Suffering from sleep apnea?

You don’t have to!

D

espite our perceptions, restful sleep is not a luxury to be enjoyed when our busy schedules allow. Rather, it’s a vital aspect of our physical, emotional, and cognitive well-being. The fact is that if we don’t get restful, recuperative sleep on a regular basis, the adverse health consequences can be significant.

Unfortunately, that’s the reality facing people with sleep apnea—a disorder that causes them to stop breathing for short periods of time during sleep.

According to Naeem Lughmani, MD, sleep medicine specialist at The Toledo Clinic, patients with obstructive sleep apnea (OSA), the most common form of the disorder, typically have a smaller airway to begin with. When they sleep, the muscles in the back of their throat relax, further reducing the size of the airway. As a result, they snore. What’s worse, they can’t get enough air with each breath, so their oxygen level drops and their body reacts as if it’s drowning, producing high levels of adrenaline. With this adrenaline surge comes an increase in blood pressure, heart rate, and muscle tone—essentially a “fight or flight” response—and then the person awakens.

This pattern can repeat over and over again throughout the night, depriving the individual of restful sleep, yet he or she is often unaware that it’s even going on. Very often, a family member or bed partner notices the problem before the sufferer does.

Symptoms commonly associated with sleep apnea include waking in the morning feeling tired, excessive daytime drowsiness, irritability or moodiness, difficulty concentrating, attention issues that increase the risk of accidents, and decrease in memory and cognitive function.

“Apnea deprives the individual of REM sleep, which is associated with psychological well-being, as well as delta sleep, which is when the body produces growth hormone and heals itself,” Dr. Lughmani explains. So, both your psychological and physical health can suffer if you

don’t achieve these sleep states or enter them only partially.

Another significant concern is that the lack of oxygen and increase in adrenaline can lead to high blood pressure, heart arrhythmias, a greater risk of stroke and other cardiovascular issues, and elevated blood sugar that’s difficult to manage. Research has also shown that apnea can have an adverse effect on the immune system.”

Furthermore, sleep apnea can be the underlying cause of health issues that may seem unrelated.

For example, Dr. Lughmani notes that waking frequently to urinate at night can actually be a sign of sleep apnea. He explains, “When the individual’s airway collapses and he or she struggles to get air in, a negative pressure is created in the chest. That negative pressure creates a vacuum that draws more blood to the heart, causing it to swell up. In response, the heart signals the kidneys that there’s too much water in the body, so they in turn begin to produce excess urine.”

Examples of other conditions that, according to Dr. Lughmani, might warrant an evaluation for sleep apnea include insomnia, depression or other mood disorders, focus problems such as ADHD, hypertension that’s difficult to treat (requiring more than two medications), heart arrhythmias, congestive heart failure, and prior stroke. “If you already have any of these problems, you should consider the possibility of sleep apnea because proper treatment of apnea can help in managing all these conditions,” he explains.

The good news is, sleep apnea can be treated effectively so that sufferers can once again enjoy healthy, restorative sleep. In fact, a retrospective study revealed that people who have sleep apnea that is being treated do just as well as people who don’t have the disorder.

involves the use of either continuous positive airway pressure (CPAP) or bilevel positive airway pressure (BiPAP) to keep the patient’s airway open during sleep. As the name implies, the former delivers air at a continuous pressure through tubing connected to a mask, while the latter delivers two levels of air pressure—a higher level during inhalation and a lower level during exhalation. Dr. Lughmani states that there are many different forms of CPAP/BiPAP technology and different ways of delivering air pressure, so if the patient is having difficulty with one form, there are always other options to try.

“Patient compliance and follow-up are crucial to success with CPAP or BiPAP, and oftentimes there are simple solutions to problems in using the technology, such as adjusting the air pressure, replacing an old mask, or tightening loose straps. And if one type of equipment makes the patient feel claustrophobic, which is a relatively common problem, we can try another type or we can use various techniques to desensitize the patient to the

claustrophobia,” he adds.

For patients who can’t tolerate CPAP or BiPAP, Dr. Lughmani notes that there are surgical options as well as dental devices for treating sleep apnea. However, they are not as effective. Also, because obesity is a major risk factor for sleep apnea, weight loss often yields significant improvement in symptoms.

Dr. Lughmani further emphasizes that there are different forms of sleep apnea, including obstructive sleep apnea (OSA); central sleep apnea, which occurs when the brain fails to send the proper signals to the muscles that control breathing; and complex sleep apnea, which combines elements of OSA and central sleep apnea. “Whatever the cause, if you are experiencing any of the symptoms of sleep apnea, it’s important to see a sleep specialist for a proper diagnosis and appropriate treatment plan. Getting started with treatment will not only help you sleep and feel better, but it might also prevent serious health complications from developing in the future,” he points out. ❦

New gorilla at the Toledo Zoo

T

he Toledo Zoo is excited to announce the addition of a new western lowland gorilla to our troop!

Sufi Bettine, a 15-year-old female, now calls Toledo Zoo home.

Sufi was born at the Bronx Zoo on December 27, 2001. She came to Toledo from the Houston Zoo this past spring on a Species Survival Plan breeding recommendation. After the standard quarantine period required for all new-to-the-Zoo animals, Sufi was slowly introduced to her new troop. Like any new addition to an established group, the process had its ups and downs, but the troop has accepted Sufi and all are now residing together on and off public exhibit. Other members of the Zoo’s gorilla troop are: 29-year-old silverback male Kwisha, 29-year-old female Johari, 23-year-old female Kitani, and 14-year-old female Nia Lewa.

Visitors can distinguish Sufi from the other females, as she is the smallest

gorilla on exhibit.

Western lowland gorillas (Gorilla gorilla gorilla) are four to six feet tall as adults and can weigh anywhere from 150 to 400 pounds. They live in the heavy rainforests of west central Africa and eat a vegetarian diet of roots, fruits, and plants. Western lowland gorillas are known to live in the smallest troops of gorillas, consisting of four to eight animals, with one silverback male, a few females, young offspring, and occasionally a few young males maintaining a three-quarterto 16-square-mile home range. Western lowland gorillas are listed as critically endangered and declining on the IUCN Red List for Threatened Species due to three main interconnected threats: habitat loss, bushmeat hunting, and human encroachment.

ollowing an injury, prolonged illness, or surgery, a period of physical rehabilitation that incorporates some combination of physical, occupational, and speech therapy is often necessary to help patients regain function and successfully resume normal activities of daily living. According to Marci Cancic, PT, MBA, Director of Therapy Services at UTMC, therapy is a conservative “first line of defense” that is not only effective in managing a wide range of conditions, but can also reduce the need for more invasive procedures such as pain injections or surgery.

However, with so many area facilities offering rehabilitation programs these days, it can be difficult for healthcare consumers to know where to turn for their own individualized treatment plan.

To help find the best fit in a therapy services provider among all the available options, ask these key questions:

Is the program outcome-focused?

Cancic explains that therapy today is evolving from a volume-based model to a value-based model. That simply means reimbursement is not based on the number of patients treated, but on the value patients receive from treatment—in other words, the quality of their outcomes. Upto-date therapy providers will be on board with this trend. “The best aspect of the value-based approach is that it allows therapists to spend more time with patients. In fact, we spend more one-on-one time with patients than most other medical professionals. Because we establish such a strong relationship with our

Ohio Living Swan Creek celebrates 25 years of pioneer spirit with open house

T

oday’s residents of Ohio Living Swan Creek, located at 5916 Cresthaven Lane in Toledo, enjoy access to resort-like surroundings; multiple levels of care on the same campus; a wide variety of social, cultural, and educational activities; and a host of other services and amenities. But in January of 1992, when the very first residents moved into what was then a brand-new retirement community, they had no idea what they were getting themselves into. In fact, this handful of early residents who had ventured into the unknown began to call themselves “pioneers.”

