lthough, burnout can affect professionals in any space, it has hit the care industry the most. It is the phenomenon of physical, mental, and emotional exhaustion, which is caused by prolonged and extensive stress. It brings with it the lack of sense of completing personal ambitions, depersonalization, and many such problems. In healthcare, the physicians are under trmendous pressures of time and work, undetermined working hours, ethical burdens and responsility of their patients’ lives, and high emotional and mental intensity. This places them under a very high risk of burnout. What have been the reasons of this? Dissatisfaction is one of the major problems that lead to physician burnout. If, how their time and energy is used dissatisfies the doctors, it leads to mental stress and self-doubt. Moreover, the patients whose conditions cannot be treated, are viewed as their personal failures sometimes. Patient management has never been easy. Other then this, there are always the external factors like long working hours, pressure, moral obligations, etc. But the most persistent complaint recorded is the stress of administrative tasks and the overwhelming regulations. These are the tasks the doctors are not taught to do and they are pretty much exhausting. Despite the wide-spread awareness about this issue, there is negligible progress and numerous physicians are facing stress, depression, physical exhaustion, and many more. Many of the doctors have reported that they have thought about suicide, someday in their life. This ailment of burnout is witnessed more in female physicians than their male counterparts. To help treat this issue, many have started arranging inspirational seminars, talks of resilience, meditation camps, yoga, and whatever they think may help. But, these solutions are not aiding enough. So, what can be done to help erradicate it from the root? Let’s look at some probable solutions. One of the solutions can be to teach the students pursuing medical education about stress management, patient handling, and many such topics right from the initial levels. The physicians should be aware about the problem of burnout and should be pushed to see a therapist or talk it out, if they have any thoughts that point to the syndrome. Today, even when they realise the problem, very less number of doctors consult a professional for treatment. The clerical and administrative loads, that they have to attend to, can be minimized with right set of individuals in their place and few government regulations. An incredible help in this can be the conversational AI, that can not only help improve doctor-patient relationships, but reduce a lot of emotional stress for the physicians. Physical burnout is one of the major problems faced by the healthcare industry. With so less practising doctors for such large population, the pressure on the existing ones keeps on rising. It is the need of the hour to find solutions that can help treat this syndrome effectively.

The genesis of this cancer institute lies in the world’s worst industrial disaster the Bhopal gas tragedy, which left a trial of death and devastation – quite a few thousand dead and tens of thousands maimed, disabled and emaciated for life – following the leakage of huge quantities of Methylisocyanite (MIC) gas from the Union Carbide’s pesticide plant on a cold wintry morning of December 3, 1984. Medical experts were almost unanimous in their view that the long term effects of this toxic gas would push up the graph of cancer incidence considerably. These warnings obviously added a sense of urgency to the long felt need of a comprehensive cancer hospital in Bhopal, one of the fastest developing towns in India.

