VOORWOORD Beste lezer, In dit boekje vindt u een overzicht van alle onderzoekslijnen binnen het Kenniscentrum Innovatie van Zorgverlening (KC IvZ) van Hogeschool Utrecht.

Bij het doorlezen zult u verrast worden door de grote verscheidenheid aan onderwerpen, maar ook doordat ze allemaal een gemeenschappelijke focus hebben: het bevorderen van de zelfredzaamheid van de patiënt, met het doel om zijn dagelijks functioneren en omgaan met zijn ziekte te verbeteren. Dit dagelijks functioneren is het hart van het ICF-model (International Classification of Functioning, de gemeenschappelijke noemer van het onderzoek binnen ons Kenniscentrum). Al ons onderzoek komt voort uit de overheersende vraag: hoe wordt de patiënt er beter van? Door te streven naar taakherschikking in de zorg en door de relatie tussen het onderzoek en het onderwijs binnen onze faculteit te intensiveren, willen we daaraan uiting geven.

Ons Kenniscentrum is in oktober 2013 geëvalueerd door een externe commissie onder leiding van dr. Guusje ter Horst. De commissie heeft aan ons KC het predicaat ‘excellent’ gegeven. Een prachtig resultaat waar we terecht trots op zijn.

Maar het schept ook verplichtingen. Bijvoorbeeld om te laten zien wat we allemaal doen, om onze onderzoeksresultaten te ‘dissemineren’ en te ‘valoriseren’. Daarom dit overzicht, dat ertoe moge bijdragen dat u als geïnteresseerde lezer een goed overzicht krijgt van onze onderzoeksactiviteiten.

Het doen van goed onderzoek berust voor een groot deel op goede communicatie. Communicatie met de patiënten die aan ons onderzoek meedoen, communicatie met promotoren en andere ‘peers’, communicatie met mede-onderzoekers, etcetera. De onderzoekers werkzaam in de verschillende lectoraten binnen het KC willen met behulp van dit boekje met ù communiceren over hun onderzoek. Op deze manier hopen zij bij te dragen aan een nog betere relatie met elkaar, met docenten, studenten, en met externe geïnteresseerden en stakeholders.

Mariska van Dijk Detection of depression in the early stage of stroke: opportunities in nursing care June 2015

Background & research question The Nursing Rehabilitation Guideline Stroke (NRGS) 21 recommendations focus on early screening, detection, and nursing interventions for post stroke depression (PSD). However, implementing clinical practice guidelines into clinical practice is often difficult. Nurses state that they need recommendations provided in a more practical form to be able to use them in daily care.

Therefore, a toolkit including screening of PSD and nursing interventions was developed and tested on its feasibility. However, no standardized instrument is available for screening on depression on patients with communicative impairments after stroke. After careful review of the literature the SODS was identified for the assessment of PSD in patients with communicative disorders.

Evidence for the psychometric properties of the SODS, however, is lacking and therefore, the psychometric properties of the SODS were investigated in the second study.

Design/methods Study 1: Detection of Depression in the Early Stage of Stroke: Feasibility and sustainability of the Post Stroke Depression–toolkit Study design: A concurrent embedded strategy of mixed methods, pre-test post-test design, in which qualitative data are embedded in a quantitative design.

Population: Nurses working on the participating neurological wards.

The main study parameter: the percentage of nurses using the PSD-toolkit.

Secondary study parameters/endpoints: − nurses' perceived barriers and facilitators to provide PSD-careThe clinical utility of the PSD-toolkit; − level of team learning; − the level of expertise of nurses concerning PSD. Study 2: Detection of Depression in the Early Stage of Stroke: Psychometric properties of the Signs of Depression Scale Study design: A cross-sectional multicenter study, using a census approach.

Population: Patients in the early stage post stroke admitted to neurological wards of a University Hospital and two general hospitals and their proxies as well as the nurses responsible for the care of the participating patients.

Study parameters/endpoints Index test: The Signs of Depression Scale revised (SODS-R).

Reference tests: 1. The Composite International Diagnostic Interview (CIDI) 2. The Barthel Index (BI) Main results to date Study 1: 150 nurses were included in the study, data on the use of the PSD-toolkit on 1,100 patients were collected, 30 nurses were interviewed, 3 focus groups were organized, patients perception on the PSD toolkit were investigated and the use of the PSD-toolkit in daily practice was examined by participating observation.

80% of the patients were screened on depression and interventions were deployed at an average of 67%. Data analysis is in progress.

Study 2: 116 patients and 107 proxies were included. Data analysis in progress.

Jeroen Dikken Development of the ‘knowledge about older patient quiz’ (KOP-Q) for nurses

Background Globally, the population of older people with multimorbidity increases. This results in an increase of older hospitalized patients. Literature shows that nurses have a negative attitude towards older patients. Attitudes are built in part on knowledge. Increasing knowledge about the elderly and old age may affect hospital nurses’ attitudes and thereby improve the quality of care for older patients. A first step in this process is understanding nurses’ current knowledge.

This understanding can be achieved by measuring knowledge using a measurement scale with good psychometric qualities. A first instrument in development is achieving good validity on content.

This study explains the content development of the ‘Knowledge about Older Patients Quiz’ (KOP-Q) measuring nursing knowledge about older patients.

Design/methods Open interviews with 7 professors and PhD, 10 nurse specialists and 5 older patients were conducted. The data were analysed using thematic analysis. Two pilot survey studies were conducted for measuring readability and face validity of the KOP-Q. Professors, PhDs and nurse specialists all had expertise in gerontology, geriatrics, and/or nursing. The older patients were aged 70 years and over, and have had one ore more hospital experiences in the last two years. All patients were able to partake in interviews. Readability was examined by a Dutch specialist and 7 nurses working on a cardiovascular ward. Face validity was tested in two hospitals with 22 nurses working on the geriatric ward.

Main results to date From the open interviews, seven themes were derived. Nurses should have knowledge about normal aging, geriatric conditions, signaling problems in old age, interventions, family care, vulnerable patients versus older patients and internal motivation for learning and reflection. Readability of the KOP-Q was good. The appropriateness for measuring nurses’ knowledge of older patients was acceptable, with a mean of 6.84 and a range from 4 (n=1) to 8 (n=7).

None of the existing tests measuring knowledge about older patients has included items covering the themes mentioned in the interviews. The first steps to validation of the KOP-Q (readability and face validity) are promising. Further psychometric testing of the KOP-Q is necessary before the KOP-Q is ready for practical application.

Roelof Ettema The PREDOCS-programme for predicting and preventing postoperative decline in older cardiac surgery patients: A stepped wedge monitored implementation study June 2016

Background & research question Given the growing number of vulnerable, older cardiac surgery patients, the preadmission PREDOCS programme was developed to reduce the incidence of postoperative complications. Also, the programme appeared feasible for patients and nurses to use in clinical practice and it is estimated that its use is already cost effective when only 0.6% to 1.6% of the patients would benefit. The programme should be built into the hospital’s cardiac surgery pathway or applied in home care. In this study the clinical effects of this complex multicomponent intervention will be evaluated.

Design/methods In a stepped wedge design in approximately 2,000 patients of 65 years and older in six cardiac surgery centres together with a home care organization in the Netherlands, the clinical effect of the PREDOCS programme is tested. The six cardiac surgery centres will be randomly assigned to a control or an intervention team, using random numbers. Patients in the intervention centres will receive usual care and the PREDOCS programme. Patients in the control centres will receive usual care only. The PREDOCS programme is delivered by a nurse in a consult at home or in the hospital outpatient department four to two weeks before the hospital admission.

Main results to date It is expected that the current incidences of postoperative occuring delirium approximately 18%, depression 15%, pressure ulcer 11%, and infection 8%, can at least be reduces with respectively 5%, 4.5%, 4% and 3.5%.

Following the intention to treat principle, multilevel regression analysis will be

performed, adjusted for baseline differences between the groups. To evaluate the costs, the total invested capacity of nurses will be measured and costs will be calculated and compared with health revenues in all patients in the intervention centres by calculating the decrease in care demand with the accompanied costs saving.

Discussion Older patients should be adequately prepared before an elective hospital admission. Despite the fact that this is in accordance with the fundamentals of nursing, this notion is unfortunately not common in nursing practice.

A monitored implementation trial with a stepped wedge design is an adequate approach with relatively low costs for measuring the effect of the PREDOCS programme. Such a design is particularly relevant in cases where the intervention will likely do more good than harm. In such a study with a complex design, careful planning and monitoring are required in order to ensure that a robust evaluation of the effectiveness of the intervention is undertaken. This will support the notion that older patients should be adequately prepared before an elective hospital admission.

Roelof Ettema Early oral nutrition intervention in homecare patients indicated for a hospital admission or recently discharged from the hospital: a clustered exploratory trail June 2018

Background & research question Screening for malnutrition is no daily practice in community-dwelling people receiving homecare, who are indicated for a hospital admission or are recently, discharged from the hospital. Although it is well known that well-nourished and hydrated patients recover more quickly and have a more positive experience of care, malnutrition often goes unrecognized. Ensuring patients receive all of the nutrients and fluids they need is vital to the delivery of good nursing care.

Therefore, we aim to explore the feasibility and effectiveness of the nutrition screening and intervention (early oral nutrition intervention), in frail communitydwelling people receiving homecare, who are either indicated for an elective hospital admission, or after hospital discharge.

Design/methods 150 community-dwelling patients receiving home care who are either indicated for a hospital admission within 30 days or who are within 30 days after hospital discharge in six nursing teams delivering homecare will be included. The six nursing teams will be randomly assigned to a control or an intervention team, using random numbers. Patients in the intervention teams will be screened for malnutrition with the Malnutrition Universal Screening Tool (MUST). Patients with an increased risk for being malnourished will receive a tailored nutrition care plan. Nursing teams with nurses working in multiple teams will be excluded.

Nurses will be trained in the use of the MUST screening and the tailored nutrition care plans.

The primary outcome will be BMI change. Patients discharged from the hospital will be followed for 30 days. Secondary outcomes are frequency of readmission

to hospital, health-related quality of life: EQ-5D, CQ satisfaction with nutritional care & care in general.

Discussion Involuntary weight loss causing malnutrition in community-dwelling older patients receiving home care who are either indicated for a hospital admission within 30 days or who are within 30 days after hospital discharge among patients is associated with disease. Malnutrition is preventable and reversible in every health care setting, but is not sufficiently recognized by hospital and community nurses. To establish the effectiveness of the nursing intervention ‘early nursing nutrition intervention’ two trails are proposed one in a hospital setting and one in a home care setting.

Roelof Ettema Individual health care needs and capabilities of older people living in the community December 2018

Background & research question There is a lack of knowledge in the home health care setting about individual health care needs of older people and general health care needs in this population, based on the following profiles: − vital older people; − older people with psychosocial problems; − older people with problems in physical health and mobility; − older people with complex problems; − frail older people.

Therefore, we aim to explore the individual health care needs of the older people living in a community and the capabilities of older people that influence their health care situation, quality of life and wellbeing.

Design/methods In a retrospective study design a phenomenon of interest will be explored in the individual healthcare needs and the capabilities of 7,394 older community dwelling people. The research will explore the full nature of these phenomena to which it is related and the explanations of the dependent and independent variables. Data will be used collected in the Utrecht region and the northern part of the Netherlands with the triage instruments Groningen Frailty Instrument, INTERMED and Groningen Wellbeing Instrument.

Main results to date In this study the data of the two triage instruments will be used to explore the unmet healthcare needs and capabilities that are needed to provide a perceived increase in quality of health care situation and wellbeing. For supporting a

proactive primary nursing care, individual health care needs and capabilities patterns of older people living in the community will be identified.

Discussion In a proactive primary nursing care, the focus of care is on prevention that is based on disease, function or care, in such a way that it encourage selfmanagement by involving patients in their own care process, demand driven care and empowering of patients. For redefinition of care in primary nursing care, it is important to know the needs and the functioning potential of older people to avoid adverse healthcare outcomes which may disturb the equilibrium and cause a change in priority of needs in the community dwelling older people.

Th贸ra B. Hafsteinsd贸ttir Leadership and mentoring programs in nursing postdoc and the impact on nursing research: a systematic review

Background & research question There is a growing shortage of nursing faculties globally. Especially nursing faculty, postdoc and focusing on nursing research is lacking attention in the literature. Postdoctoral training of nurses is important if nurses are to pursue a successful carrier in nursing research. Although faculty scholarship and nursing research is a continuing priority in nursing academy, increased competition makes it difficult for nurses to develop a sustaining career in nursing research.

This impacts research productivity. Leadership and mentoring models and mentoring programmes have been described as important in stimulating rapid growth and productivity in faculties. Based on this background the aim of this review was to systematically provide an overview of the literature investigating nursing leadership and/or mentoring models/programs and and the impact they have on the professional development of nurses postdoc and research.

At the base of the research lies the following question: what is the impact of nursing leadership and/or mentoring models/programs on the professional development of nurses postdoc and research?

Design/methods This systematic review was conducted according to the Preferred Reporting Items of Systematic reviews and Meta-Analyses (PRISMA) statement.

The electronic databases PubMed (Medline), CINAHL, Embase and The Cochrane Library were searched for eligible studies up to July 2014. The following Medical Subheadings term (MeSH term) as well as a title and abstract term (TiAb)- key words, based on DDO (Determinant, Domain, Outcome), were used.

Domain: nurse, nurses, postdoctoral, nursing academia; Determinant: leadership (model, program, strategy, intervention, training, course), mentors; mentoring, mentor, professional development, faculty orientation programs, faculty development, Outcome: nursing faculty, research development, nursing research, nursing research output, research capacity building, research leadership, research metrics, research output.

Inclusion criteria: study design: all designs appropriate for the research question; participants: nurses postdoctoral; Interventions: leadership and/or mentoring models, programs, strategies, focusing on professional development of nurses postdoc and aiming to improve research output; outcome: any outcome (defined outcome); language: English; time limits: no time limit.