As it turns out, these pioneers had staked their claim in some very exciting territory. The fledgling community’s growth was exponential and proceeded in three phases: a luxury apartment building, spacious ranch-style villas, and a healthcare center, the last of which opened in December of 1994.

Over the next 20 years, expansion continued apace. As Ohio Living Swan Creek Executive Director Ann Roller explains, “The community added multiple dining venues featuring chef-prepared meals cooked to order, the KridlerMeloy Wellness Center with state-ofthe-art fitness equipment, as well as a campus center and a chapel with a full-time chaplain and regular religious services. Amenities also expanded to include a hair salon, walking

trail, dog run, library, technology center, and woodshop. In addition, our care continuum grew to include services such as assisted living, memory care, rehabilitation, and outpatient therapy.”

In celebration of their 25year anniversary—and to honor the pioneer spirit of those original residents— Ohio Living Swan Creek held a pioneer-themed open house on Saturday, September 16, from 11:00 a.m. to 2:00 p.m. Current residents and staff and their families, prospective residents, past board members, and key vendors who have done business with Ohio Living Swan Creek over the years were among those invited.

“This special day featured a wide variety of attractions, including demonstrations of different pioneer skills by volunteers from Sauder Village; horse-and-carriage rides with Clydesdales pulling the carriage; musical performances by Ragtime Rick, Tapestry, and a fiddler and banjo player; as well as plenty of food and drink, including hamburger sliders,

Johnny cakes, fruit pies, and root beer floats,” says Sarah Schnurr, Regional Director of Sales & Marketing for Ohio Living Swan Creek.

Of course, the open house included plenty of fun activities for kids to enjoy, as well. Examples included petting the Barnyard Bunnies, building a barn out of Lincoln Logs, a rope-ahorse event, and even a makeshift candy store.

Most importantly, the open house gave many area seniors and their families the opportunity to visit Ohio Living Swan Creek, tour the facilities,

ollowing menopause, approximately 60% of women experience a condition known as genitourinary syndrome of menopause (GSM). GSM is made up of multiple symptoms including vaginal itching, irritation, and dryness. Additional symptoms might include urinary urgency, dysuria, and increased nighttime voiding.

The most significant symptom, however, is painful intercourse due to vaginal atrophy. As a result, intimacy is affected and interpersonal relationships can become strained.

Unfortunately, many women in this population suffer in silence, believing it to be an inevitable part of the aging process.

According to urogynecologist Dr. Andrew Croak, Director of the Northwest Ohio Center for Urogynecology & Women’s Health, GSM develops due to the reduction in a woman’s estrogen level following a menopausal state. “The whole pelvic floor is very estrogen dependent,” he explains. “As a woman’s estrogen level decreases and the receptors are no longer fed, the blood supply to those tissues decreases and they go from being lush, thick, and well-moistened to being thinner, dryer, more delicate, and much more sensitive. In some cases, the tissue is so thin and tender, that a simple swipe from a q-tip can actually cause bleeding.

Dr. Croak notes that breast cancer survivors are among the women who are highly vulnerable to GSM. “We have had very poignant stories of women who, in treating their breast cancers with anti-estrogens or oophorectomy, had their GSM symptoms become extremely severe.

o one likes the idea of getting older, but like death and taxes, aging is a certainty for us all.

What’s more, our overall population is getting older. By the year 2030, older adults will account for around 20 percent of the population—double what it is now. Two factors are contributing to this phenomenon: longer life spans (more and more Americans are living longer) and the aging of the baby boomers.

While we can’t avoid getting older, how we age is another matter altogether. As most people are aware, many factors can impact our quality of life as we age. Examples include chronic diseases such as high blood pressure, heart disease, dementia, and diabetes; inactivity; social isolation; depression; economic hardship; taking multiple medications; and poor diet. This being “Eating Well,” let’s focus on that last point—poor diet—and discuss five key nutritional strategies for keeping your mind sharp and fighting frailty as you age:

1. Get sufficient protein

As she ordered banana-nut bread and stuffing for her dinner at Bob Evans, my motherin-law would say, “I ate chicken yesterday!” Gosh, I miss her!

As you age, it’s normal to lose muscle mass and mobility. But you can fight back with a balanced diet that distributes your protein intake evenly throughout the day—every day. Aim for 20 to 30 grams of protein per meal.

For breakfast, work in eggs, Greek yogurt, low-fat cottage cheese, and milk-based smoothies. For lunch and dinner, think peanut butter, cottage cheese, tuna, chicken, hamburger, low-sodium lunch meats, milk, and yogurt. If you exercise regularly, this dietary tactic will help with your body’s muscle-building process.

2. Skip the soda pop

I recently had an elderly patient who

drank one to two liters of Pepsi per day but ate very little food because “I’m just not hungry!” Keep in mind that sugary and caffeinated drinks tend to deliver a lot of extra calories and cinch the appetite. Some studies even suggest that sugary drinks increase inflammation in the body and contribute to faster aging.

Instead of drinking sugary soda, learn to love water—64 ounces a day over and above other drinks. If you’re not thrilled with the taste of the water from your tap, try reverse-osmosis or filtered water.

3. Consume Omega-3s

These fish oils support heart health, brain function, and memory during aging. Also, eating fish more than twice a week has been linked to improved muscle strength. Now, I’m not talking about the fried perch served up at your local VFW hall—no matter how delicious it might be. Choose sardines, tuna, salmon, herring, and halibut. And be sure to consult your doctor before taking fish oil pills.

4. Build a healthy gut

As you age, so does your gut. And as your gut ages, beneficial bacteria and other microbes that reside there tend to decrease their activity and get supplanted by more harmful bacteria that can weaken your immune function and increase your risk of obesity, type 2 diabetes, and constipation.

to nourish good bacteria. Try fresh produce from your local farmers market or a community supported agriculture (CSA) farm.

5. Build strong bones

Mobility depends upon strong bones, and in order to stay strong, your bones need more than just calcium.

Other nutrients they demand include Vitamin D, magnesium, and DHA. Daily exercise is highly encouraged as well. Choose a well-rounded bonehealth supplement and try to get your calcium from sources like lowfat dairy products, canned seafood, edible bones, and kale.

Many adults in this country are robust and active throughout most

of their senior years. However, there are those individuals who are neither energetic nor fit, suffer from multiple medical ailments, and ultimately become frail. The fight against frailty begins with good nutrition and daily exercise habits. Pay careful attention to the foods you eat and the exercise you choose. Also, try to maintain close social contacts and spend time with others. Being with people, eating right, and feeling strong all have a positive effect on your well-being and morale. Of course, checking on your nutritional health on a regular basis makes good sense as well. ❦

Do you know the dangers of distracted driving?

S

afe driving is a skill that demands our complete concentration. Allowing one’s attention to drift from the road, even for a brief moment, can have tragic consequences. Unfortunately, all too many of us continue to tempt fate by driving while distracted.

