Operating Lease Save Money and Stay Ahead of the Technology Game Operating Lease is an alternative means of gaining access to the latest technology for the practice, without the burden of redundant or fully depreciated equipment in your practice, holding you back commercially and financially at a significant tax and financial advantage*. Operating Lease for Financing Medical and Dental Equipment, especially Imaging, Diagnostics, Healthcare IT, Management IT and Communications is particularly relevant for businesses that recognize and understand the need for regular technology upgrades and appreciate the advantages of renting (as payments in operating lease are described) as opposed to buying. Operating Leases are essentially long term (typically 3-5 years) arrangements, and the advantages include lower overall cost, offbalance-sheet accounting, and multiple options during the rental period and at the end-of-term. Often the rental payments over the term of an operating lease can actually be less than the purchase price of the equipment. The lower cost is due to the lender taking a residual risk position (Your Price Point Medical Director can explain this in more detail). This results in lower payments and lower total cost, because the borrower isn’t committed to a residual payment or loan completion. With Operating Leases, the lender/financier owns the asset, so it doesn’t appear in the borrower’s balance sheet. From a tax point of view, the lender claims depreciation, and the borrower deducts all payments in full as an operating expense. Equipment under these arrangements can often be upgraded to further enhance cash flow and stay ahead of a shorter technology curve. With Operating Lease Finance, you have the option to upgrade anytime during the term by simply extending the term, increasing the payments, or a combination of the two.
Other facilities require the settlement of the existing loan, which can include the additional expense of break costs, and entering into a separate, new facility, which may or may not meet term and rate requirements for the original equipment. Operating Lease end-of-term options are: •
Return the equipment/system to the lender without further obligation;
•
Extend the term with reduced payments;
•
Upgrade or replace the equipment/system with an extended term, adjusted payments, or both; or
•
Purchase the asset at the then fair market value.
The last option can be contentious unless there is a fairly active second-hand market for those types of assets. While the lender owns the equipment they can also be responsible for its upkeep and maintenance. Terms here may vary according to guarantees etc, but this is certainly a feature that adds further value to this option. Speak to your Henry Schein Halas representative when considering this. Systems such as Imaging, Diagnostics, clinical tools, Ultrasound and IT have rapid pace of technological improvements that enhance performance or clinical utility. We believe the cycle to be 5 years from a technology standpoint and this coincides with the Operating Lease term for most items. Financially, clinically, marketing and patient experience, Operating Lease is the way to go. Background to Tooth Dental
Tooth Dental, located just 5 km from Brisbane CBD in Ashgrove has just celebrated its first birthday, with the practice growing from strength to strength and establishing itself well within its locality. Tooth Dental is owned by Dr Lou Waldie, a local dentist who graduated from the University of Queensland. Having practiced in the area for a number of years, Lou decided it was time to branch out and establish her own practice and she hasn’t looked back since then.
The practice has moderate size entrance and reception area with toys for children and mobile charging facilities for modern patient convenience. Take a closer look and you may even notice the Tooth Fairy mail box for the children where the Tooth Fairy occasionally leaves messages, adding to the dental experience of their younger patients.
Lou’s core philosophy is to deliver a warm and inviting family environment, while providing comfort and care for the patient and forming integral part of the local community. With her key value for the practice being to treat each and every patient how you would like your mother to be treated, with a warm caring personal experience. Approaching the practice it is clear that the team at Tooth Dental offer a fun, warm and friendly environment, which helps ease the patients experience and visit to the practice. The practice has a simple layout with two small surgery rooms as well as a fully compliant OPG x-ray room and dedicated sterilisation and a plant rooms. The fit out of the practice has clearly been considered in detail, with a consistent theme and quality evident throughout.
IN•PRACTICE Lou expressed the importance of using the latest technology in order to provide patients the best experience and most accurate diagnosis, with the convenience of being in-house. To provide this service, Dr Waldie operates both Planmeca Pro-X Intra Oral X-ray’s (one in each operating room) and a ProMax Scara 2 OPG with integrated Cephalostat arm.
