A plan of care Once hospice is involved you can start receiving care right away. We devise a treatment program customized for each patient, to make the most of every day. Because life expectancy can be very limiting when you are in pain, HPH Hospice makes relief of chronic, severe pain priority number one. When pain is taken away, time can become a blessing. Because life can be very limiting when you care for a loved one night and day, hospice care provides support.
When the burden of care is lifted, you can breathe again. Most patients of HPH Hospice choose to be cared for in the familiar surroundings of their own homes. And we agree, there’s no place like home. After a thorough evaluation by the physician, nurse and social worker, we furnish medications and supplies to allow the patient the best possible quality of life. In fact, with their pain and symptoms relieved, many of our patients continue to participate in outings, hobbies and other activities they enjoyed before they became ill. Wherever you live, we’ll be there for you. HPH also provides hospice care in assisted living facilities, nursing homes and in our hospice care centers.
We are there to help sort through the mass of paperwork that comes with illness. There with a wheelchair, oxygen or other medical devices required to manage a patient’s illness and provide added comfort.
