Selling a dental practice
5 Steps to an optimised outcome
Dr Padma Gadiyar developed her own formula that has helped dentists to create multi-million dollar practice empires.
“
“
In the last 5 years there is a marked increase in the number of corporates buying dental practices in the Australian marketplace.
H
aving been a clinician and multi-practice owner myself, preparing your practice for sale and creating an exit plan can be a very sensitive topic to many dentists. Thoughts of selling your practice can be filled with emotions, doubts and fear (as well as possibilities of a great lifestyle and enjoyment). Doubt creeps in when you are second guessing if you are doing the right thing, if the timing is right. There is also fear about letting go of something you have built and what you would do after selling. For these reason preparing your practice for sale correctly doesn’t always top the priority list, especially when dentists are busy buying or growing a practice. However, starting with the end in mind will not only benefit the business when preparing for sale, but it will also lead to greater innovation, planning and forward thinking. It will give you a lateral view of creating more wealth, adding more value to your clients, your employees and creating a legacy. In order to get the best outcome for the sale of your dental practice, you will need to understand the current landscape and utilize the five key steps to an optimised outcome that I have outlined below. The lifestyle awaits of travel, golf, spending time with family and friends if you do.
Overview of the current landscape In the last 5 years there is a marked increase in the number of corporates buying dental practices in the Australian marketplace. They are looking for 3 - 4 year old practices and will ensure that no single clinician’s contribution will be a majority of the gross revenue of the business. At a bare minimum, no single clinician can gross over 50% of total practice revenue.
There is also a new breed of hungry buyers entering the marketplace, who want to buy good practices with growth potential and patient base. For more information in regards this particular topic, please see my article featured in the Yahoo Finance website (https://finance.yahoo. com/news/professional-practice-sales-providesopportunities-191900288.html). These buyers are happy to pay you a premium price for your years of hard work, providing that you take advantage of the opportunity correctly. I believe in keeping my practice ready for sale each day every day, which helps me be on top of my game every time. The first question you need to ask yourself is: Who am I selling my practice to? This question is highly relevant because whether the new buyers and you will be aligned and have a good working relationship is critical as most of the buyers want the existing owner or practice principal to stay and do it over the next 6-24 months. Finding a great broker who has their finger on the pulse of the Australian market place is imperative to asking this question. This can be a challenge some brokers are stuck with conventional way of selling. Their focus is commission (usually between 4-7%) rather than getting the best price for your practice, the best buyer match or helping you build a multi-million dollar practice empire. Unfortunately for this reason over 60% practices take over 6-18 months to get sold. In saying that, it’s not totally their fault. The reason for this is LACK OF CLARITY on what the seller wants for the practice and their overall goals. The right strategy from the right consultant is critical because a consultant will not only help you get the practice ready for sale but also help get your practice sold quickly to the right buyer with minimum headaches. So get someone to help you.
Article: Selling a dental practice
S
tep 1 - Get a market appraisal done 2-3 years before thinking of selling. This is to understand where you stand today and what you need to do in order to achieve the desired result. It also gives you an idea on the current market trends and prices.
Step strategy to a chieve th e best outcom e
Step 2 - Talk to your accountants and legal team to get your tax structures and contracts and leases right. They will also have to advise on how to invest the money that you generate from the sale wisely so you make the most out of it (instead of the tax office). Make sure your accountants and lawyers have a sound knowledge and involvement in the dental industry and have the expertise to guide you. Have a discussion with your key decision makers and advisors on your plan. Step 3 - Look into upgrading the interiors of the practice. New lights, carpets, paint and new trendy dÊcor material is low cost investment and will give a rejuvenated look. Step 4 – The new breed of buyers will investigate your digital presence. Have a good website and an active Facebook and Instagram pages as a starting point.

patient base, the reputation that the practice has earned over years.
Step 5 – Plan, implement a document management system. Your system should include infection control, equipment service, getting new patients, customer service, phone call conversations. It doesn’t need to be perfect. Involving your team in succession planning to give them time and freedom to decide their plans too. You may be worried they would leave, however in my experience they are much more comfortable and supportive when you discuss your plan with them. This step also broadens your markets for non-dental buyers too who can just run the business.