Studies were selected by two researchers independently (TBH/AvdZ) who screened titles, abstracts and full text papers if they proved relevant for inclusion. The methodological quality of the included studies was assessed using the Cochrane list of bias tool. Data were extracted using a data extraction form including the items: authors, year of study, country, design, sample, setting, description of a leadership/mentoring program (aims, length in months, issues addressed, meetings, participants); outcomes; instruments; results.

Main results to date The search strategy resulted 1,345 titles. After screening the titles and abstracts, the full text paper was read. At the moment the last step of the screening is being conducted. Further results will be presented.

Pieterbas Lalleman Nurse middle managers’ daily work and the hindering and helping influences of nursing habitus on their leadership an ethnographic case study in four hospitals in the Netherlands and the U.S. December 2018

Background & research question Nurse middle managers (NMMs) are considered key players in new models for improving the quality and efficiency of care. Due to their hybrid identity as ‘professional’ managers, they can relate to both managerial and professional worlds and span boundaries between these worlds. In this study we use Bourdieu’s theory of practice to describe NMMs’ daily work in hospitals. By focusing on the notions of habitus, capital and field, we are able to demonstrate how nursing habitus as system of dispositions and as an output of the professional socialization of nurses in various fields influences NMMs’ daily work. We also show how differences in fields result in different appreciations of capital. By using Bourdieu’s work, we are able to go beyond the individual competencies paradigm of nurse leadership and provide a more dynamic portrayal of NMMs’ work. Moreover, we are able to go beyond the ‘two window metaphor’, as we demonstrate that NMMs use a wide variety of dispositions rather than just limit themselves to ‘two windows’ of the professional and management world. Spread over various articles we address several research questions: 1. What is nursing habitus as seen in NMMs’ daily practices?

2. How does nursing habitus help or hinder the leadership of NMMs? 3. How is nursing habitus articulated and nursing leadership portrayed in the context of the four different sites?

4. How do NMMs experience being shadowed?

Design/methods This study is based on a Bourdieusian perspective and rooted in the Managerial Work and Behaviour Approach (MWB). In order to observe the dispositions of

habitus in action in Magnet and non-Magnet hospitals in both the Netherlands and the U.S., a total of four in-depth ethnographic case studies were carried out. In total 16 NNMs where shadowed (i.e. four at each site), a method common in MWB research which is particularly suitable for the in-depth study of organizational practices. The researcher closely followed each participant for four days and spent approximately 560 hours observing over a period of 19 months in 2010-2012 Main results to date Eight dispositions of nursing habitus are found. These are caring, clinical, collegial, teaching, professional, scientific, administrative and control dispositions. Together these dispositions play an important role in the behavioral repertoire of nurses and in the daily practices of nurse middle managers. The frictions and compromises between the dispositions varied at each site. The eight dispositions where not a static representation of habitus. Some dispositions gave more capital than others depending on the field they were articulated in. Answering the call for help, a manifestation of a caring and a clinical disposition are seen as essential elements of a nursing habitus and helped nursing leadership, as did a scientific disposition.

Sigrid Mueller-Schotte “Old and well-functioning�; determinants of visual functioning and preservation of functioning in elderly (PROFIEL /VF-PROFIEL) June 2016

Background & research question Being able to function and live independently at older age is an important factor in keeping good quality of life (QOL) and vitality. Vision affects several domains of functioning, such as mobility, the ability to perform activities of daily living (ADL) and instrumental ADL (IADL), mental state and QOL. Changes in vision progress slowly over time and are attributed to normal aging.

This process of gradual deterioration of the visual function is often not detected.

However, early detection and timely treatment can delay the progression of ocular disease or restore visual function thus preserving vision-related QOL (VRQOL). The aim of the PROFIEL/VF-PROFIEL study is to gain insight in factors contributing to the preservation of visual functioning and visual factors contributing to physical, cognitive, and psychosocial preservation of functioning in the elderly to enable them to stay independent.

Design/methods The PROFIEL-study has a longitudinal study design. Information was collected on medical history, lifestyle, somatic, cognitive, functional and psychosocial status. The vision-related study (VF-PROFIEL) is an extension of the PROFIELstudy with a cross sectional design.

Research question 1: ocular straylight: a determinant of quality of life in the elderly?

Study design: a cross-sectional population-based study.

Population: 120 men and women aged 50-89 years.

Endpoint: the Dutch version of the National Eye Institute Visual Functioning Questionnaire (NEI-VFQ-25) was used to assess VRQOL.

Main study parameter: Ocular straylight measured using the Oculus C-Quant instrument.

Data analysis: Linear regression models will be used to assess the association between SL modeled continuously and cut off at the recommended fitness-todrive value (SL ≼ 1.4 log(s)) and VRQOL, adjusted for confounders.

Research question 2: visual functioning and basic and instrumental activities of daily living in independently living elderly.

Study design: a cross-sectional population-based study.

Population: 120 men and women aged 50-89 years.

Endpoint: the KATZ-questionnaire was used to assess basic and instrumental activities of daily living.

Main study parameter: visual acuity, contrast sensitivity, ocular straylight and visual field.

Tuğba Çoban DIKSAP research study (Prevention of Diabetes by a group of community pharmacies in the Eastern Part of the Netherlands) 2019

Background & research question The research group "Innovation of Care Processes in Pharmacy" is developing a research study to prevent or to delay the diagnosis of diabetes mellitus type 2 (DM2) through innovative, state of the art lifestyle interventions. Those lifestyle interventions induce behaviour change and stimulate participants’ health; they are based on the the newest technology (e.g. Lifewatch V).

The aim of the study is to determine the effectiveness of lifestyle interventions by means of the newest (eHealth) technology, performed by community pharmacists as lifestyle consultants in collaboration with other health care providers, to reduce the risk of developing diabetes mellitus type 2 in high risk groups or to delay the disease diagnosis.

Design/methods The study will be a randomized controlled trial (RCT). The organization of the study is as follows. First of all, people will be measured for their weight, waist circumference, fasting oral glucose tolerance etc. in their pharmacies. After determining their risk profile, people with a relatively high risk of DM2 will be invited to participate in the study. Subsequently, participants will be divided in an intervention group (n=60) and a control group (n=60). Participants in the intervention group will get a lifestyle consultant (their own pharmacist) and will be subjected to the lifestyle interventions. These lifestyle interventions will consist of e.g. a healthy diet and physical activities. The control group will merely get written information about the positive effects of healthy diet and physical activities on their risk to get DM2.

Both groups will initially be followed for 2 years. At the end of this period, the

results of both groups will be compared to determine the effectiveness of the lifestyle interventions. Main outcome measure will be the delay of DM2 diagnosis.

Main results to date The preliminary start date for this study is 2015.

Discussion The study will be done by a group of community pharmacies in the eastern part of the Netherlands (region Salland). The study can be regarded as a pilot that will provide an initial indication of the implementation possibilities of this innovative idea in the Netherlands.

Rik Ensing HomeCoMe-programme: improving medication management post-discharge in hospital-to-home care transitions by pharmacists October 2016

Background & research question Adverse drug events (ADEs) and other drug related problems (DRPs) are common in all healthcare settings. For patients, ADEs generally are the most invasive type of DRPs. However, other DRPs such as under-prescribing or nonadherence may also result in patient harm leading to unplanned hospital (re)admissions. Moreover, several studies indicate the occurrence of a high number of readmissions in the days to weeks following discharge.

Although a guideline on transfer of drug related information has already been developed in the Netherlands, both hospitals and primary care providers are still struggling with implementation. In addition, it might be questioned whether providing information on medicine use at discharge alone is sufficient. Previous studies, however, used interventions within the hospital or within the community setting only, which could still result in discontinuity of care. Thus, it seems likely that the reduction of DRPs, improvement of patients’ medication knowledge and adherence are most effectively addressed in a multifaceted integrated transmural intervention.

Pharmacist interventions have proven to be effective in reducing the number of DRPs and ADEs. By introducing a home-based medication management program within the first week after discharge DRPs could be effectively prevented, since patients could slip back into old medication routines, or new problems may arise, such as emerging ADEs due to medication changes made during hospitalization.

Therefore, the main objective of this study is to investigate the capability of a Home-based Community pharmacist-led Medication management (HomeCoMe) program to identify DRPs (including suboptimal medication adherence, lack of knowledge and ADEs) in recently discharged patients.

Design/methods A prospective cohort study with a nested case control analysis will be performed. 150 patients will be included to receive an in-hospital medication reconciliation program complemented with a pharmacist home visit. The followup is 6 months. Possible DRPs will be identified during the home visit. The community pharmacist will perform onsite action or will contact the responsible physician to solve any identified ADE or other DRP. Main results to date 100 pharmacist home visits have taken place and initial analysis is starting in September 2014.

Discussion This study proposes an innovative approach for preventing early readmissions and managing post-discharge DRPs (including ADEs), an issue in healthcare with a known clinical as well as economic burden. Thus far, of the interventions shown to be effective, most have had both an in-hospital and posthospitalization component. Therefore, we think the integrated care plan in the HomeCoMe study, incorporating an in-hospital medication reconciliation and review program which is complemented with a home-based medication management program, has the potential of significantly improving patient safety, reducing health care costs and contribute to optimizing current clinical practice regarding care transitions.

Ellen Molewijk Hypothyreoidie-patiënten: hypo maar niet happy Juli 2015

Background & research question Hypothyreoidie kenmerkt zich door het verlies van de schildklierfunctie. In het bloed is dit te zien aan een verhoogd thyroid stimulerend hormoon (TSH) en een verlaagd niveau vrij thyroxine (FT4). Het functieverlies wordt behandeld door orale inname van het schildklierhormoon levo-thyroxine (T4). Dit is het inactieve prohormoon van waaruit het actieve schildklierhormoon liothyronine (T3) in het lichaam wordt gemaakt. De dosering levo-thyroxine wordt stapsgewijs getitreerd totdat de waarden van TSH en FT4 weer binnen het referentiebereik vallen. Het klinkt allemaal erg simpel: je mist een hormoon en vervolgens neem je het betreffende hormoon gewoon in. Probleem opgelost, zou je zeggen.

In de literatuur zijn aanwijzingen dat de kwaliteit van leven van hypothyreoidiepatiënten verminderd is ondanks behandeling met levo-thyroxine die bloedwaarden binnen de referentiewaarden brengt. Dit suggereert dat er restklachten blijven bestaan, terwijl de schildklierfunctie volgens de bloedwaarden ‘goed’ is. Dit is een belangrijke constatering die we verder willen substantiëren, omdat artsen vooral behandelen op de bloedwaarden. In de NHG-standaard staat weliswaar dat gezocht moet worden naar meer optimale dosering en/of eventueel het toevoegen van liothyronine (T3), indien de patiënt nog klachten ervaart, maar dit wordt nog weinig toegepast én het is nog niet duidelijk of dit de restklachten werkelijk vermindert. In de praktijk blijkt hypothyreoidie niet ‘top of mind’ te zijn bij artsen, zowel in de fase van de diagnose als ook bij de behandeling; de klachten worden niet erg serieus genomen.

De onderzoeksvraag luidt: op welke wijze kan bij zorgprofessionals en bij het grote publiek meer bekendheid gegenereerd worden over hypothyreoidie en

hoe kan de behandeling van hypothyreoidie-patiënten in Nederland worden verbeterd?

Opzet/methode In het onderzoek worden de kwaliteit van leven (QOL), inclusief symptomen van hypothyreoidie, en de tevredenheid over de behandeling/zorg gemeten bij hypothyreoidie-patiënten in Nederland. Bij wijze van vooronderzoek worden diepte-interviews gehouden met patiënten en artsen, teneinde vragenlijsten op te zetten en te valideren. Na het verwerken van alle feedback wordt de definitieve vragenlijst samengesteld. Deze wordt naar de MES gestuurd, en na goedkeuring in heel Nederland uitgerold via de patiëntenvereniging, huisartsen en apotheken (2014-2015CD).

Na deze periode liggen er nog allerlei andere onderzoeksvragen klaar. In het geheel omvat het onderzoek de volgende onderwerpen: 1. de kwaliteit van leven; 2. de tevredenheid over de behandeling; 3. de arbeidsparticipatie; 4. de epidemiologie van hypothyreoïdie-patiënten in Nederland; 5. de perceptie over deze aandoening en behandeling door artsen; 6. de bekendheid van hypothyreoïdie in Nederland.

Belangrijkste resultaten tot nu toe De QOL-vragenlijst is nu getest bij tien patiënten. In het komende semester wordt de vragenlijst aan artsen voorgelegd en daarna uitgetest bij patiënten.

Esther Du Pon DESTINE trial – Diabetes Education & Selfmanagement To INcrease Empowerment 2017

Background & research question Accounting for the growth and ageing of the Dutch population, the effect of current (high) risks of diabetes, predominantly caused by detrimental life style changes in recent decades and an expected further growth of the number of known patients with diabetes, requires a decrease of the workload per patient for care providers as the growth of health care providers will not keep up with the number of patients. We set up another study that started two years ago in which we aimed to increase patient’s self-management by offering remote care services, which would result in a reduction of the individual caseload. However, other studies mitigated expectations: those services would probably not result in high usage numbers within the targeted population. This proved to be the case. To improve usage of remote services supporting self-management, different strategies need to be developed in order to try to interest and reach different types of patients. Our current hypothesis is that implementation, supported by PRISMA (self-management) training, will lead to a better use of the offered remote care and self-management supporting services.

Design/methods This study investigates the effects of the voluntary use of a web portal and its educational content with or without the support of the PRISMA program in a sequential randomised controlled study. Participation in this study implies that patients are being followed for the duration of the study (two years). At four time points (0, 6, 12 and 24 months), patients are asked to fill out questionnaires. Other measurements are already being performed during usual care. The first two consultations with the practice nurse (PN) during the study, usually within 12 months, will be recorded by an unmanned camera. Those images are subsequently analysed for the information exchange in the consultations.

