IN A FIELD AS complex as medicine, it’s nice to find a little help. That’s the idea behind a local group of healthcare consultants called Clarity Care Advocates (opposite page), and it’s the same principle that guides us in publishing this list of the best local physicians in the business. Indianapolis Monthly’s annual Top Doctors guide makes it easy to find a great specialist. The whole thing starts with a question that’s about as reliable a measure of quality as it gets: Which doctors do physicians turn to for their own care? Castle Connolly Medical Ltd., a healthcare research and information company, compiled this year’s honorees by asking physicians to recommend someone they would see themselves or suggest to a family member in Central Indiana. More than 850 licensed medical professionals in the area, representing 64 specialties and hundreds of expertises, met Castle Connolly’s qualifications. This year’s list for IM considered physicians in Marion, Hamilton, Madison, Boone, Johnson, Hendricks, Hancock, Morgan, and Shelby counties. Castle Connolly was founded in 1991 by a former medical college board chair and president to help identify America’s top doctors and hospitals. Its established nomination survey, research, screening, and selection process, under the direction of an MD, involves hundreds of thousands of physicians as well as academic medical centers, specialty hospitals, and regional and community hospitals across the nation. The nomination process is open to all licensed physicians in America. Those doctors are able to nominate physicians in any medical specialty (except fields in which patients rarely have a choice, such as emergency medicine or anesthesiology) and in any part of the country, as well as indicate whether the nominated physicians are, in their opinion, among the best in their region or the nation in their medical specialty. Castle Connolly then carefully screens doctors’ educational and professional experience before making its final selections. Doctors do not and cannot pay to be considered for Castle Connolly’s Top Doctors designation. The result: Castle Connolly identifies the top doctors in America and provides detailed information about their education, training, and special expertise in its paperback guides, online directories, and “Top Doctors” features in national and regional magazines. The selected physicians may also appear online at castleconnolly .com, or in conjunction with other Castle Connolly Top Doctors databases online and/or in print. About 18 months ago, a man in his early 60s with a bad knee reluctantly walked into Eric Monesmith’s Greenwood office. The man said that he’d had his other knee replaced five years ago, while living in Pennsylvania, and the post-operation recovery had been awful. In addition to the general soreness and frustration of rehab, the opioid his doctor had prescribed did little to alleviate his pain—instead, the pills made him sick and delirious. The man told Monesmith that the ordeal had been so trying, he’d put off having his other knee replaced as long as he could bear. Monesmith, an orthopaedic surgeon with OrthoIndy South, told the man he understood and could help him. All the patient had to do was open his mouth and say “ahhhh.” With a swab of the inside of the man’s cheek, Monesmith was able to order a DNA test that could help the doctor tailor the patient’s postsurgical drug regimen. “Before, it was all just guesswork—if this doesn’t work, just give them a higher dosage,” says Monesmith. “This takes the guesswork out of it.” When Monesmith began his career 18 years ago, total-joint replacements, usually knee and hip, required months of recovery, often on morphine. Patients today are out same-day or next-day—but the post-op pain meds are still a problem, especially with America’s current opioid addiction crisis. But a few years back, a sales representative for a blood thinner asked Monesmith if he was interested in genetic testing to prevent clots. “I didn’t have much use for that,” says Monesmith. “So I asked him, ‘Does the testing do anything else?’” Monesmith found what many doctors and researchers have discovered: By decoding a person’s genetic markers, labs can decipher how well that patient’s liver will be able to metabolize opioids—either fully, partially, or not at all. Further, the result also indicated how sensitive the person’s pain receptors are. With those two pieces of information, Monesmith would have a clearer idea of how much post-op painkiller to prescribe before the surgery is even performed. After years of testing and compiling data,

Monesmith says about a third of patients either don’t metabolize narcotics fully or at all, so he tweaks the dosage to quell their pain. For the rest, he knows not to overprescribe. Overall, he thinks he’s been able to limit the period of time people spend on pain meds—a crucial step in avoiding dependency and addiction. Monesmith says the only real drawback to DNA testing is that, since it’s considered “investigational,” most insurance plans won’t cover the $250 expense. He says that for many of his patients afraid of pain or drug dependency, that out-of-pocket price is more than worth it. That includes the 60-something man from Pennsylvania, whose test revealed that he did not process pain meds as quickly as others. “His second surgery was like night and day from the first,” says Monesmith. “He was cogent and alert and in minimal pain because the meds were doing their job. He told me, ‘If I had had this five years ago, I wouldn’t have been so afraid the second time.’”

DOCTORING THE TRUTH The worst lies you tell your physician. BY KELLY KENDALL

“I’m not depressed.” Mental illness is sneaky. “Depression can affect how you heal from procedures and how you respond to certain medications,” says Kelly Kasper, an OB/GYN with IU Health University Hospital.

