Please introduce yourself and background in the International Private Medical Insurance industry: John Casey: I am the President & CEO of Pacific Cross Insurance & Pacific Cross Health Care in the Philippines and CEO of Pacific Cross International, the group holding company, so I have regional, strategic and operational responsibilities. Our goal is to establish and build an ASEAN common branded health and travel insurance business, to become market specialists and a serious competitor in international and local health insurance. In the early 2000s, I worked with an international consulting firm which specialized in financial services. I then joined the Pacific Cross Group. Throughout much of the 1990s, I worked with a major international Australian life insurance and fund management business in both Australia and the Asian region. Prior to that, I worked with a major Australian bank in establishing the pioneer bancassurance business in Australia. In respect of IPMI, there is a wide range of benefits, definitions, policy provisions and wording, pricing and underwriting differences. This is no different from other parts of the world, although in many developed countries medical insurance contracts are becoming more standardized, driven by regulatory and market demand. Of course, IPMI products must adapt to changing requirements. The challenge for providers of IPMI is to “understand locally and deal internationally”. It becomes increasingly complex in dealing multi country. Nevertheless, products and benefits must reflect market demands and needs. At the same time, clients and distributors also have to assume similar duty of care in relation their own responsibilities when dealing with IPMI.

We have to keep up with that and ensure that we are able to maintain comprehensive cover for our clients whilst ensuring reasonable pricing. That’s the challenge and it keeps life interesting! One of the new issues that insurers are grappling with is the emergence of the very high cost of selected drugs, implants and other treatments, so it is not just the high cost of medical inflation. Insurers have to deal with growing numbers of medical treatments and protocols which are not necessarily generally recognized and accepted in different countries. Should these treatments be covered or not?
John Casey: Global mobility and travel risks have always been with us, but yes the nature of some of the risks has changed or become more complex. There are also simply new types of risk we all now have to deal with, be they new health risks or safety issues. The speed with which IPMI products deal with change and uncertainty will be key, as will ensuring IPMI products remain viable for the insurance provider. Historically, some insurers have been successful, although there have also been mistakes and failures. The Pacific Cross Group provides travel insurance for a lot of people in Southeast Asia – whether within their own countries, or overseas. Can you tell us more about your travel business? John Casey: Yes, we cover a large number of customers in our different operations in Asia Pacific. In 2016, we issued more than 150,000 Travel Policies for individual, family and group travelers. That translates to hundreds of thousands of travelers in 2016 alone. Our products provide worldwide coverage or just local, depending upon country. We have different plans to suit different types of travel – whether one is traveling within one’s countr y, or internationally. With technology and with travel becoming more affordable, we have also ventured into selling our travel insurance online to make it more convenient for people to buy our products. We have received a very positive response in this area. We make sure our plans offer benefits that can give our clients peace of mind, wherever they are. Major costs are largely about treatments and medicines. These days, overutilization is not widespread, although there is an increasing trend in key hospitals to follow certain treatment protocols that will increase insurer costs. There is also some dishonesty at various points of the sale and medical service cycles, depending upon market. With careful management and strong hospital partnerships in a number of countries, we feel comfortable that we can respond to the cost challenges through the next years in a way that keeps our products and services readily available to our customers at all times.

Aetna Pioneer is aimed at individuals and families with Aetna Summit supporting members on employer-sponsored plans. The modular products offer five base plan levels - Aetna Pioneer/Summit 1750, 2500, 4000, 5000 and 5000+. Overall annual benefits range from USD 1,750,000 to USD 5,000,000. All plans provide full cover for inpatient treatment, cancer care, and emergency medical evacuation as standard. The Health Minister of Kuwait Ali Al-Obaidi announced Monday that the fatwa and legislative department has given the green light to setting up a separate healthcare system for expatriates under a new medical insurance scheme. A draft contract has been approved by the fatwa department, which the ministry plans to sign with the Health Insurance Hospitals Company, in order to provide healthcare services to expatriates. With an estimated 2 million + expatriates in Kuwait the contract will help ease the burden put on the current healthcare system and is in line with the government’s program of action and the state development plan to develop a national healthcare strategy. The Health Insurance Hospitals Company is a public shareholding firm, and based on the health insurance law of 1999, will provide medical care for expatriates through various hospitals and primary medical centers. Although no date has been set for the launch, the project is pending approval from the Audit Bureau. Once approved, the ministry will start implementing the scheme by providing access to healthcare services to expatriates at hospitals and medical centers which are specialised to treat expatriates and foreigners. Presently, the majority of expats in Kuwait must obtain
health insurance from the Ministry of Health by paying up to KD 50 per annum. Expats must also pay additional fees for a variety of medical procedures including surgeries, hospital stays and radiology examinations. Expatriates also pay KD 1 for each visit to public clinics and KD 2 to casualty sections at hospitals.