However, we are conscious of the fact that there is always scope for improvement and a lot has yet to be done in view of the opening of new and newer frontiers of medical knowledge and updating of various disciplines. A good number of patients reporting at advanced stages have also been considerably benefited by the treatment and have shown remarkable improvement. Several terminally ill patients got considerable relief from pain as a result of proper application of palliative care, a newly emerged holistic modality in the treatment of cancer. While awareness about other diseases is spreading rather rapidly, awareness vis-a-vis cancer is unfortunately still moving at a snail’s pace. The result, therefore, is disastrous. People of all ages, sex and section report-in for the treatment of this disease quite late. Even doctors, intellectuals and similar enlightened groups are no exception. The situation is worse in Madhya Pradesh because of extreme backwardness and ignorance resulting in total indifference towards growing scourge of cancer. Adding to this is the financial burden that a patient has to bear, even with an insurance. Anticipating the costs related to cancer treatment adds to the mental strain and sometimes patients are even unwilling or unable to pay for the treatment that could save their lives. With new therapies and treatment procedures being found for cancer, the expenses still remain. Making cancer care affordable for everyone is very necessary to improve the global health conditions. 1. What was your inspiration behind starting HCG? What has been the journey of HCG since then? I always had the vision to make a difference in the country through my work. In this regard, I went to the US for higher studies and was fortunate to be enrolled for a residency fellowship at the MD Anderson Cancer Center. That is where I learned the ropes of setting up a cancer center and developing partnerships. I started the Bangalore Institute of Oncology (BIO) which was the first private initiative in cancer care. Exploring how oncology shaped in the country, the needs and areas of technological gap and affordability, I put together the hub-and-spoke model for developing HealthCare Global Enterprises Limited (HCG). Since then we have made a huge paradigm shift in cancer care across the country. Presently, we have 26 centers pan India. 2. What are the main services that HCG provides? How have they benefitted your patients? Major contributing factors contributed to this success? HCG has been at the forefront of the fight against cancer. We have a multidisciplinary team of specialists with expertise in specific fields of cancer care. We provide all the treatment modalities like radiation, surgical and medical oncology, under a single roof, which gives us an edge when it comes to customizing treatment options, based on every case. We believe that we must change how we treat cancer and research is the first step to help us do that. We can cite many examples of patients who have benefited from the application of genomics research to their treatment. Instead of a “hit and miss” approach that some doctors use, I look forward to the day when across India, oncologists have the knowledge and technical expertise to treat cancer the right way, the first time. That is my onco-philosophy, and that is how we treat cancer at HCG. It is genomics – an understanding of the genome of a cancer cell and the use of that knowledge to choose the treatment that allows us to have a personalized approach to cancer medicine. model helps us to maintain an integrated approach to cancer care. With our spokes located in Tier 2 and 3 cities, we are successfully able to make the care easily accessible to the patients across the country. Our center of excellence in Bangalore serves as the hub providing access to centralized quality control and assurance services, establishing treatment protocols across the network, providing centralized treatment planning, and tele radiology services, and providing access to advanced technologies and specialized procedures. 3. Amongst so many competitors, how has HCG stood tall and differentiated itself from others, all these years? There were cancer centers run by the governments or trust hospitals and multispecialty hospitals with cancer wing. HCG came in as a private enterprise focusing on hi-end oncology care. By bringing in hub-and-spoke model, creating multiple centers across the country, we made cancer care accessible, affordable and treated the ‘right way first’ with high-quality treatment in Tier 2 and Tier 3 cities. We have made advanced cancer care a reality for millions of people across the country. And that’s what makes us different from others. Our value-based medicine - chosen as a case study by the Harvard Business Review and the World Bank - has been able to bring down the cost of cancer treatment to almost a tenth of global prices in some cases while achieving the same outcomes. 4. Kindly tell us in detail about the transplant services that you provide. How have these helped your patients and the industry? HCG offers Liver and Bone marrow transplant services. We are the first in Asia to do a bloodless bone marrow transplant for an American 60-year-old patient suffering from Non-Hodgkins Lymphoma of mantle cell type in the bone marrow. The patient was not keen on undergoing a blood transfusion. After having undergone chemotherapy at New York, he came for a consultation at HCG as no


one was able to help him out with a bloodless transplant. The benefits of this process are that there is no need for donors, fewer chances of spreading infection, reduced cost and no chances of severe diseases that are otherwise possible in blood transfusion. The patient was charged almost US$100,000 (approx. INR 45 lakh), but he only paid around INR 15 lakh at HCG. The right treatment and affordable care make us the go-to hospital for cancer treatment. 5. Having stood against cancer for years, how do you think India has progressed in providing better cancer treatment for all? What more needs to be done? Cancer still is one of the dreaded ailments. We have seen significant changes in the approach of cancer care in the last couple of decades. It has moved from evidence-based medicine to personalized care with the help of improved modalities for treatment leading to better outcomes. The advent of advanced new technologies has changed the way doctors and patients can approach radiation, making it a safer and more effective form of treatment. The use of artificial intelligence – that has taken the healthcare industry by storm helps in improving early detection of breast cancer, and it is helping scientists to predict what combinations of the existing drugs may work in different types of cancer. 6. What are various scientific and technological developments in healthcare industry? How do you ensure to stay updated and leverage these to provide better care? An unparalleled pace of technological advancement has already made inroads in the healthcare sector. Innovations in the digital space have now enabled individuals to remotely monitor heartbeat, blood pressure readings, rate & depth of breathing and a host of other things. Technologies are aiding doctors/physicians in diagnostics and determining the problems.