Hans Vehof Characteristics and self-reported patient health effects of news messages containing beneficial pharmaceutical innovations: preliminary research draft October 2018

Background & research question During the last two decades, patients gained broad access to news messages about pharmaceutical innovations. A quick scan of literature shows a plurality of sources, among which are research institutes, health intermediaries, governments, patient interest organizations, general practitioners and other health professionals. Furthermore, patient fora, social media and nonprofessional health advisors contribute to an overwhelming quantity of available information.

News messages about pharmaceutical innovations may affect determinants of patients’ mental, social or physical quality of life. Possible effects may depend on constructs such as perceived hope and stress, expectations, dissatisfaction, adherence, health information seeking behavior or illness perceptions.

In order to reach optimal communication between news message sources and groups of frail patients, and to contribute to an increased health related QOL, the following main research question is formulated: what are quantitative and qualitative characteristics of communication of news messages containing beneficial pharmaceutical innovations to patients with a life threatening disease, and what are the effects of patient exposure to these messages on self-reported health related quality of life?

Design/methods Time period: four years study; mid 2014 - mid 2015 literature study, desk research and focus group interviews; mid 2015 – mid 2017 longitudinal observational study with follow up after 6 months, 12 and 24 months; mid 2017 - mid 2018 data analysis and additional article writing.

Subjects: inclusion criteria: diagnosed with breast cancer, diabetes mellitus type II or renal failure; age 18 years and older; living prognosis >6 months.

Recruitment: general practitioners, GP registrars, GP organizations, pharmacists, patient interests organizations, virtual knowledge center UPPER. Sample size: observational study: 800 breast cancer patients; 800 renal failure patients; 800 DM2 patients. Focus group interviews: 3 x 10 breast cancer patients; 3 x 10 renal failure patients; 3 x 10 DM2 patients.

Measured survey variables: • demographic variables (age, sex, ethnicity, marital state, education, urbanization, religion/philosophy, postal code and/or SES); • determinants of physical, social and mental health as measured by the short form 36 and/or disease specific QOL-questionnaires ; • survival; • treatment and disease outcome expectations; • treatment and disease outcome satisfaction; • adherence, measured with the MARS; • disease perceptions with IPQ-K beliefs about medicines with BMQ; • health information seeking behavior; • self-reported exposure to news messages containing beneficial pharmaceutical innovations.

Data collection and analysis: Online and paper based survey. Data collection method will be recorded as a variable. Randomness of missing data will be recorded as MAR MNAR MCAR. Data entry, analysis will be performed in resp. SPSS and SAS. Multiple regression and mixed models will be used to estimate causality between – stratified- independent and dependent variables. Proportional hazards models are calculated to assess differences in survival. Attention will be paid to possible confounders: availability of pharmaceutical innovations; demographic variables (e.g. age, gender); baseline disease characteristics (e.g. diagnose/ prognosis).

Welke meetinstrumenten zijn valide en reproduceerbaar bij kinderen en jongeren met spina bifida die gebruik maken van een rolstoel, op het gebied van fysieke fitheid (aerobe capaciteit, anaerobe capaciteit), functioneel rolstoelrijden, vaardigheid in het rolstoelrijden en fysieke activiteit?

2. Wat zijn de relaties tussen en binnen de niveaus functie en activiteit/participatie (ICF-CY) bij kinderen en jongeren met spina bifida die gebruik maken van een rolstoel?

3a. Wat zijn belemmerende en bevorderende factoren voor fysieke activiteit bij kinderen en jongeren met een lichamelijke beperking, bekend vanuit bestaande literatuur?

3b. Wat zijn belemmerende en bevorderende factoren voor fysieke activiteit bij kinderen en jongeren met spina bifida, vanuit het perspectief van de kinderen en jongeren en de ouders?

Opzet/methode Vraagstelling 1 en 2. Dit cross-sectionele onderzoek bestond uit twee delen. Het eerste deel bestond uit een studie naar de validiteit en reproduceerbaarheid om praktisch toepasbare meetinstrumenten te ontwikkelen voor de kinderfysiotherapeutische praktijk. Het tweede deel bestond uit het beschrijven van de mate van fitheid en activiteit en de relaties tussen en binnen de niveaus functie en activiteit/participatie (ICF-CY) bij kinderen en jongeren van 5 – 18 jaar met spina bifida die gebruik maken van een rolstoel.

Vraagstelling 3a. Systematische literatuurstudie waarbij zowel kwalitatieve als kwantitatieve studies geĂŻncludeerd werden.

Data is verzameld door middel van focusgroepen en semigestructureerde interviews en geanalyseerd door twee onafhankelijke onderzoekers met behulp van een thematische analyse. De data is verwerkt in het “Physical Activity for Persons with a Disability” model.

Belangrijkste resultaten tot dusver Er is een overzicht beschikbaar van de belemmerende en bevorderende factoren voor fysieke activiteit bij kinderen en jongeren met een lichamelijke beperking ( vraagstelling 3a) en bij kinderen en jongeren met spina bifida (vraagstelling 3b).

Voorlopige analyses van de psychometrische eigenschappen van de te ontwikkelen meetinstrumenten laten positieve resultaten zien, definitieve analyses zullen de daadwerkelijke psychometrische eigenschappen beschrijven (vraagstelling 1), hier wordt momenteel aan gewerkt.

Analyses voor het bepalen van de relaties tussen en binnen de niveaus functie en activiteit/participatie (ICF-CY) bij rolstoelrijdende kinderen en jongeren met spina bifida zullen volgend jaar uitgevoerd worden.

Discussie Met de resultaten van dit onderzoek kunnen toekomstige interventiestudies ontwikkeld worden die de effectiviteit van de gevonden belemmerende en bevorderende factoren voor fysieke activiteit onderzoeken. Tijdens dit interventieonderzoek kunnen additionele psychometrische eigenschappen als het “klinisch relevante verschil” en “responsiviteit” van de ontwikkelde meetinstrumenten meegenomen worden. Een belangrijke beperking binnen dit onderzoek is het cross-sectionele karakter.

Marike Boonzaaijer Gross motor Development of Infants using home-Video registration with the Alberta Infant Motor Scale (GODIVA): A Feasibility Study January 2015

Background & research question Measuring of motor development longitudinally is preferred to cross-sectional measuring in young infants, but can be burdensome for child and parents. In the two-year GODIVA-study, sponsored by SIA-RAAK, a telemetry method using video recordings made by parents to observe gross motor development is being validated. If this method is successful, we want to use this method for research purposes to develop longitudinal motor trajectories for the Alberta Infant Motor Scale (AIMS). In this design parents will be asked to make several home videos of their infant over a longer period of time. To help parents doing so an accessible smartphone application for filming and uploading is developed by students of our Technical Faculty.

At first the telemetry method will be applied for research purposes. We hope it will eventually become a helpful tool for clinicians in the field. An important question in this study is about the operational feasibility.

The research questions regarding the parents are: • Are all parent able to make useable home videos of their infant when they are guided by instructions?

• Can they upload their videos to a safe streaming server when this is facilitated by a smartphone application (‘the GODIVAPP’)?

• How do parents experience this method in regard to their infant's motor development?

• Are parents motivated to engage in a longitudinal follow up? This means they have to make several home video’s during a longer period.

The research questions regarding the pediatric physical therapists are: • Could the telemetry method become a helpful tool for PPT’s?

• What is necessary for a successful implementation of the method in the field?

Design/methods To find answers to all of these questions a ‘mixed method’ is being used. Data will be retrieved in the validation study (February 2014 - July 2014) as well as the longitudinal pilot (September 2014 - June 2015).

Methods used in the validation study: • questionnaire for parents (N= 60); • interviews with parents (N= 10); • focus groups with pediatric physical therapists (N= 22) (hospitals/ private practice).

Methods used in the longitudinal pilot: • three-step-test interview with parents; • questionnaire for parents; • pilot in outpatient PPT department in hospitals; • digital feedback from parents on the application (‘the GODIVAPP’).

Main results to date Data collection is still in progress. The first result are expected in November 2014.

Ellen van Dam Validation of test results on the Alberta Infant Motor Scale (AIMS) based on a video made by parents of the gross motor abilities of infants July 2014

Background & research question Assessing motor development determined by a one-time observation involves a risk of under- or overestimation. Therefore, multiple observations in time are needed to define the motor developmental trajectory of an infant with more accuracy. Since realizing this in an outpatient pre-planned setting can be burdensome for parents and infants, benefits can be expected of assessing gross motor development based on home video recordings of the infant, made by parents. This also fits today’s society in which digital facilities are available and used more and more.

The research question for this study is: what is the concurrent validity of assessing gross motor abilities of infants, aged between 2 weeks and 19 months, based on a video made by parents at home, compared with observation on site by the pediatric physical therapist, using the Alberta Infant Motor Scale (AIMS)?

Design/methods The project is approved by the local Ethics Committee. The study sample consists of a heterogeneous group of 60 infants in the age from 2 weeks to 19 months, equally divided into three age groups: 22 weeks or less, 23 through 36 weeks and 37 weeks or more. Families meeting the following criteria are included: parents may have a question or concern about the motor development of their infant, parents have appropriate understanding of the Dutch language and both parents give informed consent. Infants with atypical motor development are excluded. Parents with a professional background being a physical therapist are also excluded. All 11 testers participating in the study

are pediatric physical therapists; all are familiar with the AIMS. They have attended a two-day training session led by an expert. The primary assessment consists a live observation of the first tester at home with the child, using the AIMS. During this assessment, parents are making video recordings of their infants on their own, but guided by tutorial material developed for this study.

Subsequently, the video made by the parents is observed by a second tester.

Both testers are blinded to the test results. The Intraclass Correlation Coefficient (ICC) and the Standard Error of Measurement (SEM) will be used to define the equivalence of the two different methods of measuring gross motor development with the AIMS. Main results to date The study is still running. At the time of writing, the results of 33 participants have been analysed. We found an ICC of 0,988 (0,973 – 0,995) and a SEM of 1,8. These preliminary results are based on a limited number of measurements and may therefore differ from the final results.

Karin van der Ende-Kastelijn, Kristel Lankhorst HAYS - Health in Adapted Youth Sports. Health effects of sport participation in children and adolescents with a chronic disease or condition September 2017

Background & research question Children and adolescents with a physical disability or chronic disease often show reduced fitness levels and physical activity patterns. In comparison to their nondisabled peers, they participate less in competitive and recreational sports.

Being less active not only affects fitness but also creates a higher risk for a variety of health related diseases and injuries. In addition, sport participation in children and adolescents with a physical disability or chronic disease may have positive effect on both (psychosocial) health and cognition. While these relationships have been reported in healthy children, these benefits have not been established in children with childhood disability or chronic childhood disease. Therefore, the aim of the HAYS study will be to describe the differences in health status and cognition between children and adolescents with a physical disability or chronic disease who participate in sports compared to their nonsporting peers.

The research question of this study is: between children and adolescents with a physical disability or chronic disease who participate in sports and their nonsporting peers, what are the differences in physical fitness, physical activity, injuries and illness (sport and activity related injuries / illness), cognition (school performance, attention, motivation) and psychosocial health (self-efficacy, quality of life)?

Design/methods The HAYS study will be a cross-sectional study comparing children and adolescents with a physical disability or chronic disease, aged 12 to 19 years, who are active in sports, to their non-sporting peers. Injuries and illnesses will be

monitored longitudinal for one year to objectify the incidence, severity and extent of injuries and illnesses. Children and adolescents with a physical disability, more specifically cardiovascular, pulmonary, musculoskeletal or neuromuscular disorders, will be included. Both children and adolescents who are ambulatory or propelling a manual wheelchair will be included. The children and adolescents who participate in sports will be recruited among different patients associations and sports clubs. Athletes will be recruited on a different range of sport levels: from top sport to recreational level.

Main results to date Submission to the METC for a non-WMO statement. Start of recruitment and inclusion as soon as this statement is issued. The aim of the researchers is to start with measuring in September 2014.

Janke de Groot Fit for the Future! Praktijkgericht onderzoek voor en met de kinderfysiotherapie 2017

Achtergrond & onderzoeksvraag Veel kinderfysiotherapeuten onderschrijven het belang van participatie in sporten spelactiviteiten voor kinderen met een lichamelijke beperking. Deelname aan fysieke activiteit is niet alleen bevorderlijk voor het opdoen en ontwikkelen van motorische vaardigheden, maar ook een essentieel onderdeel van een gezonde leefstijl in het kader van preventie van overgewicht, diabetes II, metabool syndroom. Naast de voordelen voor de lichamelijke gezondheid draagt deelname aan fysieke activiteit ook bij aan de cognitieve, sociale en emotionele ontwikkeling van het kind. In de praktijk blijkt het niet alleen lastig om kinderen met een lichamelijke beperking actief te krijgen, maar vooral ook om ze daarna actief te houden.

Eerste doelstelling van deze aanvraag is een aanzet te geven tot de ontwikkeling van een kennistransfersysteem (KTS) getiteld “Fit for the Future�, waarin bestaande kennis vanuit onderzoek in samenwerking met het werkveld wordt vertaald naar toepasbare kennis voor het werkveld. In eerste instantie vindt kennissynthese plaats ter ondersteuning van het kinderfysiotherapeutisch klinisch redeneren op het domein van fitheid en activiteit voor kinderen met een lichamelijke beperking en specifiek uitgewerkt voor kinderen met cerebrale parese en spina bifida.

De ontwikkeling van nieuwe kennis uit dit project zal betrekking hebben op: 1. Interventies en meetinstrumenten m.b.t. (sportspecifieke) rolstoelvaardigheid en fitheid bij rolstoelrijdende kinderen.