What constitutes “distracted driving”? The NHTSA defines it as “any non-driving activity a person engages in that has the potential to distract him or her from the primary task of driving and increase the risk of crashing.” Essentially, doing anything

that takes your eyes off the road (visual distraction), your hands off the steering wheel (manual distraction), or your mind off what’s going on around you (cognitive distraction) is considered distracted driving.

Chatting on a cell phone and texting while driving are certainly major forms of driver distraction that have justifiably garnered their share of attention lately, but they aren’t the only activities competing for our attention on the road. Other examples of common driver distractions include activities like changing the

station or music selection on your car stereo, taking a bite of fast-food hamburger or a sip of coffee or soda, checking your makeup (or, worse, applying makeup) in the car mirror, reading a map, conversing with passengers, improperly secured pets moving around inside the vehicle, or, depending on the location of the viewing screen, even watching a video.

Texting while driving is particularly worrisome because it simultaneously involves visual, manual, and cognitive distraction.

Owing to their lack of experience, which can lead to poor judgment at crucial moments, the drivers most at risk of being involved in a fatal distraction-related vehicle crash are those under 20 years of age. Unfortunately, drivers in this age group are also the most likely to communicate through texting. Of course, teenage drivers aren’t the only ones who are being distracted by technology. More and more drivers of all ages are using cell phones and other handheld devices while behind the wheel.

What about hands-free cell phones?

Aren’t they considerably safer than handheld phones? While that would seem to make sense, hands-free devices do little if anything to reduce

distraction-related accidents. While they may address the issue of manual distraction, they do nothing to eliminate the cognitive distraction associated with cell phone use. In fact, based on the findings of a University of Utah study, using a cell phone while driving, whether it’s handheld or hands-free, delays a driver’s reactions as much as having a blood alcohol concentration at the legal limit of .08 percent.

Another myth is that some people deal better than others with driving distractions because they are simply better at multitasking. The truth is, good “multitaskers” have, through practice, become skilled at switching from one task to another rapidly. However, they are no better than the rest of us when it comes to performing more than one task simultaneously. Hence, whatever they might think, they are no less likely to

be involved in a distraction-related motor vehicle accident.

While many states have enacted, or are soon to enact, legislation that prohibits certain driver distractions, such as talking or texting on cell phones, laws alone aren’t enough.

Distracted driving is a problem for everyone on the road, and we all have a stake in solving it. Each of us must do our part to set aside distractions when we drive. After all, no phone call, text message, song, or bite of food is so important that it’s worth putting yourself or other drivers at risk of serious injury or death. It’s especially important for parents of teenage drivers to discuss the dangers of texting, cell phone use, and other forms of distracted driving with their teens as well as to provide a positive example by avoiding distractions whenever they climb into the driver’s seat. ❦

Planning a safe and healthy Halloween

or youngsters all across our nation, the month of October means but one thing—Halloween is on its way. On this one special night of the year, kids get to fan out through their neighborhoods, dressed in outlandish costumes, with the goal of collecting a sack full of sugar-laden goodies.

However, in their haste to accumulate as many treats as possible before the “witching hour” arrives (8:00—when the trick-or-treating ends in our community), kids are also quick to forget some basic rules of safety. That’s where parents can make all the difference with a little planning.

Safety starts with the costume Deciding what to be and putting the costume together is almost as much fun for kids as the actual trickor-treating. This is also the point at which parents should get involved to ensure their little ghouls get off on the right foot where safety is concerned.

Costumes should be bright, reflective, and short enough to prevent tripping, entanglement, or contact with flame (e.g., the candles in those porch Jack-O-Lanterns). It’s a good idea to incorporate a flashlight and bands or strips of reflective tape into the costume’s design so your child will be clearly visible to motorists.

Law-enforcement authorities also suggest that emergency-identification information, including your child’s name, address, and phone number, should be discreetly concealed within each child’s costume or worn on a bracelet.

Masks are often considered an integral part of the Halloween ensemble. However, since they can limit a child’s field of vision, wearing face makeup and/or a decorative hat is a better alternative. All elements of the costume, including wigs and handheld accessories, should be

clearly labeled as flame resistant.

Simulated weapons—knives, guns, etc.—are best left at home. Too often, tragedy results when a toy weapon is mistaken for the real thing.

On Halloween night When Halloween night finally arrives, make sure your kids eat a nutritious meal before hitting the streets. This simple step will discourage them from gobbling up half their Halloween booty before they even reach home.

Home fire safety must also be considered. Make sure electrical circuits aren’t overloaded with Halloween lighting or decorations, and position Jack-O-Lanterns at a safe distance from drapes, decorations, flammable materials, or any areas where costumed children will be walking. To eliminate the risk of fire from Jack-O-Lanterns altogether, light them with chemical glow sticks instead of candles.

If your children are old enough to trick-or-treat without parental supervision, make sure you know the neighborhoods they will be visiting and the exact route they will take to get there. Agree upon a time for them to return, and equip them with a cell phone so they can call or text you in the event of any unforeseen delays or call 911 in the event of an emergency.

ctober is National Breast Cancer Awareness Month, so this month’s article is in support of all the efforts being made throughout the nation related to cancer.

Cancer has affected almost every adult in the US. This statement is true in the respect that each of us knows someone who has or had cancer. Many of us have a loved one, a friend, or a casual acquaintance that has been diagnosed with some form of cancer.

Individuals going through treatment experience many challenges, one being fatigue and weakness.

The side effects of chemotherapy, radiation, and medication often include reduced energy levels, fatigue, neuropathy, and reduction in self-esteem. An oncology nurse with one of the local clinics stated, “Fatigue is a significant factor we are seeing in 90% of our patients.”

We all know medical management is critical and the main priority,

while side effects, such as fatigue, are lower on the list of key areas to address. The question is, who can help address cancer patients’ low energy level, fatigue, and diminished self-esteem? Physical therapy can have a positive impact on individuals as they go through this journey.

Studies have shown that the appropriate amount and type of exercise can be beneficial for those experiencing cancer and cancer-related fatigue in all stages of the recovery process.

This can lead to improved strength, endurance, and functioning; increased independence; and decreased anxiety, depression, and pain.

The benefits and goal of physical therapy are to improve function,

strength, endurance, self-esteem, and physical independence. Heartland Rehabilitation Services, Outpatient Physical and Occupational Therapy, offers at our clinics a Cancer Related Fatigue (CRF) program.

This program includes a comprehensive consultation including posture, gait, strength, and sensation analysis along with identifying fatigue, activity, and pain levels. A treatment plan is established by the patient and clinician together. The treatment intervention will focus on stretching, breathing, low to moderate aerobic exercise, and strength training. Individuals who have had surgery may also experience pain and tightness at the surgical site due to scar tissue buildup. Skin integrity and scar tissue management is another area that Heartland clinicians address. Individuals involved in the program progress at their own pace and tolerance.

Heartland sees individuals at varying times in the cancer-treatment journey: pre-operatively, post-operatively, during chemo and radiation, after completion of treatment, and even years later. Individuals who exercise and move as long as they possibly can will experience a positive

H

ow did our mothers do it—cook three family meals a day for years?

It’s not like that in our world today! Lately, I have become aware that many people do not eat regular meals. Gone are the meals of our childhood, when we ate breakfast, lunch, and dinner. I remember that when I was growing up, all the kids in my neighborhood and my school ate breakfast, either packed a lunch or walked home for lunch, and then enjoyed a home-cooked meal for supper.

