Shape up and wake up your skin

ello, “Millennials”! Are the skin changes related to stress, pollution, smoking, and prior sun damage and tanning booth visits starting to show on your facial, neck, and chest skin?

How about 10!

O

ver the past few months, I have used this column to address issues greater than sports, yet through sports.

Much has been happening around the world that affects all of us in various ways, and knowing that, I wanted to bring a positive swing back into creating a healthier America. With the holiday season quickly approaching, people will be spending extra time with their families, so I feel this is a perfect time to focus on how tennis provides many positives to individuals and families.

Recently, I came across the Tennis Industry Associations top 10 Reasons to Play Tennis: 1. A longer life—Scientists and doctors around the world point to tennis as one of the healthiest activities you can participate in because it delivers overall physical, mental, and emotional gains. It will add years to your life.

2. A happier life—Tennis engages your mind and body at the same time, unlike a traditional workout at the gym. Because tennis players are more confident, healthier, less stressed, and more socially interactive, they’re just plain happier.

3. It’s social—On and off the court, you are guaranteed to make friends through tennis. The sport encourages

interaction, communication, and just plain fun. It’s great for making business connections too.

4. The whole family can participate—No matter the age, gender, ethnicity, ability level, or fitness level, tennis is perfect for families to play together. Few sports can offer the opportunities for families that tennis can.

5. Tennis teaches life lessons—Participants, particularly youngsters, develop a work ethic, learn sportsmanship, accept responsibility, manage mistakes, enhance discipline, learn to compete, cope with pressure and adversity, develop healthy habits, learn teamwork, and more.

suggests it may generate new connections between nerves, promoting a lifetime of continued development of the brain.

9. Fitness—The constant movement and action in tennis will burn calories, build muscle, increase bone strength, improve flexibility, fine-tune coordination, and enhance overall health and fitness.

10. It’s a sport you can play for your whole life—There’s no other sport you can play throughout your lifetime—from the earliest of ages to your latest years—that gives you the physical, mental, emotional, and social benefits tennis gives you.

Now, as you read through these reasons why tennis is so good for an individual, it is hard to argue that it would not be worth a try. From my previous articles you know the challenges we face in society when it comes to being healthier and socially stronger. Tennis makes us stronger in both areas—and then some.

This is just a simple article to stimulate your thinking on the reasons to try tennis. There are many facilities throughout the Northwest Ohio area that would love to provide you this opportunity. Reach out to them and give the sport of a lifetime a try.

St. Francis de Sales School launches leadership program through a global leadership symposium

O

ver several years, St. Francis de Sales School has dialogued with local leaders in the fields of medicine, business, law, engineering, church, education, banking, and civil service concerning the current educational and formational needs of our community. Many noted that employees are entering the work force with excellent technical knowledge, but too few show interest in leadership or possess the skills. Employees are needed who can motivate others, adapt to change, and possess the ability, education, humility, integrity, temperament, and confidence to lead, whether hundreds of employees or just a few coworkers. St. Francis de Sales is launching the Salesian Leadership Initiative to address these concerns.

The initiative is comprehensive, faith-based, and age-appropriate.

While many career and leadership skills are honed through collegiate

degree programs, internships, residencies, and in-service days, the seeds of good leadership—self-confidence, self-discipline, empathy, flexibility, and integrity—germinate in childhood and adolescence. Our student body of 12- to 18-year-old boys provides an unrepeatable opportunity to lay the groundwork for great leaders. The Initiative is further enhanced by the flexibility of being a private school; the caliber of our students; the motivation, engagement, example, and personal success of our parents and alumni; and the expertise and dedication of our Oblate and lay faculty and staff.

The Salesian Leadership Initiative inspires future leaders of self and others. It promotes the acquisition and practice of the skills, virtues, and motivation students need as future

husbands, fathers, citizens, employees, and employers. The initiative engages the entire school community: students, faculty, and staff. It involves teacher training and follow up, appropriate additions and adjustments to current course and extracurricular activities, student formation, expert coaching, alumni mentoring, group activities, service trips, individual and group reflection and integration, and outcomes assessment. In addition, qualified juniors and seniors may elect to participate in a more focused, community-based leadership program.

The Global Salesian Leadership Symposium is the launch of this initiative. The symposium will take place at the Seagate Convention Center, 401 Jefferson Ave. in Toledo, on Monday, November 6, 2017, from 9:00 a.m. to 2:45 p.m., for all St. Francis juniors and seniors. Alumni leaders from around the world as well as community leaders will speak to the St. Francis students about leadership qualities, including perseverance, unity, optimism, self-acceptance, loyalty, and integrity. ❦

2011 study published in the New England Journal of Medicine showed that screening reduced mortality among a select population of highrisk individuals who were all between the ages of 55 and 74 with at least 30 pack years of smoking (1 pack year is the equivalent of having smoked 1 pack of cigarettes per day for a year) and who quit within the last 15 years. The study participants, who were screened with a low-dose CT once a year for three years, showed a reduction in mortality of about 20 percent specific to lung cancer.

Studies show chelation is powerful ally in fight against “age-related” ailments

ost people are well aware of the fact that as they get older, their vulnerability to a wide range of diseases increases. However, contrary to popular misconception, most of these “age-related” disease states are not a natural consequence of the aging process.

According to cardiologist James Roberts, MD, of Comprehensive Heart Care, many of the diseases we associate with aging, such as high blood pressure, high cholesterol, and cardiovascular disease, are driven by oxidative and inflammatory stress—in other words, too many free radicals and too much inflammation, which the body interprets as infection.

Dr. Roberts explains that a variety of influences contribute to oxidative and inflammatory stress, including being overweight, eating a poor diet, and not getting enough antioxidants.

“Toxins are also a major contributor,” he says. “In fact, we’re in a ‘soup’ of toxicity today and it’s making us sicker and sicker. The body absorbs these toxins readily but isn’t as efficient at

getting rid of them. This isn’t necessarily a problem when we’re young, but over time these toxins build up silently and can lead to diseases as we age. For example, we know there are links between lead exposure and hypertension, heart disease, and kidney disease. Cadmium exposure is linked to these same diseases as well as an increased risk of breast cancer.”

Dr. Roberts notes that toxins can take a toll among the younger population as well. “Autism didn’t exist 50 years ago, but today it’s a major issue because of all the toxins we’re exposed to,” he says.

The oxidative and inflammatory stress responsible for these and other illnesses can be reduced in a number of ways, one of which is a controversial treatment called chelation therapy, offered at Comprehensive Heart Care. During chelation, the patient is given antioxidant vitamins and minerals and a synthetic amino acid called EDTA (ethylene diamine tetra-acetic acid) is delivered intravenously. working synergistically would add to the benefits of the gold-standard therapies. Vitamins were shown to help somewhat, but in the group receiving chelation we found an 18 percent reduction in the fiveyear adverse-event rate. Among the study participants with diabetes, the results were even more dramatic. Their five-year adverse-event rate fell by 39 percent. What’s more, among those who were chelated and received vitamins and minerals, the five-year death rate fell by 50 percent,” says Dr. Roberts.

No side effects were noted in the TACT study, and chelation has not been found to cause damage to the kidneys.

However, notwithstanding the safety and demonstrated benefits of chelation, the FDA has yet to approve the treatment and Medicare and other insurance providers are still unwilling to pay for it. Dr. Roberts hopes that will change after the next TACT study, for which he is currently enrolling patients. “The FDA wants to see replication of the original study, so we’re repeating it,” says Dr. Roberts, “but this time just with patients who have had a heart attack and have diabetes—the group that showed the greatest benefit in the first study. Once again, participants will get chelation versus placebo and high-dose vitamins versus placebo to determine the effect on their event rates.

Dr. Roberts is optimistic that the results of the second TACT study will confirm the efficacy of chelation and ultimately lead to FDA approval and insurer coverage of the therapy. “If we can get chelation into the hands of more and more physicians, everyone will benefit. Detoxification should be our focus before anything else, and it’s important to start treatment early, for example before patients with kidney disease are on the brink of dialysis. But before we can reach that potential, we need to get the FDA, Medicare, and other insurers behind it,” he says. ❦

Dr. Roberts’ interest in chelation developed a quarter century ago when he learned that some of his patients were having the procedure done without his knowledge. At the time, he insisted that chelation wouldn’t work because that’s what he had been taught in medical school.

But these individuals were actually getting better.

It was at the request of one of these patients that Dr. Roberts decided to learn more about chelation. He recalls, “The first patient I personally treated was struggling while on the Heart Transplant List.

Her angioplasties had closed, her grafts had failed, and she was experiencing daily chest pain. At that time, patients waited up to six months for a heart transplant and many died while waiting. So this patient asked me about chelation and I agreed it would be a good idea. Chelation wasn’t available in Toledo then and she wasn’t stable enough to travel, so she urged me to take the chelation course and treat her. With chelation, she stabilized to the extent that she was able to survive long enough to obtain the heart transplant.”

Today Dr. Roberts teaches other physicians how to perform chelation and is one of a relatively small number of providers nationwide who offer this therapy.

In addition to Dr. Roberts’ success, a growing body of research supports the benefits of chelation.

