McNairy County Health and Rehab opened in 1985 and currently houses 126 beds. The facility’s transition hall was remodeled in 2012.

The transition hall is home to a gym with state-of-the-art equipment. In each room, there are flat screen TV’s with free cable.

Other amenities include hardwood floors, large bathrooms and electric beds with pressure reducing mattresses on the beds.

We have a young and energetic therapy department on staff to help patients with their therapy and rehabilitation process.

The transition hall at McNairy County Health and Rehab is specifically for short stay patients. At our facility, 85% of our skilled patients go home with our own TN home health agency. This ensures the continuity of quality care for our patients.

Introduction: The Journey to Better Health

“Tennessee has the ability to improve our individual health and the overall health of our state and we can start now,” Gov. Bill Haslam said. “We have high rates of behavior-related diseases, and by focusing on changing unhealthy behaviors we can improve our quality of life and also reduce the cost of health care and the cost of doing business in Tennessee.”

Small changes in our everyday lives can lead to improved health.

Our goal in this special section is to begin a conversation regarding healthier lifestyles in McNairy County.

For the first time in our country’s history, the next generation is expected to have a shorter life span than the current one.

According to the State Department of Education’s website, this prediction is based on the fact that childhood obesity is at unprecedented levels nationwide. Tennessee has the third highest rate of pediatric obesity in the United States and ranks 44th among the 50 states in health outcomes for its adult populace.

Since health habits learned during the formative years are crucial to preventing negative health outcomes later in life, early intervention among school-aged youth is necessary and essential in reducing these alarming trends.

The Centers for Disease

Control and Prevention define childhood “obesity” as those children who are in the 95th percentile of all children’s combination of weight and height measurements. Children who are defined as “overweight” include children who are in the 85th to the 95th percentile of all children measured.

There has been some progress made to reduce the rate of childhood obesity in the state, however, there is much more work to do.

Overweight and obese students face immediate health problems, such as high cholesterol, hypertension, Type 2 diabetes, insulin resistance, poly-cystic ovarian syndrome, as well as emotional issues. Excess weight in adolescence carried into adulthood also predisposes youth for serious adult health risks such as coronary disease, stroke, gallbladder disease, some types of cancer and osteoarthritis of the weight–bearing joints.

Healthier Tennessee Invites Citizens to “Start Now”

First phase of statewide initiative aims to raise awareness and build movement

The Governor’s Foundation for Health and Wellness, through its Healthier Tennessee initiative, is inviting Tennesseans to “start now” and take steps toward “a healthier you, a healthier Tennessee,” the new call to action that’s part of a multi-faceted, statewide campaign launching today.

“Tennessee has the ability to improve our individual health and the overall health of our state, and we can start now,” Gov. Bill Haslam said. “We have high rates of behavior-related diseases, and by focusing on changing unhealthy behaviors, we can improve our quality of life and also reduce the cost of health care and the cost of doing business in Tennessee.”

Healthier Tennessee’s inaugural campaign combines key messaging with engaging television and digital advertising, social activation and community outreach to connect with Tennesseans. Initially, the campaign seeks to raise awareness of the relationship between a person’s behaviors and their health, and invites all Tennesseans to join the movement to make Tennessee a healthier state at healthierTN.com.

“A high level of public awareness and understanding about behavior-related diseases and ways to combat them will be essential if Tennesseans are to make positive changes in their behaviors and live healthier,” Governor’s Foundation for Health and Wellness CEO Rick Johnson said.

A new 30-second television commercial will begin airing in regular rotation on networks throughout the state. The spot illustrates how Tennesseans can make changes in their everyday activities that will improve their health, such as trading screen time for being active with your child, choosing healthier foods when shopping, and not using tobacco.

In the coming weeks, healthierTN.com will grow to include tools, resources and other features. Programs to encourage and enable greater numbers of Tennesseans to be more physically active, eat healthier foods in the right portions, and not use tobacco are also being implemented on a local level across the state and will increase as time goes on.

“Behaviors are not created overnight and they will not be changed overnight. It will take a long-term, sustained effort, but Tennesseans can start now to make small changes that can reduce their risk of chronic illnesses such as hypertension, stroke and cardiovascular disease,” Johnson said.

The Governor’s Foundation for Health and Wellness, through its Healthier Tennessee initiative, announced in January an innovative, web-based “Small Starts” tool to enable Tennesseans to make small changes in their everyday lives that can lead to improved health, but the state is among many local healthcare experts who dedicate their lives every day to improve the health of our community.

