Registration Form 2012

The Department of Languages Other Than English (LOTE) Instructions for filling out the ACIS 2012 Registration Form Deadline to submit form and check: June 01, 2011 1. Make sure that your child’s name is printed exactly as it appears on official documents/passport and on any future paperwork for the trip.

4. Fill out the form carefully in pen and please make sure everything is legible 5. Group Leader: Fratto 6. Group # 6330 7. Departure Date: 02/16/2012 8. Program Names: Insider’s France / Florence and Rome / Madrid y Andalucía Your child must select a trip that reflects the current foreign language he or she is studying.

9. Protection Plans can be added afterwards if you are unable to decide.

10. Write out a check to ACIS in the amount of $195.00. Registration forms and checks will be mailed to ACIS after June 1, 2011 if we receive sufficient registration forms for each trip -25 students per trip. If not: A meeting will be held if we need to combine two trips in the event that one or two of those excursions has less than 25 students; Checks will not be sent unless parents/guardians approve of any possible combined tour. OR Checks will be returned.

12. The registration form and check are to be dropped off in Room 108/110 at the Fran Greenspan Administration Center (Central Office) on or before June 01, 2011.