For instance, in three studies, people with chronic kidney disease who were chelated actually stabilized, delaying or preventing the need for dialysis, while people in the control group got worse.

Also, the $32 million TACT (Trial to Assess Chelation Therapy) study, funded by the National Institutes of Health, proved the effectiveness of chelation in treating heart disease.

t’s time once again to bid a fond farewell to the growing season and put our gardens and landscapes to bed until spring. Along with cool temperatures and reduced daylight, late fall brings its own set of gardening chores to be tackled before winter’s snow begins to fly. Gardener’s need to tidy up borders and beds, wrap up any remaining fall planting, provide winter protection for tender plants, and properly clean and store their tools and equipment.

Prep your perennials—Once the first hard frost hits and the foliage on your herbaceous perennials has completely withered, it’s time to cut them back to the ground. However, hold off cutting back any plants that still have green leaves, since they are still performing photosynthesis and transporting energy to their roots to be stored until spring.

If you’d like your garden or landscape to provide a little winter interest, you might choose to avoid cutting back any plants that have attractive seed heads or stems or fruits or seeds that will attract foraging, winter-weary birds.

Keep on planting—It’s getting late in the season for fall planting, but there’s still time left. As long as the soil is workable, you can plant many varieties of trees, shrubs, hardy perennials, and spring-flowering bulbs.

They won’t have much time to establish a good root system before winter sets in, so be sure to water them well right up until the ground freezes and give them a nice insulating blanket of mulch.

Make the most of markdowns—This is also a great time to take advantage of marked-down nursery stock to fill in those empty spaces in your garden or landscape. In late fall, garden centers commonly offer good, viable plant material at a considerable discount.

The plants may not look like much right now, but most will come back in spring just as well.

If you’re not too particular about the variety or color of spring-flowering bulbs, you can often get terrific deals on large quantities of “mystery bulbs”—those that fall out of the bulk bins as customers rummage through them or bagged bulbs that have lost their labels. You can still distinguish a tulip from a hyacinth or a crocus from a narcissus, but the colors and blooming periods will be anyone’s

guess. If you’re looking to naturalize a remote corner of your garden on a low budget, mystery bulbs may be a good solution.

Put winter protection in place— Mulch will be your biggest ally in preventing winter casualties. Apply it liberally around the base of each plant. With grafted roses, be sure to mulch over the bud union (the point where the plant has been grafted to the rootstock), which is very sensitive to freezing. If the bud union is left exposed, the rose could die back past the graft, and you may end up with a rose of an entirely different color growing up from the roots in spring.

If you choose to protect your roses with Styrofoam rose cones, keep in mind that they allow no air circulation, so mold can quickly result if the air temperature climbs too high.

Be prepared to remove them during unexpected warm spells.

Burlap is also very useful for winter protection. Use it with stakes to build a wind block around tender plants in exposed areas. A burlap frame combined with the use of an anti-desiccant spray will give broadleaf evergreens a good level of protection against winter damage.

Treat your tools right—Last but certainly not least, it’s time to clean up and prepare your trusty gardening tools for a long winter’s nap. Fill a large bucket with sand, and mix in some light motor oil. After removing as much dirt and debris as you can from your tools, dip the business end of each repeatedly into this mixture.

The sand will scour off any remaining dirt, and the oil will provide a rust-protective coating. If some tools are already rusted, sand off the rust before dipping.

Prepare your gas-powered tools for winter storage by either running them dry or adding a fuel stabilizer to the fuel tank. This will make starting a snap next season. Late autumn is also a good time to take power tools in for any necessary repairs, adjustments, or sharpening. Don’t postpone repairs until next spring when the shops will be inundated An investment in the future—This may seem like a substantial to-do list—especially when you thought you were done with gardening for the season—but if you tackle these chores now, you’ll be prepared to hit the ground running next spring. ❦

But Dr. Perry offers a pioneered form of the procedure called In-Office Balloon Sinuplasty, which is performed right in the office using only a topical and local anesthetic. Not only is In-Office Balloon Sinuplasty more convenient and cost-effective than surgery at a hospital or surgery center, but it’s also just as safe and effective. What’s more, patients do not have to worry about the risk of side effects or complications associated with general anesthesia. Dr. Perry notes, “The experience is much like going to the dentist’s office to fill a cavity.”

Non-invasive or minimally-invasive fat removal: Which choice is right for you?

r. Banker from Luxe Laser Vein and Body Center offers some insight into non-invasive and minimally-invasive fat-removal options available to patients, and why a patient might be a better candidate for one or the other. Procedures in each of these broader categories can play an important role in helping people achieve the look they desire, and can be used together when appropriate.

In order to make the best decision among the available fat-removal options, Dr. Banker points out that you should always speak with a physician that has both options, thus avoiding any bias.

Non-invasive fat removal According to Dr. Banker, non-invasive fat-removal techniques do not involve breaking the skin or placing anything underneath the skin. “These procedures are aimed at individuals who want zero downtime and zero recovery time. They can literally be done over your lunch break, and then you can go right back to your

daily activities with no restrictions or lifestyle changes required,” he says.

The tradeoff for this exceptional level of convenience is that the results are slightly less dramatic than what you can expect from procedures performed under the skin. “Results will be noticeable but won’t necessarily reach the same level with a single treatment that you can with a minimally invasive procedure. However, the cost of non-invasive procedures is significantly lower and probably accurately reflects this tradeoff,” Dr. Banker adds.

The ideal candidate for non-invasive fat removal is someone who seeks a moderate result with no downtime

and wants to be price-conscious in making the decision. According to Dr. Banker, the premier non-invasive fat-removal technique available—and the one he has found to be most effective—is CoolSculpting. This method involves treating areas of stubborn fat with low temperature to destroy the fat cells, which are then cycled out of the body naturally.

Minimally-invasive fat removal Not to be confused with invasive procedures requiring full-blown surgery, such as gastric bypass, minimally-invasive fat removal procedures are performed in the outer layer of tissue. These techniques produce a more dramatic result compared to non-invasive methods because the surgeon actually goes under the skin and physically removes as much fat as possible.

Dr. Banker explains, “Minimally-invasive fat removal is performed

by inserting a small laser fiber under the skin, melting the fat, and then physically removing it from the body at the time of the procedure. The gold standard in this category is Smart Lipo.”

Minimally invasive fat-removal procedures do require some recovery and downtime. Patients will also need to adhere to certain restrictions afterward, such as avoiding lifting heavy objects as well as strenuous workouts/activities. The cost for these procedures also tends to be higher than for non-invasive techniques, again because more fat is being removed and a more dramatic final result is being achieved.

Proper precautions and home safety assessment help Laurels client maintain rehab gains

C

lients who come to The Laurels of Toledo for rehabilitation following surgery, injury, or illness can expect the therapy staff to do their utmost to help them regain function and get back to their normal activities of daily living. The right exercises and cutting-edge therapy equipment are vital components of that process. However, they don’t represent the entire rehab picture. In order to protect a good clinical/surgical outcome, maintain the gains achieved during rehab, as well as make a safe transition back home, it’s essential for rehab clients to observe certain precautions every step of the way.

For James Green, 67, who recently came to The Laurels of Toledo for rehab following spinal-fusion surgery, that meant learning innovative ways to function at home day to day so he could avoid bending and heavy lifting. Green had been suffering with cervical myelopathy—narrowing of the spinal canal that puts pressure on a nerve, causing severe pain—and was hospitalized from September 28 through October 3 for surgical

fusion of the C5 and C6 vertebrae in his neck. “Before the surgery, I could hardly tolerate standing and whenever I reached for something up high, I felt a shock throughout my whole body,” he says. “I was also in a pool league at the Hunt Senior Center but had to stop because it was too painful to play.”

Laurels licensed certified occupational therapy assistant Cristina Nickle, who worked with Green throughout his inpatient rehab, recalls, “When James arrived here on October 4, he was wearing an Aspen cervical collar to stabilize his neck and had been given typical restrictions for back surgery, including avoiding lifting objects weighing over ten pounds and avoiding bending over. So part of my job was recommending adaptive equipment and techniques he could use in order to protect his spine while performing activities like dressing and bathing. For example, he uses a reacher and long-handled shoehorn for dressing and a long-handled sponge for bathing his lower body.”

Nickle notes that rehab clients are often perfectly capable of comprehending and reiterating the restrictions and precautions they’re supposed to observe, but then discover they’re not as easy to apply to the functional activities they perform every day.

To help identify areas in the home environment that might present a challenge or safety risk, The Laurels

offers home-safety assessments that are conducted before clients are discharged.

“One of the challenges we noticed right away when we assessed James’ apartment is that the entrance to the complex is about 500 feet away from his room. So we advised him to use a seated walker so he can stop and sit down if he starts to get tired or feels unsteady on his feet. Another issue we encountered was the fact that James has a bathtub rather than a walk-in shower. He has difficulty lifting his legs, so stepping into the tub wasn’t a safe option. To overcome that, we recommended that James use an extended tub bench that has two legs inside the tub and two outside.

