‘Designer Babies’ Future of Parenthood?

H

ave you ever imagined that you and your partner visiting a maternity and fertility clinic and while you are sipping your co ffee you are o ffered a menu card, descriptions of embryos? Yes, that’s right! There are hundreds of containing descriptions of the embryos that you can pick from, all made by In- vitro Fertilization (IVF); from you and your partner’s egg and sperm. Thus, handing it over to you, will you choose it? Choosing the eye color, height, complexion, physic, or immunity of your baby could be a possibility in the near future. Although this idea of cherry-picking your baby’s phenotype and talents fills most of us with a sense of dismay. Yet, some are overpowering the taboo in a proposition to create ‘the perfect child’ or ‘a designer baby’. If there’s any kind of future for ‘designer babies’, it might look somewhat like this. DNA editing could one day develop into an option for parents wanting to ‘alter the genetic characteristics of their child.’ Well this day is not far now. The advancements in Genetics, Biotechnology, and Genetic Engineering have opened the doors wide open to manipulate and conceive customized babies. While technologies like CRISPR Cas9 or RNA silencing combined with In-vitro Fertilization could play an enormous role in curing susceptibilities to cancers and dementia, or devastating genetic illnesses, scientists have not ruled out the cosmetic uses as well. The ethical repercussions of manipulating human embryos are huge. Many scientists have raised queries over these technologies and they have been termed as a ‘slippery slope’ towards the alteration in human embryos. At present, researchers are only permitted to genetically edit human embryos for 14 days for research purposes, after which they must be destroyed. And it is illegal to implant them into a womb. Due to the high possibility of misuse of these technologies and the interference it will create in the natural reproductive process, ethics have not yet legalized the full-fledged study of these technologies on human embryos. CRISPR Cas9, a genetic editing technique, which acts like a molecular scissors to snip away bad DNA and interchange it with healthy code, is reported to be most sought out procedure that may pose as a prospective tool. However, there have been reports recently that have suggested that the technique might cause wide-ranging mutations in the DNA away from the anticipated site. The researchers have warned the scientific community that this may lead to important genes being switched on or off, which may cause devastating conditions. On a irrefutable note, it is safe to say that although these technologies will pose as promising prospect for parents who wish to customize the genotype and phenotype of their babies; the ethical and scientific concerns need to be addressed with utter scrutiny to make ‘designer babies’ a reality.

OoWomaniya is a platform developed with the notion to provide women a chance to share and read about issues in “An anonymous and non-judgmental environment.”

Established by Sneh Bhavsar (Co-founder & CEO) and Krutika Katrat (Co-founder & CAO), the company is providing comprehensive healthcare information to women, and creating a revolution in the way women access this information. More than 7000 women have received help from doctors via online consultation and workshops, through OoWomaniya’s Health and Wellness Network. 1.What is OoWomaniya? What’s the idea and concept behind your startup? OoWomaniya is an online community exclusively for women to access information, services and expert consultations on their physical, mental and emotional health and well-being. OoWomaniya was incepted with an idea to let women share their issues anonymously in a safe and non-judgmental environment. The platform on web and mobile evolved over the period to create a strong, trustworthy forum over 4 years for women to access credible information and services regarding their health and wellness. 2. Who are the faces behind this startup? Sneh Bhavsar, 34, Co-founder & CEO (Chief Executive Officer)— Sneh pursued his graduation in Information Systems Management from University of London an worked in the IT industry for over 7 years before he plunged into staring his own business. Sneh has 12+ years of experience in IT and ITeS working with technologies, business analysis, process design, product planning, product design, management and recruitment. He is a member of Global Shapers Community, an initiative by the World Economic Forum and leads several initiatives personally and professionally. Krutika Katrat, 28, Co-founder & CAO (Chief Acquisition Officer)— She has pursued her M. TECH (Integrated) in Biotechnology from Pune and has pursued internships and projects while being a student at Zydus Research Centre, Zydus Manufacturing unit and Cadila’s Plant Tissue culture lab. She has worked as a research trainee in a DBT/DST project at ACTREC, Tata Memorial Centre, Navi Mumbai in the field of cancer biology and structural biochemistry before taking a plunge into entrepreneurship. 3. How have they helped build up this platform? Sneh and Krutika started OoWomaniya together with an intention to work in preventive healthcare combining their expertise. Sneh has contributed largely toward the product, technology and design of the platform along with managing the strategy of marketing, planning and acquisition of resources. and the solutions we created – we struggled to monetize the idea for some time. With several ups and downs, and finally pivoting to the community platform – we raised an angel round in 2016 and also won a couple of grants.