Workflow was also a clear consideration of the team at Tooth Dental. For example the Sterilisation room exhibits a clear flow from dirty to clean, as you progress from the washing area on the left, through the sterilisation process to the autoclave and then to packing and storage. The Equipment
To ensure maximum patient comfort, Lou opted for the latest technology in Planmeca’s treatment unit range - the Compact iTouch with an Over Patient Balanced delivery system. Lou had not used the Planmeca range previously, but was recommended it by Practice Manager Phoebe. Besides the recommendation, Lou was drawn to Planmeca due to its attention to patient comfort, reliability and affordability. In addition, Lou selected super soft Planmeca ultra upholstery, which uses visco-elastic memory foam to provide the ultimate in comfort and support for the patient, ensuring they have the most comfortable experience possible. The practice has two operating rooms with identical equipment and layout which allows for ease of transition between patients and operators across the two rooms. This also increases the efficiency of sterilising and rotating the instruments. The Planmeca Compact iTouch chairs with their small footprint and over patient delivery fit perfectly in the modestly sized operating rooms and allow easy navigation around the room and patient.
With a suite of Planmeca dental equipment, Lou achieves full end-to-end integration using Planmeca’s Romexis software. This incorporates her chairs, imaging and data management, meaning relevant patient information is always at hand. Experience So Far
Lou and her team have been very pleased with the ergonomic value and ease of use of the continental delivery system, which was new to her, previously utilising a Sirona Toneao unit with hanging tubes. In particular, Lou praises the integrated Panasonic intra oral camera, which coupled with the integrated monitor and Romexis software, allows her to display before and after pictures to patients detailing the work she has done. The Future of the Practice
The team at Tooth Dental are heavily involved in supporting their community, including sponsoring local football teams with subsidised mouthguards, visiting local kindergartens to teach the kids about the importance of oral hygiene and brushing properly, and even providing free dental treatment for the homeless during the Tooth Dental - Rescue Days. Moving forward, Lou’s plan for the practice is to continue to build up the reputation and profile of the Tooth Dental within the local area and to increase the customer base, continuing to provide the caring, family and patient friendly feel, and exceptional dental care. Furthermore, Lou seeks to continue working with the local community, helping to grow awareness through education on the importance of oral hygiene and care.

Selecting a Treatment Unit. In Australia we’re fortunate to have access to the world’s best brands of dental treatment units. With this in mind, there are many choices for the purchaser to make regarding the four main elements below that make up the complete treatment unit.
Now consider the patient seat. Is a standard patient seat the best option, or is a knee break going to provide great benefit to you and your patients? Do you require armrests or a specific colour to compliment your practice decor? There are many customisable options so don’t be afraid to ask if you have a preference.

So how are you going to make a decision on what might be best for you? As a part of your thought process you might consider brand allegiance - whether you’ve enjoyed exemplary performance from your existing chair. Or you may have seen a chair in a colleagues practice, or a supplier showroom. Then again you may be more practicality driven, with a specific ergonomic design or feature set that you’re seeking. In reality we expect it’ll be a mix of all the above with preferences influenced by your own experiences and those of your colleagues. Dental Chair The two main considerations when selecting a dental chair should be the operators comfort and chair access, followed by the comfort of your patient. Your comfort is very much paramount. Before deciding ask yourself, can you sit under the chair and maintain great posture when working in all parts of the oral cavity? Can you place the foot control where you need it? Remember you will use this chair full time for the next 10 (or more) years so you need to be 100% sure you’re comfortable and well positioned. For the patient, chair comfort is more about being nicely supported and cushioned by the chair during treatment. Not feeling bent, twisted or falling back is important. It’s true that for most people dental treatment is stressful, so providing a chair that facilitates patient comfort removes one stress trigger, and then the rest is up to you. 24
Your skill as a clinician is complimented by the quality of your instrumentation. Smoother cutting, polishing, scaling and diagnosis instruments not only make your job easier, but noticeably improve patient comfort during treatment. Use of quality instrumentation enhances patient wellbeing as the tasks are completed more quickly. Likewise you benefit from the heightened clinical efficiency. There are a wide variety of dental element configurations, each offering their own distinct advantages. These include over the patient (OTP) with hanging tubes, OTP with balanced tubes (often called whip or continental), under the patient, cart, wall/cabinet mounted and other variants. Your choice may be influenced by numerous factors including available space, room design and your individual style. Here are the aforementioned configurations in more detail: Hanging Tubes Hanging tubes (pictured right) are by far the most common style of dental element in Australia. They’re convenient, familiar and widely available. The delivery system can be over the patient arm, under the patient arm, from the cabinet or on a cart. As the lines hang beneath the dentist element it is possible to incorporate a tray for instruments and materials. Despite the numerous advantages, hanging tube deliveries do have some weaknesses too. As they are hanging, you must carry the weight of the instrument and its line on your wrist. Furthermore dropped instruments can easily hit the floor resulting in a greater potential for damage.