Using the bench, he can sit down on the outside, steady himself, and then swing his legs into the tub,” says Nickle.

support when standing or walking in the kitchen, and always keeping a seat close at hand, whether it’s a seated walker or a chair, so they can sit down immediately and avoid a fall if they start to feel unsteady. To help clients with lifting restrictions visualize the maximum amount of weight they can safely manage, she explains that a gallon of milk weighs eight pounds, so they shouldn’t attempt to lift anything heavier than that.

Nickle observes that Green is very conscious of energy-conservation— walking with a slow-and-steady pace and knowing when to stop and take a rest—which gives him an advantage when it comes to fall prevention. “Also, he has recruited family and friends to help with chores like laundry and housekeeping. Of course, he knows not to reach for items up high and is very careful to avoid bending,� she says.

For his part, Green is grateful to have learned about all these tools and techniques that simplify daily

activities, stating, “They showed me easier ways to do a lot of the everyday things we all take for granted, such as getting in and out of bed, bathing, dressing, and cooking meals.�

Green also credits the rehab team at The Laurels for getting him out of pain (he no longer takes any pain medications) and for helping him get back on his feet. As of this writing, he is interested in returning to The Laurels for outpatient therapy—taking advantage of the facility’s free shuttle service—and is no longer required to wear the Aspen collar while sleeping. “The main thing now is that I want to be able to get back outside and walk in the park, and I’m really looking forward to playing pool again,â€? he says. â?Ś The Laurels of Toledo, located at 1011 Byrne Road, accepts Medicare, Medicaid, and all private commercial insurances.

cupuncture is effective in treating a wide range of conditions.

Some of them might even surprise you. Here are the top five:

1. Infertility For thousands of years, acupuncture, in combination with other adjuncts of Oriental Medicine, has been successfully used to treat infertility.

Researchers are beginning to study the effects that acupuncture produces in a woman’s body.

Anovulatory infertility, which is infertility caused by lack of egg release during the monthly cycle, is one of the most common causes of infertility.

A recent article in the Journal of Taishan Medical College noted that acupuncture treatments in combination with traditional clomiphene therapy increased pregnancy rates, increased blood flow through the uterine arteries, and reduced side effects caused by the clomiphene.

Pregnancy rates were notably higher and side effects were greatly

decreased over the traditional clomiphene plus estradiol cypionate and dydrogesterone protocol when just acupuncture and clomiphene were used.

When adding acupuncture to IVF treatments, an increased successful implantation rate of 15 to 17% has been reported.

2. Chronic inflammation Inflammation comes in many forms.

You jam your finger on a door jamb, and within minutes it’s red, sore, and warm. This is acute inflammation, and it is a sign that the body is beginning the healing process as blood and proteins flow into your damaged finger.

Many studies have shown that acupuncture is effective for many types of pain. Researchers have had to dig down to try and figure out the mechanism by which acupuncture works. Recent studies have demonstrated that acupuncture stimulates the release of neurotransmitters, including opioids, in the central nervous system. Acupuncture has also been shown to cause changes in beta-endorphin production via the hypothalamus. Beta-endorphins and natural opioids are the body’s own pain-suppression system.

4. Nerve repair Carpal tunnel, Bell’s palsy, peripheral neuritis, sciatica, and radiculopathy are just a few examples of nerve damage in the body.

Acupuncture assists in the promotion of nerve healing. According to one major study, published in Neural Regeneration Research, nerve conduction tests of acupuncture patients with nerve injuries showed “an effective response” in 80% of patients participating in the study.

The results indicate that acupuncture treatments promote functional repair after nerve injury.

5. Failed back and neck surgery syndrome Patients seem to be pouring into physicians’ offices with complaints of chronic pain localized to a past spinal surgery. This complaint has become so prevalent that doctors have named it “failed back surgery syndrome” (FBSS).

Typically, a patient will have back surgery and initially get favorable results. But after a few years, the pain returns to the surgical site with a vengeance.

At this point the pain seems to involve a chronic inflammation of the soft tissues at the surgical site.

This type of back pain is not correctable through additional surgery, and patients are frequently referred to pain-management centers.

One recent study involved 80 patients. The improvements noted included a reduction in back or neck pain and leg or arm pain. Also noted were improvements in sensory and

motor nerve impairment. Patients who were previously heavily impacted in their ability to perform activities of daily living found they were able to do much more after treatment.

Recent advances in acupuncture include a technique called micropulse acupuncture. This technique has proven particularly effective in controlling pain of chronic injuries and FBSS. In this technique, traditional acupuncture needles are placed in points near the old surgical site.

A micropulse generator attached to the needles sends a micro electrical pulse down the needle into the tissues.

This has the effect of stimulating the regrowth of micro blood vessels into the area, allowing the body to restructure fibrous scar tissue into a healthier connective tissue and releasing trapped inflammatory acids contributing to the chronic pain.

Modern research has demonstrated acupuncture’s effects on the nervous system, endocrine and immune systems, cardiovascular system, and digestive system. By stimulating the body’s various systems, acupuncture can help to resolve pain and improve sleep, digestive function, and sense

of well-being.

Fried turkey safety tips

T

he trend of deep frying the Thanksgiving turkey continues to grow all across our nation, and with good reason. Deep frying the ol’ gobbler seals in flavor and juices so it turns out delightfully tender, succulent, and delicious. What’s more, deep frying cooks the bird in a fraction of the time it takes for conventional oven roasting—only about three to four minutes per pound.

But deep-fried turkey also has a potential sinister side. Lowering a turkey into a big vat of scalding-hot oil makes for a highly combustible situation to say the least. Each year around Thanksgiving, news stories about turkey-fryer-related fires, injuries, and even deaths begin to pop up. So, if you intend to deep fry your bird this Thanksgiving— and would prefer to stay out of the headlines—be sure to observe the following safety tips:

Read the fryer manual While most turkey fryers function in essentially the same way, there are certain differences among the various models. Before attempting to use your new fryer, be sure to read the manual thoroughly to familiarize yourself with all the included components as well as the manufacturer’s safety precautions and recommendations for proper use.

Set up the fryer outdoors Under no circumstances should a turkey fryer be used inside your home, garage, or any other enclosed area.

Instead, set up the fryer outdoors on a flat, level surface that is out of the wind and a good distance from any structure or combustible material, such as dry leaves. The best place to set up frying operations is on dirt or concrete—never on a wooden deck or any other type of wooden platform.

Have a fire extinguisher handy No turkey frying should take place without a fire extinguisher close at hand—and make sure it’s rated for use on oil fires.

If an oil fire should occur, the last thing you’d want to do is try to extinguish it with the garden hose.

Small fires can grow into large, unmanageable ones very quickly, so if you have any doubt about your ability to extinguish the fire, call 911.

Protect your hands and eyes To protect your hands and arms from spattering oil and the hot surfaces of the frying vat, wear heavy duty oven mitts. It’s also a good idea to don goggles or other protective eyewear before deep frying the bird.

Smaller birds are better The bigger the bird, the greater the potential for hot oil to overflow and cause a fire or major injury. Not to mention, bigger turkeys tend to burn on the outside before the inside is cooked completely. The USDA

recommends a maximum size of 12 pounds for turkeys destined for the deep fryer. If one 12-pound bird won’t feed all your guests, don’t risk going larger. Instead, fry a second bird while the first one is resting.

be sure to remove the neck and bag of giblets from the body cavity as well as any plastic bindings and that plastic button that pops up when the turkey is done. Do not stuff the turkey before frying.

Don’t overfill the oil

The risk of tipping the frying vat or causing an overflow is greatest when you’re immersing the turkey in the oil and raising it out. Never rush these operations. Even a completely thawed and dried bird will cause some spattering as you lower it into the hot oil. Lower too quickly, and the vat can easily bubble over.

Simply “eyeballing” the oil level before lowering the turkey into the vat often results in an overflow of oil, which can easily ignite if it makes contact with the cooking flame. The best way to determine exactly how much oil you’ll need is to perform a simple water-displacement test.

Place the turkey in the empty vat and fill it with water to a few inches above the bird. Then, remove the turkey and measure the depth of the water. That measurement will tell you how high the oil level should be before the turkey is added. Once you’ve made your measurement, drain the water and dry the vat completely.

Prepare the turkey properly Before frying, make sure the turkey is completely thawed and pat it dry—inside and outside—with paper towels. Remember, any moisture will cause the oil to spatter. Also,

Cook the turkey properly Food safety is just as big a concern when frying a turkey as when oven roasting it. Heat the oil to 350 degrees, and cook the turkey until its internal temperature reaches at least 165 degrees in the breast as well as the thickest part of the thigh.

Never leave the fryer unattended From the moment the burner is lit, the turkey fryer must not be left unattended. Also, the frying area should be ruled off limits to children and pets. The oil will remain dangerously hot well after you’re done frying, so this rule must remain in effect for several hours post cooking. Also, don’t attempt to dispose of the oil or store it until it has cooled completely. ❦

The role of physical therapy in managing diabetes

I

have been writing the last few months about specific diseases such as Alzheimer’s and cancer and the role physical therapy plays in addressing the individual’s ability to perform functional daily living tasks. This month I’ll continue to focus on the role of physical therapy in the management of chronic disease, specifically diabetes.

I want to start by defining the term “chronic disease.”

