perfect for CAD/CAM

For dentists using CAD/CAM technology, having an open, dry, illuminated field in which to work is essential. The Isolite system is a perfect tool for providing such an ideal environment keeping the oral cavity “wide open and well lit.� The Isolite aids dentists in their CAD/CAM operations by enabling quick, easy isolation. It provides continuous illumination, evacuation, and retraction in a single device. Dr Thomas Hirsch, Director of Clinical Relations and cofounder of Isolite Systems believes that his ability to place restorations in one visit makes his patients more compliant, because they are much more likely to get a procedure done if they only have to come to the dental office once. Since incorporating CAD/CAM into his practice, Hirsch has found that the Isolite and CAD/CAM technology are an excellent fit for each other. For example, some CAD/CAM systems will not take images if any movement in the frame is detected. The Isolite solves this problem by ensuring

that the tongue and cheek are securely retracted and do not obstruct the view of the teeth. Having a dry, open field also allows clinicians to capture digital images of upper and lower quadrants in one fell swoop, which can dramatically cut procedure time. Additionally, using Isolite dental isolation relieves the clinician of the time-consuming, and sometimes arduous, task of having to place a rubber dam. The Isolite can be connected and operational in only a few minutes. The super-soft, single-use mouthpieces used with Isolite make for a more comfortable experience for the patient, and provide significant safety advantages as well. Isolite mouthpieces are latex-free, and are available in six sizes to fit the spectrum of patients from paediatric to large adult.

GC Cerasmart The perfect alternative for cases of erosion and restorations on implants The development of CAD/CAM technologies in the laboratory has not only improved the comfort and quality of work but also made it possible to work in a more standardised way. New materials such as hybrids have opened up alternative ways to treat complex and difficult cases. One of these materials – Cerasmart – has proven to be very effective in those indications. In this article, Christian Thie, CDT, explains why he chooses to use this material. Why and when do you use CAD/CAM restorations in your laboratory? And what do you think are the additional values of CAD/CAM restorations compared to conventional prosthetic treatments? Most laboratory work can now be done using CAD/CAM technology, thanks to the great range of systems and materials available. It is now easier to plan and design using CAD/CAM, especially large, complex restorations that take considerable skill, time and effort to be modelled by hand. This makes it possible for us to achieve better fit and adaptation than with traditional methods, which are more prone to uneven quality. The results with CAD/CAM are now much more predictable. In which cases do you choose to use Cerasmart restorations? Eroded teeth that also present a loss of vertical dimension require a restorative material with high flexural strength. But if the material is too hard it could be detrimental to the antagonist teeth as well as the TJM because of the high masticatory load. To treat those cases, the choice fell naturally on Cerasmart, a material that can buffer masticatory pressure, inducing little volume wear on the antagonists, but that still has sufficient flexural strength to prevent reduction of the vertical dimension. Patients with complex restorations on implants often present a difficult occlusion with only part of the antagonists still in place. Cerasmart is a particularly good alternative in these cases because it produces monolithic restorations with constant physical properties and reduced chipping.

What are the advantages of Cerasmart in terms of indication, handling and aesthetic outcome? As mentioned, the choice of indication is dictated by the physical properties of the material. Furthermore, Cerasmart is etchable and can therefore be cemented adhesively. In terms of handling, the material is easy to work with using diamond burs and rubber polishers. A high gloss can then be obtained with DiaPolisher Paste. As for aesthetics, Cerasmart has proven to be a very versatile product. This hybrid material can be designed to be very thin if necessary and, depending on which aesthetics are wanted, different finishing options are available: it can be polished in the original monolithic shade or individualised with GC Optiglaze Color, or even layered with composite using a cutback technique. Tips on how to make the use of the material easier To characterise restorations, the surface texture can be created with diamond burs followed by the application of Optiglaze Color. Alternatively, Cerasmart restorations can be created in a monolithic way. Here, the polishing is done with goat-hair brushes and DiaPolisher as the polishing agent. The high-gloss finish remains after steam cleaning. Thanks to its unique physical properties, Cerasmart is a perfect alternative to ceramics in many difficult cases. Used with Optiglaze Color, it also becomes a very effective and aesthetic alternative that constitutes a new long-term solution for many clinical indications.

Secure bonding of CAD/CAM materials: A compact

clinical guideline

Dr. Julián Conejo, Dentist | Philadelphia, USA For the clinical long-term stability of full ceramic restorations, a resilient and reliable adhesion after adhesive fixation is essential. However, different materials must be conditioned and adhered appropriately for the material. Dr. Julián Conejo (University of Pennsylvania, School of Dental Medicine, Philadelphia, USA) explains step-bystep the material-specific protocols for feldspar, glass, hybrid, and zirconia ceramics and demonstrates the differences.

Selection of the bonding system "An adhesive bonding is indispensable for feldspar and hybrid ceramics," according to the recommendations of clinicians. Although full crowns of zirconia-reinforced lithium silicate glass ceramics and zirconia can also be conventionally cemented, Julián Conejo recommends this procedure only in the case of a retentive dissection form and in treatment situations with suboptimal drainage. Insufficient composite and amalgam filling of tooth 16

Conditioning the tooth substance Julián Conejo performs the clinical conditioning steps as follows: "If the dissection is mainly in the enamel, I allow the phosphoric acid to act for 20 seconds. After that, I apply the adhesive components." If the dissection is in the dentine, he recommends a self-etching bonding system. "No phosphoric acid is required for these adhesive systems, which prevents sensory disturbances after the adhesive conditioning," says Julián Conejo.

Conditioning the restoration "To obtain friction, feldspar and hybrid ceramics must be etched with hydrofluoric acid for 60 seconds, and zirconia-reinforced lithium silicate glass ceramic for 20 seconds," reports the dentist and scientist. He further explains, "A rough surface of the ceramic restoration is necessary for a good micromechanical retention. If this step is omitted, it can lead to adhesive failure." The silane bonding agent should be massaged in for 60 seconds to achieve a chemical bond to the restoration material.

Protocol for zirconia Julián Conejo points out the deviating conditioning protocol for zirconia: "This material should be sandblasted for 20 seconds with an alumina grain size of 30 to 50 micrometres and less than 2.8 bar." A primer (bonding agent) with the phosphate monomer MDP is recommended for a high adhesion to zirconia.

Summary "Every step is of great importance for the adhesive incorporation of ceramic restorations," according to Julián Conejo's conclusion on adhesive bonding. The flawless surface of the restoration should be properly conditioned. The instructions for use of the adhesive and bonding systems have to be followed, and only a careful drainage of the working field allows a reliable adhesive attachment.