7. What are your major threats and challenges? What has been your strategy to overcome these? Even though things have improved over the years, in the past, regulatory issues and lack of transparency in the system made it difficult to get the necessary plan approvals and in making sure that the governance in each state is addressed. When it takes 2-3 years to build a single cancer centre, these concerns really hamper our execution of multiple cancer centres across India. Very few people in the world have managed to get it right, including MD Anderson which is a single centre. Getting the right talent was also a challenge that we overcame by making it doctor-centric. We rope in doctors who are not just treating the disease but also show interest in research. Bringing in the necessary technology is a huge motivational factor for these doctors. 8. What have been your major accomplishments? Please mention any awards or accolades you have received so far, if any. We are the first hospital in India to receive the Golden Peacock Award – in Innovation Management. We have bagged the award of ‘The Most Innovative Single Speciality Healthcare Entity’ by VCCIRCLE; won the Frost & Sullivan Oncology ‘Leader of the Year’ 2 times in a row; won ‘Best Health Care Group of The Year’ by BMA. In 2014, we also won the Best Marketing Campaign for SelfV Survivor Stories at the Asian Healthcare Leadership Awards 2014. 9. What are your future plans for the company? Where do you see the company in the years to come? Presently, we have 26 oncology hospitals across the country, and we are coming up with our centers in Delhi, Mumbai, and Kolkata. We are also into multispecialty and coming up with our fifth center. We feel our fertility chain Milann is rated number one and see significant opportunities to grow. In the near future, we are looking forward to creating a multi-specialty with entrepreneurs.

We would like to know about your interesting journey, from an outpatient clinic to a Multispecialty hospital to the success height BBH is at today. The biggest asset we have today is that of an implicit trust of the community around us. Trust is not earned overnight; it is a result of

credible honest work over the years. The support and encouragement of the community is invaluable. This comes about when the community feels that there is a ‘value add’ because of the hospital. Integrity and professional excellence have been hall marks of the institution. Being relevant to the community will also mean being up to date: constant innovation, keeping up with the changing realities and demands, staying abreast of technological advancements are aspects of this. 2. At BBH, what are the novel services and solutions that benefit the patients? What are these benefits that they reap? Facilities • Approximately 25% of our beds (72 of them right now) are related to intensive care. There is currently a big need for a provision of well-equipped high quality intensive care in hospitals. It was this realization that drove us to augment and increase the number of these beds. • Comprehensive Cancer Care: Number of malignancies is going up by the day. There is a felt need for a complete cancer care – all under one roof. Radiation Therapy, Medical Oncology, Surgical Oncology in all specialties are the features of HCO. A state-of-the-art Bone Marrow Transplant Unit is being established and should be ready very shortly.

• Mother and Child Care: Bangalore Baptist Hospital has the highest number of deliveries per month for any private hospitals in Bangalore. In fact, it is much more than that of private medical colleges in the city. High risk obstetrics, advanced gynae surgeries, neonatal and paediatric ICU, paed neurology, paed cardiology, paed pscyciatry, paed genetics, and paed surgical services are being offered. • Reproductive Medicine Unit: World-class IVF facilities with world class results • Comprehensive Cardiac Sciences: Interventional Cardiology and Cardiothoracic surgical services are being offered. Complex interventions are being done on a regular basis. • Geriatric Services: BBH recently received the “Older Patient Friendly Hospital” award. Our geriatrician Dr Benjamin received the Vyoshreshtha award from the Vice President of India in 2018. • Nephrology Services: Almost 1000 haemodialyses are done every month. Senior nephrologists and urologists are part of the team. • Spine surgeries, joint replacements, bariatric surgery