What about corporates then? They offer a lot of money and multiples of 4,5 or even 6 times the EBITDA. Right!!! Of course, they do and then they lock you into their contract to work for them and keep the same gross and profitability for next 3-5 years. Most corporates are looking at practices with 3-4 chairs, 250k EBITDA (Earnings Before Income Tax, Depreciation and Amortization) it is an approximate measure of a company’s cash flow based on the company’s income statement. Just think for a moment: most of these corporates are owned by Private equity firms, law firms, investment bankers, accountants etc. If they did not see a potential in the dental industry would they invest in this healthcare business? They are just paying your profits in advance and securing your cash cow to produce golden yield for their future. Your years of hard work needs to be well rewarded. If you value it the right way you will get an outcome you desired to achieve.
The most common exit options 1) Selling your practice to an individual buyer (often a dentist looking to step into practice ownership) 2) Acquisition or merger with a corporate and work for them for another 3-5 years. Usually you get paid in the multiples of 4-6 times 3) Bring in a partner, a long term associate or third party. This can also be a part of staged sell out.

Most buyers are looking for a practice with high growth potential and good rate of return. Once you have ticked off all the 5 steps, the question is: what is your practice worth? You can get an appraisal done by a niched dental broker or a registered valuer with experience in dental industry or even a dental accountant. I am sure you are eager to know this (If it’s an established practice goodwill plays a very important role). By goodwill we mean the name, the established loyal

Traditional dental operator chairs cause the spine to curve into a “C” shape, placing strain on the lower back.
T
hose involved in clinical practice, be it as a dentist, hygienist or assistant are prone to back issues. These include lower back and neck problems which relates to how we position ourselves while rendering treatment. Differences can be denoted between those rendering treatment (dentists and hygienists) and those assisting during treatment based on how they tend to position themselves. Hayes reported in a systematic review that the most prevalent regions for pain in dentists have been shown to be the back (36.3-60.1%) and neck (19.8-85%).1 A meta-analysis reported a combined prevalence of 53.90% for thoraciclumbar pain, 41.23% for lower back pain and 41.39% for neck pain reinforcing the issues those treating patients in the dental field are subjected to. 2 The musculoskeletal pain mechanism has been attributed to poor ergonomic work posture and position adopted by dentists in relation to their patients. 3 The prevalence of musculoskeletal disorders among dental personal is high and a critical need exists to enhance awareness regarding correct working postures. 4 Why do we have a high tendency towards lower back and neck pain? Most practitioners subconsciously tend to lean towards the patient when rendering treatment to improve visualization.
Figure 1: Curvature of the spine when using a traditional flat chair forms a “C” shape, creating strain at the lumbar region.
Additionally, the practitioners tends to rotate and bend their head to better visualize the maxilla, instead of better positioning the patient and using indirect visualization (mirror) is a frequent occurrence. As the practitioner ages and it becomes more difficult to see small items they tend to lean in further placing more strain on the neck and lower back. Use of loupes helps improve visualization but also forces the practitioner to sit more upright due to the focal distance of the loupes. When the practitioner wearing loupes leans in closer to the patient the field of view blurs (goes out of focus) subconsciously forcing them to sit in a more upright position to bring the subject into focus. Unfortunately, most dental chairs due to their design still allow the practitioner to lean forward while rendering treatment. This causes stress to be placed on the lower back, the spines weakest area. Traditional dental chairs cause the spine to curve into a “C” shape placing strain on the lower back. (Figure 1). This unnatural curvature of the spine increases stress at the 4th and 5th lumbar disks, the classic trouble spot in the back. The ideal angle between the spine and hips is 45-50 degrees before strain is noticed in the lumbar region. Use of a flat chair tends to create an angle of 90 degrees causing the back to form the “C” shape to compensate leading to the tell-tale lumbar issues.
Figure 2: Curvature of the back while utilizing the Bambach saddle chair places the spine into a natural “S” shape, relieving
Taking the backache out of practicing
Figure 3: The Bambach saddle chair is available in three configurations: assistant with arm rest (left), doctor/hygienist (center), and front desk (right). A foot ring may be added to the assistant’s chair if desired.