A chronic disease is a disease that persists for a long time, lasting three months or more by the definition of the US National Center for Health Statistics. Chronic diseases generally cannot be prevented by vaccines or cured by medication, nor do they just disappear.

Before getting into the role that physical therapy has in chronic disease management, I want to share some facts on chronic disease and specifically on diabetes.

Chronic diseases are responsible for seven of ten deaths each year, and treating people with chronic diseases accounts for most of our nation’s healthcare costs according to the Centers for Disease Control.

What is diabetes? Diabetes is a chronic disease that affects how your body turns food into energy. Most of the food you eat will be broken down into sugar (also called glucose) and released into your bloodstream.

Insulin, a hormone produced by the pancreas, acts like a gatekeeper or key to let the blood sugar into your body’s cells for use as energy. If you have diabetes, your body either doesn’t make enough insulin or can’t use the insulin it makes as well as it should. When there isn’t enough insulin or cells aren’t responding and not using the insulin, then too much sugar stays in your bloodstream, which over time can cause serious health problems.

Diabetes is broken into three categories: Type 1 diabetes is caused by an autoimmune reaction (the body attacks itself by mistake) that stops your body from making insulin. It’s

usually diagnosed in children, teens, and young adults.

Type 2 diabetes occurs when your body doesn’t use insulin well and is unable to keep blood sugar at normal levels. Most people who have diabetes have this type. It develops over many years and is usually diagnosed in adults (though increasingly in children, teens, and young adults). Type 2 diabetes can be prevented or delayed with healthy lifestyle changes, such as losing weight if you’re overweight, healthy eating, and getting regular physical activity.

Gestational diabetes develops in pregnant women who have never had diabetes.

If you have gestational diabetes, your baby could be at higher risk for health complications. Gestational diabetes usually goes away after your baby is born but increases your risk for type 2 diabetes later in life.

There’s also a state called prediabetes—a serious health condition where blood sugar levels are higher than normal but not high enough yet to be diagnosed as diabetes.

The big picture To understand the scope of our nation’s diabetes problem, consider the following CDC statistics: • More than 30 million people in the United States have diabetes, and one in four of them don’t know they have it.

• More than 84 million US adults—over one-third—have prediabetes, and 90% of them don’t know they have it.

• Diabetes is the seventh leading cause of death in the United States (and may be underreported).

• Type 2 diabetes accounts for about 90 to 95% of all diagnosed cases of diabetes; type 1 diabetes accounts for about 5%.

• In the last 20 years, the number of adults diagnosed with diabetes has more than tripled as the American population has aged and become more

overweight or obese.

• The total direct and indirect estimated cost of diagnosed diabetes in the United States in 2012 was $245 billion.

• Average medical expenditures for people with diagnosed diabetes were about $13,700 per year. About $7,900 of this amount was attributed to diabetes.

These are some significant, overwhelming, and frightening facts with the trend increasing and not decreasing!

Diabetes risk factors You are at risk of developing prediabetes or type 2 diabetes if you are overweight; are age 45 or older; have a parent, brother, or sister with type 2 diabetes; are physically active fewer than three times a week; or have ever had gestational diabetes or given birth to a baby weighing more than nine pounds.

Also, people with diabetes are at greater risk of developing complications if they smoke, are overweight or obese, are physically inactive, or have high blood pressure, high cholesterol, or high blood glucose.

Physical therapy and diabetes Physical therapy can play an instrumental role in managing diabetes.

Physical therapy addresses the areas of safe, effective exercise; healthy lifestyle; skin management; fall prevention; and pain and neuropathy management.

Exercise/physical activity is a key for the prevention of type 2 diabetes and, if you have diabetes, can be the key to controlling blood sugar, improving blood pressure, lowering cholesterol, improving mood, lowering anxiety, and reducing the risk of heart disease and stroke.

The American Diabetic Association recommends at least 150 minutes of moderate-intensity activity per week (aerobic and resistance training), with no more than two consecutive days without exercise. The ADA also recommends a reduction of sedentary time by breaking up prolonged sitting.

The physical therapist will work with the individual to develop an exercise program involving aerobic

and resistance exercises, as well as communicate that activity is more about decreasing medication needs and reducing peaks and valleys in blood sugar than about getting in shape or losing weight.

To encourage a healthy lifestyle, the physical therapist will provide education and coaching that supports healthier eating, including decreasing or eliminating high-sugar foods/drinks, reading and understanding food labels, and making healthier food choices.

The physical therapist can also assist with skin management. Individuals who are insulin-dependent and have to administer multiple insulin injections daily can develop scar tissue at the injection site over time. The therapist can assist in breaking down this scar tissue, which will allow better absorption of insulin into the bloodstream, and can educate the individual on pressure point avoidance and skin and wound care.

Pain and neuropathy management is another area that a physical therapist can address. As a result of nerve

impairment, diabetics often have pain and lack of feeling (neuropathy) in their feet and hands, and physical therapy will provide interventions that focus on increasing blood flow to these areas, which may improve sensation.

Yet another aspect of diabetes management that a physical therapist can address is fall prevention.

Individuals with neuropathy in their feet have a higher risk of falls. The physical therapist can work with the patient to address gait patterns, footwear, and balance.

Again, physical therapy can help individuals with diabetes by improving their ability to move, perform daily activities, reduce pain, lower blood sugar, heal associated skin problems, and lead a healthier lifestyle. ❦ Heartland Rehabilitation Services’ goal is to assist you in focusing on your health and wellness for life. ecently I spoke with my niece, Mary, who told me they should be in their new house at the end of October, a month short of a year to the date when their home burned to the ground. She sounded hopeful that all will go well, and even though they have had some setbacks, she sounds ready to face the challenges of making a new house a home for her and her family. Praise God! It’s become too commonplace for bad news to greet us on a daily basis.

Besides personal traumas, at least somewhere in our neighborhood, our city, our state, our country, or our world, some tragedy or trauma is being experienced and shared immediately in the media.

Bad things have always happened on this planet, and the difference seems to be that we learn about these challenges as soon they happen and sometimes even while they are unfolding. How do we keep our wits about us with all these problems constantly coming at us? How do we care for our souls when such awful things are happening to our family, neighbors, friends, and community?

It seems to me that Maya Angelou has an excellent answer to the questions posed above. We can hope that things will get better or remain in fear of what is yet to come. It’s up to us to decide if we will invite hope or fear to stay with us as we face the future. I’m so glad my niece chose hope to help her work through the many changes that she and her family faced when the house burned down and she continues to face the new challenges with hope.

Hopefully, hope is there to stay and will sustain them now and in the future because none of us knows what the next days, weeks, months,

or years will bring.

If we choose fear, we will be unable to act because fear keeps us from acting in ways that would be helpful in changing this world in which we live.

I know that denial is a coping mechanism that helps us deal with difficult situations. It becomes dangerous when we use it as the only way we deal with problems. It is dangerous because it doesn’t allow us to make the changes we need to make to help ourselves or others deal with life and its many twists and turns. You may have heard a person who has a terminal illness say how she didn’t really live until she learned she had only a few months or a year to live. She faced death, looked death in the face, so to speak, and changed her life accordingly. The death bed conversion, the ability to say “I’m sorry, I forgive you” comes only when a person faces what needs to be changed.

There are issues in our society that need to be faced in order for change to take place. I can live with some of them not changing as long as we as a society have at least attempted to change them by honest and open dialogue.

I know that change is difficult for people; it’s difficult for me. I also know that for large institutions, like churches, educational institutions, healthcare systems, and governments, change is extremely difficult and slow to come. I live with the hope that change will come, things will get better, maybe not in my lifetime but surely when enough people work together for a better and brighter, more peaceful world and a healthier planet. Dr. Tita notes that the “low-dose” aspect of CT lung cancer screening is important because the radiation dose produced by a conventional CT scan, done yearly over a long period of time, is unacceptably high for routine exposure. To ensure patient safety, the level of radiation was reduced so that it’s roughly equivalent to that of a mammogram. “We got to where we are today by first determining that yearly CT scans diagnose lung cancer and increase survival, then developing the technical ability to lower the CT radiation dose for screening purposes, and finally getting insurance companies and the CMS on board,” he says.

Since Dr. Tita began ordering screening CT scans for patients at high risk, he has found malignant nodules in the lungs of several individuals who

were completely asymptomatic and were then treated effectively. “Just as with lumps or other irregularities found on mammograms, most nodules found in the lungs are not malignant. In fact, about 30 percent of Ohioans have benign nodules in their lungs from prior exposure to histoplasmosis, which is a fungal infection acquired by inhaling spores that are present in soil contaminated with bird droppings. However, some nodules are going to be malignant and we want to catch and treat those as early as we possibly can,” he says.

With respect to misdiagnosing benign nodules as cancer, the concern is not only that it causes the patient needless anxiety, but also that it can lead to unnecessary biopsies and other procedures that come with their own set of potential complications. So, as with any type of health screening,

doctors recommending CT screening for lung cancer have to balance the benefits of early diagnosis against the risks associated with overdiagnosis.

“However,” says Dr. Tita, “the bulk of evidence seems to show that the benefits of catching lung cancer early outweigh the risks of unnecessary procedures.”

