Girls Just Want to Have Fun! So back in January when I asked you what you wanted more of, how many of you said fun!? This was probably the empty nesters that have spent most of their lives taking care of everyone else. Now they find themselves just wanting to enjoy life. Of course we do! And my suggestion is just do it… whatever that is. Do you want to spend more time with your kids, your grandkids, your girlfriends, your husband, your cousins, your dog, yourself!? Decide you are going to have fun in everything you do, even during everyday activities. You can have fun while driving to work with the radio cranked up and singing at the top of your lungs (Cyndi Lauper’s Girls just want to have Fun would be appropriate)! You can have fun shopping for groceries…riding the cart down the aisle and saying hello with a big smile on your face to everyone you see. Watch and listen to a video of a baby laughing! Run through the sprinkler with your kids or grandkids! Having fun can be silly or serious. Maybe it’s preparing for and running a marathon. Maybe it’s taking piano lessons and having your first recital when you are 70. Maybe it’s volunteering with hospice. Calling a friend you haven’t talked to in a long time. Use your imagination. If you aren’t sure what to do for fun, sit down with your journal and do a little 2

brainstorming. Think back when you were a child and recall all the things you dreamed about and wanted to do. Make a list of all the things you love to do and haven’t done for a long time. FIRST STEPS The purpose of First Steps is to bring together families and professionals to coordinate services to benefit the child. All families qualify for services if their child is age 0 – 3 and is experiencing a developmental delay or a diagnosed physical or mental condition that has a high probability of resulting in delays. What is a developmental delay? Developmental delay means a delay in one or more of the areas of childhood development as measured by diagnostic instruments and standardized procedures. The areas of development include; cognitive development, physical development (including hearing and vision), communication development, social and emotional development, and adaptive development which includes eating, dressing, toileting and other areas of personal responsibility. Cognitive development refers to the child’s ability to learn new tasks and activities. It is often referred to as IQ or intelligence. Physical development includes skills such as standing, crawling, walking and other physical activities, as well as grasping, reaching, touching, etc. Communication skills include both the pronunciation of words and the use of learned language. Communication refers to the ability to produce communication as well as the ability to understand what is said. Social and emotional development looks at a child’s ability to play with others, the ability to separate from parents, the willingness to explore the environment, and other activities that allow a child to be involved with their peers. A diagnosed physical or mental condition that has a high probability of resulting in delays refers to children who have genetic or chromosomal abnormalities (such as Down Syndrome), a neurological disorder (such as cerebral palsy), a congenital disorder or sensory impairment (including vision and hearing), a neurological development in the newborn period and low birth weight or failure to thrive. To qualify for services through the First Steps

program, a child must exhibit a 25% delay in one or more of the developmental areas, or exhibit a 20% delay in two or more areas, or has a diagnosed physical or mental condition is likely to result in a developmental delay. Eligibility is determined by a multidisciplinary team of professionals who are trained and qualified to administer the assessments tools and are able to interpret the results of the testing. The determination is done annually during a face-to-face meeting with the child’s family and the professionals who have conducted the assessments. Once eligibility is determined the types of services that a child will receive will be decided. The services available are wide ranging and designed to help the child and family to address the problems the child is experiencing. The services are written into an Individual Family Service Plan that provides documentation of the meeting and the decisions made. The Individual Family Service Plan is reviewed and revised on a predetermined schedule. If a child makes adequate progress and no longer exhibits a delay in development, services will be discontinued. If the child reaches age 3 and continues to exhibit a delay, he/she will be referred to the local school system so that services can be provided through the school’s early childhood program. The goal is to make the transition from the First Steps program to the public school early childhood program as seamless as possible. You can find additional information at www.in.gov/fssa.

At The Applied Behavior Center for Autism, we utilize the evidence-based practice of applied behavior analysis (ABA) to improve the lives of children and families affected by autism spectrum disorders. Research strongly suggests that intensive early intervention leads to optimal outcomes in Janine Shapiro, children with autism. CCC-SLP, BCBA As a result, most of our patients receive nearly forty hours of ABA therapy per week! The Applied Behavior Center for Autism’s highly trained therapists hold degrees in fields such as psychology, education, and communication science and deliver therapy on a 1:1 ratio. Services are funded through private insurance and many families pay little or no cost out of pocket! Below are a few areas of difficulty experienced by our patients that are successfully treated on a daily basis at The Applied Behavior Center for Autism. Language Language impairment is one of the hallmark features of children with autism spectrum disorders. We thoroughly assess all new patients using both formal and informal measures to determine their current language levels. Based on the child’s assessment and input from the family, we create a specialized language intervention program that focuses on functional language. We recognize that without an effective language system, a child is only able to communicate through undesirable behavior. Therefore, for those with little or absent language, we immediately begin teaching requesting skills. Requesting provides people with control over their environments and the ability to request often eliminates the need for a child to engage in problem behaviors, such as tantrums, to gain access to desired items and activities. Requesting supplies the tools necessary to appropriately escape and avoid undesired activities. When a child who previously was not able to ask for his favorite items learns to request, his quality of life instantly skyrockets! For children who present with higher