Healthcare professionals are committed to lifelong learning and adhere to a set of professional ethics in their practice, focused on professional competence, improving quality of care for all patients while maintaining confidentiality and assuring patients are informed and consent to care and treatment.

Medicine is not a perfect science, and navigating the healthcare continuum can be overwhelming. However, one of the most important things a person can do is to follow doctors’ orders and be compliant with the

plan of care.

A patient has rights and responsibilities which can be clarified with the physician.

One of the most important responsibilities of the patient is to provide full information about

dates back as far as Hippocrates, an ancient Greek physician considered one of the most outstanding figures in the history of medicine.

Physicians and other healthcare professionals take the Hippocratic Oath, swearing to practice medicine honestly, upholding a number of professional ethical standards.

Holistic medicine is underpinned by the concept that there is a link between our physical health and our more general ‘wellbeing’. In a holistic approach to medicine, there is the belief that well-being relies not just on what is going on in the body physically in terms of illness or disease, but also on the close inter-relation of this with the psychological, emotional, social, spiritual and environmental state of the patient. These different states can be equally important. They should be managed together so that a person is treated as a whole.

When an individual makes a commitment to improve his or her health, they also need to consider their physical, psychological, social and spiritual health. If you are thinking of improving your health, the best advice is to start where you are today, and make a commitment to gain a better understanding of how your actions impact your health.

According to Dr. Margaret Chan, DirectorGeneral World Health Organization, “We face some alarming disease trends at the global level, and some striking recent events, with global causes, that are directly related to nutrition. Both point to the need to give nutrition much greater attention in discussions about health, whether in rich societies or the developing world.”

As the saying goes, we are what we eat.

Chan also states the role of behavioral factors, notably unhealthy diet, physical inactivity, tobacco consumption and the harmful use of alcohol as key risk factors for noncommunicable diseases, which constitute a rapidly growing burden. Chan also notes that promotion of mental health constitutes an important component of overall health promotion.

Promotion tells us how a healthy lifestyle makes a difference. The evidence for the importance of healthy lifestyles is now overwhelming. New research conducted in Iran shows that lifestyle changes in diet and levels of physical activity improve the health of entire communities. Results of the research have appeared in the January 2009 issue of the Bulletin of the World Health Organization.

In the research project, called “A Healthy Heart,” mass media was used to educate people about healthy nutrition, food labels, the introduction of half portions in fast food restaurants and healthy snacks in schools. To encourage people to undertake physical activity, the government announced automobile-free days and built bicycle lanes in cities. Smoking was banned in the workplace.

Diet, physical activity and smoking behavior were assessed annually for four years in the intervention areas and for three years in the control area. Changes were most notable in the diet of the research participants. In one area, 14% of participants had a healthy diet at the beginning of the study. This increased to 30% after four years. Time spent on leisurely physical activity increased from 81 minutes to 181 minutes per week. Smoking declined but not significantly.

Noncommunicable diseases such as cancer, cardiovascular disease and diabetes account

for 35 million deaths each year. This is 60% of all deaths, and these diseases have common risk factors: tobacco use, inappropriate diet and physical inactivity. By avoiding these risk factors in the first place, we can prevent the majority of deaths due to those diseases.

In McNairy County, the top ten leading causes of death from 2007 to 2009 (along with number of deaths) were: heart disease (294), cancer (220), accidents (79), chronic lower respiratory diseases (77), stroke (68), Alzheimer’s disease (29), diabetes mellitus (24), influenza and pneumonia (24), suicide (17) and kidney disease (13) according to the Tennessee Dept. of Health, Division of Health See START THE JOURNEY, 14C

One could hypothesize that since the county received a high rank for the physical environment, including drinking water safety, daily fine particulate matter, access to recreational facilities, access to healthy foods and fast food restaurants, that our environment would lend itself to foster a healthy lifestyle.

Dr. Armstrong, WHO, stated that small changes in behavior, such as diet or physical exercise, really make a difference to an individual’s health. “We believe that prevention is the best way to answer the problem. We can prevent these diseases by small changes in people’s behavior. For example, 30 minutes of moderate physical activity, the equivalent of a brisk walk, can reduce your risk of heart attack by up to 50%. Increasing your fruit and vegetable consumption can reduce your risk of colon cancer by up to 50%. So these changes are real, they are achievable. What we as WHO, and of course governments are encouraged to do is create the policies and the environment

2014 HEALTHCARE GUIDE for people to be able to make these small choices. The health benefits are there, they are real, they are measured and they are cost effective.”

In our local area, we have the full array of services, from prevention to treatment to rehabilitation and maintenance, required to support the optimum health of our community that makes up the Health Care Continuum.