If lung cancer screening does lead to a diagnosis, several effective options are available for treatment. “Surgery is generally the most effective approach in earlier stages, but it’s not an option in advanced stages. Chemotherapy, though generally not curative, may extend survival and cause some regression. In patients who aren’t candidates for surgery—for example, those who have poor lung function or are in overall poor health—stereotactic radiation therapy can be very effective at controlling localized tumors. We offer this treatment at the Mercy Health – Perrysburg Cancer Center, and it’s generally very well tolerated,” Dr. Tita says.

The ABCDs of Medicare: What covers what?

rying to figure out the different parts of Medicare can be a confusing process. That’s because each part (A, B, C and D) provides a different set of benefits. When you’re ready to enroll, it’s important to understand those differences so you can make the best decisions. Here’s a brief explanation to get you started:

Medicare Part A Provided by the federal government at no cost to most people, Medicare Part A covers care you receive in a hospital or other inpatient or long-term care facility.

This includes nursing services, a semi-private room, meals, medications, and other hospital services and supplies. Medicare Part A also covers a stay in a skilled-nursing or rehabilitation facility after a qualifying hospital stay of at least three days. Medically necessary home healthcare and hospice care are also covered. You are eligible to begin receiving benefits when you turn 65.

Get me out of here: emergency planning for those with a disability

I

was glad I was not at the Las Vegas concert the night the gunman tragically shot innocent people.

I thought about people there who had to evacuate quickly. This must have been a frightening and challenging thing to do with thousands of people, full of shock and fear.

Then I thought, “What would I do with my electric cart, trying to drive out of the area in the night, with people running all over?” Besides trying to leave a dangerous area and go to a safer place, I know there would be the challenges of physical barriers

for those in wheelchairs or using carts or canes.

Physical barriers are structural obstacles in natural or manmade environments that prevent or block mobility (moving around in the environment) or access.

Examples of physical barriers include steps and curbs that block a person with mobility impairment from entering a building or using a sidewalk, non-paved walkways where a person might get stuck in soft earth, and just trying to negotiate leaving quickly. It can be challenging using your wheelchair

or electric cart when people might be running all over and you have to leave without hitting a pedestrian.

Accessible emergency management I checked with the Federal Emergency Management Agency (FEMA) to find out what type of plan they had for those who use wheelchairs, electric carts, or walkers and canes when they find themselves in a weather catastrophe or safety crisis. FEMA did have a plan for those who had mobility limitations.

Here’s how FEMA is facilitating accessible emergency management: • Preplanning for evacuations to include accessible transportation for people using wheelchairs and other mobility devices.

• Equipping each Disaster Recovery Center with communication devices that allow people who are blind or have low vision and people who are deaf or hard of hearing or have other communication access needs to receive disaster information first hand.

• Improving our disaster housing strategies and guidance so that

people with disabilities have access to general shelters and temporary housing that meet their needs and helping to keep families together during an emergency.

• Providing practical cot and sleeping arrangements for evacuees with disabilities; assisting them with personal care, hygiene, and specific dietary needs; and accommodating service animals alongside disaster survivors in the shelter.

• Registering disaster survivors for federal assistance with mobile devices, enabling Disaster Survivor Assistance Team members to register survivors in the field.

• Prior to a disaster, if there is enough warning, prepositioning disability integration advisors as part of the Incident Management Action Teams.

What I found particularly helpful was FEMA’s We Prepare Everyday video, which targets all communities and shows people with disabilities taking charge to prepare themselves and their families for emergencies.

The video provides equal access and includes open captioning, a certified deaf interpreter, and audio description for viewers who are blind or have low vision.

Make a plan How might a disaster affect you?

Could you make it on your own for at least three days? After a disaster, you may not have access to a medical facility or even a drugstore, so it’s crucial to plan for the resources you use regularly and what you would do if those resources are limited or not available. Additional planning steps should include: • Create a support network. Keep a contact list in a watertight container in your emergency kit.

• Be ready to explain to first responders that you need to evacuate and choose to go to a shelter with your family, service animal, caregiver, personal assistant, and your assistive technology devices and supplies.

• Plan ahead for accessible transportation that you may need for evacuation or getting to a medical clinic. Work with local services, public transportation, or paratransit to identify your local or private accessible transportation options.

• Inform your support network where you keep your emergency supplies; you may want to consider giving one member a key to your house or apartment.

• Contact your city or county government’s emergency management agency or office.

Many local offices keep lists of people with disabilities so they can be helped quickly in a sudden emergency.

• If you are dependent on dialysis or other life-sustaining treatment, know the location and availability of more than one facility.

in your home that requires electricity, talk to your doctor or healthcare provider about how you can prepare for its use during a power outage.

Wear medical alert tags or bracelets.

If you have a communication disability, make sure your emergency information notes the best way to communicate with you.

If you use an augmentative communications device or other assistive technologies, plan how you will evacuate with the devices or how you will replace equipment if lost or destroyed. Keep model information and note where the equipment came from (Medicaid, Medicare, private insurance, etc.).

If you use assistive technology devices, such as white canes, CCTV, or text-to-speech software, keep information about model numbers and where you purchased the equipment, etc.

Plan how you will communicate with others if your equipment is not working, including laminated cards with phrases, pictures, or pictograms.

Keep Braille/text communication cards, if used, for two-way communication.

The U.S. Department of Health and Human Services online tool at HealthIT.gov helps people locate and access their electronic health records from a variety of sources.

Plan for children with disabilities and people who may have difficulty in unfamiliar or chaotic environments.

Healthy eating with diabetes

N

ovember has been designated Diabetes Awareness Month— and with good reason. Diabetes has reached epidemic proportions and impacts millions of lives in our nation. According to the Centers for Disease Control and Prevention, 29.1 million US adults have diabetes and one out of every three people will develop diabetes in their lifetime.

Heightened awareness of diabetes is also important because there’s a lot of confusion out there about how to manage it effectively, especially when it comes to diet.

We do know that the foods we eat can make a big difference in blood sugar control, but figuring out what to eat is one of the biggest challenges when someone is diagnosed with diabetes. So let’s take a closer look at this relationship between diet and diabetes.

Simply stated, healthy eating for type 1 or type 2 diabetes doesn’t really differ from how we all should be eating anyways. And, your healthy foods and meals should be ones you enjoy so it’s easier to commit to eating them long term.

Years ago, the “diabetic diet” was very rigid and restrictive, but nowadays we know much more about

carbohydrates and digestion and we recognize that all foods can fit into a healthy plan. Diabetes-friendly meals should consist of the same healthy foods recommended for everyone—whole grains, fresh fruits and vegetables, lean meats, and low-fat dairy.

What are carbohydrates and what do diabetics need to know about them? Carbohydrate is the nutrient in food that affects your blood sugar levels the most. Carbs are broken down into simple sugar in your gut and absorbed into the blood.

If you do not have enough insulin, the blood sugar has nowhere to go and it builds up in the bloodstream.

This does not mean that all diabetics need to avoid carbohydrates. Carbs are our best source of fuel and the only fuel our brain likes. Diabetics do, however, need to pay attention to the source, portions, and timing of carbs.

Choose complex carbs as opposed to simple carbs—such as candy, cakes, and pies—which have sugar in simple form and can be digested easily. Better choices are steel-cut oats, whole-grain breads, brown rice, whole-wheat pasta, fresh fruit, vegetables, and low-fat milk. If, once

in a while, you’re going to work in a piece of cake, brownie, or cinnamon roll, then you’ll need to eat less carb at your meal than you usually would.

So, skip the mashed potatoes if you know you’re going to have a piece of birthday cake after dinner.

When counting carbs, planning is key. Look at your normal meals, count up the carbs, and make adjustments accordingly. Good sources for this information include nutrition facts labels, recipe information, restaurant websites, smartphone apps, and online food databases. Also, keep in mind that the carb budget differs by gender. Women should not exceed four carb choices, or 45 to 60 grams of carb per meal, while men should limit themselves to five carb choices, or 60 to 75 grams of carb per meal.

Even drinking 100 percent fruit juice can be problematic—with just four ounces of juice containing 11 to 15 grams of carb.

Consistency counts, too. Keep to a mealtime routine, never skipping meals, especially if you’re on medication. It’s best to eat three meals and, perhaps, one to two snacks per

day. Whether you can eat snacks or not depends on your meal spacing, the medications you’re taking, and your blood sugar control. Your blood sugar targets should be 80 to 100 mg/dl when fasting or before meals, and less than 180 mg/dl two hours after a meal.

Keep in mind that blood sugar rises after a meal, so spacing out eating times gives blood sugar a chance to come back down between meals.

Also, avoid eating one large meal per day as it is really hard for blood sugar to come back down afterward.

