72 year old female with persistent soft tissue swelling along the dorsal aspect of the index finger after accidentally striking a cactus with her hand.

Diagnosis: Cactus Thorn Foreign Body with Surrounding Granulomatous Reaction

Ultrasound is an excellent imaging modality for evaluation of superficial soft tissue lesions, including detection of foreign bodies.

Different foreign bodies have different appearances but usually display a non-organic contour and often have surrounding hypochogenic granulomatous tissue which may or may not be hyperemic.

Clinical context of penetrating injury is very helpful.

Relationship of the foreign body to surrounding structures such as neurovasculature, as well as potential associated injury, can be seen on ultrasound.