“Yes, this paperwork all looks correct.” By now, you may have been asked by a doctor’s office to fill out your medical records on a computer at home before your appointment, rather than talking through all those details with a physician or nurse. “It becomes very depersonalized,” says Ronald Suh of Urology of Indiana. “It takes on the feel of a survey, and I find that patients—and sometimes providers, too—tend to not treat the information seriously.” Doctors need to know about changes you might downplay on the forms, so take filling them out seriously.

Hey, we get it. When you’re wearing a skimpy hospital “gown” while being interrogated about your body and life choices, it’s tempting to gloss over the facts. But when you fib about how faithfully you take your prescribed medicine, or swear you’ve stopped smoking while crossing your fingers behind your back, the doctor can’t give you the best possible treatment. Doctors have heard it all—and they need to hear it from you. “Patients are vulnerable when seeing physicians,” says Michael Agostino, a physician with Otolaryngology Associates, “and those who are not capable of telling the truth need the most mercy.” Here, common whoppers to avoid.

“I listed all medicines on my chart.” Over-the-counter pills and herbal remedies count. “People are always unclear about how these can cause an adverse effect on your body and lead to bleeding disorders, liver and kidney disease or failure, and worsening depression,” says Tamika Dawson-Knox, an IU Health physician.

“I drink two beers a day.” Knowing your real alcohol and drug intake is key, says Larry Micon, a St. Vincent surgeon. If you’re admitted to a hospital, withdrawal symptoms can mimic worse complications.

“My teen and I are best friends—they tell me everything.” “No, my kid doesn’t pick their nose!” DJ Trigg has heard this one plenty during his work as a pediatric otolaryngologist at St. Vincent. “Then we turn to the child literally with a finger in their nose,” he says. Trigg has seen a few children who were damaging their nasal septum.

You’d be surprised. “Most adolescents—really, most people—keep some information private,” says Mary Ott, a physician at Riley Hospital for Children. That’s why many doctors ask to speak confidentially with their teenage patients, away from parents’ prying ears. Don’t interfere. The information your teen gives or receives can be life-saving.

Franciscan Health’s cancer center has a secret weapon: a medical director who knows what it’s like to be the parent of a leukemia patient. BY ALICIA GARCEAU

When Peter Garrett, a boardcertified radiation oncologist and the medical director of oncology with Franciscan Health Indianapolis, was in his last year of residency in the early 1980s, his 9-monthold son was diagnosed with leukemia. Over 11 months, the infant spent more than 270 days in the hospital. “We were dealing with finishing my

schooling [and] exams, his illness—he was quite critical on many occasions,” says Garrett. At the end of his son’s chemotherapy, Garrett and his wife were given the option of maintenance medicine with probable relapse, or a then-new treatment called a bone-marrow transplant. They opted for the transplant, and their older son, who was then just 5 years old and a match, was the donor. For a time, both of Garrett’s children were in the hospital. His son survived and now has a 2-yearold of his own, but the ordeal left its mark on Garrett and his family—and continues to influence the way the oncologist practices medicine. That’s why 35 years later, Garrett hopes a survivorship program he championed for Franciscan Health will be a step toward putting him out of a job. 76

The state-of-the-art Franciscan Health Cancer Center, which opened five years ago, offers top-notch treatment to patients through multidisciplinary clinics and cutting-edge clinical trials, Garrett says. But his ultimate goal is to reduce cancer through the hospital’s multifaceted survivorship program, Well Beyond. It combines care coordination, palliative care, survivorship planning, spiritual care, support groups, educational programs, and genetic testing. “[The program is] something I feel passionate about, meaning I don’t want someone else to go through what our family went through,” Garrett says. Physician Erika Rager and advanced practice nurse Kim Ziner run the program, which is offered at no cost to patients. The hospital and its foundation jointly fund the program. Part of the Well Beyond survivorship program that has proved quite popular is Moving Beyond, a 12-week series that involves exercise, along with nutritional, psychological, and financial counseling. Moving Beyond is free to family members of patients. “So much stress is placed on caregivers and partners that oftentimes their health can be affected as well,” Garrett says. Participants are surveyed throughout the program to measure healthy lifestyle behaviors, cancerrelated distress, and how they are managing self-care. They often see increased confidence and improved quality of life, which can help prevent cancer. Approximately 150 participants complete the program every year. With all the innovation in cancer treatment and care, it’s an exciting time to work in the field, Garrett says. But he’s not content to stop there. He’s set his sights on developing a prevention and screening center for Franciscan Health that will cut down on the incidence of cancer significantly. Until then, Garrett draws upon his experience 35 years ago. “Anybody who walks in the front door of our program, I view them as a family member,” Garrett says. “That’s how I want to treat them and how I would expect to be treated.”