2. Meetinstrumenten voor het meten van fysieke activiteit bij kinderen met een lichamelijke beperking.

3. De effecten van een bewegingsstimuleringsprogramma bij kinderen met een lichamelijke beperking.

Opzet/methode Centraal in het design staat samenwerken met het werkveld en andere kennispartners. Het consortium bestaat uit kinderfysiotherapeuten van het kinderbewegingscentrum van het Wilhelmina Kinderziekenhuis, de mytylscholen, revalidatiecentra, de 150 kinderfysiotherapiepraktijken verbonden met Stichting Fitkids en de beroepsvereniging Nederlandse Vereniging voor Kinderfysiotherapie (NVFK), die ruim 1.600 kinderfysiotherapeuten in Nederland vertegenwoordigt. Een andere werkveld partner is K-J Projects, een bedrijf dat zich richt op activiteiten rondom de beeldvorming ten aanzien van mensen met een beperking. Kennispartners in dit project zijn de Hogeschool Utrecht, de lectoraten Leefstijl en Gezondheid en Digitale Architectuur, het UMCU locatie Wilhelmina Kinderziekenhuis, het kenniscentrum van Revalidatiecentrum de Hoogstraat en de afdelingen revalidatiegeneeskunde van het VUMC en het Erasmus MC. Ook hebben we goede contacten met de patiĂŤntenvereniging de BOSK en de afdeling sportgeneeskunde van het UMCU. Binnen dit project staat kennistransfer centraal, waarbij we “Kennissynthese: een handleidingâ€? (Bos & van Kammen) gebruiken als uitgangsdocument. Het draait daarbij niet uitsluitend om de informatie uit wetenschappelijk onderzoek: deze dient altijd gecombineerd te worden met informatie over de praktijk waarin de resultaten en beleidsmatige aspecten gebruikt moeten worden.

Belangrijkste resultaten tot dusver Drie METC- en/of niet-WMO-plichtige aanvragen zijn afgerond. De validatie van de Vitamove is afgerond. Scholingsmodule voor het bewegingsstimuleringsprogramma is klaar. Een eerste concept van het digitale KTS is gepresenteerd.

Het eerste symposium heeft plaatsgevonden. De eerste werkveldbijeenkomsten hebben plaatsgevonden. Tot nu toe actieve deelname van 40 praktijken en 20 studenten.

Hans Heneweer Lifestyle intervention and measuring quantity and quality of work. Research in an occupational setting March 2016

Background & research question Various publications have addressed the negative consequences of impaired health, illness, and disease for work ability and productivity loss at work. Besides impaired health conditions, different work characteristics, such as high physical or psychosocial work demands, may be related to productivity loss at work and absenteeism. Many factors affecting work ability can be influenced actively by lifestyle interventions aimed at physical activity, smoking, alcohol, food and relaxation.

The aim of this study is to explore the association between lifestyle interventions, i.c. physical reconditioning and dietary advice and the improvement of the quality and quantity of work performed on an daily basis. • What is the effect of a weekly physical training program, supported by a dietary advice, on the quality and quantity of work performed on an daily basis by the individual employee? • What is the proportion of variance within the improvement of scores on quality and quantity of work, explained by scores on physical training and physical, emotional and social lifestyle factors?

Design/methods Longitudinal study with repeated measurements at 6, 12 and 24 months and an estimated duration of 3 years. The population consists of employees on various degrees of sick leave and employees with an increased risk profile for sick leave of the police department of Utrecht (NL). Participants are referred by a company doctor and included into the program after ruling out conceivable health related risk factors for physical exertion.

Preliminary analysis revealed significant, (low up to moderate) correlations (rp) between scores on the Rand 36 (PCS) and VO2peak / BMI and the selfestimated emotional ability to perform work. Both scores on quality and quantity of work are significantly correlated with scores on the Rand 36 (MCS), VO2peak and BMI. Self-estimated physical work ability is moderately correlated with self-estimated emotional work ability, quantity and quality of work.

Achtergrond & onderzoeksvraag Uit de reviews uit de HALYNed, fase 1 Fit for the Future en gesprekken met de praktijk, blijkt er een behoefte te zijn om fysieke (in)activiteit (soort en mate van activiteit) objectief te meten bij lopende als rolstoelrijdende kinderen met een beperking. Op dit moment is er geen valide meetinstrument om fysieke activiteit objectief te meten dat toepasbaar is voor de praktijk bij kinderen met een fysieke beperking, ambulant of rolstoelrijdend.

De onderzoeksvraag luidt: hoe kun je fysieke activiteiten valide en objectief meten bij lopende en rolstoelrijdende kinderen en jongvolwassenen met een fysieke beperking?

Opzet/methode Ontwikkeling van het meetinstrument voor het meten van fysieke activiteit (content validity) gebeurt hoofdzakelijk op basis van een review: het meten van en deelname aan fysieke activiteit bij rolstoelrijdende kinderen. Daarnaast vinden gesprekken plaats met het werkveld, experts en rolstoelrijdende kinderen en hun ouders. Het werkveld wordt niet alleen betrokken bij de ontwikkeling van de test, maar ook bij de studies rondom reproduceerbaarheid en toepasbaarheid binnen de praktijk.

Een tweede stap is het valideren van dit nieuwe meetinstrument (Activ8). Het validatieproces bestaat uit criterion validity, waarbij het nieuwe meetinstrument vergeleken gaat worden met de gouden standaard. De gouden standaard is de gevalideerde accelerometer (VitaMove) uit de voorafgaande studie (fase 1 Fit for the Future). Voor het valideren zal bij 30 kinderen de Acti8 vergeleken

worden met video-opnames en gelijktijdig ook de VitaMove. De kinderen zullen zowel ambulant als rolstoelrijdend zijn met een fysieke beperking en kinderen die zich normaal ontwikkelen.

Belangrijkste resultaten tot dusver Gestart is met de ontwikkeling van een validatieprotocol voor de Activ8. De Activ8 zal het nieuwe meetinstrument mogelijk gaan worden voor het inzetten van het meten van fysieke activiteit in de kinderfysiotherapiepraktijk. Zo spoedig mogelijk wordt er een onderzoeksprotocol ingediend bij de METC van het Erasmus MC, voor een niet-WMO-verklaring, in samenwerking met R. van der Berg-Emons. De wens is om met de validatiestudie te beginnen in september 2014.

Discussie De Activ8 wordt eerst gevalideerd en als blijkt dat dit een valide meetinstrument is voor het meten van fysieke activiteit, zal het ge誰mplementeerd worden in de kinderfysiotherapeutische praktijk. Op deze manier is het mogelijk om objectief de fysieke activiteiten van lopende en rolstoelrijdende kinderen met een beperking te kunnen meten.

Franรงois Maissan Clinical decision making in patients with neck pain (CLICK) study

Background & research question Neck pain is a very common complaint in the Netherlands. In 2000 the period prevalence of twelve months of neck pain in the Dutch population of 25 years or older was 31,4%. Prevalence of chronic neck pain complaints of a duration longer than three months was 14,3%. Neck pain sets out one percent of all costs in the health care sector. Well executed physical therapy may diminish the socioeconomic burden. The main research question is: can we identify physical therapeutic interventions which improve specific impairments and/or activity limitations in patients with non-specific neck pain?

Design/method Two approaches will be taken on to achieve identification of interventions. If these approaches fail, a new study with a high level of external validity, namely a practice based evidence design, will be executed. The first two approaches are identification from scientific publications, to be specific RCTs, secondly by asking clinical experts using a Delphi method.

Preliminary outcome of the review with meta-analysis is an overview of 70 included RCTs. The effect of interventions on specific impairments were inconclusive or missing. The effects on activity limitations, measured with selfrating questionnaires, vary between studies. Unfortunately, there is considerable evidence that self-rating is not equal to the real performance. Therefore through self-rating outcome alone it is impossible to identify if an intervention improve the actual activity limitation.

The expectation of the Delphi is that there will be clinical expertise of intervention leading to specific therapeutic effects. However this remains to be demonstrated through external valid scientific research.

Therefore an extensive cohort study with a practice based evidence design will be carried out. Two important conditions for this kind of research are: having insight in the measurement error of the participating physical therapists and, secondly, objectifying the clinical reasoning process in the same way with the same outcome measures. Hence, in advance of the cohort study, a reliability study is needed. At the same time the participants need to be trained in objectifying there clinical reasoning proses the same way. Obviously it must be demonstrated that this happened. Thus appropriate research will be conducted.

Main results to date The early signs are that we know little of the impact of our physical therapy interventions in patients with non-specific neck pain in a scientific proven way.

Next to the current methods like RCTs, new more external valid methods need to be investigated if they are of any use to underpin the effect of our physical therapy interventions.

Jacqueline Nuijsink Gross motor development of infant using home-video registration with the Alberta Infant Motor Scale (GODIVA) 2015

Achtergrond en onderzoeksvraag Aanleiding voor het GODIVA-project was de vraag van kinderfysiotherapeuten om een methode te ontwikkelen waarmee de motorische ontwikkeling van jonge kinderen via filmbeelden, gemaakt door de ouders zelf, gevolgd kan worden in de tijd. Centrale vraagstelling is een objectieve, kwantificeerbare observatiemethode te ontwikkelen, die weinig belastend is voor ouders en kind, waarmee kinderfysiotherapeuten snel en efficiĂŤnt het verloop van de vroege grofmotorische ontwikkeling (dus van omrollen, loszitten of lopen) bij kinderen in de eerste anderhalf jaar van hun leven kunnen volgen. Voor deze toepassing wordt gebruik gemaakt van een bestaand instrument, de Alberta Infant Motor Scale (AIMS).

Opzet/methode Om aan deze vraag te kunnen voldoen is een consortium samengesteld waarin naast twee faculteiten van de HU (Faculteit Gezondheidszorg en Faculteit Natuur en techniek), de volgende partijen deelnemen: drie afdelingen kinderfysiotherapie van grote ziekenhuizen (Wilhelmina Kinderziekenhuis, VU medisch centrum, en St. Antoniusziekenhuis), de NVFK (beroepsvereniging kinderfysiotherapeuten), de regionale werkgroep kinderfysiotherapie (als vertegenwoordiger van 21 kinderfysiotherapiepraktijken), Leidsche Rijn Julius Gezondheidscentra, en de UU, faculteit Sociale Wetenschappen/Pedagogiek.

Hiermee ontstaat een stevig netwerk rondom professionals, opleiders en kennisinstellingen die zich bezig houden met de ontwikkeling van het jonge kind. Het onderzoek past binnen het thema Zorg en technologie en sluit goed aan bij het nieuwe missiethema van het Kenniscentrum Zelfredzaamheid.

Het consortium heeft de vraag van de kinderfysiotherapeuten uitgewerkt in drie innovatie- en onderzoeksprojecten: (1) een validatieonderzoek waarbij het beoordelen van motoriek via door ouders gemaakte video-opnames wordt vergeleken met observatie door therapeuten ter plaatse, gebruik makend van de AIMS; (2) een haalbaarheidstudie naar technische, juridische en operationele aspecten; (3) een pilot-onderzoek naar longitudinaal gemeten ontwikkelingsprofielen bij gezonde kinderen.

Zowel docent/onderzoekers als studenten en kinderfysiotherapeuten zullen deelnemen in de projecten. Deze projecten zijn voorwaardelijk voor latere onderzoeksprojecten waarmee werkveld en onderzoekers normwaarden en speciale profielen kunnen gaan onderzoeken.

De verwachte concrete resultaten voor het werkveld zijn: een bruikbare videoobservatiemethode voor testen en monitoren van ontwikkeling. Het onderwijs krijgt zo een webportal voor het veilig uploaden van filmmateriaal en een beelddatabank die breed ingezet kunnen worden bij het opleiden van toekomstige kinderfysiotherapeuten. Beide infrastructuren kunnen ingezet worden in vervolgonderzoek. Naast de concrete producten ontstaat meer kennis en inzicht in de (longitudinale) ontwikkelingstrajecten, een nieuwe manier om naar ontwikkeling te kijken. De samenwerking tussen de partners faciliteert een wisselwerking tussen de kenniscentra Hogeschool Utrecht (HU) en Universiteit Utrecht (UU), de masteropleiding (Kinder)Fysiotherapie HU en professionals: kinderfysiotherapeuten werkzaam in ziekenhuizen en in de eerstelijns praktijken en gezondheidscentra. Zij maken alle gebruik van elkaars competenties en vormen met elkaar een doelmatig netwerk rondom ontwikkeling van jonge kinderen.

Resultaten tot dusver Er zijn videoprotocollen en instructiefilms gemaakt. Er is een applicatie ontwikkeld (GODIVAPP) voor uploaden en downloaden van video’s. Het eerste symposium voor het werkveld is gehouden.

De validatiestudie is bijna afgerond (zie abstract Ellen van Dam). De ontwerp- en haalbaarheidsonderzoeken zijn in volle gang (zie abstract Marike Boonzaaijer).

De longitudinale studie is in voorbereiding.

Janke Oosterhaven PREPGO study: Pain rehabilitation in patients with low levels of health literacy - Explaining dropout in a chronic pain management program October 2016

Background & research question Chronic pain has a negative impact on quality of life, daily functioning, mood and sick leave. One in five adults in the Netherlands (2,25 million) suffers from chronic pain. Chronic pain rehabilitation programs have been shown to be effective. Unfortunately, patients with chronic pain dropping out of chronic pain programs is an important problem in those programs. Early dropouts more often have poor treatment outcomes and cause higher medical costs for society. In scientific research it remains unclear why and how many patients with chronic pain drop out in chronic pain rehabilitation in the Netherlands.

The aim of the PREPGO study is to get more insight into the mechanisms of dropping out. The central research question is: which socio-demographic and clinical factors predict early “dropping out” in patients with chronic pain being treated in a chronic pain management program? The hypothesis in the PREPGO study is that chronic pain patients with low levels of health literacy have higher dropout rates than chronic pain patients with higher levels of health literacy.

Low levels of health literacy may be related to maladaptive illness beliefs and unrealistic treatment beliefs, which in turn may lead to dropout.