A less stressful alternative:
Conclusion:
An alternative to the flat traditional dental chair, is the Bambach Saddle Seat. Saddle chairs help preserve the natural “S curvature of the spine removing strain from the lumbar area. (Figure 2) Additionally, due to the seat shape the spine-hip angle is kept at an optimal 45 degrees allowing back and thigh muscles to be at their most relaxed positions. Utilization of the Bambach saddle chair was shown to maintain an acceptable working posture which may reduce the development of work-related musculoskeletal disorders.5
Doctors and hygienists spend a large portion of their day seated rendering treatment. Over time stress and strain in the lumbar region and also the neck can lead to degenerative changes that can affect not only how we practice but the quality of life both in and out of the operatory. The Bambach chair positions the operator in the correct position, with optimal angle between the spine and hips relieving stress in the lumbar area. Less physical stress when rendering treatment results in a better health especially as we age.
Additionally, with proper posture during treatment stress and strain is taken off of the shoulders. When utilizing a flat chair, the operator while leaning forward will elevate the elbows while working. This placing increased stress on the shoulders which is transmitted to the neck leading to orthopedic issues over time in this area and fatigue after shorter periods of work. Whereas, with the Bambach chair the operator is seated more upright with the elbows positioned closer to the body in a more natural position. This takes strain off the shoulders and fatigue during long treatments or busy days so is not felt as readily as with flat chairs.
References:
Typically, when seated in a flat chair, the operator tends to position themselves so that their knees are at the same level as the hips (horizontal and parallel with the floor). Proper positioning in the Bambach chair so that the hips are at the optimal angle requires that the knees be positioned slightly lower then the hips to create an angle with the horizontal plane. This aids in removing loading of the lumbar region. It is also recommended that if the practitioner is planning on changing their loupes or acquiring a set, that the working distance be measured while seated in the Bambach chair and the patient ideally positioned. This will ensure the ideal working distance.

Is your practice compliant?
How to get your dental practice compliant: The 4 Components of Compliance
Written by Megan Sharpe, General Manager – Practice Solutions ANZ Prime Practice Compliance
Is your practice compliant?
I
nfection control breaches should never ever be taken lightly. With most practices achieving an average of 77% compliance rate when audited, the risks are very real and concerning. Dental practitioners are required to comply with all Dental Board of Australia and Dental Council of New Zealand policies and guidelines. This includes ensuring mandatory infection prevention guidelines are fully implemented in your practice. This obligation cannot be delegated – responsibility to complete the task may be, but ultimately, the accountability remains with the clinicians. Therefore, it is imperative that all clinicians are always familiar with all elements of infection prevention protocols required. To help prompt you to see if your practice is compliant, here’s a quick overview checklist of what you must do to be compliant.
1: Documentation Every practice must have the following documents in either hard copy or electronic form (includes guaranteed access), and all dental practitioners must have access to these documents. Infection Control Manual Your practice must have/ develop an infection control manual that includes detailed information about procedures specific to the daily routines at your practice. It provides a reference point for all members of the dental team and should also be used for training all dental staff. This manual should be a comprehensive document that contains all your Infection Control policies and procedures that is aligned with current guidelines. Visit the link below to find out a comprehensive scope of what your policies and procedure manual must include. http://articles.primepractice.com.au/blog/ what-should-your-infection-prevention-policyprocedures-manual-include In addition to your practice’s Policy & Procedure Manual, there must be access to the following documents in your ‘IP library’ at the practice:
»» The Australian Dental Association Guidelines for Infection Control – Third Edition »» The Australian Guidelines for the Prevention and Control of Infection in Healthcare published by the National Health and Medical Research Council »» AS/NZS 4815:2006 Standards and/or the AS/ NZS 4187:2014 Australian/New Zealand Standards It is important to keep in mind that your manual and all associated documents should not live on bookcases gathering dust. In fact, they should never be in mint conditions at all. Instead, a ‘well loved’ manual should constantly be reviewed, implemented and used in training, onboarding new team members, regularly reviewed and regularly updated accordingly. When updated, a Staff Register document should have signature and dates indicating a routinely update process to ensure each team member adheres to all protocols of being compliant. Need help identifying if your practice’s documentations are up-to-date and implemented correctly? Visit the link bellow to find out more: https://compliance.primepractice.com.au/manualspolicy-procedure-manuals/#inpagecontactform
2: Validation of Sterilisation Validation is “a documented procedure for obtaining, recording and interpreting the results required to establish that a process will consistently yield a product complying with predetermined specifications”. Essentially, it is the recipe to produce the same result every time. Because sterilisation is an entire process, not just a single event, all aspects of the process must be validated; not only the steriliser and the loads passing through it, but also the cleaning and packaging procedures prior to sterilisation, and the subsequent storage of sterilised items. In order to ensure appropriate sterilisation of items in the surgery, your dental practice must thoroughly understand what a validated process means and how to fully implement a validation process.