How did you start OoWomaniya? What was the motivation behind starting this company? As we were extremely concerned about the healthcare systems and how we can use the technology to create an intervention – we conducted a few workshops with the community. These workshops were the starting point for our research and we built a prototype on web to receive stories from women about their day-to-day issues. We found women’s health issues getting ignored significantly and major factors include social and cultural practices in India. Realizing it to be a long battle – we started with the venting platform for women to share their physical and emotional health issues. Feedback from women was a huge motivation to pursue the idea further and create a query to cure solution for the Indian women.

5. The name is quite interesting. Why did you name it OoWomaniya? This name is inspired by one of the songs in a regional language where Womaniya means a woman who is independent and takes her own decisions. That is at the core of women’s well-being. 6. How OoWomaniya is different than other websites in the same domain? What makes OoWomaniya standout amongst the rest? OoWomaniya is the only platform which is exclusively focused on women’s over all health and wellbeing. It doesn’t encourage the market driven ideas of beauty, image and persona for women. OoWomaniya focuses on core healthiness, emotional wellness and freedom of choice for women. OoWomaniya also attempts to eradicate the myths and taboos around social and cultural practices which affect women’s well-being. 7. Establishing a startup is no piece of cake. What amount of time and money did you invest in this venture? We have been working on our startup for more than 4 years now. We had our share of struggles with the subject, with the perception towards women’s health and also sustaining the venture as a business. However, we were extremely confident about the problem

8. How do you think this venture has revolutionized the way a woman approaches doctors? Online consultation is not a new concept but it is important to create an ecosystem around it in order to make the women feel safer, secure and nonjudgmental. Also, women’s health issues are evolving rapidly due to the lifestyle and food and hence it is also essential that this ecosystem also educates them primarily about the issues they might be facing. OoWomaniya empowers women with the information on health issues and resources to take informed decisions about their well-being. 9. What’s your future plan? Our web and mobile platforms are revamped and our immediate goals is to reach out. We will be acquiring more users in coming months, expand the community across India and add more features and solutions related to women’s well-being in center. 10. Urinary Problems in U Gynaecology rinary problems are the most frequently encountered complaints of gynaecological patients visiting the the outpatient department. Several causes contribute to the varied presentations and types of the urinary problems. The establishment of a proper diagnosis involves a detailed history, meticulous examination and a complete urological investigation. Common urinary problems that patients present with include Urinary Tract Infections (UTI), retention of urine, difficulty in micturition, painful micturition, increased frequency of micturition, difficulty in controlling urination, involuntary passage of urine on cough or sneeze, abdominal pain associated with micturition and increased night frequency of micturition. Let’s concentrate on a few of these complaints in this article. Urinary Tract Infection (UTI) It can be an infection in any part of the urinary system from kidneys to ureters to bladder and/or urethra. Most common method of infection in women is ascending from the urethra

into the bladder, due to anatomical reasons – urethra is shorter in females than males, and is closer to vagina and anus. Some factors like poor personal hygiene, sexual intercourse, catheterization, pregnancy, and menopause make women more susceptible to UTIs. Symptoms and signs of UTI are painful micturition, increased frequency, urgency (strong urge to urinate), burning sensation, cloudy or blood stained urine, foul smelling urination, mild fever and/or passage of frequent small amounts of urine; if the infection reaches the kidneyhigh fever with or without chills, nausea, vomiting and pain in back radiating on one side. Diagnosis is made by characteristic symptoms and by urinary examination- urine may contain pus and organisms. Culture and microscopy of the discharge helps in establishing the diagnosis. Treatment consists of administration of large quantities of fluids, at least 2.5 litres/24 hours, especially in cases of cystitis (inflammation of bladder) and appropriate antibiotics. Patients are advised to avoid all irritants like deodorants, vaginal