TREATMENT UNITS Balanced (Whip/Continental)
Operating Light
The popularity of balanced (whip or continental) delivery systems is growing steadily. The system lends itself to over the patient delivery, and with correct design will support the weight of the instrument and tubing without pulling on your wrist. They are a lighter touch, and the ergonomic benefits of this delivery system for the operator are well documented. Furthermore they also provide a flat easy to clean barrier/handpiece rest (not hanger) and will never allow a dropped instrument to make it to the floor.
The operating light is an essential part of any dental treatment unit.
Under the Patient Delivery The under patient delivery, as the name would suggest, supports the dental element from an arm this protrudes from beneath the dental chair. This configuration offers a few key benefits. Firstly this configuration, depending on the chair, may be capable of ambidextrous use (two or ten o’clock position). Furthermore with the right unit choice and room design this arm can even be hidden away over at the nurses’ side. This benefits both patient and clinician as the used instruments are out of sight on exit and out of the way of the patient, and allows the assistant to begin resetting the unit, decreasing down-time between patients. Similar to over the patient, this delivery system most commonly utilises hanging tubes. Cart & Cabinet/Wall Cart and Cabinet/Wall deliveries, as mentioned earlier, typically utilise hanging tube systems. Carts offer significant mobility of the delivery system, however this is limited by an umbilical from cart to the main unit on the floor. Cabinet or wall mounts, like cart, offer the versatility of separating the instruments from the chair, reducing visible clutter. These may also be mounted to the rear of the chair to facilitate a 12 o’clock practicing style. With this in mind, the dentist can also work from nine o’clock to three o’clock.
Lights can be unit mounted (typically), ceiling (sometimes) or wall (rarely) mounted. If your unit is unit pedestal based, the light is normally mounted to the pedestal which is firmly bolted to the floor, meaning there is no movement of the light if the patient moves during treatment. If the light is mounted to the chair, as with American styled units, many practices opt for the ceiling mounting option to eliminate light movement during the treatment. Our light discussion then moves to the quality of the light itself. Do you need bright white light? What about light positioning? Is the light stable and movement free? Can the light intensity be easily adjusted? Does the light spread to the entire operating field? Patient comfort should also be considered. Does the reflector control the light field so as to not dazzle the patient. Does the light feel warm or heat the patient? A great light features illumination characteristics that also take into account patient comfort, as apart from your masked face, it’s about the only thing in their field of view. The Assistant Side. For treatment units with spittoons consideration for hygiene and maintenance is paramount. How easy is it for the nurse to clean? Does the assistant element come with automated hygiene features? The assistants’ side instrumentation will also affect your workflow. High level assistance keeps your operating field clear, and for that your nurse will need an air/water 3F syringe combined with high volume and saliva ejector suctioning. Often suction is overlooked, however having better control can reduce aerosol clouds, likewise features such as being able to isolate the suction while it is out of the mouth can also be helpful to reduce noise around the patient’s head. With so many options available, purchasing a treatment unit can appear daunting. At this juncture, the knowledge and experience of the Henry Schein Halas Equipment team is invaluable. Laser-Lok overview Laser-Lok microchannels is a proprietary dental implant surface treatment developed from over 25 years of research initiated to create the optimal implant surface. Through this research, the unique Laser-Lok surface has been shown to elicit a biologic response that includes the inhibition of epithelial downgrowth and the attachment of connective tissue.2,3,4,5,6,7,8,9,10 This physical attachment produces a biologic seal around the implant that protects and maintains crestal bone health. The Laser-Lok phenomenon has been shown in post-market studies to be more effective than other implant designs in reducing bone loss.11,12,13,14
Unique surface characteristics Laser-Lok microchannels is a series of cell-sized circumferential channels that are precisely created using proprietary laser ablation technology. This technology produces extremely consistent microchannels that are optimally sized to attach and organize both osteoblasts and fibroblasts.15,16,17,18,19,20,21,22,23,24 The Laser-Lok microstructure also includes a repeating nanostructure that maximizes surface area and enables cell pseudopodia and collagen microfibrils to interdigitate with the Laser-Lok surface.