The good news is, people with diabetes—type 1 and type 2—can live long and healthy lives. It’s no longer the death sentence it was back in the 1930s and ‘40s. But whether you’re a new diabetic or have had the disease for some time, there’s always room to improve your eating habits as well as your blood sugar control. And you have a lot to gain by doing so—including avoiding long-term complications of diabetes such as blindness, kidney failure, heart disease, and peripheral vascular disease. Don’t worry if you don’t get it all right immediately and every day. It’s not something you’re going to master overnight. And if you find you need help, don’t be afraid to ask for it. To increase awareness, the UTMC Eleanor N. Dana Cancer Center will present “Lymphedema: From Head to Toe” on Monday, November 13, in the Radisson Hotel Mahogany Ballroom. The Radisson Hotel is located on UT’s Health Science Campus. The event will feature Guenter Klose, an internationally known expert on lymphedema therapy who lectures extensively at professional conferences, physician meetings, and lymphedema support groups in the United States, Europe, Asia, and South America.

Certified in MLD/CDT from the renowned Foeldi Clinic in Hinterzarten, Germany, Klose was instrumental in establishing the field of lymphedema therapy, as well as training in the U.S. He also is the founder of Klose Training & Consulting, LLC, a lymphedema therapy training program based in Lafayette, Colorado.

People with conditions that can lead to lymphedema—from cancer to venous leg ulcers to chronic wounds—will benefit from Klose’s presentation. Local therapists and lymphedema product manufacturers also will be on hand to share

information.

Bethany House receives $20,000 grant during Domestic Violence Awareness Month

T

hroughout Domestic Violence Awareness Month in October, The Mary Kay Foundation awarded $20,000 grants to 150 domestic violence shelters across the United States, the District of Columbia, Puerto Rico, and Guam for a total of $3 million.

Local shelter Bethany House was chosen to receive one of the annual grants to further the organization’s efforts to combat domestic violence and provide services in Northwest Ohio. This year’s highly sought-after grant slate brings the total investment in The Foundation’s annual shelter grant program to nearly $42 million.

Bethany House will use the grant funding to provide long-term transitional shelter and support services

for victims of domestic violence and their children. Other recipients use the unrestricted funds for basic operating expenses, hiring much-needed personnel, completing repairs and renovations, or adding programs and resources based on the unique needs of their shelter and the clients they serve.

“At Bethany House, we are incredibly grateful for support from The Mary Kay Foundation and its continued commitment to break the cycle of domestic violence,” said Deidra Lashley, Executive Director. “With their support, Bethany House can do more to raise awareness about domestic violence in our community, provide essential

resources for escape and recovery, and bring about lasting change for victims and their children.”

In Lucas County, it is estimated that a call for domestic violence is placed to 9-1-1 every 28 minutes.

Domestic violence is affecting a staggering number of families locally and nationally.

: I’ve noticed tinnitus, or ringing in my ear, and many of my friends have told me they’ve experienced the same. What should I know about it?

A

: Recently, a surplus of attention has been given to tinnitus and ringing in the ears because of the high percentage of adults affected by the condition who are looking for answers. They also want to learn how to handle tinnitus and hearing loss, which is sometimes associated.

Tinnitus affects over 50 million Americans according to the American Tinnitus Association. It’s described as ringing, buzzing, roaring, hissing, or whistling sounds, which may be in one ear or both in the presence of no external noises. The sound can vary in volume and intensity and is most noticeable when you are in a quiet environment. Many patients are aware of it at night when they are trying to fall asleep. For the majority of us, tinnitus may be barely annoying, but there are others who describe it as extremely disruptive to their life.

caused your tinnitus. It isn’t a disease but a symptom that can be caused by several things, such as exposure to loud noise, impacted cerumen, ototoxic drugs along with several medical conditions, and aging. A complete physical examination and tinnitus evaluation by an audiologist, including a hearing test, is recommended.

We have been seeing patients with tinnitus for several years. Tinnitus is treatable, and the approach to treatment may vary depending on the cause. For instance, if it’s due to a medical condition such as high blood pressure, the goal should be to reduce your blood pressure and, in turn, reduce the tinnitus. If the ringing is due to a related hearing loss or from damage caused by exposure to excessive noise, there are hearing aids that have the ability to provide relief through “Zen” programming.

And, of course, it’s also important to recognize that stress can increase the presence of tinnitus. Managing your stress through relaxation measures or exercising will reduce the effect of the tinnitus. No drug has

KNOWLEDGE IS POWER Mercy Health Cancer Centers offer genetic testing

T

hough most cancers are not clearly linked to the genes, it is known that certain forms of cancer are more likely to have a hereditary association than others. Among these are breast, ovarian, colon, and uterine cancers. Thanks to advances in genetics, however, it’s now possible to test individuals for gene mutations that put them at increased risk of developing these or other cancers.

To help assess patients’ cancer risk and promote more individualized screening and treatment protocols, Mercy Health offers genetic counseling and testing at the Mercy Health Cancer Centers.

Who qualifies for genetic testing to assess cancer risk, and how is this determined? According to licensed genetic counselor Michelle Hall of the Mercy Health Cancer Centers, “We look primarily at the types of cancer that occur in a family and the age at which they were diagnosed. Any time we see a cluster in each generation of a family, whether on the mother’s or father’s side, and it’s occurring under age 50, we know there could be a genetic link. For example, there may be several females in a family who developed breast cancer at a younger age. Another red flag is seeing cancers in a family that don’t occur commonly in the general population, such as ovarian cancer, pancreatic cancer, or male breast cancer. We find a hereditary association only about five to 10 percent of the time, but it’s always important to follow up on that red flag.”

For this reason, Hall stresses the importance of knowing your family’s health history. She encourages individuals to talk with their relatives to learn what conditions might run in their family, including cancer.

This information can be valuable to

determine whether genetic testing is indicated.

Prior to testing, an initial consultation is held for the purpose of educating the patient. Hall notes that it’s very helpful at this time for the patient to meet with a credentialed genetic counselor or other individual who specializes in genetics to learn what the process entails and to discuss the potential results, which are typically received within a few weeks of testing.

Discovering a genetic predisposition to a certain cancer (or cancers) does not mean the patient is predestined

to get the disease, Hall emphasizes.

“However, knowing which cancers he or she is predisposed to can really help guide our screening recommendations. Oftentimes, that means beginning screening at a younger age and screening more frequently.

For instance, if someone is found to have a genetic predisposition to breast cancer, we may want them to begin breast cancer screening as early as age 25 because their lifetime risk is much higher than the average—between 50 and 85 percent higher. It’s so important for us to modify medical management for these individuals so we can detect any developing cancer at a very early stage through increased screening,” she adds.

In addition to guiding screening recommendations, the results of genetic testing can even help guide cancer treatment. The genetic profile of a given cancer is vital information doctors can use in predicting which cancers might—or might not—respond to a particular form of chemotherapy or immunotherapy. Patients, too, can use the results of genetic testing to help determine which preventive measures might be appropriate to lower their risk.

Hall cites the example of patients determined to be at high risk of de-

veloping breast cancer who choose to undergo bilateral mastectomy. “Prophylactic mastectomy isn’t necessarily a requirement in this situation, but it’s important to discuss all options and make that information available.

When patients are considering this option, we will schedule them to meet with a breast specialist or breast surgeon to discuss the pros and cons, the types of surgery available, and the reconstructive options. It’s a very individualized decision, and we want to make sure it’s an informed one,” she says.

Though genetic testing for cancer offers a host of benefits, it’s not necessarily appropriate for everyone. As Hall explains, it’s very uncommon to find that a cancer is hereditary, so only a small subset of individuals actually needs testing. Also, those who qualify to be tested need to understand that there are limitations to the testing as well as potential emotional and psychological consequences to learning one is at increased risk for developing certain cancers. “Some people don’t want to hear that they’re genetically predisposed because this knowledge causes a lot of anxiety or they don’t want that burden placed on their family members,” she observes.

Furthermore, Ashanti’s services can be provided wherever clients call home—whether it’s in their home, at one of Sunset’s communities, or in another assisted living, independent living, or skilled nursing facility. “For our residents, this means they can remain in their home, in familiar surroundings with friends, family, and a supportive staff,” Young observes.

Though hospice care is reserved specifically for patients who are terminally ill and are no longer receiving curative treatment for their underlying disease, it can be beneficial long before life’s end is imminent. In fact, the sooner in the end-of-life process that hospice care is initiated, the sooner symptoms can be managed and stress reduced, allowing patients and their loved ones to make the most of their time together. “We encourage folks to engage in the conversation to learn

about their loved one’s end of life wishes. Starting that conversation can be difficult, but using a gentle approach, reassuring the individual that hospice focuses on quality of life and comfort, may help,” says Young.

A Walk in the Park

here is an ancient parable about six blind men who went to see an elephant. One bumped into the broad side of the elephant and declared it was like a wall. The second felt a tusk and said it was long and smooth like a spear. The third felt the trunk and found it very like a snake. The fourth grabbed a leg and decided an elephant is like a tree. For the fifth, the ear meant the elephant is like a fan. The sixth felt the tail and pronounced the elephant to be like a rope.

The moral is that your personal experience may be true but it is inherently limited. Few of us can see the whole elephant and, therefore, perhaps we should exercise some humility before passing judgment or leaping to conclusions.

“Yeah,” said Shirley, “can you think of anybody who might want to take that advice to heart?”

I suspect that she was dropping a rather broad hint, but I don’t want to go leaping to conclusions until all the facts are in.

The day before we left for Alaska, our friend Sharyn called to ask where we were going on our summer trip.