services, pulmonology, andrology, head and neck oncology, rheumatology, endocrinology are some of the services offered. • Large Community Outreach Services: One million people are served in Bangalore Rural District and another million are covered in the 12 slums of Bangalore. Palliative care services reach out to people in need of terminal care, a great felt need. District disability rehabilitation center in Bangalore Rural District is run by BBH in collaboration with the government. • Post graduate residency courses in 11 specialties (DNB courses) are offered. BBH was conferred the ‘Best Teaching Hospital’ national award a few years ago. • We hope to continue to partner with other collaborations. One such example is our collaboration with Baylor University in the U.S. We were recipients of the USAID grant in the year 2015-16 which resulted in the building of a state-of-theart ‘Simulation Education

Research Centre’. High fidelity mannequins and facilities have helped in the education of nursing students, nurses, post graduate medical students, and other health care professionals. 3. BBH is known for its 46 years of Quality, Service, and Care. How have you managed to remain on the top of the ladder for all these years? Association with Christian Medical College, Vellore, has been a very positive factor in the growth of BBH. Commitment to the quality journey very early on has helped chart its course towards greater heights. BBH was the first mission hospital in the country to be NABH and NABL accredited. When Baptist Hospital received this award, it became part of the

first one hundred hospitals thus recognized in the country. Health Communication Workshops were conducted all over the country in various hospitals, medical colleges, and even defense establishments. Institute of Healthcare Excellence has been initiated (IHE-BBH) in Baptist Hospital to provide a forum for exchange of ideas and to share knowledge. 4. Being focused on compassion and charity, how is BBH helping the underprivileged Section of the society? Also, tell us about your Community Health Division (CHD). Being relevant to the disadvantaged is central to the existence of Bangalore Baptist Hospital. 18% of the total budget is reserved for the care of the poor and the disadvantaged. This is true within the hospital and also in the large community health network outside the confines of the hospital. The higher paying patients do ask for more frills and better ambience, but they pay more and thus, are able to help subsidize the poor. At BBH, the concessions are reserved only for the ‘general subsidized’ category. On an average, the patient mix is that of 35% private patients to 65% general patients. 5. BBH claims to be the first mission hospital in India to be accredited with NABH and NABL. What led to this? There has always been a realization that there has to be a palpable commitment to quality. When BBH received NABH and NABL accreditation, it was among the first 100 of the country. BBH was also the first mission hospital in the country to be thus accredited. Nursing Excellence award was given in 2018. I am reminded about the story of the mountain climber who was lost in a blizzard. After days of frantic search and rescue efforts, as his team members erected a stone in his memory, they inscribed the following sentence: “When last seen, he was still climbing”. That, in a nutshell, crystallises the BBH story. We hope we will never stop climbing. Quality is a journey; not a destination.

6. At the position it has achieved, BBH must have been adorned with various honors. Just last year, a 128 slice high end CT scan machine and a digital mammography machine with facility for stereotactic biopsies were added on. PHISHING ATTACK A Real and Ubiquitous Threat for Healthcare Organizations

T

hough it seems that phishing is an outdated mode of hacking, the brutal truth is that it is still alive and is a real threat to the healthcare industry. In fact, it has become a preferred method for hackers to breach healthcare organizations to steal valuable medical data and has devastated the healthcare industry over recent years. The primary purpose of a phishing attack is to gain a grip in the organization by infecting a computer or other endpoint. A recent data breach investigations report has found that ďŹ nancial pretexting, i.e. obtaining ďŹ nancial information under false pretenses and phishing has represented around 95 percent of all breaches, with email being the key element. Attackers use various social engineering tricks to catch the victim into clicking on the malicious link.