THE
4
COMPONENTS
This process can be outlined in four relevant steps: Step 1: Establish protocols and procedures Step 2: Determine the ‘cold spot’ in the sterilising chamber (Empty Chamber Study / Heat Distribution Pattern Test) Step 3: Validating the sterilising process Step 4: Implement a QA program.
3: Infection control management plan Each practice must also have an Infection Control Management Plan (ICMP) – a documented plan to prevent or minimise the risk of infection in relation to declared health services for everyone who has contact with your practice. This includes your patients, staff and team members, and all other persons who has engaged or come in contact with the practice.
» Details about the provision of training in relation to the ICMP for your entire team at your practice » How often the ICMP is to be reviewed (this must be reviewed at intervals of not more than one year) » The name of any person who is responsible for providing advice about and monitoring. The effectiveness of the ICMP. Your ICMP should always be accessible to all staff members and also written in a way that is easily understood by everyone engaged at your practice.
4: A quality assurance program Continuity of quality care can only be assured if all staff undertake and perform tasks in the same manner – Thus it is an important aspect of successful infection prevention management to be performing regular assessment/audits to ensure that this is the case.
Your practice’s ICMP must detail out: » The infection risks associated with the provision of declared health services » The measures to be taken to prevent or minimise the risk of infections » How the operator is to monitor and review the implementation and effectiveness of those measures 40 || EQUIPMENT SOLUTIONS
A comprehensive quality assurance program should be implemented to support and demonstrate a “checking, proving and recording” methodology in your practice. Use a series of checks and balances to ensure the policies and protocols outlined in your practice manual are being implemented accurately.
Henry Schein NSW Showroon has an entirely section dedicated exclusively to Steri Solutions and compliance
There’s a difference between “thinking” that your team is doing what they should be, and “knowing” that they are indefinitely doing so. Choose and appoint an Infection Control Coordinator. Give them adequate training and support. Empower them to be successful in this expanded function their role. Utilise an Online self-assessment / audit tool; preferably one that formats a comprehensive Audit Report and provides recommended corrective actions for implementation as applicable where “semi” or “non-compliant” actions have been observed. Visit the link below for an Online self-assessment: Visit: https://compliance.primepractice.com.au/ onlineselfassessment/ Alternatively, you could also use an existing Quality Assurance checklist and do so manually – just keep in mind that your manual checklist needs to address all aspects as outlined in your written policy and procedure manuals. Be sure to keep all completed Quality Assurance Reports on file to support audit processes and show documentation of any ongoing quality assurance programme. As Compliance is an organic process, the four components mentioned above should always be reviewed regularly and updated.
Regulatory obligations state that dental practitioners must practice in a way that maintains and enhances public health and safety by minimising or preventing the risk of the spread of infectious disease. It is also required that each practice provides regular infection prevention training at least every 12 months. Do not compromise on your own health and safety. Be able to reassure your patients, your team and your own self that you are safe. Get compliant now.

