Voting forms must be on original newsprint form and filled out completely. Any incomplete or incorrect ballots will be disqualified. 3. Each vote is a voluntary donation of 25 cents. Please mail forms to the Herald & Review, P.O. Box 311, Decatur, IL 62523 or drop it off at the Herald & Review office, 601 East William Street, Decatur, between 8:00am - 5:00pm weekdays.* All proceeds from the votes will go to support literacy through the Herald & Review Newspaper in Education program. 4. The Herald & Review reserves the right to make all final decisions concerning all matters of administration, procedures and eligibility. All entries become property of the Herald & Review. 5.