Design/methods The PREPGO study is a “mixed methods” research; both qualitative and quantitative research methods are used. This study acknowledges the need to take account of patient perspectives in chronic pain rehabilitation programs. The chronic pain patients themselves are the research subjects in this study. The starting point of the PREPGO study is a systematic literature review about patient characteristics related to dropping out of chronic pain management

programs. In a longitudinal prospective quantitative study, all patients who start in a chronic pain program are followed throughout the program. From the patients with chronic pain who drop out in the pain rehabilitation program, quantitative data (social demographic data, depression (HADS), health status (SF 36), pain catastrophizing (PCS), pain self efficacy (PSEQ), pain intensity/ impairment (PSK), illness perceptions (IPQK) and treatment beliefs (TBQ)) will be collected to find out which factors predict dropout. With qualitative research methods more in-depth information will be collected on illness beliefs, treatment beliefs and health literacy levels and reasons for dropout in patients with chronic pain in a pain rehabilitation program.

Main results to date The output of the mixed methods research on dropout mechanisms will be used to construct a theoretical model explaining reasons for dropout in chronic pain management programs. A screening tool will be developed which can be used for the intake in the chronic pain rehabilitation. First results of the PREPGO study are expected in 2015.

Jacqueline Outermans Investigating and stimulating long term walking activity in stroke: Underlying mechanisms for walking capacity in chronic stroke September 2015

Background & research question Worldwide stroke is a major cause of disability (Roger, 2011), specifically impaired gait (Mayo, 1999) that stroke survivors perceive to be a significant problem (Pound, Lord, 2004). It has been shown that over 20% of community dwelling stroke patients show reduced walking activity in the community over time (Van de Port, 2006). A recent meta-analysis (Field, 2013) showed that people after stroke take a mean of 4,355 steps a day, which is only half to two third of the current recommendation for people with a disability of 6,500-8,500 steps a day (Tudor-Locke, 2011). A recent review (Billinger, 2014) describes the increased risk for components of metabolic syndrome due to low physical activity in stroke and emphasizes the benefits from exercise programs and increased physical activity.

However, increasing walking activity in stroke survivors may not be easy to implement without expanded knowledge and understanding of reasons and motivation for physical activity in general and walking specifically as well as profound knowledge on underlying mechanisms for walking.

The purpose of this study is the exploration of barriers and facilitators for walking activity from the perspective of community dwelling individuals after stroke in the chronic stage and exploring underlying mechanisms for walking.

Design/methods 1. Systematic review of the literature : “How strongly is VO2peak correlated with walking speed and walking distance after stroke?� Meta-analysis using the Hunter-Schmidt method.

Qualitative study: “Perceived barriers and facilitators for walking in community dwelling individuals after stroke.� Using semi-structured individual interviews and focus groups, performing a thematic content analysis utilizing the framework approach (Ritchie, Spencer) 3. Cross-sectional analysis of underlying factors for walking capacity in community dwelling stroke survivors in the chronic stage. Measures independent: VO2peak, O2 consumption during walking, balance, functional strength, gait symmetry, perceived exertion, falls efficacy, fatigue, depression and physical activity. Measure dependent: walking distance.

Main results to date The systematic review showed a low to moderate combined correlation coefficient between VO2peak and walking capacity, indicating that other factors influence this association.

Marleen Sol Ontwikkeling van een valide en betrouwbaar meetinstrument voor het meten van rolstoelvaardigheid bij kinderen in een handbewogen rolstoel September 2015

Achtergrond en onderzoeksvraag Dit onderzoek is een onderdeel van het project ‘Fit for the Future!’. Uit de review uit fase 1 van dit project en uit gesprekken met de praktijk blijkt rolstoelvaardigheid een belangrijke invloed te hebben op het niveau van activiteit en participatie van rolstoelrijdende kinderen. Voor rolstoelgebonden volwassenen zijn er al meerdere meetinstrumenten en trainingen voor rolstoelvaardigheid. Voor kinderen is er op dit moment echter geen valide en betrouwbaar meetinstrument om de rolstoelvaardigheid in kaart te brengen.

De onderzoeksvraag luidt: hoe kan je valide en betrouwbaar de rolstoelvaardigheid bij kinderen in een handbewogen rolstoel meten?

Opzet/methode Ontwikkeling van een meetinstrument voor het meten van rolstoelvaardigheid gebeurt op basis van een review: ‘ontwikkeling van een rolstoelvaardigheidstest voor kinderen: een combinatie van onderzoek met de mening van experts’.

Hiervoor wordt de literatuur doorzocht op rolstoelvaardigheidstesten voor rolstoelrijdende volwassenen. Uit deze resultaten volgt een lijst met testitems geschikt voor volwassenen. In samenwerking met het werkveld wordt de relevantie, reproduceerbaarheid en toepasbaarheid in de praktijk van deze testitems voor rolstoelrijdende kinderen bepaald.

De tweede stap is het bepalen van de test-hertest, inter- en intrarater betrouwbaarheid van de rolstoelvaardigheidstest bij rolstoelrijdende kinderen in de leeftijd van 7 tot 18 jaar. Hiervoor zal de test bij 30 rolstoelrijdende kinderen tweemaal worden afgenomen binnen een periode van twee weken. De resultaten worden digitaal vastgelegd.

Belangrijkste resultaten tot dusver Gestart is met de ontwikkeling van een rolstoelvaardigheidstest. In november 2014 zal het afnemen van de rolstoelvaardigheidstest bij 30 kinderen voor het betrouwbaarheidsonderzoek worden afgerond.

Discussie Er wordt op dit moment een rolstoelvaardigheidstest ontwikkeld. Als blijkt dat dit meetinstrument betrouwbaar en valide is, kan het worden ingezet in de praktijk en het onderzoek. In verder onderzoek kan dit meetinstrument gebruikt worden voor het evalueren en verbeteren van rolstoelvaardigheidstrainingen voor rolstoelrijdende kinderen.

Marlies Welbie Health literacy in physical therapy. How to adapt and use a questionnaire in a Dutch and Turkish low health literate patient population December 2015

Background & research question Health care develops towards a more patient centered care approach (PCC).

Patients are increasingly required to take a more active role in making decisions about their health. To do this effectively, patients need to be able to seek, understand and use health information. This concept is referred to as ‘‘health literacy’’ (HL). Poor HL is associated with reduced patient activation. The tendency towards passivity complicates interaction between patient and provider and undermines the possibilities for receiving PCC. Little is known about the patient's role in PCC and how patients can be empowered. This study aims to contribute to filling this knowledge gap.

The hypothesis underlying this study is that adapting and using a questionnaire to help patients with poor HL to order, prioritize and express their symptoms and limitations will help them to take a more active part in patient-providerinteraction during the diagnostic process of physical therapy treatment.

The aim of this study is to adapt a screening questionnaire and develop a tool which helps Dutch and Turkish physical therapy patients with poor HL who report pain to be more capable to take an active part in the decision making process within the diagnostic phase of physical therapy in The Netherlands.

Achtergrond en onderzoeksvraag Stemkwaliteit is niet eenvoudig te definiĂŤren, want de beoordeling ervan blijkt een multidimensionaal proces te zijn. Dit maakt het ingewikkeld om een dergelijke kwaliteitsbeoordeling uit te voeren. Perceptuele beoordeling is nog steeds de belangrijkste pijler voor het beoordelen van stemkwaliteit, maar de betrouwbaarheid is sterk wisselend. Om in de toekomst een betere beoordeling van de stemkwaliteitskenmerken roughness (R) en breathiness (B) te kunnen geven moeten objectieve meetmethoden worden ontwikkeld ter ondersteuning van perceptuele beoordeling.

Doel van het onderzoeksproject: 1. Literatuurstudie: welke relevante akoestische metingen werden in de wetenschappelijke literatuur reeds gerapporteerd voor de objectivering van de stemkwaliteitskenmerken R en B? 2. Literatuurstudie: van welke akoestische metingen is de correlatie met R en B reeds bestudeerd?

3. Meta-analyse: welke akoestische metingen hebben voldoende gewogen correlatie met R en B in doorlopende spraak (DS) en in een aangehouden vocaal (AV)?

4. Retrospectief onderzoek: kan er een multivariaat model op basis van diverse akoestische metingen ontwikkeld worden om R en B afzonderlijk te objectiveren in DS en in een AV? Opzet/methode 1) Retrospectief onderzoek op basis van uitgebreide literatuurstudie van de jaren 1960 t/m 2013 in peer-reviewed literatuur en het opstellen van de resultaten in twee meta-analysen (doelnr. 1 t/m 3).

Mieke Beers Ontwikkeling van de fonologische woordmaat Phonological Mean Length of Utterance (PMLU) voor de klinische praktijk Doorlopend onderzoek

Achtergrond en onderzoeksvraag Bij het analyseren van fonologische stoornissen wordt gebruik gemaakt van een fonologische procesanalyse (bijv. Metaphon) of een fonologische contrastanalyse (bijv. FAN). Beide beoordelen de accuraatheid in de productie van consonanten en vocalen, maar houden beperkt rekening met de invloed van de woordstructuur.

Variabiliteit in de klankproductie (consistente en inconsistente fouten) kan echter afhankelijk zijn van lexicale factoren, zoals de lengte en de complexiteit van het hele woord. De Phonological Mean Length of Utterance (PMLU) is een maat die van een set woorden de gemiddelde lengte berekent op basis van het aantal (in)correcte consonanten en vocalen. Van de PMLU kunnen andere fonologische woordmaten worden afgeleid. Bij normaal ontwikkelende Nederlandstalige kinderen van 1-4 jaar blijkt de PMLU toe te nemen met de leeftijd (Rodenburg– Van Wee, 2013). Als kinderen langere en complexere woorden gaan produceren met toenemende accuraatheid van de klankproductie, stijgt de PMLU. Dit onderzoeksproject vormt onderdeel van de onderzoekslijn ‘Fonologische Interventie bij kinderen met spraak- en taalontwikkelingsstoornissen’. Het project PMLU beoogt een klinische toepasbare maat te ontwikkelen waarmee in de eerste diagnostische fase een uitspraak kan worden gedaan over de vervolgdiagnostiek. De PMLU is een klinisch toepasbare maat, indien deze discriminerend, betrouwbaar en efficiënt is.

Eerste resultaten bij 15 kinderen van 4-6 jaar met fonologische stoornissen laten zien dat de PMLU een ontwikkelingsachterstand kan aantonen ten opzichte van zich normaal ontwikkelende kinderen, dat er per kind vooruitgang kan worden gemeten en dat een PMLU berekend kan worden over een relatief kleine sample woorden uit de spontane taal.

Opzet/methode Vervolgonderzoeken naar de klinische toepasbaarheid van de PMLU zullen zich richten op: • uitbreiding van de referentiegroep zich normaal ontwikkelende kinderen ouder dan 4 jaar; • validering door vergelijking met FAN-analysemethode; • nader onderzoek naar de vereiste samplegrootte bij kinderen met fonologische stoornissen; • geschiktheid van PMLU bij bijzondere groepen, zoals slechthorende kinderen en meertalige kinderen; • implementatie in de webapplicatie FAN 2.0.

Resultaten tot dusver Met de resultaten uit de verschillende deelonderzoeken wordt beoogd om de PMLU als maat geschikt te maken voor toepassing in het logopedisch werkveld.

Door het meten van de PMLU over een set woorden uit de spontane taal van het kind moet de logopedist een beslissing kunnen nemen over het wel of niet uitvoeren van uitgebreide handelingsgerichte diagnostiek bij kinderen met fonologische stoornissen.

Discussie De implementatie van de PMLU in het logopedisch werkveld biedt de logopedist de mogelijkheid onderbouwde keuzes te maken voor de inzet van logopedische zorg bij kinderen die door ernstige spraakproblemen moeilijk verstaanbaarheid zijn. Gerichte diagnostiek en behandeling bij kinderen die deze zorg het meest nodig hebben zal een belangrijke bijdrage leveren aan het verbeteren van de communicatieve redzaamheid van kinderen met fonologische stoornissen.

Achtergrond en onderzoeksvraag Kinderen met fonologische stoornissen hebben ernstige spraakproblemen en zijn hierdoor vaak moeilijk verstaanbaar. Dit kan leiden tot beperkingen in hun communicatieve redzaamheid. In de logopedische praktijk vormen zij het grootste deel van de populatie: circa 70%. Handelingsgerichte diagnostiek bij kinderen met fonologische problemen vindt plaats door het uitlokken van losse woorden of op basis van een analyse van lopende, spontane spraak.

Logopedisten ervaren beperkingen bij het vaststellen van behandeldoelen en behandelvormen voor de interventie bij kinderen met fonologische stoornissen.

Doel van deze onderzoekslijn is om fonologische diagnostiek en daarvan af te leiden behandelvormen inzichtelijk en toegankelijk te maken voor een brede doelgroep van professionals die zich bezig houden met de diagnostiek en behandeling van kinderen met fonologische stoornissen. De communicatieve redzaamheid van deze kinderen is een belangrijk doel voor de logopedische zorg.

In Nederland wordt handelingsgerichte diagnostiek bij kinderen met fonologische stoornissen op woordniveau verricht door analyses met een benoemtaak of door analyse van de spontane spraak met het computerprogramma FAN uit 1995. Hoewel verouderd, wordt dit programma nog steeds gezien als relevant voor de klinische praktijk. Het is uniek, omdat het een analyse biedt van woorden uit het alledaagse taalgebruik van het kind. Een FAN-analyse geeft een volledig beeld van de mogelijkheden en de onmogelijkheden in de spraakproductie van een kind. Het oorspronkelijke computerprogramma is onlangs beschikbaar gekomen als webapplicatie FAN 2.0.

Onderzoeksprojecten binnen de onderzoekslijn zijn gericht op : 1) Implementatie van de webapplicatie FAN 2.0 in het logopedische werkveld door het verbeteren van de gebruiksvriendelijkheid en de bereikbaarheid en door het uitbreiden met andere analysemethodes voor het beoordelen van de ernst en de aard van de fonologische stoornis.

2) Ontwikkeling van behandelmethodes en/of behandelvormen voor de logopedist bij kinderen met fonologische stoornissen.