Her follow-up question was, “What do the Alaskans think about all the glaciers melting?” Now, what is the very first thing that pops into your head when you think about Alaska? Spectacular scenery? Wildlife?

Hunting or fishing? Eskimos? Aurora borealis? Sled dogs? Midnight sun?

Elegant cruises? Far be it from me to pontificate on what the Alaskans think about anything. Still, we have

talked with quite a few Alaskans on two summer trips that took us just about everywhere the pavement goes. Frankly, I don’t know what any of them think about the glaciers melting because not a single one of them ever mentioned the subject.

Like people in the Lower 48, though, they did have quite a few comments about the weather that fell into two categories: those who were tired of the constant drizzle and those who optimistically set about enjoying their active lifestyles with no concession whatsoever to the weather.

These people, though, were not representative of our experience of the huge elephant that is Alaska. I see the elephant as more like a kid on a sugar high. Alaskans don’t seem to cultivate the quiet, contemplative life. They love active outdoor pursuits—hiking, bicycling, kayaking, fishing, boating, and running.

Oh, yes, running for sure. We were miles from anywhere on the highway from Seward when a young woman came running toward us at the side of the road. She wore a competition number pinned to her shirt. Where could she being going? Better yet, where could she be coming from?

A few minutes later (that is, several miles farther north at highway speed) there was another young woman.

Snoring through the ages

artin Luther King Jr. (or was it Martin Lawrence?) once famously said, “Judge a man by the content of his character, not by the cacophonous sound he produces while he sleeps.”

Notwithstanding these words of wisdom, many among us continue to practice a form of discrimination so insidious that it crosses all racial and ethnic boundaries. Of course, I’m talking about snorism—the irrational fear and hatred of people who snore.

Snorists are silent sleepers who cannot abide noisier—some would say “more audibly creative”—sleepers and will attempt on a nightly basis to drive them out of their comfortable beds and onto lumpy sofas or recliners in separate rooms. These “sleep segregationists,” as someone probably calls them, achieve their objective of a dull, soundless slumber through various means, chief among

them being sharp elbow thrusts to the snorer’s ribs at 10- to 15-minute intervals throughout the night.

While there is no “typical snorist,” spurious studies suggest that a relatively high percentage of them are wives of middle-aged, slightly overweight, roguishly attractive, intelligent, high-achieving men. Exactly why wives turn into snorists is unclear, though self-proclaimed “ism” expert and eye-patch model Lars Windowrattler suggests, “It might be ‘cuz they’re jealous or they think the snoring will summon evil spirits.”

Whatever the cause, snorism appears to be firmly entrenched in our culture. But it wasn’t always thus.

Mounting evidence indicates that snorers were actually held in high esteem throughout much of human history.

For example, ancient drawings

discovered on cave walls in the Neander Valley may or may not indicate that our troglodyte ancestors viewed snorers as holy men who channeled the spirits of powerful animals and brought good fortune, victory in war, and successful hunting to the clan.

One can almost hear the nighttime conversation between wife and Holy Snorer husband now: “Wake from sleep, husband!”

“What wrong, wife? Saber-tooth in cave?”

“No! Grizzly bear spirit in nose again! Husband protect wife with bear spirit! Wife proud and grateful!”

“Husband happy!”

During the Salem Witch Trials in colonial Massachusetts, snorers were considered divine for their uncanny ability to discern innocent women from the Devil’s consorts via the notorious “Ordeal of Nighttime Thunder.” Women suspected of witchcraft were forced to sleep alongside a known snorer for an entire week without complaint. Those who couldn’t withstand the ordeal or awakened the snorer to suggest an alternate sleeping position were deemed witches and promptly executed. The rest were exonerated of all accusations—and later executed just to be on the safe side.

Perhaps snorers’ proudest historical moment was in June of 1944, just prior to the D-Day invasion. In an effort to deceive the Germans into believing the main Allied landing would be at Pas-de-Calais instead of Normandy, a certain Private First Class Max Dullroar, renowned for his snoring prowess, was airdropped into

the region, promptly anesthetized, and ordered to “snore with extreme prejudice.” His nocturnal rumblings were so profound that the Germans believed a column of Allied tanks had already arrived at Pas-de-Calais and shifted their attention and defenses there, opening the door to a decisive Allied Victory in Normandy. This operation, code named “Snore-titude South,” helped turn the tide of World War II. Despite snorers’ storied history and their tireless efforts in support of peace and liberty, snorism has once again arisen in our midst. Shamelessly snorist wives across the Fruited Plain have taken to tormenting their weary, sleep-deprived husbands throughout the night—poking their ribs, kicking their shins, and ordering them in shrill voices to “Roll over on your side!”

Winter running tips

A

s the winter months approach, running outdoors in cold conditions is not as appealing as the warmer months to many of us. Using these helpful winter running tips will make your workout safer and, hopefully, maintain your desire and commitment.

Cold and icy conditions are the ideal scenario for slipping and injuring a muscle or even breaking a bone.

It’s advisable to warm up indoors before you hit the pavement and to be especially vigilant if the conditions are icy or wet. When running in cold weather, warm up slowly and gently. Your muscles are cold and need a chance to warm up to prevent injury. Most wise runners use the winter season for maintenance runs. You are increasing your risk of injury if you try to do speed work in the severe cold.

you must run in an icy area, think about putting spikes over your running shoes, but make sure you have proper instruction before hitting the pavement.

Choose the proper clothing to keep you warm and comfortable while still maintaining your performance.

Synthetic and breathable fabrics will wick perspiration away from the body to the outer surface of the garment where it can evaporate more easily.

You want to dress in layers with the layer closest to your body being a light-weight wicking fiber. Also, wear a warm hat when running in cold conditions. Approximately 40 percent of body heat is lost from the head, which affects the whole body’s circulation.

Cold weather running can be dehydrating. Make sure you do not reduce the amount of fluids you drink just because you are not sweating as much as 4:03 in the warmer weather. Cold 3/10/17 PM weather can mean more urination,

which, by itself, is a source of fluid loss.

It is imperative that you remember to maintain adequate hydration before and after running in cold weather to ensure your muscles recover. Remember to use moisturizers and sun block to prevent blister formation and redness on your skin and face.

Running in the winter months can be enjoyable if you dress appropriately and use your best judgment on the running conditions. Remember, the winter time is perfect for maintenance runs. eing able to communicate with our loved ones is truly one of life’s greatest joys. When the family comes together during the holiday season, being able to hear and understand clearly becomes even more treasured. Everyone wants to enjoy the chatter around the Thanksgiving dinner table and be delighted listening to the little children as they open their presents to see what Santa Claus left them on Christmas morning.

Don’t we all want to enjoy experiencing these events?

But what if you have hearing loss? Hearing loss can make the holiday season very frustrating, for you and your family both. The holiday season is a time of joy and laughter that surrounds us for a few months every year. Whether you will be visiting your in-laws or spending quality time with your own family, chances are there will be lots of noise throughout the days. This sounds like a perfect recipe for family fun, but it can actually be exhausting for some people.

For people with hearing loss, the noise of laughter and multiple conversations happening at the same time can actually be isolating. What should be a happy time can become one of great sadness instead. If you can’t hear well enough to fully participate in the conversations and festivities, you may decide that it’s just easier to sit on the outskirts of the conversation and become a spectator instead of a participant, an outsider instead of a contributor. At times like these, hearing loss can be devastating.

It doesn’t have to be that way!

Think about having your hearing assessed to determine what kind of hearing loss you have. If the hearing loss can be helped with hearing aids, now is the time to take care of it so you can enjoy this year’s busy holiday season. The best gift to yourself and your family is to take care of your hearing difficulties without delay.

Here are five ways to hear more and enjoy this holiday season:

1. Wear your hearing aids This one may seem obvious, but with the hustle-and-bustle of the holidays, it’s easy to walk out the door and forget. Even if you’re not fond of them, wearing your hearing aids really does help. Experiment in similar situations prior to the gathering, and test out different settings. If you don’t want to experiment in public, try it at home.

2. Don’t fake it If you’re in a conversation with a close friend or family member and are having difficulty hearing, let them know. It’s better to have five memorable conversations than ten that you can’t recollect. Just remind yourself that your friends and family are there for you and want the holidays to be just as, if not more, enjoyable for you.

3. Take a break Introverts aren’t the only ones who benefit from a hiatus during the holidays. Stepping away for a few minutes from the sounds of holiday family gatherings will give your brain and ears a break. Repair to the restroom, step outside on the patio,

or relocate to a quieter room with less noise. This suggestion can actually help re-energize and refuel you for another round of holiday greetings.

4. Bring your sense of humor Along with the gifts, this is something you won’t want to forget when you leave the house. While you may not hear every laugh or kitchen ding, try to laugh a little and be grateful for the loving friends and family around you. And who knows, you may even get Oscar the Grump to crack a smile!

5. Set reasonable expectations Whether this is your first holiday season with hearing loss or your tenth, it’s always important to keep the situation in perspective. Unmet

expectations can lead to frustration and disappointment, neither of which you want to allow to damper this cheerful time of year.