BIOHORIZONS Different than other surface treatments Virtually all dental implant surfaces on the market are grit-blasted and/or acid-etched. These manufacturing methods create random surfaces that vary from point to point on the implant and alter cell reaction depending on where each cell comes in contact with the surface.10 While random surfaces have shown higher osseointegration than machined surfaces,11 only the Laser-Lok surface has been shown using light microscopy, polarized light microscopy and scanning electron microscopy to also be effective for soft tissue attachment.2,3,4,5,6,7,8,9,10

The Laser-Lok surface has been shown in several studies to offer a clinical advantage over other implant designs. In a prospective, controlled multicenter study, Laser-Lok implants, when placed alongside identical implants with a traditional surface, were shown at 37 months post-op to reduce bone loss by 70% (or 1.35mm).11 In a retrospective, private practice study, Laser-Lok implants placed in a variety of site conditions and followed up to 3 years minimized bone loss to 0.46mm.12 In a prospective, Universitybased overdenture study, Laser-Lok implants reduced bone loss by 63% versus NobelReplace™ Select.13
Months Post-Op In a 3-year multicenter perspective study, the Laser-Lok surface showed superior bone maintenance over identical implants without the Laser-Lok surface.11
Latest discoveries The establishment of a physical, connective tissue attachment to the Laser-Lok surface has generated an entirely new area of research and development: Laser-Lok applied to abutments. This provides an opportunity to use Laser-Lok abutments to create a biologic seal and Laser-Lok implants to establish superior osseointegration15 – a solution that offers the best of both worlds. Alternatively, LaserLok abutments can support peri-implant health around implants without Laser-Lok. Multiple pre-clinical and clinical studies support both of these concepts.5,6,7,8,9 Laser-Lok abutments can inhibit epithelial downgrowth, physically attach connective tissue to protect and maintain crestal bone. Most recently, the combination of Laser-Lok abutments, implants and platform switching was shown to regenerate crestal bone surrounding the implant.5 BioHorizons is committed to someday being able to provide implant products that predictably achieve zero bone loss. Clinical Case Study
A New Zealand experience: from DIAGNOdent to DIAGNOcam Dr Yvonne Vannoort. Principal Dentist, Sanctuary Dental. Sanctuary is a general practice with a team of five dentists and three hygienists located in Botany, Auckland.
As a KaVo DIAGNOdent user for many years, I have been curious about the further advantages of decay detection that the KaVo DIAGNOcam would give to me, my clinical team and of course our clients. The quantitative measure that the DIAGNOdent gives is very useful to help diagnose decay and provide a record to compare and monitor at subsequent visits. Those high pitched whistles of the DIAGNOdent, when decay is present, do get the patientsâ&#x20AC;&#x2122; attention. So how does the DIAGNOcam compare? The DIAGNOcam uses diode laser technology at 780nm wavelength. The tooth becomes the conductor of the light and this is why these images are so interesting and actually quite beautiful! Cracks and carious lesions block the light transmission and appear
as dark areas. A digital video camera records the image and displays it live on a monitor. After having used the DIAGNOcam for a couple of months now, the visual impact is amazing. Our clients are intrigued by the images and by saving the images we have a record that we can refer back to later and compare if we need to. The DIAGNOcam shows both occlusal and interproximal decay, and there is no need to clean the surface first. The cases below illustrate some of the uses of the DIAGNOcam.
Case one There is an obvious cavity in the distal of tooth 14. Note the mesial surface of tooth 15. This is an example of how decay may not be clear on a bite wing. On the DIAGNOcam image, however, it is obvious. Note also that the decay has progressed beyond the enamel dentine junction in the DIAGNOcam image and you can see this when the cavity is exposed. Fractures show well with the DIAGNOcam too. This is the same client. Note the fracture in the mesial marginal ridge of tooth 16.

One method of streamlining and maintaining the integrity of the process is to implement a bar-code tracking system linked to the practice management software such as EXACT by Software of Excellence. To ensure that instruments and reusable equipment were sufficiently reprocessed in the event of an audit or incident there is information that must be recorded and associated with the patient. An excerpt from Australian Dental Association, Victorian Branch ADAVB Publication 2005 states: “Reprocessing of reusable medical and surgical instruments and equipment, and maintenance of the associated environment states that procedures should be in place to link steriliser batch information of a critical
item that has been sterilised, to the patient. Each critical item or tray shall have batch control identification which shall designate the following:“ (a) Steriliser identification code (if there is more than one steriliser within the office-based healthcare facility) (b) Date of sterilisation (c) Cycle or load number The Mocom autoclave range are designed to generate labels and integrate a label printer to facilitate cycle details being uploaded to a patient data management software. If a cycle is unsuccessful, the autoclave will not print/generate a label.