The pain in Ashlie Olp’s leg started in 2001. At one point, the endurance runner could barely walk across her bedroom due to the searing sensation. Yet even with a medical degree, she found it difficult to navigate the healthcare system. Olp, a primary-care physician, bounced from doctor to doctor, feeling rushed during appointments. When she needed an immediate appointment, she couldn’t get one. She struggled to get through the front-office staff to communicate with her doctor. The final straw, though, may have been a massive and confusing bill after a surgery—which turned out to have been unnecessary. Finally, after 11 years, she was properly diagnosed with external artery iliac endofibrosis. Olp decided the current way she was practicing medicine was DOA. Earlier this year, she shuttered her familymedicine practice and set to work building a new one that she hopes will make it easier for patients to communicate directly with her, allow her take more time with them, and nix confusing insurance statements and inflated bills. Olp opted for a direct-primary-care model, which allows patients to purchase healthcare from her rather than contracting with a third party. She won’t take insurance. Instead, patients pay a monthly membership to have unlimited access to her services: traditional office visits and telemedicine appointments, texts, and phone calls. The cost is $100 for adults and $35 for those 26 and under. “So much of primary care is people coming in because they just have questions about nutrition or weight loss or stress,” Olp says. “They don’t need a bunch of tests. Dialogue can be done remotely. They don’t have to take a half day off work, come to my office, and wait on me for 45 minutes.” But they will pay for healthcare twice—insurance premiums on top of Olp’s monthly fee. Olp says her model saves money in the long run, though, because, barring a big emergency, you shouldn’t have to touch your deductible. She accepts HSAs when allowed to by federal law. For someone with complex medical issues who needs a lot of time with a doctor, the attention and ease might be worth it. Olp saw 2,800 patients at her former practice; she’ll take just 600 at her new one, allowing her to devote 45 to 90 minutes to each appointment instead of the standard 15. hen a patient arrived at St. Vincent’s new neighborhood hospital in Noblesville this summer complaining of flank pain, he was checked in, seen by a doctor, and had a CT scan, which was read remotely by a radiologist elsewhere in the St. Vincent system. The digital image confirmed what the doctor suspected: The patient had a kidney stone. He received pain medication and was discharged with an appointment to see a specialist. Sure, it sounds like a standard emergency-room visit, with one exception. This all happened in about an hour. “Our philosophy is an ER visit doesn’t have to take forever,” says Richard Fogel, the chief clinical officer for St. Vincent and a practicing cardiologist. The goal of a microhospital is to make emergency and in-patient care more accessible. This isn’t a new level of care that falls somewhere between immediate care and the ER. It’s a fully functional hospital, one that just happens to be smaller and more nimble. At just 17,000 square feet, the St. Vincent Neighborhood Hospital packs a lot under its roof. There’s a waiting room with a Zen-like vibe, modern furnishings, and a mini fridge stocked with complimentary bottles of water. Beyond a door, there are seven rooms on the emergency side for pediatric and adult patients and eight in-patient rooms for adult care, all with windows

offering natural light. An imaging room contains the latest X-ray and CT scanners. There’s a pharmacy and a lab, too. At the center of it all is the medical staff ’s station, which is again bathed in natural light from the plentiful transom windows above. The place doesn’t just look different than traditional hospitals; it functions differently, too. The goal is to get each emergency patient through in an hour or less. How is it done? First, though the waiting room is beautiful, the reality is most patients won’t spend much time there because all that’s required at the front desk is a name, date of birth, and reason for visit. Once in a room, patients never wait more than 15 minutes to see a doctor. Next, the imaging center and laboratory sit mere steps from patient rooms, where X-rays are often performed using portable equipment. Results from imaging and blood tests—two items that lead to hold-ups in a traditional hospital ER—are delivered in 20 minutes. Even specialists, while not on-site, are within arm’s reach through telemedicine. A high-resolution machine allows a specialist to see and hear as if examining the patient in person. Ninety percent of patients seen at the facility are treated and discharged. The 10 percent who require a higher level of care are stabilized and transferred to one of the nearby partner hospitals. Neighborhood hospitals are a growing trend nationwide, and St. Vincent plans to open new ones in Avon, Castleton, and Plainfield by April 2018. Franciscan Health debuted its first microhospital this year, on the far-south side. The catch? Procedures at these places can be billed at hospital rates, which are more expensive for the same treatment at a non-hospital site, such a surgical center or quick clinic. But for some, more choices are just what the doctor ordered.