My mother packed my dad’s lunch for work and sometimes lunch for school for one of the four kids in my family. But that tradition seems to be just a memory. Today, so many people are skipping meals, eating food that is fast and not nutritional, or trying to catch a free meal at a local lecture or social gathering.

As we become adults, we need to eat just as much as we did when we were growing up as children.

It might sound basic, but we have to eat. Adrienne Justen, RD, LD, a registered dietitian with Genacross Lutheran Services, agrees. “Good nutrition is very important for all adults. It is especially important for older adults who are ill or who have been diagnosed with a chronic disease. Malnutrition can lead to a number of health problems, such as weight loss, tiredness, muscle weakness, depression, poor memory, and higher risk for infection and anemia,” she says.

I know that when I am hurrying in the morning and late for work, I tend to skimp on breakfast, and it shows. I cannot concentrate on my work, I take much longer to do simple tasks, and I am distracted because I am hungry. Doing any physical tasks, like picking up papers, moving my legs, or opening my desk drawer, seems to be a little off kilter. I do not have the proper fuel for my body!

Why don’t we eat?

Time seems to be one culprit.

With two-income families, parents find cooking a meal challenging. Children are busy with school, athletic and other after-school practices, or social events. Factor in doctor appointments, dental checkups, and church services, and when you finally get home after a day, you might be too tired to eat, let alone cook a meal. There is no time for cooking, and more nutritious home-cooked meals are replaced with the often less-nutritional fast-food meals.

Some people do not eat regular meals because they keep very loose schedules—a three-time-a-day meal schedule is the furthest thing from their mind. There is a culture of persons who are retired, or who live alone, and stay up into the “wee hours” of the night since they do not have to report to work anymore. Some often eat late at night and wake up not too hungry. Their meal schedule might change every day, and often it consists of fast food, junk food, or non-nutritious foods because they

are eating for convenience and at irregular times.

Others outright say that they do not like to cook. It really is another job. There is planning a menu, shopping for food, preparing it, and cleaning up after a meal. Cooking can be creative, and many like the art of cooking, but it certainly is not for those who have low energy or do not want another job in their lives.

They enjoy a cooked meal but do not want to do it.

Some are just tired of cooking; they do not want to cook since they raised families and are done with cooking, even for their empty nest

of two. Others have health challenges—they recently got home from a surgery or rehab and they are not strong enough to cook. Sometimes medications cause them to lose their appetite.

How can we make sure we eat?

The opioid epidemic and seniors

f youâ&#x20AC;&#x2122;ve ever been hurt or had surgery, youâ&#x20AC;&#x2122;ve probably been prescribed pain medication. Thatâ&#x20AC;&#x2122;s the standard treatment of care, right? Well, for some this plan of care has an unplanned resultâ&#x20AC;&#x201D;addiction. You may have heard about the opioid epidemic in the news lately, and you should know that it can especially impact older adults.

Consider this: The Centers for Disease Control and Prevention (CDC) reported that almost 62 million patients had at least one prescription for opioids filled or refilled in 2016.

According to the Henry J. Kaiser Family Foundation, Ohio leads the nation in opioid overdose deaths, with 3,050 reported in 2016. With these statistics it is not surprising that opioid abuse is now considered an â&#x20AC;&#x153;epidemic.â&#x20AC;?

What exactly are opioids?

Opioids are strong prescription medications that block pain signals to the brain and are often used to treat short-term pain caused by an injury or surgery. They can also be used to help manage long-term or chronic pain caused by cancer or back or nerve pain. The problem with opioids occurs when your body gets used to having the drug and requires more and more of it.

Opioids are prescribed by physicians because they do provide pain relief. They can also have side effects such as sleepiness, confusion, nausea (feeling sick to the stomach), and breathing problems. Taking just one large dose could cause serious breathing problems that could lead to death.

Some common types of opioids include oxycodone (OxyContin, Percodan, Percocet), hydrocodone (Vicodin, Lortab, Lorcet), morphine (MS Contin), codeine, fentanyl, hydromorphone (Dilaudid), methadone, and diphenoxylate (Lomotil).

Alternatives for pain management You should know that there are safer options for pain relief. Depending on the cause of your pain, opioids may not be the best treatment for you. Ask your healthcare provider about nondrug treatments for pain, such as exercise and physical therapy. If those donâ&#x20AC;&#x2122;t help, ask your healthcare provider if you could try over-the-counter pain relievers, such as acetaminophen (Tylenol), or anti-inflammatory medications, such as ibuprofen (Advil, Motrin) or naproxen (Aleve, Naprosyn).

Prescription precautions If an opioid medication is prescribed for you, make sure you take it as directed and store it in a locked drawer or cabinet to prevent others from taking it. Call your doctor if itâ&#x20AC;&#x2122;s not working or if you experience any adverse side effects.

You should also call your doctor if you fear you are becoming dependent on it. If you need information on addiction treatment, call the Substance Abuse Treatment Facility Locator at 800-662-HELP or visit findtreatment. samhsa.gov.

If you donâ&#x20AC;&#x2122;t finish your prescription or have an expired prescription in your home, be sure to dispose of the remaining pills safely. Some Walgreens pharmacies have disposal kiosks, and many law-enforcement agencies have drop boxes available.

Do you know who your beneficiaries are?

W

hen was the last time you checked your beneficiary designations for your individual retirement account (IRA), employer retirement plan, annuity, or life insurance policy?

If you haven’t examined it since the account was set up, you are not alone.

Due to changing circumstances and shifting priorities, you may find that your named beneficiaries are no longer in keeping with your estate plan or wishes. If you have switched jobs, become a new parent, divorced, or survived a spouse or a child, your current beneficiary designations may need to be updated.

Consider the “What If?” In the heat and emotion of divorce proceedings, for example, the task of revising one’s beneficiary designations can fall through the cracks. A court decree that ends a marriage also terminates the provisions of a will, but it does not automatically revise the beneficiary status of an employer-spon-

sored retirement account or an IRA. Some financial institutions automatically cancel the designation of a spouse as the beneficiary of an IRA in the case of divorce, but not all do. So, for example, if an IRA owner remarries and has a new family but fails to change the beneficiaries on the account, the original beneficiary may have a legal claim to the assets in the event of death.

Also keep in mind that the law requires that a spouse be the primary beneficiary of a 401(k) or a profit-sharing account unless he or she waives that right in writing. A waiver may make sense in a second marriage if a new spouse is already financially set, and the children from the first marriage may need the money.

Steps to stay current To ensure that your beneficiary designations are current and up-to-date, consider the following steps:

1. Make a list of all accounts that have named beneficiaries. This may include 401(k) plans, 403(b) plans, 457 plans, IRAs, pension plans, life insurance policies, annuities, and bank accounts.

2. Contact the plan administrator or financial institution that maintains or services your account to verify your current beneficiary designations. You may want to do this with the help of your tax advisor or estate-planning professional to ensure that these documents are in sync with other aspects of your estate plan.

3. Keep it safe. Store this list in a safe place with your other estate-plan documents, such as your will, health care proxy, and power of attorney, and make sure your designated executor has a copy.

4. Register for online access. If you do not already have online access to your accounts with beneficiary designations, consider registering so you can view and update your account information whenever you need to.