Ultrasonics vs. Medical Grade Washer Written by Louisa von Heynitz; SCICAN Product Manager, lheynitz@scican.com
I
n 1916 The Medical Summary journal reported, “The possibility of the transmission of disease through the medium of dental instruments has probably been considered by every occupant of the dentist’s chair.”1 One can only assume that healthcare workers were considering that same risk throughout their day at work. Within a dental clinic, one of the most potentially hazardous steps in the reprocessing cycle is the cleaning of contaminated instruments; and yet, in 1976 instruments were still only cleaned manually with a short handled brush, sometimes only disinfected and not sterilized, and then stored unwrapped.2
It wasn’t until the late 1980’s that Occupational Safety and Health Administration (OSHA) recognized the hazards that healthcare workers were being exposed to and released the first mandatory standard infection control procedures for dentists. This was then followed by the issuing of the final Mandatory Bloodborne Pathogens Standard in 1991. While these were critical steps in protecting healthcare workers from hazards faced in their occupation, OSHA’s sole focus are the employees and their standard does not address patient safety. Thus, to protect both the patient and the healthcare workers, the Centers for Disease Control and Prevention (CDC) issued their first Infection Control Recommendations for dentistry in 1993, and subsequently the first Dental Infection Control Guidelines in 2003.3 Thankfully infection control guidelines have come a long way since then and, as patients, we trust that everything is being done to protect us from infections and diseases caused by improper reprocessing and cross contamination. The patient’s safety and that of every healthcare worker in the dental office, needs to be the number one priority. There are two methods to clean dental instruments:

manual and automated cleaning. Manual cleaning of instruments is not recommended because of the risk it presents to the healthcare worker. In both the 2003 guidelines and the 2016 Summary of Infection Prevention Practices in Dental Settings, the CDC recommends using automated cleaner such as ultrasonic cleaners, or medical grade washers “to improve cleaning effectiveness and decrease worker exposure to blood.” 4 A 2016 study done by the Dental Advisor showed that ultrasonic solutions can become up to 70 times more contaminated over the course of the day when the prepared solution is used several times to clean instruments. Furthermore, the average microbial contamination at the beginning of the day was 2190 cfu/mL (Table 1), which is 83% higher than the EPA regulated microbial levels for potable water. This finding suggests that even when changing the ultrasonic solutions regularly there is still residual contamination from previous runs, requiring the ultrasonic chamber to be cleaned thoroughly at the end of each work day.5 A medical grade washer, on the other hand, uses fresh water and cleaning solution for each cycle. In addition some medical grade washers, such as the SciCan’s HYDRIM units even have a self-cleaning cycle that can be run periodically.

Figure 1. Microbial contamination of ultrasonic cleaning solution
When using an ultrasonic there are still many steps involved in the preparation of instruments, such as pre-soaking and scrubbing instruments to remove any gross debris, loading, unloading, rinsing, inspecting, sorting, and drying the instruments before preparing them to be sterilized.6 All these steps involve handling the contaminated instruments thereby increasing the risk of being exposed to blood and body fluids, as well as the risk of sharps injuries. Alternatively, when using a medical grade washer, the process only involves loading, unloading, inspecting and preparing the instruments for sterilization. Ultimately a medical grade washer is the safer and more effective option, greatly decreasing microbial contamination and the risk of exposure and injuries. A medical grade washer is also more time efficient. Medical grade automated washers such as the HYDRIM, were designed as a solution to improve the dental office’s business operations and protect healthcare workers and patients. For example, the HYDRIM C61w G4 fits six full-size instrument management cassettes, allowing an office to clean and dry 120 instruments within only 36 minutes. According to DentistryIQ, “the time saved by automated washing compared to ultrasonic cleaning is approximately one hour of labor for every nine procedural set-ups.”
HYDRIM C61w G4 shown open and a HYDRIM L G4 shown closed.
Not only are medical grade washers like the HYDRIM time efficient, but that time savings translates into cost savings. DentistryIQ calculated, “The increased efficiency translates to the HYDRIM C61w G4 paying for itself in about 38 months;”7 depending on the number of operatories in the office. Having a medical grade washer means that the time spent completing the many steps involved in ultrasonic cleaning can instead be used to complete other pressing tasks. Another benefit of medical grade washers is that most have the ability to electronically log your cleaning cycle data. For example, SciCan’s G4 technology’s DataLogger automatically stores all of the cycle data for the lifetime of the unit onto a USB and online to be accessed at any time. This feature saves the office time by removing the need to manually log cleaning cycles. Moreover, it provides the office the peace of mind of always being prepared for any audit. Adding a medical grade washer to your office means that you can rest easy knowing that your patients and staff are safe, that you are meeting your region’s infection control and prevention recommendations, and that you are saving money. Safety is priceless; a medical grade washer is a worthy investment for today’s standard of care.

Data Security and what you need to know....
On February 22nd the country’s first data breach notification law dubbed the Notifiable Data Breach (NDB) scheme came into effect.
What does that mean for my Practice?
Under the Act, a notifiable data breach is a data breach that is likely to result in serious harm to any of the affected individuals. The Act defines a data breach as occurring when any personal information held by an organisation is lost or subject to unauthorised access or disclosure. Under the new law, any organisation or business with an annual turnover of $3 million (or private sector health service providers) are obliged to notify individuals whose personal information is involved as soon as possible after becoming aware of a breach.
Data breach myth busted..
There is a huge misconception that hackers are the number one cause of data breaches. This is mainly because we are often exposed to media reports about large hacking cases, such as the Sony hack in 2014 where hackers stole huge swaths of confidential documents before leaking them online. The actual leading cause of data breaches -- and often the most detrimental because it can disrupt business continuity -- has been identified as unintentional human error.