Design/methodes De onderzoekslijn bestaat uit verschillende lopende en geplande projecten op het gebied van: 1) Implementatie webapplicatie: waar liggen de knelpunten? Wat zijn de wensen van de gebruikers? Welke vormgeving past bij het doel van de webapplicatie?

2) Uitbreiding webapplicatie: behandelmogelijkheden, andere fonologische meetinstrumenten, forum, FAQ’s.

3) Ontwikkeling fonologische behandelmethodes: inventariseren wensen gebruikers, ontwikkelen apps.

4) Onderzoek: uitbreiding normering, effectonderzoek bij nieuwe doelgroepen (CI), differentiaaldiagnostiek ernstige spraakproductiestoornissen.

Belangrijkste resultaten tot dusver Eerste projectresultaten betroffen de implementatie van de fonologische woordmaat PMLU, waarmee ontwikkelingsachterstand van kinderen met fonologische stoornissen kan worden gemeten.

Op de langere termijn beoogt de onderzoekslijn te komen tot een gebruiksvriendelijk instrument voor fonologische analyse en aansluitende diagnostiek geschikt voor gebruik door logopedisten, klinisch linguĂŻsten en taalspraakpathologen, en ontsluiting van kennis over interventie bij fonologische stoornissen voor een brede doelgroep van gebruikers.

Discussie Door de fonologische diagnostiek gebruiksvriendelijk en bereikbaar te maken en aan te vullen met een behandelmethode en/of behandelvormen, kan deze dichter bij de logopedische praktijk worden gebracht. De onderzoekslijn beoogt innovatie van de technologische mogelijkheden voor de zorg (diagnostiek en behandeling ter bevordering van de communicatieve redzaamheid) bij kinderen met fonologische stoornissen en bevordert de technologische ondersteuning van de professionals die betrokken zijn bij deze vorm van logopedische interventie.

Gerda Bruinsma Effectiveness of speech-language therapy in young children with developmental language disorders January 2018

Background & research question This study will investigate the effectiveness of speech and language therapy in preschool children with developmental language disorders receiving special education services.

A developmental language disorder severely affects communication skills and has adverse effects on all aspects of a young child’s development. Little is known about the effectiveness of different service delivery models combining therapy and special education in children with developmental language disorders. Results of effect studies are contradictory and in most cases aimed at older children. Moreover, there is a lack of research on child and therapy factors influencing effectiveness.

Research questions: 1. What is the effect of speech and language therapy on language performance of 4 and 5 year old children with developmental language disorders in different educational settings?

2. What child factors, therapy factors and educational factors are predictors of growth in language performance?

3. What is the grammatical development of children receiving speech-language therapy on schools for special education during one year?

4. What is the effectiveness of Communicative Language Intervention (CLI) on grammatical development in comparison to usual speech-language therapy in children visiting schools for special education?

Design/methods This study consists of two parts, a cohort study and an effect study.

Part 1: cohort study During one school year, the effect of speech-language therapy and educational support on language performance will be measured. A solid and efficient monitoring system will be designed to collect data on language skills and influencing factors.

200 children, age 4 and 5 years, scoring ≼ 1,5 SD below the mean on standardised language tests will be included. Measures on standardised language tests will be collected from the children's files (at Time 0 = start of the school year and Time 1 = end of the school year). Child, therapy and educational factors, such as intelligence, socioeconomic background, amount and frequency of intervention and class size will be registered. Regression analysis will be applied to predict factors for language growth.

Part 2: effect study Effectiveness of Communicative Language Intervention (CLI, Van den Dungen, 2007) will be studied using a time series design. 24 Children from different schools for special education will be randomly assigned to an intervention group and a control group. Measurements of grammatical development in spontaneous language will be collected at short intervals during one year. On Time 1 - Time 5 all participants will be receiving usual care. Subsequently the intervention group will receive CLI according to a protocol on Time 6 - Time 8.

Measurements on Time 9 and Time 10 will take place after the CLI intervention.

The growth curve of grammatical development of each child in the intervention group will be analised, making each child serve as its own control. In addition growth curves of the intervention group and the control group will be compared.

Main results to date The study is in a preparatory stage, so there are no results available yet. Data collection will start in September 2014. The results will lead to improvement of care and education that is offered to children with developmental language disorders.

Ellen Gerrits Effectiviteit woordenschattherapie op basis van semantische scripts

Achtergrond en onderzoeksvraag Tien. Dat is het aantal nieuwe woorden per dag dat jonge kinderen zonder oefening of behandeling leren. Bij kinderen met een specifieke taalontwikkelingsstoornis is dit niet zo vanzelfsprekend. Hoe kunnen deze kinderen wel meer nieuwe woorden leren? Auris, Hogeschool Utrecht en de NSDSK onderzoeken of deze kinderen meer nieuwe woorden leren tijdens een nieuwe geprotocolleerde woordenschattherapie, dan tijdens de behandeling die nu vaak wordt aangeboden (usual care). De Viertakt voor het aanbieden van nieuwe woorden van Verhallen en Verhallen (1994) vormt de basis in het woordenschatprotocol. De Viertakt bestaat uit vier fases: voorbewerken, semantiseren, consolideren en controleren. In de geprotocolleerde therapie zijn alle stappen uitgeschreven; van doelen, werkwijze en materialen tot verhaaltjes of situaties met doelwoorden. Hierdoor kan worden gegarandeerd dat de kinderen vaak aan te leren woorden worden blootgesteld. In dit project wordt de effectiviteit van deze werkwijze onderzocht. Het project levert daarmee een bijdrage aan evidence based logopedisch handelen en de effectiviteit van woordenschatbehandeling in de behandelgroepen en de klassen van kinderen met een taalontwikkelingsstoornis.

Opzet/methode In een pilot study is een geprotocolleerde woordenschatinterventie met semantische scripts vergeleken met het reguliere woordenschatinterventieprogramma bij peuters met een specifieke taalontwikkelingsstoornis in peuterbehandelgroepen van de NSDSK. Kinderen in de leeftijd van 2 tot 4 jaar met een specifieke taalontwikkelingsstoornis zijn willekeurig toegewezen aan de geprotocolleerde woordenschatinterventie (n =12) of aan de controleconditie (n

= 10). Logopedisten boden gedurende vier weken met drie sessies per week van 15 minuten aan de experimentele groep geprotocolleerde woordenschatinterventie. Daarnaast volgde deze groep het reguliere interventieprogramma.

De controlegroep volgde alleen het reguliere interventieprogramma. Bij alle kinderen is de expressieve woordkennis van de themawoorden met een voor- en nameting getest.

Momenteel wordt er bij een grotere groep peuters een randomized controlled trial voorbereid. Hierin komen meer woordenschatthema’s aan de orde en wordt een grotere groep logopedisten, werkzaam in verschillende instellingen, betrokken.

Belangrijkste resultaten tot dusver In afstudeeronderzoek van studenten Logopedie zijn al eerder positieve effecten gevonden van de didactiek ‘Met woorden in de weer’ bij kinderen van verschillende leeftijden met een taalontwikkelingsstoornis. Uit de resultaten van de pilot study naar geprotocolleerde woordenschatbehandeling met semantische scripts blijkt dat de peuters in de experimentele groep significant meer woorden leerden dan de peuters in de controlegroep.

Discussie Er zijn sterke aanwijzingen dat peuters met specifieke taalontwikkelingsstoornis profijt hebben van een sterk gestructureerde woordenschatinterventie bovenop het reguliere interventieprogramma. Om de resultaten van de pilot study te generaliseren naar peuterbehandelgroepen binnen andere instellingen, wordt deze onderzoeksopzet herhaald bij een groter aantal participanten door het lectoraat Logopedie: participatie door communicatie van Hogeschool Utrecht en studenten van de opleiding Logopediewetenschappen, in samenwerking met de Koninklijke Auris Groep en de NSDSK.

Esther van Niel Effectiviteit van compenserende therapie bij oudere kinderen met taalontwikkelingsstoornissen 2018

Achtergrond en onderzoeksvraag Kinderen met een specifieke taalontwikkelingsstoornis hebben veel moeite met het begrijpen en uiten van taal zonder aanwijsbare oorzaak. Deze stoornis heeft negatieve gevolgen voor het kind op academisch en sociaal-emotioneel niveau: deze kinderen hebben verhoogde kans op leerproblemen, gedragsproblemen en minder maatschappelijk succes dan kinderen zonder taalstoornis. Deze stoornis komt voor bij 7% van de kinderen en kan zich dusdanig hardnekkig manifesteren dat deze levenslang aanwezig blijft.

De logopedist is de primaire behandelaar van deze groep kinderen.

Logopedisten merken dat oudere kinderen (vanaf 7 jaar) weinig profijt (meer) hebben van stoornisgerichte therapie (behandeling gericht op het verhelpen van de stoornis). Vanuit de literatuur krijgen logopedisten weinig handvatten voor de effectieve invulling van de behandeling van oudere kinderen: effectonderzoek heeft zich voornamelijk gericht op jongere kinderen. Uit interviews met logopedisten blijkt dat zij compenserende therapie gaan inzetten. Hierin leert het kind strategieën en versterkt het andere vaardigheden om de communicatieve redzaamheid en participatie te verbeteren. Voorbeelden zijn het leren van leerstrategieën (leren leren) en metacognitieve strategieën (bewustwording en leren reflecteren op hoe taken worden aangepakt).

Dat logopedisten bij oudere kinderen metacognitieve strategieën inzetten wordt bevestigd door onderzoek uit het Verenigd Koninkrijk, maar onduidelijk is of deze resultaten te generaliseren zijn naar de Nederlandse situatie. Het is onbekend welke inhoud Nederlandse logopedisten geven aan compenserende therapie. Daarnaast is onduidelijk wat het effect is van het inzetten van

compenserende therapie. Gezien de hardnekkigheid van een taalontwikkelingsstoornis en de negatieve gevolgen die het kind hiervan ondervindt is het noodzakelijk dat begeleiding adequaat en effectief wordt vormgegeven.

De onderzoeksvraag luidt: wat is het effect van compenserende therapie op de taalvaardigheid en communicatieve redzaamheid van oudere kinderen met een specifieke taalontwikkelingsstoornis? (a) Welke inhoud geven Nederlandse logopedisten aan compenserende therapie en welke strategieën/vaardigheden vinden zij het meest effectief voor de taalvaardigheid en participatie van het kind? (b) Wat is het effect van het aanleren van metacognitieve strategieën bij oudere kinderen met een taalontwikkelingsstoornis op de aandachtsvaardigheden en de taalverwerking?

Opzet/methode Deelvraag (a) Participanten: logopedisten uit alle werksettings (vrije vestiging, speciaal basisonderwijs, speciaal onderwijs, speciaal voorgezet onderwijs) waarin oudere kinderen worden behandeld.

Meetinstrument: enquête op basis van literatuurstudie en gesprekken met ervaren logopedisten die werken met oudere kinderen. Met een enquête worden veel logopedisten in Nederland bereikt. Zo ontstaat een compleet beeld van de huidige praktijk in Nederland.

Deelvraag (b) (in ontwikkeling) Participanten: oudere kinderen (vanaf 7 jaar) met ernstige taalontwikkelingsstoornissen, behandelend logopedisten, leerkrachten van de participerende kinderen.

Meetinstrumenten: zelf ontwikkelde observatielijst voor transfer van aandachtsvaardigheden, Test of Everday Attention for Children (TEA-Ch), Clinical Evaluation of Language Scales (CELF) 4 NL; onderdeel tekstbegrip, begrippen en aanwijzingen volgen en cijfers herhalen; eventueel zelf ontwikkelde vragenlijst met Patient Recorded Outcome Measures Interventie: aanleren van metacognitieve vaardigheden met therapieprogramma LinC. Design: controlled time series effectstudie. Met meerdere baseline-metingen wordt de ontwikkeling in kaart gebracht, waarna de interventie plaatsvindt en metingen achteraf worden verricht om het effect en de retentie te bepalen.

Ingrid Singer DigiTaal: innovatie van taaltherapie met games

Achtergrond en onderzoeksvraag Games bieden mogelijkheden om de doelmatigheid en effectiviteit van taaltherapie te vergroten. Taaltherapie wordt doelmatiger doordat games de mogelijkheid bieden tot automatische monitoring van antwoorden, herhaalde oefening van alleen incorrecte onderdelen, en een directe weergave van leeropbrengsten. Taaltherapie wordt bovendien effectiever door gemotiveerde herhaalde oefening middels het wedstrijdelement, een passende moeilijkheidsgraad door verschillende 'levels' en impliciet leren door zelfstandig te gamen.

Aansluiten bij de belevingswereld van het kind is een voorwaarde voor effectieve taaltherapie: het zorgt voor motivatie, aandacht en betrokkenheid.

Logopedisten zetten apps al volop in tijdens de behandeling. Experts zijn van oordeel dat apps op tablets veel voordelen bieden binnen een behandelsetting van kinderen met een taalontwikkelingsstoornis, omdat apps kinderen motiveren en aanspreken. Ook de eenvoudige interface, lage kosten en brede beschikbaarheid worden gezien als voordelen. Het is dus zaak om te onderzoeken of het inzetten van apps past binnen een effectieve behandeling van taalontwikkelingsstoornissen.

De kennis en ervaring uit het project leveren inzichten op over evidence-based gaming in taaltherapie en zijn inzetbaar in het hele taalonderwijs. Op hogeschoolniveau betekent dit dat de verworven kennis zal terugvloeien in het onderwijs van meerdere faculteiten. Binnen de HU zijn dat de faculteiten Gezondheidszorg, Communicatie en Journalistiek (Multimedia design) en Educatie.

Opzet/methode Voor het onderzoek worden kwalitatieve en kwantitatieve methoden ingezet, zoals focusgroepen, survey-onderzoek, literatuuronderzoek, systematic review en systematische observaties. Studenten participeren in deelprojecten, zoals het ontwikkelen van meetinstrumenten, onderzoeken van het effect van gebruik van apps op het gedrag van het kind en de logopedist, en onderzoek naar de ervaringen van (ouders van) patiĂŤnten en therapeuten met het gebruik van games in taaltherapie.