5. Consolidate. If you have changed jobs and left your assets in your

former employers’ plans, you may want to consider moving these assets into a rollover IRA. Consolidating multiple retirement plans into a single tax-advantaged account can make it easier to track your investment performance and streamline your records, including beneficiary designations.

ince 2001, the Northwest Ohio Silent Witness Project has been committed to honoring the memories of girls and women from our community who were victims of domestic violence and murdered by current or former spouses, dating partners, or stalkers. During this time, the BGSU Women’s Center housed and maintained the Silent Witness Project.

As of August 2017, the Northwest Ohio Silent Witness Project has relocated to the Bethany House domestic violence shelter, sponsored by the Sisters of St. Francis of Sylvania. Dr. Mary Krueger, Retiring Director of the BGSU Women’s Center, will assist Bethany House to ensure the Silent Witness Project continues to honor those murdered in domestic violence.

The annual Unveiling Ceremony

will take place on Monday, October 2, 2017, at 7:00 p.m. at the Franciscan Center, 6832 Convent Blvd., Sylvania, Ohio.

OTTERBEIN Doctor-patient communication…Can we talk?

S

ee if this sounds familiar: You have a medical appointment. You check in at the front desk on time and take a seat. You wait for quite a long time, and then finally your name is called.

You go to the exam room, and the nurse comes in, asks a few questions, and takes your blood pressure. You wait some more. The doctor comes in, asks a few more questions, writes a prescription, and is gone. You leave the office with more questions than answers. You aren’t quite satisfied.

Patient-physician communication is very important for successful healing. An analysis from The Joint Commission, a nonprofit that accredits US healthcare organizations, faulted “inadequate communication”—both between physicians and between patients and their doctors—in more than 70 percent of sentinel events, or adverse health outcomes not related to the natural course of a patient’s

illness. This means that if we can improve the communication and effectiveness of the visit to a physician, there is a greater chance for a positive outcome.

What can a patient do?

As a patient, you are not powerless when it comes to effective communication in the exam room and after you leave. There are things you can (and should) do to ensure your voice is heard and your concerns are addressed.

Come prepared. Know what you intend to talk about before you arrive for your appointment. Write down your concerns and questions, and make them specific. The more pointed your questions are, the more direct answers you’ll get.

Prioritize your concerns. Know that your doctor may not have time to answer all 50 of your questions,

so ask them in order of importance.

“When a patient prioritizes their concerns, it tells me they have very specific health interests and they understand we’re both working with limited resources,” says Dr. Nirmal Joshi, chief medical officer for Pinnacle Health System. “It also tells me you respect my time and allows us to focus on what concerns you most about your medical situation.”

Don’t be afraid to ask for another appointment. Ultimately, you want all of your concerns addressed with ample time and attention. So rather than trying to rush through your list of questions, if you don’t finish, ask for a follow-up appointment.

Dr. Joshi says it’s not uncommon for patients to remember something they wanted to talk about as the

doctor is getting ready to leave the room. In that situation, the doctor has a choice—address the concern quickly, possibly sacrificing good clinical care, or set up another opportunity to discuss the issue. As a patient, you can take the reins by requesting a follow-up.

Be willing to communicate outside the exam room. “Never underestimate the power of communication that’s not necessarily face-to-face,” says Dr. Joshi, who recommends patients ask their doctors whether they are willing to email. “Many physicians love that. I personally love that. That freedom allows me to instantaneously communicate with patients when I have the time for them.”

Email or communication through an online patient portal or telephone number is particularly useful when a face-to-face appointment has already occurred and you have questions about what was discussed or your treatment instructions.

Bring someone with you. “If the person getting care happens to be elderly or is otherwise incapable of asking questions in an assertive way, I strongly advise them to bring someone along,” Dr. Joshi says.

When a caring son or daughter is present, for instance, Dr. Joshi says the expectations are very high, and he is most likely to respond in kind.

In other words, the added person can serve as a medical advocate for the patient who may otherwise just accept that “doctor knows best.”

The bottom line: Your time with

your doctor is limited, so making every moment count is crucial. Though it may seem like 15 minutes can’t possibly be enough, learning to use these precious moments wisely will help you get the most out of each appointment.

Rnning tips for beginners

A

re you interested in running but afraid of the first steps? Here are a few tips to get you off on the right foot and on your way to enjoying the sport of running. Most important, consult your physician before starting any exercise program.

As a runner, the most important piece of equipment is your pair of running shoes. Before starting your running program, visit a shop that specializes in running shoes, such as Dave’s Performance Footgear, for an assessment of your needs and a personalized fit.

One of the most common complaints of a new runner is, “I can’t breathe.” Focus your energy on exhaling rather than inhaling. As your need for air increases, think about forcing the air out of your lungs rather than sucking it in. This will make you less tired than when you are huffing and puffing and will also help prevent hyperventilation.

In the beginning, take frequent short runs after resting sufficiently.

Your runs should last long enough for your body to get a workout but still be able to recover in time for your next run. If you over exercise and your muscles do not recover to their original condition, you will not gain any benefit. Strength is gained during rest when your muscles repair the fibers that are broken down during exercise. But remember, every runner is different when it comes to the amount and frequency of exercise that is appropriate. Listen to your body.

As a novice runner, take quick steps rather than long ones. You will not tire as fast if you take short, fast steps rather than long, slow ones, especially when you are tackling

hills. When running downhill, lean your body forward in order to try to keep your body perpendicular to the hill.

Novice runners need motivation and a schedule. Join a beginner running group, or seek out a running partner. Some runners find that taking music along on their workouts helps make running fun. When using headphones, though, play the music at a reasonable level so you can hear traffic noise and remain alert to potential hazards. Speaking of hazards, if you are running alone, carry identification and always run against traffic, preferably on sidewalks or running paths.

One of the best ways to improve and get more enjoyment out of running is to enter road races. The experience of finishing a running race is exhilarating. Start with the goal of a 5K (3.1 miles) road race, “fun run,” or a relay race with your running friends. Races can provide the foundation for enjoying a long life of running.

hanks to advances in imaging technology, breast cancer is being detected earlier and earlier these days.

As a result, treatment with advanced therapies is being initiated sooner, patient outcomes are improving, and mortality from this disease is on the decline. Earlier detection also means that a much greater percentage of breast cancers can be removed successfully via breast-conserving lumpectomy versus mastectomy.

Interestingly, early detection of breast cancer also presents a certain challenge to the surgeons whose job is to locate and remove the tumor with minimal damage to surrounding healthy tissue. O

ver the past few months, I have been fortunate enough to attend several tennis industry events that discussed a wide variety of topics.

The most shocking topic that continues to come up over and over (and not just in tennis) is how inactive the upcoming generations are overall. As parents, industry professionals, and beyond, we all most challenge ourselves to do our part to change this trend of inactivity. Here are a few items that were shared with the attendees: • The generation that is coming up behind us is the first generation predicted to live five years less than their parents.

• Physical inactivity is number four on the list of causes of death (more than smoking).

• Less than 25% of kids ages six to 12 are active three or more times a week.

• 48% of all high schools have no PE. • In 1990, not a single state had over 10% obesity.

• People who exercise regularly have a 40% lower risk of Alzheimers.

• 150 minutes of exercise is really all that one needs per week.

Now, the items listed above are certainly interesting and, personally, they really hit home, not only as an industry person, but also as a parent.

Over the months that I have written articles, I have occasionally pushed the sport of tennis and, guess what, based on the above statistics, I am once again going to do so. This time the approach is simple: We need to change the trend of inactivity in America, and tennis is a great way to do it.