What steps can I take to ensure we are protected?

Dr Anikó Ball used to bend and twist over her patients for decades, unaware of her harmful work postures and wondering why she was suffering chronic pain in her neck, back and left shoulder.
What we didn’t learn in dental school As dental students, we were highly trained in diagnostic and clinical skills, but there was no training in how to organize our bodies to gain access to our patients mouths without compromising our musculoskeletal health. As a result, we bend and twist over patients in any which way, without paying attention to ourselves.
Biomechanical Design Although there are fundamental biomechanical design principles governing movement and posture, we are not trained in how to co-operate with our design and we are not provided with a “User Guide” at birth. Anyone who drives a car knows what happens when you don’t co-operate with its mechanical design. Speeding along a freeway in first gear or going up a steep hill in fourth gear results in engine damage and costly repairs. We receive training in how to co-operate with the car’s mechanical design when we have driving lessons, learning to drive in the correct gear. We don’t receive training in how to drive ourselves in the right gear.
Cumulative Trauma When our posture and movement patterns are not co-operating with biomechanical design, cumulative trauma occurs.
Soft tissue damage happens over time at a cellular level, when forward and downwardly directed, unilaterally collapsed dental work postures are held for prolonged periods, as we do in dentistry. Repetitive and prolonged twisting of the trunk results in spinal compression and inefficient use of muscles and joints. This is the reason why there is such a high incidence of chronic pain in the dental profession. Pain and discomfort are late warning signs of cumulative trauma.
Dental Ergonomics Ergonomic research and design have focused on dentists’ work environment - the Outer Ergonomics, overlooking the importance of their efficiency in posture and movement - the Inner Ergonomics. How a dentist sits on a stool and bends over the dental chair is as important as the stool and chair design. The way instruments are held is as important as the shape and the weight of the instrument. Although ergonomically designed surgery layout, dental chair, stool, equipment and instruments are important, without knowledge of your Inner Ergonomics (understanding how your body is designed to work) you are literally driving yourself in the wrong gear in clinical practice, at a desk and in everyday activities.

Body Map Update
A
lthough anatomy is part of the curriculum at dental school, functional anatomy is not taught. Functional anatomy is essential for understanding in a practical way how the body works in posture and movement. We move according to a mental picture of our body – the Body Map. Unfortunately, most of us have faulty body maps, as these were formed when we played with teddy bears and toy soldiers which have anatomically incorrect joints. Incorrect mapping of joints results in harmful posture and movement patterns. You need to know where your joints are and how they work, not just intellectually but as a practical blueprint for posture and movement For example, if you map your hip joints incorrectly (the hip joints are situated lower and more medial than most people think) then you are bending forward over the patient at the lumbar spine, which is not designed for such a movement.
68 || EQUIPMENT SOLUTIONS
If you are looking at your mobile phone, with your neck bent at the level of the chin, which is where most people point to when asked where they think their head sits on top of the spine, you are asking the cervical spine to perform a task that is contrary to its biomechanical design. Such repetitive strain over time results in intervertebral disc damage and chronic back and neck pain. Updating your body map with correct data on where the atlanto-occipital joint (a rod going through the ears marks the level at which the head nods) and the hip joints are, enables co-operation with biomechanical design. Flexing the head and trunk at the actual joints reduces the risk of pain. Recruiting the appropriate joint for the task, for example bending the arm at the elbow (hinge joint) to bring a mobile device closer, instead of bending at the cervical spine (gliding joints), also reduces chronic pain.
The Principles of the Alexander Technique
Tips for a long dental career
Suffering chronic back and neck pain for over thirty years as a dentist, I tried all available medical and physical therapies. Unfortunately, all of them focused on treating my symptoms without dealing with the cause of the problem and resulted in short-term relief.
»»
When I finally came across the Alexander Technique, a method for consciously altering habitual postural behavior, I realized I was driving myself in the wrong gear. I was bending and twisting my spine, compressing intervertebral discs for prolonged periods and over-tensing muscles.