contraceptives and douches. Alkaline drinks and citrates are helpful. Menopausal women may benefit by supplementing oestrogen (hormonal) creams to improve the atrophic (thin and shrunk tissue) state of vagina and urethra. Some preventive measures can be followed to avoid UTI: like use a 100% cotton inner wear, maintain proper personal hygiene, urinate after intercourse, use proper antibiotics as advised, cranberry juice can help in decreasing number of UTIs, have plenty of oral fluids, use lubricated condoms without spermicide creams and most importantly proper toilet training should be followed. Coming to the next common urinary problems which most women elicit in their history either as chief complaint or on enquiring include frequency of micturition, urgency to urinate and/or inability to control urination or involuntary passage of urine (incontinence). Incontinence affects nearly 400 million people worldwide. Studies have found the incidence as high even in younger population, affecting nearly 24% women between ages 18-44 years. Urinary

In women, pregnancy, childbirth, and menopause are major reasons for increased prevalence of incontinence. Although Urinary Incontinence is not a life threatening problem, it negatively affects the quality of life; may cause emotional problems like depression, anxiety, stress, and embarrassment in public impacting their travel, exercises, and sexual activity as well. Thus it is an important public health problem which needs to be addressed not just medically, but socially also. The common causes which are postulated to precipitate urinary incontinence are genital prolapse (weakening of genital tissues leading to their prolapse outside the vagina), type of childbirth, and longer duration of delivery, number of children, urogenital infections, and injuries to the genital regions, genetics and menopause.

tress Urinary Incontinence (SUI) is the commonest cause of urinary incontinence noted worldwide. SUI is involuntary loss of urine from the urethra when there is increase in intra- abdominal pressure, which occurs during physical activities like coughing, sneezing, running, lifting heavy weight or exercise. Diagnosis is made during detailed history of the symptoms, along with important past history relating to injury to the genital tract (example during child birth); followed by meticulous examination of the patient in supine (lying down), standing, and squatting positions. Urodynamic tests focus on the bladder’s ability to hold urine and empty steadily and completely, that is, a study that assesses how the bladder and urethra are performing their job of storing and releasing urine. Sonography and cystoscopy (a tube with a camera and light to visualize the inside of the bladder) are other tests which may be individualized as per the patients. Treatment options for Stress Urinary Incontinence are non-surgical and surgical. Non- surgical therapies include behavioural therapy like bladder training, fluid, and diet modification, Kegel’s exercises and Pelvic floor muscle training and drug therapy. However, surgical therapy is well established and over 200 different surgical procedures are described. Most effective and durable long term therapies are Surgical. The goal of the surgical treatment is to provide sufficient urethral support to prevent urine from leaking in cases of increased intra-abdominal pressure. When determining the optimal surgery for a patient, factors to be considered include type of SUI, bladder capacity, severity of the problem, associated other conditions which may require concomitant surgery (like prolapse). There are many modifications since the advent of surgeries for SUI, to name the most common ones performedKelly’s plication, retropubic suspension ( Burch colposuspension), Laparoscopic Procedures, Trans Vaginal needle suspension procedures, sling surgery ( mid- urethral polypropylene mesh, Tension free Vaginal Tape, Trans Obturator Tape), bulking agents, etc. Sling surgeries are the most popular and easy to perform surgeries. The choice of the surgery depends on the cause of urinary incontinence and thus to ensure optimal efficacy of the selected procedure with reduction in risk of complications, careful patient selection is a must. The other common complaints are increased frequency of urination- day and/or night, inability to reach the washroom when there an urge to pass urine (urgency) and inability to hold the urine before reaching the toilet with some amount of leakage of urine (urge incontinence). In these patients detailed history especially of any associated medical conditions (like diabetes, high blood pressure on treatment) is vital. Urinary infection is also ruled out in patients with these complaints. After the diagnosis is confirmed, the treatment is mainly pharmacological and non-surgical. The underlying medical condition should be addressed along with the treatment for the urinary complaints; minimizing the side effect profile of these medicines. The treatment is always individualized depending on the severity of the problems. When patients have mixed symptoms along with SUI, non-surgical or pharmacological methods an are preferred as first line treatment followed by surgery for SUI ( if needed).Thus, to conclude, urinaryc symptoms are commonly encountered problems in gynaecological practice and it is of utmost import e to focus on its prevention along with treatment, eliminating their negative effects on the quality of life.