Below you will see the top 10 reasons to play tennis that were shared with us, and I think all of you can see how they assist with changing the trends.

1. Get fit and lose weight. An hour of singles play can burn 580870 calories.

2. Live longer. Playing just three

From alertness to tactical thinking, tennis enhances the neural connections in your brain. Kids who play tennis regularly get better grades.

7. Learn to solve problems. Tennis is a sport that is based on evaluating angles, geometry, and physics to get the best results, which translates into better problem-solving off the court.

8. Enjoy family and friends. Tennis is great for the whole family no matter what the age. With minimal equipment needed and plenty of courts nearby, it’s easy to bring a friend or find one at the courts.

9. Develop teamwork and sportsmanship. From doubles play to team and league play, tennis develops your ability to communicate and work together.

10. Improve social skills. Tennis outperforms all other sports in developing positive personality characteristics.

We all need to do our part to encourage our society to get more active once again. Parents and children alike, I challenge you all to contact one of the local tennis facilities in the area and give tennis a try. Doctors, I challenge you to potentially prescribe tennis as a healthy exercise. New pulmonary program helps patients breathe easier at The Manor at Perrysburg

F

ew medical symptoms are as distressing as the inability to breathe freely and fully. Unfortunately, for people with chronic health conditions that cause respiratory problems, such as COPD, emphysema, congestive heart failure, or asthma, difficulty breathing can be an everyday reality.

Recognizing the high degree to which breathing difficulties can impact patient comfort and quality of life, The Manor at Perrysburg, a member of the HCF Management family of companies, recently launched a state-of-theart pulmonary program that will help patients with respiratory ailments breathe a lot easier.

“We can treat anyone with a pulmonary issue, whether restrictive or obstructive, ranging from patients who use CPAP or BiPAP to those requiring full ventilator or tracheostomy care,” says Lori King, RT, RCP, Director of Respiratory Services for The Manor at Perrysburg. “We’re also poised to nip small respiratory problems in the bud before they become bigger problems. For example, if a bug is going around and one of our residents develops bronchitis or a bad cough, we can take steps to prevent it from advancing to something more serious, such as pneumonia.”

King knows firsthand what pulmonary patients experience. Diagnosed with asthma as a child, she is well acquainted with the alarming sensation of being unable to catch one’s breath. To describe what it feels like, she suggests that people try breathing in and out through a coffee stirring stick for a few minutes. “That’s what it’s like to have an asthma attack or other form of respiratory distress.

It’s a very panicky feeling,” she says.

The new pulmonary program is directed by pulmonologist Dr. James Tita, who heads a multidisciplinary team of dedicated professionals consisting of a respiratory therapist; physical, occupational, and speech therapists; nurses; social services; and dietary services—all of whom are focused on getting the patient back to the highest possible level of pulmonary function.

As King explains, the team can teach patients different breathing

techniques, for example diaphragmatic breathing, as well as ways to keep the airways open and exercises that will strengthen both inspiratory and expiratory function in order to increase the volume of air moving in and out of the lungs.

Of course, the most important member of this multidisciplinary team is actually the patient. “Any pulmonary problem is manageable if the individual is willing to work at it, so patient compliance with treatment is essential to a positive outcome,” King says.

In addition to a multidisciplinary team of professionals, the pulmonary program boasts the most advanced respiratory equipment. For example, the ventilator used is the state-of-the-art Trilogy unit manufactured by Phillips/Respironics, which is very small, lightweight, and quiet in operation. This last characteristic ensures patients can get restful sleep without the disruptive noise produced by traditional ventilators. The Trilogy unit is also highly versatile, transitioning from CPAP (Continuous Positive Airway Pressure) to BiPAP (Bi-level Positive Airway Pressure) to full ventilation.

The new pulmonary program at The Manor at Perrysburg couldn’t have been introduced at a better time. Many members of the baby boomer generation were smokers and now have chronic pulmonary diseases, such as COPD, emphysema, or congestive heart failure, so the demand for pulmonary services in various healthcare environments is considerable and will only increase as this population ages.

Summing up the primary goal of the new pulmonary program, King states, “Here at The Manor at Perrysburg, the patients are always number one, and our objective with this new program is to make every breath they take easier. Genesis Village tournament proves videogames can be healthy for the body, mind, and spirit

ideogames often get a “bad rap” for encouraging inactivity, a critique that one might argue is justified when it comes to many young people today. However, when a group of seniors and their supporters gathered together at Genesis Village on August 28 for a Wii Bowling tournament, the scene was anything but sedentary.

Meeting that day for a little friendly competition were the Bowling Stones, coached by rehab staff from The Laurels of Toledo, and the 300 High Rollers from Michaelmas Manor. Spirits ran high as players of all fitness and mobility levels demonstrated their virtual bowling prowess. Some bowled from a standing position, others from a wheelchair, but all brought their own unique moves to the game.

Though everyone played to win, it was clear from the laughter and friendly banter between the teams that fun and camaraderie were their

true objectives.

Julie Leidel, Rehab Services Director for The Laurels of Toledo, remarks that Wii Bowling is not just an opportunity for fun, but also a great

way for seniors to stay active.

“Therapists like Wii Bowling because it offers visual, auditory, and kinesthetic feedback,” she says. “In fact, if patients are in formalized therapy, the game can actually be integrated right into their plan of care. But one of the best rewards is socialization, having something in common to do with their peers.”

Laurels physical therapy assistant Brad Weaver agrees, adding, “The great thing about Wii Bowling is

that the mechanics of the game are so similar to real bowling. A lot of our team members used to bowl for different clubs around town, but it got to the point where the ball was too heavy for them to manage.

For Bowling Stones member Rubie Williams, age 74, a regular bowling ball most decidedly became too heavy to manage. In fact, the last time she tried real bowling, the weight of the ball pulled her over and she fell in the lane. So, she got into Wii Bowling at a local senior center and then joined the Bowling Stones when she moved to Genesis Village. “I just love it!” she enthuses. “Some days you do really well, and other days you don’t, but you always have a great time nonetheless.”

resident Georgia Bayer, age 90, has been involved in Wii Bowling for three years now and says she loves the game simply for the thrill of it all.

“This is such fun! It’s great exercise, and I really enjoy the camaraderie with other teams,” she says. Georgia notes that she’s been off her game lately—possibly because she’s letting go of the ball too soon—but she’s working hard to overcome that habit so she can improve her score.

Thomas Williams, 53, a member of the visiting 300 High Rollers, joined the team because he was looking for a way to stay active rather than just sit in his apartment. He quickly discovered that he loves Wii Bowling and has taught many other people how to play the game. “I’ve been involved for about 10 years now, and I find that Wii Bowling improves me both physically and mentally,” he says.

teammates that Thomas has mentored is Ina Lyons, 80. As a former member of two bowling teams with over 40 years of experience, Ina learned to play the virtual version of the game quite rapidly. She, too, appreciates how Wii Bowling helps keep her active and healthy. “I’m 80 years old, and exercising is what keeps me going,” she states. “I’m always ready to play. I love the sport, and I love the camaraderie and sense of belonging I have with my teammates.”

Of course, it’s not just the competitors who enjoy Wii Bowling tournaments like the one held recently at Genesis Village, but also the spectators—many of whom are family members who come to cheer on their loved ones and simply join in the fun. For example, Anita Smith

regularly attends tournaments to cheer on her mother, Mary Smith, 93.

Anita says that Mary likes all sports, is very competitive, and enjoys Wii Bowling for the motivation and spirit.