Alexander Technique and apply them in clinical
The Alexander Technique is a cause oriented, evidence based kinaesthetic method for overcoming and preventing occupational chronic pain. Practically speaking it a User Guide for movement and posture in co-operation with biomechanical design. Using the Inner Ergonomic principles of the Alexander Technique, I learned to change harmful posture and movement habits in all activities, especially in clinical dentistry. Out of a desire to contribute to the dental profession and save my colleagues from suffering occupational chronic pain, I undertook a 1600-hour part time training course at the Melbourne Alexander School and founded “Optimum Dental Posture”. I train dentists and their teams in Optimum Dental Posture Programs as well as in their own practices. I am passionate about reducing the incidence of occupational musculoskeletal disorders in dentistry. My vision is to have ‘Ergonomics & Wellness in Dentistry’ training programs included in the undergraduate curriculum in all dental schools as an intervention to reduce the risk of occupational chronic pain, to ensure long and productive dental careers and to improve quality of life.

It is hard to treat what you cannot see. This CBCT clinical case was acquired using the Morita Veraview X800. This patient experienced intense pain in the upper left maxilla. Clinical examination revealed no abnormal findings. A conventional radiograph revealed nothing to explain the patient's pain. A Cone Beam CT scan was taken and the assessment revealed a significant, previously undisclosed extensive root resorption defect on the buccal side of tooth 26. The diagnosis of root resorption could not have been made without CBCT imaging.

Cleaner than the Himalayan air
C
ompressed air is indispensable in dental practices. For instance, it ensures a long-term adhesive bond when dry blowing in restorative therapy and prosthetics. More importantly it can reduce pathogen contamination at the place of treatment and helps avoid infections. But this presupposes that the compressed air itself is completely hygienic. Simple filters alone are not enough. The following article explains the important factors in dental air. Compressed air is required in many applications in dentistry. It drives a whole range of appliances and components – from turbines in handpieces and pressure moulding machines for the pressure moulding technique up to the control of precision milling cutters in the laboratory. Not least, dental air is used for drying the treatment site directly in the mouth of the patient. But in all these areas of application various factors have to be taken into account and are connected with risks which should not be underestimated.
Water as a risk factor Compressed air with an excessive water content reduces the adhesive strength of dental adhesive. There is also a risk of corrosion of the driven instruments. In addition, moist air – especially if it is warm too – creates the ideal environment for bacteria, viruses and fungi: Alarm stage red! The dentist must always expect the presence of open wounds in the patient’s mouth. If the finest particles are also added to this, together with water and oil they can form an emery paste that acts like sand in a gearbox. It impairs the functioning of all bearings in the turbine, disrupts the air circulation in turbine heads and destroys the bearings and shaft. The cooling required for rapidly rotating components is diminished, causing the wear of many components. Even the patient can become aware of this in the form of unpleasant musty odours.
Himalayas mountains are claimed to have one of the freshest and purest air in the planet
Oil as a risk factor
Industrial solution
Simple appliances in the industry use tried-andtested oil lubrication. It reduces the wear of piston, bearings and shafts and supports the work with low-pulsation, constant air output. But this also means potential exposure to residues of oil droplets, oil mist or oil/water emulsions.
Air is used as a propellant medium in industrial compressors. It must achieve certain volume flows and pressures yet does not necessarily satisfy special hygienic requirements. Mainly the task is only in transportation, assembly, control, etc.
This potential contamination also involves several undesirable risks. The viscous lubricating oil can add to the low viscosity oil of dental turbines and, especially after partial oxidation and resin deposits when exposed to heat, the gaskets in turbine heads can stick. This leads to total failure of the turbine. Any oil film in the patient’s mouth will prevent the adhesion of composites. which can result in the failure of the restorative therapy.
Stricter conditions are imposed by the foodstuffs or pharmaceuticals industries – and, naturally, the most restrictive regulations must be observed where air is used as the working medium for treatment in immediate contact with hard and soft tissues directly on the patient.
Article: Compressors for the dental market
Cleaner than the Himalayan air
Durr Quattro is the ideal solution for dental laboratories and large dental practices
The dental solution Dental compressors typically have an integrated air drying process. Modern membrane drying systems work the most efficiently. Firstly they bring the compressed air to room temperature in a cooler. Next the air flows through thin membrane fibres that further dehumidifies it. Dry purging air on the other side of the membrane fibres absorbs more moisture. This means that three times more water is separated than in conventional systems. Furthermore, compressors with conventional drying systems can only utilise around 50 percent of their capacity as the drying medium must be regenerated at regular intervals. This is not necessary in the membrane-drying unit – the compressor can run non-stop 24 hours a day. A membrane-drying unit provides dependable, highquality dental air at all times.