Belangrijkste resultaten tot dusver In 2013 hebben studenten Logopedie een checklist ontwikkeld waarmee logopedisten kunnen beoordelen hoe geschikt een app is voor taaltherapie. De checklist is gebaseerd op literatuuronderzoek en een onderzoek onder gebruikers. De volgende stap is onderzoek naar de validiteit en betrouwbaarheid van deze checklist.

Uit een scoping-studie naar literatuur over het inzetten van games in taaltherapie blijkt dat kinderen met en zonder taalontwikkelingsstoornis taal kunnen leren door games te spelen op een pc. Kinderen met een normale taalontwikkeling, maar ook kinderen met een ontwikkelingsachterstand leren door het gebruik van apps op een tablet vaak evenveel of meer taal te leren dan wanneer ze de gebruikelijke instructie krijgen. Deze uitkomsten zijn gepresenteerd op het ICPLA in Stockholm. Een volgende stap is het uitvoeren van een systematic review naar het effect van pc-games op de taalontwikkeling bij kinderen met een taalontwikkelingsstoornis. Er zijn nog geen effectstudies gedaan naar het gebruik van apps in taaltherapie bij kinderen met een taalontwikkelingsstoornis.

Discussie Het gebruik van games in taaltherapie lijkt een logische stap, maar vereist wel het nodige onderzoek in samenwerking met de praktijk. Het project DigiTaal is opgezet om het inzicht te vergroten in de werkelijke kansen en mogelijkheden van het selectief inzetten van games in taaltherapie.

Jeanet Smilde Peiling naar kennisbehoefte bij JGZprofessionals omtrent taalachterstand en toeleiding naar VVE Juli 2014

Achtergrond en onderzoeksvraag Voor- en vroegschoolse educatie (VVE) is onderwijs voor zeer jonge kinderen met een taalachterstand. Artsen en verpleegkundigen in de jeugdgezondheidszorg (JGZ) hebben een essentiële rol in het signaleren van taalachterstanden en de doorverwijzing naar VVE. Taalscreening vindt plaats op diverse contactmomenten, door gebruik van screeningsinstrumenten zoals het Van Wiechen-onderzoek en SNEL. Wanneer er, op basis van de gebruikte taalscreeningsinstrumenten, twijfels zijn omtrent het taalniveau van een kind, kan een kind doorverwezen worden naar VVE of naar het logopedisch spreekuur. De toeleiding naar VVE blijkt (landelijk) niet optimaal te verlopen.

Opdat specifieke bijscholing en lokale protocollen kunnen leiden tot optimalisering van de toeleiding tot VVE, is inzicht nodig in de huidige kennis(behoefte) en werkwijze. TNO Leiden, Hogeschool Utrecht en GGD Hollands Midden hebben hiertoe een peiling uitgevoerd naar de kennisbehoefte van deze professionals bij GGD Hollands Midden. Cofinanciering voor het project is verkregen via de Akademische Werkplaats Publieke Gezondheid Noordelijk Zuid-Holland.

De onderzoeksvragen luiden: wat is de kennisbehoefte bij professionals om een blootstellings(taal)achterstand adequaat te kunnen signaleren en door te kunnen verwijzen naar VVE? Hoe vindt verwijzing naar VVE binnen de regio plaats? Wat zijn verschillen en wat zijn overeenkomsten?

Opzet/methode Online enquête onder 158 JGZ-professionals van GGD Hollands Midden. De projectregio GGD Hollands Midden is onderverdeeld in vier (sub)regio’s,

namelijk regio Midden Holland, regio Duin- & Bollenstreek, regio Leiden en regio Rijnstreek.

Eindproducten: presentatie, factsheet en rapport Belangrijkste resultaten tot dusver Respons: 62% (= 99 vragenlijsten retour) Kennis: er zijn hiaten in kennis over normale taalverwerving en kennis over taalontwikkelingsstoornissen. Basale kennis over meertaligheid is bij de meerderheid aanwezig. Er zijn geen grote verschillen tussen de verschillende regio’s.

Screening: Van Wiechen en SNEL zijn bij een ruime meerderheid bekend, maar er is onduidelijkheid over de vervolgstappen bij niet-pluisscores. Scores op screeningsinstrumenten worden niet altijd genoteerd in het dossier. De ‘Werkinstructie JGZ / VVE’ van GGD Hollands Midden is bij 2/5 niet bekend; er zijn regionale verschillen. Niet iedereen kan een GGD-logopedist raadplegen bij vragen. Hier is wel behoefte aan.

Verwijzing: er is veel onduidelijkheid en onzekerheid over de verwijzing naar VVE, m.b.t. doelgroep, verwijzingscriteria en taaldiagnostiek.

Toeleidingsformulier Verwijzing VVE PSZ/KDV is bij 1/3 onbekend en wordt door 1/5 niet gebruikt.

Advisering: een ruime meerderheid is op de hoogte van een aantal goede taaladviezen. Het gebruik van GGD-informatiefolders is beperkt.

Scholingsbehoefte bij medewerkers: in alle regio’s bestaat duidelijke behoefte aan bijscholing en extra ondersteuning bij taalscreening.

Conclusies: 1. Veel onduidelijkheid over het VVE-beleid! (doelgroepen, criteria) 2. Regionale verschillen in werkwijze 3. Verbetering van toeleiding mogelijk Discussie Aanbevelingen: • bijscholing van JGZ-professionals; • iedereen mogelijkheid bieden een logopedist binnen GGD te kunnen raadplegen; • beleid VVE: meer uniformiteit in criteria en toeleiding; • beleid t.a.v. ‘eigen‘ screeningsinstrumenten: wel of niet gebruiken?

Ansam Barakat EHealth program for the management of the health status of type 2 diabetes mellitus patients. A personalised approach to enable the management of type 2 diabetes mellitus 2019

Background & research question Type 2 diabetes mellitus (T2DM) is one of the largest chronic diseases. 347 million people worldwide have diabetes. T2DM accounts for more than 80% of all cases of diabetes; its incidence continues to increase worldwide and it has serious short-term and long-term implications due to glycaemia fluctuation. A better regulation of glycaemia reduces the risk of experiencing the complications of T2DM. Early research shows that the regulation of T2DM disease could be managed by physical activity (PA) and dietary patterns. Despite the given advice to control glycaemia through PA and diet, there does not currently appear to be much evidence that the given advice significantly improves glycaemic control, body mass index or blood pressure and that the given advices are achievable for all patients.

New technology, such as eHealth applications, offers some exciting new opportunities to expand on the success above and counter some of the difficulties. In a rapport of The Dutch College of General Practitioners, eHealth will play a larger role in the communication between patient and professional and management of chronic diseases (https://www.nhg.org/nhg-e-health; (LHV; NHG, 2012). eHealth could be used as tool to manage T2DM. Early research indicates that eHealth technology and namely mobile health intervention could be a first step to better management of type 2 diabetes.

This PhD-project aims to use an eHealth intervention program for the management of the health status of T2DM patients. This program takes the form of a personalised mobile health intervention program, which will induce a change in daily physical activity and diet for T2DM patients. The main research

question of this PhD-project is: can a personalised mobile health program induce a change in level of daily physical activity and diet, which in turn improve or stabilize the health status in persons with T2DM patients?

Design/methods In this PhD-project the Medical Research Council (MRC) framework and the Chronic Care Model (CCM) will be used as theoretical background models to the develop and evaluate this project. For the realization of the project, a randomized controlled trial (RCT) will be conducted.

The improvement or stabilization of the health status includes hemoglobin A1c (HbA1c), body mass index and quality of life. A stable or even better management of the health status will likely result in reduced medication use and enhancement of self-management, participation to society and a reduction in health care costs. In terms of results for health care professionals, by using a mobile health monitoring system, the quality of care could be higher.

Discussion An expected return of this PhD-project is a valid and reliable mobile health monitoring intervention program to achieve a stable or even better management of the health status of T2DM patients. This mobile health monitoring program is expected to promote daily physical activity as well as diet as described by the professionals for patients with T2DM. By using a mobile health monitoring system, the quality of care could be increased, due to better insight in the condition of the patient, which provides foothold for more personalised advice to patients.

Background & research question Within the next decade the number of care professionals (in the Netherlands) will no longer be sufficient to deal with the increasing demand for care. eHealth, for example screen-to-screen contact instead of regular face-to-face, offers older adults a solution to age in place and is claimed to be a solution for the increasing demand for care. Despite the apparent positive effects of this technology, there are several barriers to implementation. One of the main barriers is largely caused by the lack of knowledge and skills of health care professionals how to use this new technology. The Unified Theory of Acceptance and Use of Technology (UTAUT) has been used as a starting point in phase 1. Theories of Entrustable Professional Activities (EPA’s) and medical competence will play a central role in phase 2, 3 and 4.

Main results to date Results phase 1: In our sample of nurses, willingness to use home telehealth was predicted by its (a) perceived usefulness to the client (β = 0.435, p < 0.001)), (b) effort expectancy (β = 0.280, p = 0.001), (c) social influence (β = 0.216, p = 0.013), and (d) cost expectations (β = 0.236, p = 0.004).

Discussion Home telehealth, signifying a transition from face-to-face care to screen-toscreen care, may contrast with the expectations health care professionals may have at the beginning of their career. Dutch health care professionals feel valued when they help patients out of bed, change their clothes, help them eat, or assist them in taking a walk (activities of daily living). The fear of losing this interaction may create a barrier for the use of home telehealth by health care professionals. Therefore, more research is needed to gain an understanding of the quality of contact when nurses provide ‘care at distance’. Furthermore, the actual effectiveness of educating nurses and / or nursing students based predictors from phase 1, has to be examined. For many years, studies in the literature have emphasized the urgency of eHealth education. Therefore, the slow development of eHealth education is remarkable.

Emelieke Huisman Quality of light in a long term care facility in the Netherlands January 2015

Background & research question Several studies describe the importance of light for our physical and psychological wellbeing. Exposure to light can affect human experience, performance, and physiology via both image-forming and non-image forming processes. (Day)light is a key concept for life enrichment care facilities. ‘Life enrichment care’ is a concept based on healing environments, which targets long term care facilities (Ltcf) rather than hospitals. This concept focuses on the well-being and quality of life of frail elderly in Ltcf. Among Lctf residents are often people who suffer from some form of dementia. For this group, the image forming and non-image forming aspects of light play a role. The image forming or visual aspects of light include the ability to perform daily tasks, and are related to the prevention of falls. The most important non-image forming aspect of light is light's influence on controlling circadian rhythms (the “biological clock”). Older adults with dementia often suffer from disturbances in circadian rhythm, which can lead to behavioral problems including nocturnal unrest. This nocturnal unrest is mostly treated with sleep medication. However, in previous research, the emphasis was on light in general and electric lighting in specific.

No clear distinction was made between electric lighting and daylight. This study investigates the effect of (day)light on the well-being of frail elderly and healthcare professionals in Ltcf. This paper describes the current quality of light at a Ltcf in the Netherlands, which is discussed with reference to currently available recommended values.

Design/methods The study was conducted in four living rooms of the psycho-geriatric department of an Ltcf in the Netherlands, in July 2012. In order to chart the

current quality of light in the Ltcf, several methods were used. The following parameters were quantified both for electric lighting and for daylight: the horizontal and vertical illuminance at relevant positions and viewing directions; vertical colour temperature at eye level of a sitting resident; and, measurement the luminance ratios for the visual effects of light. To determine the contribution of daylight in illuminating the four living rooms, one series of measurements was done with the electrical lighting switched on and one series with the electrical lighting switched off.

Main results to date In the current situation, about half of the measured positions meet the recommended values of at least 750 lux for the horizontal and vertical illuminance. In the several studies, a minimum horizontal and vertical illuminance value of 750 lux is recommended. However, in other studies values of 1,000 lux are mentioned. Values of 750 lux were mainly reached in areas relatively close to the window. For positions further away from the window, the illuminance values notably decreased. The measured colour temperature in the living rooms were between 4,000 K and 5,500 K, with some peaks up to 7,000 K. A minimum colour temperature value of 5,000 K is recommended, and is usually reached when only daylight is present and when viewing in the direction of the window.

The measurements in this study took place during summer; it is expected that the illuminance levels in winter do not reach the recommended values. The results will be used as input for the development of another light strategy for Ltcf.

Marianne Sinoo Identification of Visual Problems among Nursing Home Residents: evaluating client records and light conditions December 2014

Background & research question According to the 2010 global data on visual impairment published by the World Health Organization (WHO), the prevalence of visual impairment is high in people aged 50 years and older. The highest estimated prevalence (> 40%) of visual impairment (blindness and low vision) has been found in the subgroup of residents of nursing homes and residential care facilities. The role of care professionals and the environment of the nursing home is described by the framework of the ICF (International Classification of Functioning, Disability, and Health). The ICF invites care professionals to communicate regarding the functioning of their clients in a “common language” Objective: reflection on visual problems in nursing homes.

Research questions in this study are: • What is the quality of the illuminance levels in common rooms and corridors in nursing homes?

• What age-related eye pathologies are reported by optometrists after eye examinations?

• What is the status of recording of information on visual problems or other eye-related data in client files?

Design/methods The optometric examinations and recorded visual problems were combined with illuminance data.

In seven nursing homes, 259 residents underwent an optometric examination.

Their client records were analysed for information regarding visual functioning.

The illuminance data were ranked to set the quality of the lighting conditions.

Main results to date Of the referred residents, 50% had problems with cataracts, retinal problems (21%), suspected glaucoma (13%), and other pathologies (16%). The information was not actual in 56% of the records. The quality of light conditions was low or moderate.

This study showed that no information on visual functioning was found in the client records of 67% (80/119) of the residents who were referred by the optometrist for additional ophthalmologic consultation. Furthermore, if information on visual problems was recorded, in more than half of the cases (56%), this involved information from the past. Additionally, the light in common rooms and corridors in all nursing homes was of low or moderate quality.

