Dear patient, Read carefully following information:


Documentation that you should submit for operation: v Confirmation from your dentist that you have solved all your teeth problems (this note refers only for the patients that will have heart valve operation). v Hospital referral for Special hospital for surgery diseases FILIP VTORI issued from your doctor from primary health care (not older than 1 month). v Refunding bills or last check from your pension. v Total medical documentation and X-Ray scans. v CD from coronarography. Note: You must bring with you all above listed documentation on the day of your receipt in the hospital.

Before you are called for operation you should: v Inform the doctor if you have had brain stroke. After brain stroke patients can’t be operated three to six months. v Follow the instructions from the doctors while canceling anticoagulation therapy (аspirin, pelentan, sintrom, andol, sinkum, plavix, zilt, sinetra, clopidogrel, marivarin, coumadin). v

Prepare at home before operation, meaning:

Take off all your hair from all your body (legs, hands, underarms, crotch, chest). It is the best to use raiser (it is not recommended to use depilation crèmes because of allergic reactions);

• Take off your nail polish and makeup; • Do not wear valuable objects (take only necessary thing: glasses, tooth

prostheses, ear apparatus, picture…); • Take with you the medications you take in their original package; v On the reception day take one family member with you that will take your personal stuff.

The administrator on info desk: v Announce the day of receipt for operation on each patient, on the phone number given by the patient. v Informs the patient for: - necessary documentation that the patient should submit for starting administrative procedure for operative treatment; - Confirmation of blood type and providing of 4 blood units if necessary. - Preoperative preparation for surgery; - All additional information about preparation, receipt and hospital stay in our clinics; - Telephone for contact with the administrator on info desk: 02/3091-493. General information: v Receipt of patients for operation is every day (except Saturday) in period between 8.30-09.00 h. v Dismissal of the patients is every day between 08.30-09.00 h. v Patient visiting – each day between 16.00 -17.00 h.