The Toledo Clinic-St. Luke’s Medicare partnership yields higher quality results

N

orthwest Ohio Accountable Care Organization (ACO), a partnership between The Toledo Clinic, St. Luke’s WellCare Physicians Group, and independent community physicians, has received some tremendous news from the Center of Medicare and Medicaid Services (CMS) regarding their clinical quality. The results of their 2016 quality and medical savings initiatives with Medicare were some of the best-rated in the Midwest.

An “ACO” is a formal method designed by Health & Human Services (HHS) for doctors and hospitals to work together and be incentivized

to raise quality and lower the cost of care. 2016 represented the first year that The Toledo Clinic, St. Luke’s Hospital, and an Independent Physician Association (IPA) of Toledo-area physicians collaborated together in the program.

As a result, this collective effort in 2016 saved Medicare millions of dollars in medical costs and, more critically, achieved an overall quality score of 97.36%. “This score represents one of the higher quality rankings in the Midwest,” stated Ian Elliot, MD, Board Chair of NW Ohio ACO. “We are very proud of the results because a lot of hard work went into the effort.”

2 pounds red potatoes, washed 2 tablespoons canola oil 4 teaspoons kosher salt, divided 8 ounces (1/2 pound) smoked bacon, diced and cooked crisp, divided 1 cup sour cream 4 green onions, thinly sliced 2 cups (8 ounces) Wisconsin aged cheddar cheese, shredded 1 teaspoon black pepper, freshly ground

Preheat oven to 350°F. Toss potatoes, canola oil and 2 teaspoons kosher salt in large bowl. Place potatoes on rimmed baking sheet, and bake until potatoes are tender, about 20-30 minutes, depending on size. Meanwhile, fry bacon until crisp.

Drain on paper towels. Set aside.

When potatoes are baked and cool enough to handle, quarter them or cut in large chunks and place in large mixing bowl. Add bacon (reserving 1/3 cup), sour cream, green onions, and Wisconsin aged cheddar. Season with remaining kosher salt and black pepper. Fold all ingredients carefully, being careful to leave the potatoes chunky. Scatter reserved bacon over.

Best served warm. Serves 8.

What all Alaskans have in common is that they are proud to be Alaskans.

In line at Costco in Anchorage, we got to talking to a woman who proclaimed that she was a “native Alaskan.” That is, she was born there. Not one of those Johnny-come-latelies from the Lower 48. The cashier, who was what is more conventionally perceived as a Native Alaskan, just rolled his eyes.

We also talked to quite a few of the Johnnie-come-latelies. They tended to be either young people looking for an active lifestyle or young families looking for a safer lifestyle.

Safer? Alaskan parents with young children know there is an outside chance their kids will get eaten by the bears or wolves or trampled by a moose during the rut. But these risks are offset by a lower probability that they will be trampled by the sex, drugs, and rock and roll culture of the Lower 48. Alaska is not quite stuck in a Leave it to Beaver 1950s time warp, but it is about as close as you can get. Plus, so far at least, you don’t see many Alaskan kids walking around bumping into things because they are totally absorbed in texting.

Are you set for snowblower season?

A

las, cold weather and the possibility of snow will soon be upon us again! We can only hope this winter proves to be an underachiever where snowfall is concerned, but just in case it doesn’t, now is the time to get prepared. For many area residents, that might mean digging out and tuning up the old snowblower—or finally investing in one to avoid getting caught off guard by another snowy onslaught.

Which brings us to an important issue: Did your snowblower start right last year? As we approach the season, here are some things you might want to do to be prepared.

Finding out that your snowblower won’t start at 6:30 a.m. on a Monday is no way to begin your work week!

• Check the sparkplug and clean or replace it.

• Check the air filter and clean or replace it.

• Check for signs of wear or

damage.

• If you didn’t change the oil last year, do it now. Oil should be changed after every 25 hours of use.

• Lubricate moving parts.

• Fill the tank with fresh gasoline.

If your blower has a two-cycle engine, carefully follow the instructions for blending oil with the fuel.

• Have your snowblower professionally tuned if you are not confident in your smallengine mechanic’s skills.

Toledo Zoo now home to orphaned cougar cubs

hree cougar cubs, also known as pumas or mountain lions, came to reside at the Toledo Zoo after the state of Washington’s Department of Fish and Wildlife reached out to the Association of Zoos and Aquariums to find the cats new homes following the loss of their mothers. The oldest cub is estimated to be 10-12 weeks old, weighs about 10 pounds, and is from southeast Washington. The younger two cubs are approximately three weeks old, weigh about three and a half pounds each, and are from northeast Washington. The younger cubs were part of a litter of four, with the other two cubs now residing at a zoo in New Jersey. Cougars (Puma concolor) are found throughout North and South America, and while they are not endangered, some populations are severely threatened.

“The Toledo Zoo is proud to provide a home to these orphaned cougar cubs. As a part of our own mission and that of AZA, we always try to meet the needs of wildlife officials in

placing orphaned native species. These cubs will not only provide a tangible connection to human-wildlife conflict, but also give Zoo guests the privilege of watching an elusive and often-villainized feline species grow and develop,” said Jeff Sailer, the Toledo Zoo’s executive director.

The younger cubs can be viewed by the public during veterinarian supervised bottle feedings at 11:30 a.m. and 2:30 p.m. each day, near the indoor viewing of elephants in Tembo Trail. Times and availability may vary. Please visit toledozoo.org for additional times and/or schedule changes.

“Viewing the bottle feedings is a particularly unique opportunity most outside of the Zoo field would never experience. We hope visitors will enjoy watching these cubs grow and explore while also gaining an appreciation for the importance of avoiding human wildlife conflict for the conservation of many incredible species, including cougars,” said Shayla Bell Moriarty, the Toledo Zoo’s director of communication. ❦

can provide not only convenience, but also relatively inexpensive insurance against the risks associated with shoveling heavy snow, such as bad backs, sore shoulders, and that unfortunate event cardiologists refer to as myocardial infarction. So what kind of snowblower is best for you?

Some people believe you can’t go wrong by investing in the best and biggest that money can buy. When dealing with snowblowers, however, sometimes the “best” is not what’s best for you. Here’s why: Large snowblowers come equipped with powerful engines—in the approximate range of 8HP to 12HP or even higher. They are generally twostage machines designed to handle up to 12 inches of heavy snow, and they are self-propelled. The larger snowblowers can handle virtually any snowfall we are likely to get.

Sounds like you can’t go wrong by choosing a machine as large as your SUV, right?

Well, maybe yes, maybe no. If you have a long, double-width driveway, you may very well need a large, powerful snowblower. But you also have to consider other factors, such as whether you’re physically capable of handling it. When you are finished clearing the driveway and sidewalk, how much space will the snowblower occupy in your garage or shed? If you have to transport it, how will you manage that? These are not arguments against buying larger snowblowers, but important factors to consider before making a purchase.

If you have only a small area to clear, perhaps a lightweight electric snowblower is better suited to your needs. These machines are useful for clearing relatively light snow from porches, decks, or patios where it would be difficult—or ill-advised—to bring a larger machine. And, because they are electric, you don’t have to worry about them refusing to start in really cold weather. Of course, unless you’re using a cordless model, electric snowblowers must be used within reasonable proximity of an outlet and you have to be careful to avoid running over the cord.

The lightweight electric is at the opposite end of the snowblower spectrum from the heavy-duty machines.

But there’s plenty in the middle to choose from!

Make mine a double!

Most snowblowers on the market are either single-stage or dual-stage,

though there are three-stage blowers out there as well. The designation refers to how they move the snow.

Single-stage machines have an auger that sweeps up the snow and throws it in a single action. They are generally light, maneuverable, easy to use, and appropriate for light- to medium-duty snowblowing on short driveways and sidewalks. They may, however, get bogged down in deep, heavy, wet snow.

Dual-stage snowblowers use an auger to move the snow to an impeller, which, in turn, pushes it out through a shoot. The auger on this type of blower turns more slowly to better control the amount of snow that is fed to the impeller. Dual-stage machines are designed to handle deeper, heavier snow on larger driveways and longer sidewalks, and some can throw that snow a long way (even onto your neighbor’s driveway, so be careful where you point that chute!).

Three-stage units have an auger and impeller just like a dual-stage, but in addition, they have a second auger positioned above the primary one.

Other options to consider

Electric starters are really handy! If you remember repeatedly pulling the cord on your lawnmower on a cool morning early last spring, you can appreciate that small engines don’t always kick over right away. With the electric-start feature, you just need to connect a power cord from the machine to an electrical outlet and then push a button or turn a key to start the unit. When the engine starts, disconnect the cord from the machine and you’re on your way.

Remote control of the chute is another helpful feature. It allows you to change the direction in which the snow is discharged without stopping.

A rectangular metal discharge chute is worth looking for because it can virtually eliminate clogging when you are dealing with wet, heavy snow.

High-intensity headlights can be useful when you are snowblowing early in the morning or in the evening. In Toledo, it is often still dark when we leave for work in the morning and dark again when we get home. If your property is well-illuminated, this might not be much of an issue, but if you have a long driveway to clear, you might appreciate the improved visibility.

Plus, when you’re clearing the apron of your driveway, those lights help alert passing motorists and snowplow drivers to your presence. ❦