Most importantly, each autoclave must have its own printer. Furthermore, to implement the aforementioned solution, you must have a barcode scanner connected to the practice management software. In conclusion, barcoding is an efficient way to both manage the sterilisation process and mitigate the risk of using unprocessed equipment/instruments. With over 20 years of dental experience working with the Royal Australian Air Force, Dr Draper has undertaken a wide range of duties ranging from dental forensic casualty identification, to instructing on tooth problems in high performance aviation. Louise is an experienced registered nurse with a background in acute surgical nursing, ensuring the practice provides an exceptional level of patient care and expert treatment. This also allows them focus on patients living with complex medical conditions, such as patients suffering from severe anxiety or those undergoing cancer treatments to ensure their oral health is preserved. The philosophy of the practice is “To provide quality, evidence based care for anybody who needs it.” From the outset, Dr Draper and his wife put a great deal of care and attention to the details when building the practice to ensure the patients have the most comfortable 64

The practice has a simple layout of with two similarly sized mirror image operating rooms, each with stunning in-chair views of the surrounding landscape. The practice was built to accommodate almost any patient. It is fully wheelchair accessible featuring widened doors and hallways, as well as full soundproofing throughout to ensure patients that suffer from high levels of stress and anxiety cannot hear treatments from the waiting room which may result in unnecessary distress. The Equipment
Dr Draper undertook extensive research to select the most suitable equipment for the practice. For Andrew, selecting a treatment unit wasn’t a simple decision as he is left handed which significantly limits the solutions on the market.
IN•PRACTICE With over 20 years’ experience using ADEC chairs in the military, it was vitally important for the chair to be reliable and affordable, as well as inexpensive to maintain and suitable for left and right handed operators. For Dr Draper the KaVo E30 was the most cost-effective unit, as well as providing the reliability and quality of the Kavo brand. This decision was cemented by the E30’s ability to convert from left to right side delivery in under two minutes and the capacity to accommodate patients of up to 185kg. Using the DIAGNOcam
At Odontologica the vision is “To provide superior quality, personalised, affordable comprehensive family dentistry for people of all ages.” With patient care of the utmost priority at Odontologica the DIAGNOcam by KaVo was ideal choice for Dr Draper. He stated he uses it three or four times a day with his patients and with almost every new examination. He finds it particularly useful with children and for patients who gag, as well as for providing a second opinion alongside the intra oral camera or x-ray. Combined with a ceiling mounted 42” 3D monitor in each operating room, it gives him the ability to clearly identify potential areas of concern to the patient and is an extremely valuable clinical and case presentation aid. Infection Control
As a member of the ADA Infection Control Committee, sterilisation equipment and procedures are extremely important within the practice and this was one of the biggest considerations when establishing the instrument reprocessing area. Dr Draper has fitted out his sterilisation room with impressive stainless steel worktops which ensure an easy to maintain hygenic working environment.
Disinfector with integrated reverse osmosis and active drying system; one of the first of its kind in Australia, allowing the practice to provide an optimal standard of infection control. The practice has opted for Zirc EZ-Jett cassettes for instruments and the EZ Jett 10 for restorative and periodontal kits. As well as Hu Friedy cassettes as minor oral surgery trays. Coupled with a Mocom Futura 22L steriliser, this allows for the highest level of efficiency, convenience and reliability. Dr Draper Expressed “We can process up to six of the EZ-Jett 10 cassettes or three of the larger oral surgery trays in one load on a B-134C cycle in about 55 minutes.” This same philosophy and emphasis on infection control is evident throughout the practice, with wall mounted sink basins and cabinetry to allow for simple cleaning and disinfection, whilst providing a professional, highquality finish to the surgeries.. Other Equipment
Odontologica has the facilities to capture in-house panoramic x-rays using their Planmeca ProOne OPG machine. Andrew stated “I have found this to be a huge benefit with the bitewing and paediatric programs being great options when you need to make a radiological diagnosis with challenging cases for which normal options aren’t the best fit, the imaging is fast with great clarity. Being able to make an accurate diagnosis immediately, on-site, every day of the year is a great aid to the clinical and business aspects of the practice.” The practice’s plant room was one of the first in Australia to be supplied with Durr Dental’s fully integrated Tyscor Pulse monitoring system which allows the user to monitor the status of the plant room equipment to ensure peak performance is delivered to the surgeries.