The continuum goes from the least intensive with home and community based services like prevention, early intervention, home-based maintenance to outpatient, home health, skilled, outpatient surgery to inpatient therapy and intensive care at the most intensive end of the continuum.

The Centers for Disease Control (CDC) provides resources for communities to develop and implement effective strategies for chronic disease prevention. There are several ingredients that are essential in preventing such diseases and achieving health equity.

Investments at local, state, and federal levels are necessary to achieve the full potential for community health efforts to promote health, prevent diseases, and achieve health equity nationwide.

Secondly, we must create a venue for citizens to learn about effective strategies.

It is recommended that each community establish a team composed of city leaders, politicians, healthcare providers, public health providers, and school officials to develop a plan for community change that addresses new challenges faced today.

Thirdly, we must mobilize networks for change, provide resources and technical assistance, and initiate interventions to achieve health equity and prevent chronic diseases with tools like the community health resources provided by CDC with step by step action guides.

Information regarding healthier communities is bountiful, such as the World Health Organization and First Lady Michelle Obama’s Let’s Move initiative (letsmove.gov), dedicated to solving the problem of obesity within a generation, which celebrities like New York Giants quarterback Eli Manning and State Senator Bill Frist are supporting.

communityhealthresources), Governor Haslam’s Foundation for Health and Wellness Healthier Tennessee initiative (healthierTN. com), and Get Fit Tennessee (getfit.tn.gov) are just a few of the resources available to our community.

In recent years, access to quality medical services has experienced significant growth in our community. In addition to a 45 bed hospital, McNairy County has a variety of facilities offering care to include minor emergency clinics, air evac services, medical centers with expanded services, clinics, doctors offices, nursing homes, assisted-living facilities, residential care facilities, outpatient rehabilitation facilities, chiropractic services, mental health offices, pharmacies, optometrist offices, dialysis centers, dentistry, home health, hospice and more.

In neighboring communities, there are hospitals and a full array of healthcare facilities that provide services to many of our local residents as well. Many of the health care providers and physicians in our county feel that the majority of healthcare needs can be met in the area, unless the person requires a very specific specialist not represented in the hospital or clinics or intensive care.

There are also a number of great opportunities for preventative health programs. We have a Senior Circle at McNairy Regional Hospital for citizens over 50 who can get access to the community center track and gym for $15 plus a $5 photo ID card, including other benefits. We also have several gyms, exercise and dance, yoga, zumba and aerobic classes, free arthritis exercise classes, massage therapy, walking trails in the parks, community centers, senior centers, not to mention walking, biking or running in nature, to help maintain good health habits and maximize health and function throughout the life span.

There are debates on the value of the annual physical exam according to Web MD, webmd. com. It may not be necessary in generally healthy people, however, exercising, keeping a healthy weight and not smoking are enough to keep most of us in good health, with or without an annual exam. We offer a range of inpatient and outpatient, emergency, diagnostic, surgical and women’s services, including our new sleep lab, podiatry, pain management, mammography, EMG’s, ENT, cataract, OB/GYN and hand surgery for our neighbors in southwest Tennessee.”

Dr. Mike Smelser, Prime Care Medical Center “Our kids are falling into a sedentary lifestyle our generation started. Activity is very, very important. If we could get people motivated, active and going to the gym, we could make a difference.”

Dr. Jim Clark, OB/GYN, The Womens Place McNairy Regional Hospital “Pre-pregnancy care is as important as pregnancy care. There are many factors that can determine the outcome of your pregnancy that can be controlled before conception. Some of these include: weight loss, smoking cessation, blood sugar and blood pressure control, preconception prenatal vitamin use and routine dental hygiene.”

There are a number of safe medications, which are non-amphetamine based, to help promote healthy weight loss before conception and after delivery. These same medications are also useful for smoking cessation and are covered by most insurance plans. Current medications can be changed to safer medications during pregnancy. During pregnancy, women should

2014 HEALTHCARE GUIDE maintain their current level of exercise activity (there are no activity restrictions for running, lifting weights or going to the gym).”

“There are many doctors who are not OB/ GYNs who currently act as OB/GYNs. Women should be sure to pick a board certified OB/ GYN to manage their care. OB/GYNs receive four years of training specific to obstetrics. Other practitioners often receive one year or less of training. OB/GYNs are also trained in surgery if the need arises.”

Dr. Shawn Pitts, Selmer Chiropractic Center “I believe what is key and critical to the debate is just like Hippocrates said, “Let food be thy medicine and medicine be thy food.” Over the past year, I lost 40 pounds doing what I already knew to do. We eat fresh foods, cook a lot, as I enjoy cooking and entertaining, so I simply started eating half the portion size I used to and get up every morning and run. I do 30 minutes of cardio and yoga and feel better than I have in years.”

