortness of breath. You may also experience muscle aches, rapid heartbeat, and ringing in your ears. If you feel faint, call your doctor. Do not consume alcohol while taking this pill; likewise, avoid red meat, shellfish, and vegetables. OK foods: flounder. Under no circumstances, eat yak. Projectile vomiting is common in thirty percent of users—sorry, fifty percent. If you undergo disorienting nausea accompanied by migraine and raspy breathing, double the dosage.”

Although Martin’s view on side effects makes light of the serious role patients have taking their pills, it tends to keep us in balance. We try to do the right things to maintain wellness, and besides medical checkups, diagnostic tests, watching our diets and keeping active, we need to look seriously at the medications we take.

The 19th-century American physician Oliver Wendell Holmes said, “If all the drugs were thrown in the ocean, everyone would be better off...except for the fish."

is a decrease in a neurotransmitter produced in the spinal cord where the nerve originates. This deficit, in turn, seems to lead to further nerve cell death that greatly inhibits the healing process.

Researchers have found that acupuncture treatments counter this neurotransmitter decrease and actually increase it in spinal cord tissue after peripheral nerve injury.

As a result, the researchers suggest that this may be an important mechanism by which acupuncture promotes the healing of peripheral nerves.

At the conclusion of the treatment study and testing, the researchers concluded that, “Our results indicate that governor vessel and local meridian acupoints used simultaneously promote functional repair after peripheral nerve injury.”