language skills, we teach how to accurately respond to increasingly more complex questions and instructions. These skills will serve as the foundation for future social and academic success. Visitors unfamiliar with Verbal Behavior often comment that our children appear to be engaged in an extended speechlanguage therapy session from the moment they arrive in the morning until the second they depart in the afternoon. Social Skills Social skills difficulties are another core deficit in children and adolescents with autism. At The Applied Behavior Center for Autism, our goals related to social development are twofold: to increase our patients’ desire to interact with others and to improve our patients’ social adeptness so that peers want to be their friends. Just as requesting can boost a child’s language use by illuminating the benefits of communication, it can also increase a child’s interactions with peers! By creating situations in which a child’s peer has an item that the child wants, therapists generate strong incentives and benefits for social interaction. For children with proficient language, we teach how to adapt one’s verbal and nonverbal behavior according to the specific peer(s) present and the setting. The social world can be extremely difficult to navigate, and our therapists coach patients to respond appropriately to its nuances and complexity! Problem Behaviors Many of our patients exhibit problem behaviors, often due to impaired communication skills. We perform careful assessment of problem behaviors to determine the function of those behaviors. Once identified, we develop and teach appropriate replacement behaviors across different environments. Be Honest and Give Choices Today I wanted to write about how to involve your child, preteen or adolescent into therapy. It can feel like an overwhelming task because more than likely they are not talking to you therefore you can’t imagine that they would share their feelings with a stranger. First and foremost you must demonstrate honesty with your child. That may look like “I have been worried about your anger and wanted to work with a professional to get some help and she asked me to bring you to talk about the issue” or it may sound like “I have been worried about your grades and you don’t seem motivated to do better....I know part of the problem is the way that I handle things so I called a counselor and he asked me to bring you.” You always own the problem as it is what we call a “family systems” issue. It is imperative to be honest because ultimately you are asking your child to be honest with you. Don’t be surprised if you meet with resistance because your child is already feeling different and probably does not want to be identified as what we call “the identified patient.”

Always give your child choices because this affords them some autonomy but also allows you to guide them in a direction that is helpful. Sometimes I have encouraged parents to tell their child that he will only have to meet with the therapist 3 times. This lets the child feel like he has some choice in the matter but also leaves the responsibility to the therapist to assess the child and decide how to relate with him and make him feel comfortable. Or I might tell the parents to explain to the child that during the initial evaluation all family members will be present but after the first session the child can determine whether to see

the therapist alone or with his parents. Again it becomes the therapist who will ask the child if she can occasionally involve the parents. I find that kids are much more amenable to do what the therapist asks as opposed to the parents.

This week I had a 15 year old adolescent in my office whose parents had tricked him to get him into my office. They had told him that they were going out for lunch and had to make a quick stop before they had lunch. That quick stop was me. You can imagine his dismay when they stopped in the parking lot and informed him that they were coming in to talk to me about his depression. As I walked them into the office I could feel the tension and sat down with them and assured him that I was sorry that his parents had not been honest with him. His parents sheepishly admitted that if they had asked him to meet with me he would have resisted. I quickly decided to give the teen back some power and after I got a brief history, I walked the parents out and insisted that I wanted to get to know their son because I knew that I would have to win him over without him feeling coerced. Initially he said there was no way he would meet with me, but then he described his severe depression and I decided to give him a choice. I assured him I could hook him up with a therapist who was a younger male and when given the choice....he picked me. Never underestimate the value of choice!

Advantages of In-Home ABA Therapy With thousands of success stories in the media and years of research to back it up, it can be easily argued that Applied Behavior Analysis (ABA) therapy is the best treatment for children with an Autism Spectrum Disorder (ASD). ABA therapy can be provided in many environments; however the most common settings are home-based or center-based programs. According to research, there is no significant difference between the two environments on measures of skill acquisition. In fact, with in-home therapy there is more flexibility, so more skills can be generalized to the child’s natural environment. Many families find that starting in home gives the child comfort to learn in their own space. In addition, other benefits of an in-home program are: • With in-home services families do not have to worry about the time and stress of driving their child to a clinic every day. • Home-based programs provide more opportunities for incidental learning by working in the child’s natural environment. • Parents, siblings and caregivers can be more involved in the child’s program, increasing

the child’s ability to learn and grow while having trusted adults nearby. • Home-based ABA therapy programs are more flexible with scheduling therapy hours, helping to accommodate children who are supported after their school day. • In-home therapy is a preferred environment for very young children. Many parents feel uneasy about leaving their small child at a facility for 40 hours a week. • With constant communication and training, parents can help further their child’s goals by implementing the techniques themselves. Quality in-home ABA therapy’s overall goal is to move the child into a typical peer setting such as kindergarten, making in-home therapy an ideal place for a child to begin ABA. Ultimately, the family must decide which factors are the most important to them when choosing an ABA therapy provider and setting.
