I have not found any information which indicates that any of the failure criteria described in 310 CMR 15.303 or in 310 CMR 15.304 exist. Any failure criteria not evaluated are indicated below.

System appears to be functioning properly under its current usage. IT IS STRONGLY RECOMMENDED THATAMANHOLE COVER BE INSTALLED TO FINISHED GRADE ON THE SEPTIC TANKAS THE COVER IS TOO HEAVY FOR 1 PERSON TO LIFT.

Liquid level is at the base of the outlet invert. The center and outlet cover on tank are seized on and can’t be removed. The inlet cover is extremely heavy and needs to 2 men to remove.