They search for public sources to fetch information about the organization in the email to make it look trustworthy. The attackers often create an urgent scene. For an instance, there is some admin IT task that needs to be done to access to their email, and then they ask us to click on the link given and the system is attacked. Medical Records are their Prime Target Today, attackers are becoming more successful at penetrating networks because their techniques are getting more precise and sophisticated. A recent survey expressed that attackers are mostly attracted to valuable medical records. Over onequarter of the organizations had experienced a successful phishing attack that had infected their network with malware. Also, attackers are very conscious of timing for phishing

emails. Hence, they follow the time strictly and trick victims. CEO Phishing At times, attackers also launch spear phishing attacks against corporate executives within a company. If they can enter the network with ease, the attackers can lay low for months collecting data on email flows. Once they have enough information, they create a fake scene of being a top-ranking executive and carry out evil schemes. This situation is termed as ‘CEO Phishing’, which is the ultimate authority scam. Phishing schemes involve impersonating an authority figure and CEO Phishing is the best example of this. What better option does the attacker have than holding the authority of a CEO? In such a case, the attackers could pose as the CEO and direct the CFO to wire money to a supplier, which is basically an account set up by the attackers. Effective Training Can Resolve the Issue Employee training is considered as one of the best ways to fight phishing and it should be done on a frequent mode. The training should be impactful, timely, relevant, and robust in the case of employees handling patients’ digital healthcare data. Also, arranging such sessions once a year does not prove enough to combat phishing and hence, such sessions should be organized often. The training should focus on training employees to look for every minute detail of a phishing attack and what not to click on in an unsought mail. This is because the email is considered as the most frustrating thing as it easily opens the door for malicious attacks. Best Security Tools Tools such as Domain-based Message Authentication, Reporting, and Conformance (DMARC) protocol

improves email security by providing greater accuracy on the sender's identity. DMARC is designed to identify forged sender addresses that appear to be from legitimate organizations by providing the accurate domain name in the 'From:' field of email message headers. It enables organizations to stop scammers from using an email domain to attempt infiltration. The protocol basically assists the organization to stay assured that an email received from the domain name is registered to servers that you own and prevents someone spoofing your email domain. Below are the techniques any organization should implement to prevent from the phishing attacks. • Audit of the Current Cybersecurity Environment Today, the vast use of mobile and IoT devices carry a huge number of threats. Hence, organizations should conduct an exhaustive and ongoing valuation of their susceptibilities. Also, it is beneficial for the companies to stay aware of the threats, keeping updated patches, and using defensive tools that protect against evolving malware. • Segment Networks One of the toughest challenges of safeguarding patient data is to keep the sensitive information quarantined from the rest of the network, making the cyber attackers difficult to reach it. Implementing segmentation employs routers, firewalls, and other various tools to restrict access to parts of the network, providing an added layer of security to PHI. • Train End Users Healthcare industry is the only industry where inside threats prove superior to those from outside an organization. Hence, to prevent accidental exposure from insiders, training employees to detect and report suspicious email activity is pivotal. It has also been believed by various healthcare companies that training can help to reduce attacks while improving confidence. There is no doubt that phishing is a significant danger to healthcare organizations. It is the most chosen method for attackers to steal medical records and to deploy Ransomware. Therefore, healthcare organizations need to mold their systems to prevent phishing attacks from arising. As discussed above, to battle phishing, organizations need to train employees on how to detect and avoid phishing emails. Adopting best security practices and deploying appropriate technology would lessen the chances that a phishing attack would succeed.