Local Red Cross recruiting volunteers to support disaster relief efforts

T

he American Red Cross is recruiting volunteers to join its workforce to help people affected by Hurricane Harvey and other disasters locally and across the country.

The Ohio Buckeye Region of the Red Cross has sent 67 volunteers to Texas and Louisiana in response to Hurricane Harvey and more are on their way. In addition, the Red Cross is beginning preparations to get workers and supplies in place in response to Hurricane Irma. The Ohio Buckeye Region will recruit and train 100 new volunteers per week to assist with the response and recovery efforts for the weeks and months to come.

Locally, volunteers are responding to daily emergencies close to home, including home fires, tornadoes, and flooding. The Red Cross is training new volunteers to help with this up-

surge of disaster relief efforts across the Ohio Buckeye Region and across the country.

Disaster relief volunteer orientations for new volunteers will be held every Thursday at 6:00 p.m. at the American Red Cross of Northwest Ohio, 1111 Research Dr., Toledo, Ohio 43614. Anyone interested is asked to call the Red Cross at 419-329-2900 prior to coming that evening.

common quality metric that breast cancer centers use is the time that elapses between diagnosis and the initiation of treatment. We’re now beginning treatment within weeks of diagnosis versus months. Even if this doesn’t change the outcome, it’s a huge comfort to patients. The time from diagnosis to treatment is an extremely anxious period for patients, so the sooner you start, the better they feel.”

Mercy Health streamlines access to vital screening mammograms

W

ith today’s hectic lifestyle placing more and more demands on our time and attention, many women are putting mammograms and other important health screenings on the “back burner” or overlooking them entirely as they attend to more pressing obligations. This is unfortunate because outcomes for breast cancer tend to be much better when the disease is diagnosed and treated in the earliest stage. And, according to the American Cancer Society, the most reliable way to detect breast cancer early is to get regular screening mammograms.

To ensure that nothing stands in the way of women getting screened for breast cancer in the timeliest manner possible, Mercy Health is now offering mammograms with no requirement for a doctor’s referral at all of their facilities that offer mammography services. “What we’re trying to do through this initiative is remove one more barrier to preventive healthcare for women,” states Sandy Michalski, MBA, BS, RT(R), Director of Diagnostic Services for Mercy Health. “We tend to do things in the moment, so when a woman realizes she’s due for a mammogram, our services are one phone call away. Requiring a doctor’s order means women have to make an additional call and potentially schedule an office visit before they can access this vital screening, which complicates the process unnecessarily and may convince some women that getting a mammogram is more trouble than it’s worth.”

Michalski emphasizes that getting a screening mammogram without a doctor’s referral does not mean the patient’s primary care physician is removed from the process. “The results of the screening are sent to the patient’s doctor, so we prefer that you have a physician we can route them to. However, women who don’t have a primary care physician can still come in for a mammogram and have our team look at the results. We can also help connect them with a doctor through our Health Link system. If any abnormalities are discovered on the mammogram, navigators at the center will work with the patient and the doctor ’s office to ensure any required additional testing is arranged,” she says.

Aqua Hut introduces two cutting edge aquatic workout programs

F

or many years, Aqua Hut has introduced Toledo-area residents to the wonders of the underwater realm through scuba-diving adventures both close to home and all around the world. Now, the aquatic experiences available at this full-service, customer-friendly dive center have expanded to include Aqua Cycling and BOGA Fit/BOGA Yoga classes.

“We’re always pushing the envelope when it comes to the courses and services we offer, and these aquatic exercise classes are really on the cutting edge,” says Aqua Hut owner Jeff Davis. “In fact, we’re one of only five full-time Aqua Cycling centers in the country. The other four are located in major cities—one in New York, two in Miami, and one in Los Angeles.”

Invented in 1993 by an Italian physical therapist to help injured athletes, Aqua Cycling delivers a powerful full-body workout—burning between 600 and 800 calories per 45-minute session—with minimal impact on the joints and no next-day soreness or fatigue. What’s more, virtually anyone can do it, from the ironman triathlete to the patient rehabbing from a total knee replacement. “Our

Cycling has been paramount in this progression for many of our clients,” says Mincheff.

BOGA Fit and BOGA Yoga classes were just added to Aqua Hut’s lineup of offerings in August of this year.

These fun, innovative programs in-

volve performing various exercises or yoga poses on stable floating mats, and the element of water movement helps improve strength, balance, flexibility, and coordination to a much greater degree than is possible on land. As with Aqua Cycling, BOGA classes are offered mornings, afternoons, and evenings, making them accessible to anyone regardless of their schedule. Mincheff notes that many clients like to schedule Aqua Cycling and BOGA Fit/BOGA Yoga classes back to back so they can get a comprehensive workout in two

45-minute classes.

Davis emphasizes that, although all of Aqua Hut’s Aqua Cycling and BOGA instructors are highly trained, certified fitness professionals, his facility is not a gym. “We’re a small, safe, intimate environment where clients can feel comfortable wearing a swimsuit and no one is going to gawk at them. We don’t have multiple activities going on in the pool at once, so the only other people you’ll see in the pool will be your classmates and instructor. Plus, since we’re not a corporate gym, we’re not bound by corporate rules. We make our own rules, and we can change them as needed to provide a better experience for our clients,” he says.

Mincheff adds, “Above all, these classes are filled with fun, laughter, and camaraderie. Many of our clients say they have such a good time that it doesn’t even feel like a workout.”

REM is a short test, taking just five to 15 minutes, and measures the correct amount of sound presented to your tympanic membrane, otherwise known as the eardrum. The hearing aids are fit to the ears and programmed via the computer.

Then a small probe device is inserted in the ear canal to measure the sound close to the eardrum to ensure the appropriate gain is applied at each frequency.

The REM allows better fine-tuning. In other words, the goal is for the hearing aids to amplify sounds at each frequency to provide as close to normal hearing as possible. Even though the hearing aids are programmed according to the manufacturer’s fitting software, you may feel for some reason your hearing aids don’t sound right to you. We take into account the fact that each person’s ear canal has a different shape, diameter, and length, so the output of sound is not always what is expected.

As an example, let’s say there is a sharp bend in the ear canal; it may dampen the sound before it reaches the eardrum as compared to another individual who has a straight canal.

REM has been around for several years, but not all audiologists have the opportunity to provide this valuable tool. It’s an added service to increase patient satisfaction and make it a pleasurable experience for the patient. The American Academy of Audiology and audiology community have made REM a best practice and a gold standard of care.

By “here” I mean in the US and other disaster areas on this side of the planet.

Wow! Can you even comprehend all that has been happening since those Category 5 hurricanes and an 8.1 earthquake in Central America have shaken our earth, our lives, and our souls? Are you not yet numb from watching all the destruction, the wind ravaging the trees and buildings, the

incessant rain covering everything, and people leaving their material possessions behind? And we’re only watching from afar; what must it be like to be in these storms and floods and earthquakes? Even if you know someone in the region of the storms, can you really comprehend what it’s like to lose so much that is near and dear?

Yes, indeed, there is a lot of holy ground where there is so much sorrow!

As so many of the reporters and government officials have noted, there have been many brave and heroic people coming to the rescue of so

many people, especially the most vulnerable. Who can forget the image of elderly residents of a care facility in Texas up to their waists in water waiting for help?

