The answerissimple.Noonewantedtochallengephysicians’ entrenchedbeliefsthatthecurrentwayofchartingisthe onlyvalidapproach. Evenanewmedical syndromeisbeingrecognizedasaffectinghealthcare providerscalledthe“AlertFatigueSyndrome.” Now,the
lawofthelandisthatdoctorsaretosearchallthesecodes totransmitclinicalinformationtotheirpeers.Noone stoppedthisinsanityandrealizedthatwehadthebasics wrong.