“One of the issues I see when I am at the grocery is people loading up their carts with processed foods. Some people argue that vegetables and meat cost more, but I would like to challenge that and ask people to ask themselves, do they really want a healthy body and if so, consider adding more fruits, vegetables and a little meat and compare the costs. We need to encourage the purchase of local foods. Right here in the county we have a new organic farm that is USDA certified for produce and poultry and will soon be adding heirloom vegetables and pork. If people would purchase from local farmers and eat healthy, appropriately proportioned meals, I believe their health would improve, and they would lose weight. This is not an uncommon discussion among my patients.”

Dr. Jamie P. Smith, OD, Eye Clinic PC “I think we really need to stress having eyes checked every two years, or depending on their health, one time a year. As we age, cataracts, glaucoma, retina problems, and macular degeneration can sneak up on us and cause problems. Glaucoma for example, does not have many symptoms, but for patients over 40 it can develop fast and cause damage to the optic nerve. Diabetics need to be checked annually.

regular exams, there are laser treatments and injections for treatment. High blood pressure is another issue that requires annual exams.

“One of the biggest problems we see is dry eye, where there are insufficient tears to lubricate and nourish the eye. However, there are many treatments available. Amblyopia, or lazy eye, occurs when one eye is weaker than the other.

A child may not even notice it. If they are not using the weak eye, it can go down in a hurry.

The child compensates, and only uses the good eye. It is hard to correct unless it is caught at an early age. A lot of times, parents don’t notice, so it is important to get checked.”

“Nearsightedness can be inherited, so there is a good possibility for a child to develop it at an early age. A parent should get their child checked, especially if they notice anything like headaches, blurred vision, redness, tired eyes, or watering, as kids don’t have anything to compare their vision to. As kids start to read and do homework, their eyes can get so tired and strained, and they give up studying, but it can just be from the strain in their eyes. A child that appears normal can have their eyes checked at age 5 or 6. School or college age children can have problems from eye strain or poor vision, so they may need to be checked.

The earlier an issue is identified, the better our chances of correcting a problem.”

“We see patients with arthritis, post-op rehab, work conditioning, industrial rehab, or work injuries, and try to help one return to work. There are also people with chronic illnesses like multiple sclerosis, cerebral palsy, lupus, and fibromyalgia that need help to complete their daily activities.

In addition, many have a history of falling or a broken hip and have issues with balance on uneven surfaces. We work on proper balance to prevent future falls and use assistive devices as necessary.”

“We encourage patients to follow our home safety guidelines to make their homes as safe as possible. Picking up throw rugs is just one example of a preventative measure. People need to be aware of symptoms that require attention, like pain that lasts over a month, limited motion, inability to reach into cabinets, inability to dress themselves, limited motion in the neck, knee pain limiting them from going upstairs or back pain that prevents them from sitting or standing for 30 minutes at a time.”

“Bodies in motion stay in motion. We need to move and change positions every 30 minutes, as so much of what we do is sedentary. The CDC recommends one hour of exercise for children daily, while adults need 2 hours and 30 minutes of moderate intensity aerobic activity and muscle strengthening activities on 2 or more days a week. I recommend doing some basic exercises every 30 minutes at work and utilizing the resources in the community to exercise and stay healthy and strong.”

There is a huge emphasis on technology and new techniques like sedation dentistry. People that are reluctant to go to the dentist due to dental anxiety can come in and have a good experience. I practice with my father, and he has been using sedation for over 30 years on both children and adults.”

“We offer Cone Beam 3D imaging, which can be very helpful when completing an implant or a miniimplant for older patients with dentures, as we can retro fit dentures to fit better. Laser frenectomy is used on a regular basis so tissue does not grow back. With a scalpel, tissue grows back. We can also use the laser on an active fever blister if in the vesicle stage and get rid of cells, so the blister will not resurface in that spot again. We use CAD/CAM technology to fabricate crowns. We can prep and crown in the same day.”

“The design of dental offices has changed, as we now have larger rooms to accommodate a support person and have computers in every operatory.

We use digital x rays, so we can show and discuss them with the patient and even take photos to share with a patient.”