There are things which one couldn’t control earlier, that is the case of some the defective genes in father, mother or in both. But with the advent of landmark IVF treatment and successive progression towards its improvements have gifted this world many popular, sometimes controversial but effective medical techniques that can prevent the transfer of faulty genes into the child and ensure a good health of baby on its arrival. Karyomapping and three parents IVF are the few techniques which have even got the acceptance (both in legal and societal aspect) in many countries, including the UK. Karyomapping People are often affected with lethal genetic diseases, which many a times transfer to their dear ones and drastically affect their health and well-being. While in the process of expecting a child, its health is always a matter of great concern for the families and especially for the parents. With hectic life schedule, bad food habits, work pressure, loneliness, depression, and more, people often suffer from many life-threatening diseases like diabetes, mental illness, and many more.

No parent, even after knowing that they are suffering from a particular genetic disorder, will ever want to let affect the health of their new baby in a negative way. In this revolutionary technique, embryos get screened before getting implanted into the mother’s womb (uterus). This method is also known as pre-implantation genetic diagnosis or PGD for short. In this process several eggs get collected from the ovaries and then are fertilized outside the body using previously extracted sperm, to produce embryos. Each embryo is then screened, on various parameters, to check its health. Karyomapping technique scrutinizes the chromosomes of the mother, father, and the reference at 300,000 different points. Through this method, doctors can find the defective embryos and precisely remove those from the IVF process. Only the embryos that are free from any genetic condition are transferred to the

uterus. In this way one can ensure a healthy life for the new born and can also reduce the probability of miscarriage, significantly. Three Parent IVF or Pronuclear Transfer Mitochondria are small organelles that sit inside our cells and provide vital energy to it. Mitochondria also play a vital role in the respiratory process. But when this significant organelle of our body gets defected, it invites a plethora of life-threatening diseases. A recent study revealed that many children every year are born with faulty mitochondrial DNA, caused by mutations in them that affect mitochondrial function. Its symptoms includes: heart disease, liver disease, kidney disease, poor growth, loss of muscle coordination, muscle weakness, neurological problems, autonomic dysfunction, and dementia. Mitochondrial disorder becomes worse when the faulty mitochondria present in the muscles, cerebrum, or nerves as these vital parts of our body require more energy as compared to other parts. Mitochondrial diseases strike early in childhood and get worse steadily and even prove fatal before adulthood. Mitochondria replacement therapy is a controversial but effective technique, which can be used to prevent the transmission of mitochondrial diseases from mother to child. This procedure is used in cases when mothers contain genes affected with mitochondrial diseases. Mitochondrial donation, also known as three parents IVF, is said to be the solution for the problem mentioned above. In this method, a third party donor, apart from the father and mother, gets involved in this well-known IVF process. The donor’s healthy mitochondria are used and the nuclear DNA from the third party is removed and replaced with DNA from the both. Many would think that when a third party gets involved in the fertility process then it may also claim some right over the child. The answer is NO. The third party donor does not have any right over the child and his/her identity is always kept as a secret. Third party donor is not even involved in the upbringing of the child. Even if we talk on genetic level, all of the 20,000 genes on the child’s 23 pairs of chromosomes come from the child’s original mother and father. The donor’s contribution here remains miniscule- less than 0.2% of the total that too in the DNA that concentrated in the mitochondria. Recently, a 34-year-old Ukrainian mother has been gifted with a healthy child with this IVF procedure, which is also sometimes known as pronuclear transfer. Although, these techniques are gaining popularity due to their effectiveness and many countries have even made these processes legal, but still these techniques have to test the waters concerning its broader acceptability. Only time will tell how far these techniques will impact both- in the domain of healthcare, especially IVF, and in the hearts of infertile couples struggling with genetic disorders. Facing Your Fears Many of times in my medical profession maximum infertile patients are frustrated so when they enter in OPD they have extreme fear about treatment. Once they get assurance about treatment, then second fear is about success. Means the infertile patient has many of fears on every step of their treatment. In theory, you know that fear is an irrational state of being. If it is irrational, then why are you fearful? Perhaps fear is recognition that you have not fully and willingly accepted yourself.