In 50% (59/119) of the referred residents, the main reason for referral was related to problems with cataracts, followed by retinal problems (25/119 or 21%), suspected glaucoma (16/119 or 13%), or other pathologies (19/119 or 16%). In nursing homes, staff awareness about past and current visual problems among nursing home residents should be raised. Professional caregivers in the nursing home as well as consulting optometrists can be seen as an important environmental factor in diagnosing eye pathologies and controlling lighting conditions.

Sigrid Vorrink EHealth ter optimalisatie van de multidisciplinaire COPD-zorg September 2015

Achtergrond en onderzoeksvraag Bij de behandeling van COPD (Chronisch Obstructief Longlijden) zijn hulpverleners uit verschillende disciplines betrokken. Het is voor de verschillende disciplines lastig om goed zicht te houden op een patiënt en zijn/haar ziekteverloop. Meestal komt de COPD-patiënt pas weer in beeld wanneer deze een nieuwe exacerbatie (verslechtering van de ziekte) krijgt.

Tijdens revalidatie wordt de fysieke activiteit (FA) van de patiënt weer op peil gebracht. Na revalidatie krijgt hij/zij een activiteitenprogramma mee naar huis.

Helaas houdt het merendeel van de mensen zich al snel niet meer aan dit programma: zij vallen terug naar inactiviteit. Inactiviteit zorgt voor kortademigheid en deconditioneren. De behandeling die de patiënt tijdens de periode van revalidatie heeft gehad wordt hierdoor weer teniet gedaan.

Momenteel zijn de controles maar een paar keer per jaar. Dit levert de zorgprofessional te weinig informatie om een adequate behandeling te bieden.

Als behandelaars van COPD-patiënten eerder kunnen zien dat de gezondheidstoestand achteruit gaat zouden ze eerder kunnen ingrijpen. Op een eerder moment zal de behandeling die gegeven moet worden minder intensief zijn dan na een exacerbatie, wat kan zorgen voor een verlaagde werkdruk en minder kosten. In de Utrechtse gezondheidsregio zijn een aantal (long)artsen, (long)verpleegkundigen en fysiotherapeuten al geruime tijd bezig om het netwerk van zorg voor mensen met COPD te optimaliseren. Hun vraag is hoe zij als zorgprofessionals effectiever kunnen werken en daarbij zo veel mogelijk rendement kunnen halen uit hun behandelingen.

Het onderzoek wil de multidisciplinaire COPD-zorg voor mensen met COPD na de revalidatieperiode optimaliseren met een laagdrempelige, betaalbare

interventie. Daarmee kunnen mensen met COPD zelf hun fysieke activiteit meten en hun bewegingsdoelen bijhouden. Daarnaast levert de interventie informatie op voor de betrokken professionals, zodat deze op afstand kunnen monitoren of een patiënt zich houdt aan het opgegeven bewegingsprogramma.

Hierdoor is het mogelijk eerder te behandelen en kan de zorgprofessional tevens goed over de tijd heen zien wat het patroon is in de fysieke activiteit van een patiënt. Nadat het fysieke activiteitenpatroon duidelijk is kan het trainingsprogramma daarop worden aangepast, zodat het beter past bij de patiënt.

Opzet/methode eHealth-interventie.

De instrumenten zijn: • een eHealth-applicatie voor smartphones die mensen met COPD kan monitoren en tegelijkertijd kan motiveren om te blijven bewegen; • een website voor de zorgprofessionals om de bewegingsstatus van de patiënt uit te lezen en te beoordelen, zodat deze de patiënt van feedback kunnen voorzien en snel kunnen ingrijpen op het moment dat dit nodig is.

Wanneer uit het promotieonderzoek blijkt dat de interventie effectief is, dan worden de producten geïmplementeerd. De opgedane kennis en de te ontwikkelen interventie zullen een onderdeel zijn van de gebruikelijke zorg aan mensen met COPD. De verkregen kennis uit het project komt ten goede aan het onderwijs aan de hbo-opleidingen: • HBO-V, de specialisatie Longverpleegkunde • Advance Nursing Practice • Fysiotherapie en aan de universitaire opleidingen: • Verplegingwetenschappen • Fysiotherapiewetenschappen • Bewegingswetenschappen De kennis en ervaring zal inzichten opleveren voor het gebruik van eHealth voor andere doelgroepen die een beperking in de mobiliteit ervaren.

Katarina Jerkovic-Cosic Innovaties in de preventieve mondzorg

Achtergrond en onderzoeksvraag De opleiding mondzorgkunde is bezig met de ontwikkeling naar een kennisorganisatie. Er zijn door het team onderzoekslijnen opgezet die zich richten op innovaties van preventieve mondzorg voor met name risicogroepen en/of groepen met een langdurige zorgvraag. Het doel is tweeledig: ten eerste gaat het om het verhogen van de zelfredzaamheid van deze patiënten, ten tweede om bewijs te leveren voor de bijdrage van de mondhygiënist als een paramedische beroepsbeoefenaar in de preventieve zorg. Hierbij gaat het om het ontwikkelen, implementeren en evalueren van innovatieve preventiestrategieën. Ook wordt er onderzoek verricht naar de rol en de bijdrage van de mondhygiënist in de mondzorg van de toekomst.

Opzet/methode De onderzoeken die uitgevoerd zijn of zullen worden maken gebruik van verschillende soorten designs (zie resultaten) en worden in verschillende settings uitgevoerd.

Het onderzoek bij de doelgroepen kinderen en ouderen wordt voornamelijk georganiseerd in samenwerking met externe partners, bijvoorbeeld de GGD, verloskundigen, basisscholen, JGZ (consultatiebureaus), verpleeg- en verzorgingshuizen, ouderenbonden, thuiszorgorganisaties etc. De HU-kliniek Mondzorgkunde leent zich voor onderzoek onder volwassen patiënten. Het onderzoek op de HU-kliniek komt voornamelijk ten goede van de onderwijsontwikkeling en professionalisering van de docenten.

Binnen de lijn organisatie van (mond)zorg zal onderzoek verricht worden naar de doeltreffendheid en doelmatigheid van de inzet van de mondhygiënisten om de positie en de bijdrage van de mondhygiënist in de mondzorg te onderbouwen.

Er zullen studies worden verricht naar kosteneffectiviteit en de meest optimale praktijkscenario’s om de preventieve mondzorg ook buiten de mondhygiënische praktijk te bieden.

Discussie Mondgezondheid is onlosmakelijk verbonden met de algemene gezondheid en het algemeen welbevinden. Ziekten als diabetes, hart- en vaatziekten en reumatoïde artritis staan in relatie met een slechte mondgezondheid.

Het onderzoek vanuit de mondzorgkunde zal nieuwe kennis onder de beroepsgroep verspreiden en daarnaast wordt er ingezet op het verspreiden van kennis over (preventieve) mondzorg onder de andere beroepsgroepen in de gezondheidszorg (bijv. medewerkers GGD, JGZ, verpleeg-, verzorgpersoneel, managers van de zorginstellingen, diëtisten, logopedisten etc). In het kader hiervan zoekt de opleiding Mondzorgkunde graag aansluiting met docenten/onderzoekers van het KC die in hun onderzoeken mogelijkheden zien om samen te werken en aansluiting hebben met het onderwerp mondzorg.

Jan Roelof Polling Why do eyes become myopic?

Background & research question Refractive errors are the most common eye disorders worldwide and the largest source of visual impairment. In particular, high near-sightedness (myopia) with long axial length is associated with a significant risk of visual complications, such as myopic macular degeneration, glaucoma, and retinal detachment. Reports have shown that the prevalence of myopia is on the rise. In the United States, the prevalence of myopia increased by 145% during the past 3 decades, and the rate of high myopia increased by 820%. In South Korea, the increase in the prevalence of myopia and high myopia was 334% and 891%, respectively.

Although the same trends are found in African and European populations, the prevalence of myopia is currently highest in Asians. In Singapore, 80% to 90% of young adults are myopic. Besides genetic predestination, numerous studies have shown that lifestyle factors, particularly education level and outdoor exposure, play a crucial role in the onset and progression of myopia. Myopia is mostly caused by growth of the eye axis during childhood. The first question is: why do eyes become myopic? This research project summarizes current insights into the environmental development of this trait.

Design/methods We aimed to investigate outdoor exposure and its influence on axial length (parameter for myopia) in children before they learn to read. In our multi-ethnic population-based birth-cohort study Generation R, Rotterdam, the Netherlands, we examined the length of the eyeball in 6,690 children at age 5-6 years. Data on time spent outdoors and near work activities were obtained by questionnaire.

Main results to date The prevalence of myopia was 1.9% (129 children), and varied from 0.9% in Dutch to 3.2% in non-western immigrants. Children who played outside less than 45 minutes a day and those spending more than 2 hours a day on near work and computer games had the longest eyes. These factors both had an independent effect on eye growth.

Discussion Children with an increased risk of nearsightedness, such as those from a nearsighted family, should be prompted to spend time outside at least 15 hours a week outside and avoid spending long hours of near work even when they are very young.

Mirjam van Tilborg Management pathways for dry eye disease in the Netherlands January 2017

Background & research question Dry eye disease or DED is a multi-factorial chronic condition, with a strong impact on visual functioning and daily life. It is difficult to define in one symptom or to diagnose by current investigative techniques and, most importantly, no single treatment works for all.

GPs, optometrists and OHPs all have a role in the management of DED in primary health care.

Estimates for the worldwide prevalence of DED gives a range from as low as 0.1% to as high as 33%. In the Netherlands there is no prevalence data available, partly due to a lack of consensus in defining DED. However, in 2013 there were 573,540 persons using prescribed artificial tear products, which can be used as an indication. Non-prescribed artificial tears by self-management were not taken into account.

The prescription of artificial tears, which is often inappropriate and ineffective, cost â&#x201A;Ź 26,635,300 in 2013, an increase of 35% since 2009.

The high impact on daily activities, particularly the impact on decreased visual functioning and the difficulties in accurate diagnosis between reported symptoms and investigative technique outcome prompted the research question: how can the diagnosis and management of visual functioning by DED patients be improved?

Design/methods Phase 1 - Dry eye management in primary healthcare in the Netherlands The design of this study was a cross-sectional, web-based survey, that focused on the management of DED by GPs and optometrists. Areas of agreement

between them in the definition and diagnosis of DED by symptoms were determined.

Phase 2 - Eye related problems at daily activities at work The design of this study was a cross-sectional, web-based survey that investigated the impact of (dry) eye problems and near work with the use of VDU in office workers at two universities.

Phase 3 - Comparison of approach to DED by healthcare workers Using a Delphi method, this research will focus on the attitude of GPs, OHPs and optometrists towards the diagnosis and management of DED, with the aim of determining recommendations towards a chronic care model by defining the roles of these professionals in co-management of DED. Phase 4 - Appropriate investigative techniques This research will focus on the investigative techniques for DED that can be used

by GPs and OHP to enhance the management options for the professional for DED patients.

Main results to date There are fundamental differences in the methods for diagnosis of DED, interpretation of dry eye-related symptoms and possible causes of developing dry eyes between the GPs and optometrists in the Netherlands, with a potential impact on consistency of patient care.

Femke de Vries Huidtherapeuten schrijven medicatie voor (HUVO); een randomised controlled trial naar het voorschrijf gedrag van huidtherapeuten en medisch specialisten bij patiënten met huidaandoeningen (acne vulgaris/eczeem).

Achtergrond en onderzoeksvraag Per 1 januari 2012 is er een wetsvoorstel tot aanpassing van wet BIG in werking getreden met betrekking tot de taakherschikking van de zorg. Op grond van dit artikel hebben bepaalde groepen beroepsbeoefenaren de bevoegdheid gekregen tot het zelfstandig uitvoeren van een aantal voorbehouden handelingen. Hierbij valt te denken aan het overdragen van routinematige medische handelingen van artsen naar physician assistants of het voorschrijven van medicatie door verpleegkundigen.

Dit laatste punt, het voorschrijven van medicatie, is een vraagstuk dat tevens speelt bij de beroepsgroep huidtherapie. Is er voor de huidtherapeut als vakbekwame deskundige op het gebied van de gezonde en aangedane huid geen belangrijke rol weggelegd in het voorschrijven van medicatie, aan patiënten met diverse huidaandoeningen? Het overslaan van de schakel arts (dermatoloog/huisarts) als voorschrijver van geneesmiddelen past immers goed binnen het profiel van de huidtherapeut als regisseur van het gehele zorgtraject bij patiënten met een huidtherapeutische hulpvraag. Met het direct toegankelijk worden van de huidtherapeut in 2011 is de eerste stap al gezet in het bieden van het complete zorgverleningstraject. Het voorschrijven van geneesmiddelen zou hierop een goede aanvulling kunnen zijn. Tevens past deze tendens, in het kader van taakherschikking, goed bij het vergroten van de kwaliteit en doelmatigheid van de Nederlandse gezondheidszorg.

Onze hypothese is dan ook dat de huidtherapeut even goed in staat is om relevante medicatie voor te schrijven aan patiënten met een huidtherapeutische hulpvraag als de arts.

Opzet/methode Een RCT zal worden uitgevoerd waarbij patiënten ingedeeld worden in twee groepen: • Interventiegroep. Patiënten met een huidaandoening worden behandeld door huidtherapeuten die onder supervisie van een arts het gehele evidence based zorgtraject aanbieden, inclusief het voorschrijven van medicatie.

• Controlegroep. Patiënten ontvangen de standaard behandelprocedure met betrekking tot geneesmiddelen van dermatologen en/of huisartsen.

Patiënten zullen worden geworven via de HU-kliniek en andere participerende dermatologie-klinieken. Voor screening, het stellen van de diagnose en consultatie wordt er gebruik gemaakt technologische hulpmiddelen zoals teledermatologie.

Belangrijkste resultaten tot dusver Femke is op 1 juni 2014 gestart met het opzetten van het onderzoek.