“From a preventative standpoint, we discuss how many sodas or sweet teas they consume and recommend they limit it to one a day. Pregnant women are particularly prone to periodontal (gum) disease due to hormonal changes associated with pregnancy. Research has found that women with periodontal disease may be at higher risk for delivering preterm, low birth weight babies. Insurance companies will often allow an extra exam for pregnant women. We recommend regular hygiene visits twice a year, and if the patient is diabetic, they may need to be checked every three to four months. Brushing is preferred for two minutes twice a day and flossing at least one time a day. Mechanical removal of debris is necessary, as a swish and spit technique is not effective. For older adults, we provide special mouth washes and toothpaste for dry mouth and sensitive teeth. Go to your dentist for tailored care and education on proper oral hygiene.”

With regular checkups, we can find cavities before they become painful and can usually treat them with small fillings. Most of the time we can avoid treatment like crowns, root canals & extractions with early detection.”

“Children should see a dentist regularly within six months after their first tooth comes in (erupts). Teeth are susceptible to decay as soon as they erupt, and it’s important to establish good oral hygiene and have positive dental experiences at an early age. Parents should begin brushing their children’s teeth with literally a “smear” of fluoridated toothpaste when teeth first appear twice a day (morning and before bed). By age three, a “pea-sized” amount of fluoridated toothpaste can be used each time they brush. Fluoride is an important, naturally occurring mineral that helps to strengthen the enamel of teeth. It can stop or even reverse the early stages of cavities. Never allow children to go to bed with a bottle or “sippy cup,” unless it contains only water.

Sweet drinks and even milk before bed can cause rapid and generalized decay, especially on the front teeth.”

“When food debris and bacteria (or plaque) are not removed by brushing and flossing, it can irritate and cause the gums to bleed (gingivitis).

them to heal. If plaque is not removed, it will harden to the surface of teeth (referred to as calculus or “tarter”). Calculus forms in a matter of weeks, and it can then only be removed by special instruments at the dentist’s office. Generally, everyone routinely has a little calculus buildup that needs to be removed. If allowed to collect, it can infect the gums and eventually the bone that supports the teeth. Deep pockets form around each tooth where more plaque and bacteria hide, causing the teeth to become loose and eventually have to be extracted. Gum disease (periodontitis) is mostly painless and can go unnoticed. However, infection from gum disease not only causes bleeding, bone loss, loose teeth and bad breath, but has also been linked to other systemic conditions such as heart disease, diabetes and stroke. People who smoke or chew tobacco, have crooked teeth that are hard to keep clean, are pregnant, or have a family history of gum disease are much more susceptible to periodontitis.”

“Gum disease is preventable with effective brushing/flossing, eating healthy foods and regular checkups. Once the bone is lost from gum disease, it cannot be re-grown. We can, however, maintain what bone remains and prevent further loss of bone and teeth with regular visits to the dentist. Nothing may be as important though as good home care.

When you go to many pharmacies, they may not have access to all of your meds, and then I have no idea how to advise regarding drugs that may be contraindicated. I encourage everyone to ask questions and know what a medicine is for and how it will work. Know why you are taking a drug and how to take it, as drugs metabolize differently in the body.”

“Ask the pharmacist about the drug you are on. Pharmacists are specifically trained to determine how drugs interact, while physicians prescribe based on disease states and know what medications treat those disease states. Some customers want to come to certain pharmacies, however, their insurance plans may require certain pharmacies. I encourage and offer flu shots for all adults and can give to a minor with a verbal order from a physician. We also offer shots for shingles with an order, however, we do not offer the pneumococcal vaccine. Many insurance companies pay for immunizations through the pharmacy. “ “We must take responsibility for our health as there is just so much a medicine can do. A person can modify their lifestyle and improve their health. People need to be more aware of their disease state, become more educated and informed without resorting to medications. With blood pressure medications, if they watch what they eat, diet and exercise, they can usually cut down on taking multiple medications when they take care of themselves and listen to their doctor.”

“At least once a month, I offer classes at the Adamsville Senior Center on topics like blood pressure, blood sugars and would love to do an after school program. I want to meet the prescription needs of the people in my community.”

People need to take their medicine as ordered, and for example, take your blood pressure medications whether you feel badly or not. Diabetic patients need to take their medications and eat right. We see a lot of adult onset diabetes in the pharmacy.”

“I recommend vaccines to everyone. Anything you can do to prevent illness is good. We can give flu shots, B12 shots and shingle shots with an order. Duplications of medications is a big problem. I recommend staying with one pharmacy so you can work with the pharmacist and prevent any drug interactions. People should really carry a list with all their medications on it. I encourage people to ask questions and be clear regarding what medications they are taking and why. It is a blessing to be in a small pharmacy where you can give more attention to your customers.”

“I give free classes once a year to the Selmer Senior Center and would love to offer more classes to the public. People can get off of certain medications with simple lifestyle changes, and that is important.”