Or the many ordinary fishing boats bringing out people and their pets from so many flooded areas in the Houston area? First responders, the National Guard, local law enforcement personnel, any and all were put to the test trying to rescue and later recover as many people as possible. What about me? What about you? What can we do to help out in this overwhelmingly difficult time in our history?

A Walk in the Park

ohnny Cash,” said Shirley. “Very nice. But your B-flat could be just a tad less flat.”

“I was going for a note of Country-Western authenticity,” I said.

“And you almost hit it, too,” she said.

We were on the beach at Anchor Point, Alaska. The sign there says “Anchor Point, North America’s Most

Westerly Highway Point.”

Across the Cook Inlet rose my vocal inspiration—the four volcanic peaks of Mt. St. Augustine, Mt. Redoubt, Mt. Iliamna, and Mt. Spurr—part of the Ring of Fire that stretches virtually all the way around the Pacific. The Ring, the most geologically active area of the globe, has more than 75% of the world’s volcanoes and experiences about 90% of the earthquakes.

“Do you suppose Johnny was thinking of plate tectonics? You know, the subduction of the Pacific Plate under the North American Plate that creates so much seismic activity?”

“Probably not,” said Shirley. “I think it is more likely he had Dante in mind. The imagery seems more theological than geological.”

“Maybe,” I said, “but you should never underestimate the complexity and profundity of Country-Western artists.”

We had been to Anchor Point in 2014, but back then we just wanted to brag that we had been as far west as you can go by highway. It is the same principle as stopping at the sign in Key West that says you have gone as far south as you can go. This year we actually stayed in Anchor Point instead of Homer, about 15 miles away. We went to Homer, too, of course, because out at the end of the five-mile gravel Spit in Katchemak Bay is Land’s End, yet another place where you have gone about as far as you can go.

At our Anchor Point campsite we were impressed when three juvenile bald eagles came to sit overhead in a spruce tree. (You can tell they are juveniles if they are not “bald”; i.e., their heads have not turned white.)

to watch bears eating salmon. Then it was to be east to Prince George and south into Washington. More specifically, we wanted to return to Columbia River wine country to visit my “cousin” Liz Mercer Elliott at the Mercer Winery as well as Fourteen Hands, Covey Run, and Columbia Crest. But that was not to be.

Huge wildfires in British Columbia had closed several roads—and there aren’t all that many roads to begin with.

At decision time, it was still possible to get through by taking some rather complicated detours, but conditions were fluid, as they say, and could be vastly different by the time we arrived at a critical junction. If the fire cut across our route, it could require backtracking for several hundred miles—assuming it was possible to backtrack. (A woman at the visitor center said she really needed to get back to California and asked how fast she could drive without getting a ticket.) Instead of speed, the option we chose was to come back the way we went: down the Alaska Highway to Mile 0 at Dawson Creek, south on the Icefields Parkway through the Canadian Rockies to Montana.

Wildfires in British Columbia closed several highways. Those that remained open were a little on the hazy side.

There were truly grand vistas along the Parkway: Columbia Icefields, the glaciers that give the road its name, Athabasca and Sunwapta Falls, Bow Lake, Peyto Lake, Moraine Lake, and Lake Louise. Plus the magnificent mountains punctuating the whole experience. And I won’t even mention the wildlife along the way: black bears and grizzly bears, bighorn sheep, Stone sheep, mountain goats, deer, elk, caribou, moose. We even got stuck in a bison jam just the way you do in Yellowstone. But I won’t mention any of that.

Speaking of Yellowstone, let us not forget that almost the entire park sits in the caldera of a volcano that promises to bust loose any time now—geologically speaking. This year there was an earthquake “swarm” of more than 2,400 beginning in June. Most were

detectable only with highly sensitive technology. In 1985 there were more than 3,000 in a three-month period.

The area has always been one of the most geologically active places in the world. Some of the most popular features of the park—Old Faithful, Norris Geyser Basin, and Mammoth Hot Springs—are a direct result. Yellowstone is the most famous tourist destination in the Ring of Fire even though we don’t usually think of it that way.

When we reached Jasper, Alberta there was haze in the air and just a whiff of smoke. Before long there was much more than just a whiff. Shirley and I think the smell of campfires is quite pleasant. The smell of millions of trees all aflame, not so much.

Driving south through Kootenay National Park to Radium Hot Springs, we entered a stretch of dense smoke that was like really heavy fog. Less intrepid souls might have been a little unnerved by driving a steep, winding mountain road unable to see… well, unable to see. We don’t know any such intrepid souls. I started to hum. Then to sing under my breath.

I fell into a burnin’ ring of fire….

At Radium Hot Springs the smoke began to thin out, and at Eureka, MT the sky was clear. But, wait, there’s more. We spent several days in northern Montana before continuing to the Tetons in NW Wyoming.

(Shortly after we left Montana, a wildfire destroyed Sperry Chalet in Glacier National Park and closed Going to the Sun Road through the park.) The Tetons are the stars of the most spectacular scenery you will find anywhere. When you can see

them. When the wind came from the right direction, the days were clear and bright. But too frequently the Tetons were enveloped in haze from wildfires to the west in Idaho, Oregon, Washington, and California.

Wildfires in the West are as common every summer as snow in Buffalo every winter. For the traveler, this can mean various levels of inconvenience, but the burnin’ ring of fire usually represents just one more story to tell the folks back home. For residents, wildfires are a serious threat to life and property. What do you do, for example, if you operate a ranch with several hundred head of cattle and/ or horses? Got enough trucks to haul them all to safety? Caring for your family, home, ranch hands, and livestock gives you cause to be grateful to the firefighters who take extraordinary risks as a matter of routine.

This year, even the population of Los Angeles was threatened.

At Gros Ventre Campground in Grand Teton National Park, our neighbors left to enjoy their day in the park. Soon after, the wind fanned their “dead” campfire back to life.

(I was tempted to make a bad pun about their “fire ring.” But I won’t.)

We went over and put a couple gallons of water on it. No harm done.

This time. Shirley and I love the Tetons and want to continue hiking the trail up to Taggart Lake that has already been burned over at least once in recent memory. About 10% of wildfires are caused by lightning.

The rest are started by people like our neighbors who are just careless.

Whether you are a tourist or resident, coping with the threat of wildfires

is never just a walk in the park. ❦

• Store the food you’ve made ahead of time in single-serving portions, which will allow you to reheat just the amount that you will eat.

• Augment meals with nutritional supplements like instant breakfast drinks, milk shakes, or smoothies.

• Accept assistance from friends or family who are willing to help you cook and prepare food.

• Get takeout from local restaurants that deliver and consider ordering extra to store as single-serving portions to be eaten later.

When we are sick and have health challenges, we might find that feeding our body “good stuff” or protein-filled and nutritious meals actually helps us function better. Can acupuncture help me with…?

s a Doctor of Chiropractic who also practices acupuncture, the biggest set of questions I get from patients is, “Can acupuncture help me with [insert condition].”

Acupuncture falls under Traditional Oriental Medicine and has been practiced continuously for over 2,500 years. The only reason we call it “Oriental Medicine” is because the Chinese invented the written word and wrote the first formal textbooks.

Acupuncture is just one type of treatment under Oriental Medicine, but it can be applied to a wide variety of health issues.

There has been a growing awareness that taking a drug for every ailment might not be the best way to go with many conditions. Patients are more than ever seeking a more natural form of treatment and shying away from the heavy deluge of prescription drugs that seems so prevalent today.