The key to breaking the fear barrier is to seek balance in your life –balance derived from values based on beliefs and heart centered thinking and behavior. When you are balanced, you can take risks; make glorious mistakes, and plenty of leaps-of-faith. In other words, you have the courage to do what comes from your heart, and you can take responsibility for what you can control, even in the face of fear. To be courageous, feel the fear and don’t run away from it. Then go and do what you fear. When my patients ask me about whether they will enjoy parenthood or not, that time I ask them that “Ask yourself- What’s the worst that can happen?” Try to imagine what would be the worst possible outcome of your fears and write it down. Not only does admitting your fears lessens its impact, but also writing down your fears is a courageous act in and of itself. Change Your Story from Infertility to Maternity Is the Total Positive Attitude If everything is perfect in your life, leave it alone if you are comfortable. But if you want to take a risk, if you feel a passion to embrace another way

So, you resist and create artificial boundaries to stay in that familiar place of resisting. This is the opposite of courage. During counseling I must try to make them bold to face each and every failure alone & be ready for next challenge. Due to this you become more courageous to taking responsibilities of your failures. Often my friends and families consult me the same. After doing the in depth research with more than 100 patients of infertility, at my clinic; I have found out the most common causes of infertility in both males and females. • Unhealthy and Insufficient Diet: This is an era of quick foods. Long working hours at office and excessive workload make them think that they should grab a quick meal instead of a complete Indian Thali. Even if they eat Indian food it comes out of office canteen. People are eating outside more and more. When we eat outside food the hygiene and quality of food is not guaranteed. Instead of fruits, nuts, milk etc. as a “To Go Snack” working people are majorly consuming sandwiches, vada pao, pav bhaji etc. which provide minimum or no nutrition at all. To suppress their hunger people take help of tea, coffee, black teas and coffees etc. This is a wrong practice. People should understand the importance of food and health. People should carry healthy snacks such as fruits and nuts along with them. • Obesity & Malnutrition: Metro cities like Pune, Mumbai, Delhi etc. has more industries and more and more people are into sitting jobs. Continuous sitting with an imbalanced diet leads to weight gain, obesity and malnutrition. • Irregular and Insufficient Sleep: Mobiles phones and extended working hours have snatched the sleep from people in today’s time. Generally people sleep and wake up at different times on weekdays than weekends which induces hormonal imbalance and leads to health complication including infertility. • Stress: Busy lifestyle, career priority, long working hours leaves a person stressful and exhausted at the end of the day. A stressful mind leads to health complications. • Lack of Physical Activity: Our facilitated world leaves no space for physical activity. For example, people own private vehicles and do minimum walking while commuting for daily works; most of the buildings have lifts and people use them even if they have to go up by one floor; most of the people don’t play sports because of lack of time. Lethargy is more prevalent and one feels lazy to do any physical activity. • Planning Past Fertile Period: Keeping career as a priority people push the baby planning to future. Our fertility decreases with age. For women the fertility window starts to close after 35 while men are fertile till late ages. But even if one partner has passed the fertile period it becomes di fficult to conceive. • Excessive or Wrong Use of Birth Control Measures: For long term child control women tend to use birth control pills. In some cases it can affect their fertility. One should opt for better and harmless ways of birth control. So far condoms have been proved as the most harmless method. • Alcohol, Smoking & Drug Abuse: In metro cities, weekend and late night parties are becoming a part of life. People consume much more amount of alcohol than the ideal allowance, which leads to infertility. Movies and television influences people to indulge into smoking and drugs. People start with these habits at early ages and by the time they make the decision of having kids; their fertile health is already gone.
