Indications for Extraction • Extensive dental decay • Serious infection • Preirradiation • Advanced periodontal disease • Teeth that interfere with placement of prosthesis. • Orthodontic reason. • Teeth in line of fracture. • Teeth associated with pathology. www.indiandentalacademy.com
Fundamental Elements • Proper position of the patient to allow doctor comfort & leverage • For maxillary extractions the upper arch should be perpendicular to the floor. • For Mandibular extractions, occlusal plane is parallel to the floor. • Always be prepared with necessary instrumentation. • Use of bite block • Reflect cervical gingiva to expose crestal bone. • All teeth should be luxated prior to forceps extraction. • Avoid excessive force • If no tooth mobility seen after various attempts, plan for surgical extraction. www.indiandentalacademy.com

Techniques and Principles Regardless of the tooth to be extracted, some common principles are Applicable. • Before engaging the forceps , a periosteal elevator should be used in an atraurmatic fashion to gently release the gingival cuff and interdental papilla. • A strong apical seating force is applied which helps snificantly increase the mechanical value and to break the periodontal ligament. www.indiandentalacademy.com
• Lateral forces cause the alveolus to expand. • In all cases, all forces used during extractions should be applied in a slow and deliberate fashion. • When routine exodontia can’t be accomplished, surgical technique must be employed.