ith so many new diseases on the loose, today, the mankind needs healthcare facilities more than ever. Even with numerous hospitals and clinical facilities opened up, they always remain busy or overflowing. We need doctors to help us get rid any kind of ailment. But, what if you are told that the place where you go for treatment is the one that can bring new diseases? However contradictory it may sound, this is a fact. Hospital-aided infections, commonly known as nosocomial infections, are the ones that we bring home from the hospitals we visit. These are present in the hospital environment and get transmitted through various sources. Nursing homes, outpatient facilities, hospitals, and clinics are the places where one can acquire such infections. They can be passed to a patient from other patients, hospital staff, and so on. These infections are caused due to non-sterile and unclean hospital atmosphere, loopholes in infection control practices, and sick employees. Various types of nosocomial infections are present. Intensive Care Unit (ICU) is the ward where patients catch infection the most. Out of all the factors, the period of stay at a hospital plays a major role in this. It is said that around one in every ten patients admitted in the hospital catches a hospital-aided infection. The patients whose medical condition is more complex have more resistance towards anti-biotic medicine and they become the easy targets. Role of Weak Immune System Patients enter a healthcare facility already sick. During illness, their immune systems are already weak. So, when an opportunist infection attacks such bodies, the bodycannot fight back. For example, let’s say a patient suffering from a viral fever visits a hospital. His/her immune system is already busy fighting the virus. Meanwhile, if the

patient catches a bacterial infection from the hospital environment, the body cannot fight back then. Hence, the infection develops and remains. When does One Realize? The symptoms of the infection start showing either in the 48 hours of admission in the hospital, after 3 days of discharge, or after 30 days of an operation. The patients are admitted to the hospitals for ailments other than the infection, but they catch the infection in the hospital surroundings. Symptoms of Nosocomial Infections The symptoms vary from type to type in the infections. Let’s look into some most common infections first. Respiratory, surgical wounds, urinary tracts, gastrointestinal are the most common nosocomial infections in people. Meningitis and pneumonia are also quite common. The symptoms for these may include fever, headache, cough, shortness of breath, vomiting, nausea, diarrhea, difficulty in passing urine, burning sensation while urinating, and wound discharge. These are various other infections and various other symptoms related to these infections. A patient must remain careful, especially after discharge from a hospital, and take immediate action once he/she feels something unusual.

Preventive Measures • A lot can be prevented if the employees as well as the visitors at the hospitals wash their hands regularly. The simple ignorance of washing hands has become a major source of starting hospital-aided infections. • Improper and excessive use of anti-bacterial drugs has to be controlled. They aid in the process of catching and retaining nosocomial infections. • The doctors and hospital staff should always be wearing overalls, like mask, gloves, and coat, to avoid touching people with bare hands and keep the process hygienic • The used syringes should be discarded and all other instruments, including the stethoscope should be disinfected or sterilized • For UTIs, catheters and bags should be changed when medically indicated. Catheters should be inserted aseptically and removed when not needed. They should be placed properly in order to enable uninterrupted urine flow. • Hospital beds, bed sheets, pillows, and covers should be changed and washed at regular intervals of time. The rooms and wards should also be properly ventilated. Such simple hygiene practices matter a lot. • ICUs should be scanned for infected patients as they can further be the source of an epidemic. Identified patients can then be isolated and treated separately. • Hospital-aided infections cannot be diagnosed until later, when the symptoms show. But, until then these can spread from people to people, resulting in an epidemic. Healthcare systems, all over the world, have not addressed this issue properly. Steps need to be taken in this direction by the healthcare industries of all the countries as well as nosocomial control programs should be initiated by the hospitals. This will not only improve health conditions, but also save the economic resources spent on these. Diagnosis and Treatment Some nosocomial infections can be diagnosed by the doctors only by looking at the symptoms in the patients. Some may suggest the patients to get their blood and urine tests done. It is important for the patients to be aware of these and report any extra discomfort they feel during their stay at the hospital or after the discharge. As far as the treatment is concerned, usually doctors suggest rest and a few antibiotics or other medication to the patients suffering from infections. It is suggested to stay isolated in order to prevent the further spreading of the infection. A person should stay hydrated, drink a lot of fluids, follow a healthy diet, and take the mediation timely till the infection is totally cured. Understanding about the infections that can be carried home from a hospital, at our end, we need to be aware of the notion – nosocomial infections. This will help in prevention, development of hygienic habits, and early diagnosis. As on the healthcare providers’ end, there are various nosocomial control measures they can take up. In the time where we hear about new diseases every day, we need to aid in preventing illnesses that can be.