90 || EQUIPMENT SOLUTIONS
In the case of conventional air drying, the compressed air is cooled first, e.g. via a cooling coil. The moisture that condenses out is “flung to the edge”, as it were, in a cyclone chamber by specifically targeted turbulence and pulled downwards into a separation tank by the force of gravity. Residual moisture is typically eliminated by an adsorption agent. This is regenerated in the shut-off phase of the compressor. Currently, there is not a single compressed air system working on the basis of oil-lubricated pistons that can guarantee 100 % oil-free air. The dental solution can, therefore, only be an oil-free compressor. In such “self-lubricating” systems, the pistons move in finely machined cylinder bores. The piston rings are made from a special compound that is distinguished by outstanding sliding properties. And where there is no oil, it cannot affect the function of a dental turbine or the adhesive strength of an adhesive.
The “Quattro Tandem” model, that produces its enormous output from two units with a total of eight cylinders, is particularly suitable for larger dental practices and dental laboratories. For clinics, Dürr Dental offers equipment with compressed air stations for 30 to 50 treatment places and beyond that if necessary. Such systems are running, for instance, at the universities of Prague, Toulouse, Santiago de Chile, Madrid and Valencia – to name The Power Tower: all in one Hygienically perfect but a few. A Power Tower combines the compressor generator, the As an option, Dürr Dental compressors can be equip compressed air tank, the suction unit and, depending on

Article: Cattani After Care
If plant room equipment could talk, it might say:
What About Me
What About Me ?
Which is why Cattani created After Care The scheduled servicing programme that gives equipment the attention it needs
O
utstanding performance and reliability – it’s a dream combination. Once found, we don’t want to give it up.
And yet, with machinery, performance and reliability are never a given, however well designed it should be. By its nature it is subject to wear and tear. Treat it well and the result is most likely better performance and longevity. Forget to look after it and it will eventually let us down – or did we let it down? No clinician wants essential equipment to fail midway through a procedure, or in fact at any time that causes inconvenience – it’s bad for patients, bad for business. So they have arrangements with manufacturers, dealers and service technicians to maintain and service equipment, thus preventing those disasters and protecting their reputations. Which brings us to an anomaly – that crucial cluster of plant room equipment, usually out of sight and therefore often out of mind, tends to get overlooked. Many technicians are proactive in caring for this equipment, but often it depends on the practice to remember and organise regular servicing. This uncertainty worries manufacturers, who fear that their equipment, through lack of servicing, will let down the owner. It’s a scenario that troubled plant room equipment manufacturer Cattani Australasia so much that they decided to develop their own structured and comprehensive servicing programme from scratch. They called it After Care, and it covers a wide range of their suction systems and compressors, ensuring that this equipment is cared for annually according to a predicted lifetime programme of servicing and maintenance. Not unlike a car servicing programme, it includes a specific set of parts and steps for each servicing, depending on the age of the equipment.
Developing After Care has been a major focus for Cattani Group CEO Greg Fox. “We’re a well-established family company with a good reputation, but of all a company’s assets, reputation is probably the most precious after a quality product. It’s beholden on me to do whatever I can to protect and enhance that reputation. To be able to do that by basically insuring our products against failure, and in that way giving our customers one less thing to worry about, means I’ve done my job and we all win.” After Care has become a pillar of Cattani’s business model. It figures prominently in training courses for dealer technicians and there’s a strong marketing focus, using the theme, “Smart Move”. Much effort has been put into gaining the support of dealers and supporting them with explanatory material and quality resources befitting such an important part of the business. “I’ve been delighted at the response from dealers, but at the same time not surprised because in a lot of ways it’s not really smart, just plain common sense. All of us – clinicians, dealers and manufacturers – have reputations to protect, so anything that intercepts problems before they occur is going to be a no-brainer. “I’m sure end users understand that any hardworking machinery with moving parts will eventually wear out, so we hope that timely replacement is part of their capital programme,” says Greg. “After that it’s up to us and our products to meet and preferably exceed expectations. “No product is perfect, however, so if there is a problem despite everything we have done to prevent it, Cattani has an incredibly knowledgeable and responsive technical team at the ready. It’s another point of difference and one I’m very proud of – one that also keeps us on track towards our collective goal of 100% reliability.”