he Health Authority Abu Dhabi (HAAD) has announced a number of amendments to the Emirate’s health insurance programs. The changes apply to those who seek treatment at a private hospital under the Thiqa plan for Emiratis, or for expatriates and their families, the Daman insurance’s Abu Dhabi Basic Plan. Procedures at government hospitals are unaffected. According to HAAD the amendments are designed to utilise the health insurance system in increasing overall efficiency across the sector to better meet current and future demands. The amendments are to be implemented with immediate effect as of Friday July 1, 2016. Changes to the ‘Thiqa’ plan stipulate that cardholders will receive 80% coverage of the fees for treatment services availed at private healthcare facilities in the Emirate. Previously they had 100% cover. Thiqa holders will continue to receive full coverage (100%) for treatment services availed at all governmental healthcare facilities in the Emirate. Thiqa will cover 50% of the cost if cardholders should seek to avail medical services outside the Abu Dhabi Emirate, however, in cases where specialized services and treatments are unavailable at healthcare facilities within the Abu Dhabi Emirate, Thiqa will cover 100% of the treatment cost. Changes also exclude coverage for ‘Orthodontic braces’ for those who are 18 years of age or above from the plan, unless there is a medical necessity identified. Thiqa holders will also be offered full coverage of IVF treatment if availed at a governmental healthcare facility, in addition to one attempt per year at any private medical facility in the Abu Dhabi Emirate.

itch Ratings has affirmed Bupa Insurance Company’s (BIC) Insurer Financial Strength (IFS) rating at ‘BBB+’ with a Stable Rating Outlook. The rating actions follows the completion of a periodic review of BIC’s ratings. BIC’s rating reflects application of a partial attribution approach where Fitch has referred financial and operational strength to BIC from its ultimate parent company, The British United Provident Association (Bupa). Excluding the uplift from this partial attribution approach, BIC’s standalone assessment of BIC’s IFS rating is ‘BB’. Bupa Insurance Limited (BIL), which is rated ‘A+’ for IFS, is a key Bupa subsidiary. BUPA’S STRATEGY IN THE INTERNATIONAL PRIVATE MEDICAL INSURANCE (IPMI) MARKET IN LATIN AMERICA A key consideration in Fitch’s partial attribution approach is its categorization of BIC as a ‘Very Important’ Bupa subsidiary. In Fitch’s view, BIC is particularly important to Bupa’s strategy in the international private medical insurance (IPMI) market in Latin America. Additionally, the disposal of BIC, or meaningful alterations of its strategic plans or direction in the IPMI market in Latin America would cause Fitch to question Bupa’s strategic direction. BIC’s ‘Very Important’ categorization also reflects Fitch’s belief that BIC has a synergistic relationship with the Bupa organization due in part to the IPMI
products marketed by BIC, the geographic diversity of the Bupa organization’s operations, capital support provided to BIC by Bupa subsidiaries and common branding between BIC and the Bupa organization. ‘Scores’ assigned to rating factors underlying BIC’s rating and the score’s relative influence on the rating are discussed below under Key Rating Drivers. Collectively, these scores, along with the ratings uplift derived from being part of the BUPA organization, support BIC’s rating. KEY RATING DRIVERS Market Position and Size/Scale: Scored ‘bb’ with a ‘higher’ influence on the rating. Fitch believes that the market for BIC’s IPMI product is small in comparison with U.S. commercial and government sponsored health insurance markets. Fitch views BIC’s target markets of high net worth individuals, expatriates and individuals that travel internationally in Latin American countries as comparatively small niche markets while recognizing that BIC maintains a strong competitive position in the IPMI market. From a size/scale perspective, BIC’s primary metrics such as membership and revenues are very small in comparison to Fitch’s universe of rated health insurers. At March 31, 2016, BIC’s enrollment was 87,646 and in 2015 the company generated $368 million of revenues. Financial Performance and Earnings: Scored ‘bb’ with a ‘higher’ influence on the rating. This score is heavily influenced by the small absolute amount of earnings BIC generates and to a lesser extent, net earnings volatility from a premium deficiency reserve (PDR) that BIC reported in 2013 that was largely reversed in 2015. Underwriting results are characterized by low benefit ratios and high expense ratios and rates of return are generally stronger than ‘bb’ category guidelines. From 2013 through 2015, BIC generated average earnings before income taxes (EBITDA) of $20 million, an average EBITDAto-revenue margin of 5.3% and average net income of $11 million. Recent revenue trends have been negative. In the first quarter 2016, the company reported an 11.7% decline in revenues compared with the prior year period. This followed 2015’s 10.9% year-over-year decline in revenues. Capitalization and Leverage: Scored ‘a’ with a ‘lower’ influence on the rating. Fitch notes that while BIC’s capitalization and leverage metrics are strong

relative to the current ‘a’ score, the company’s capitalization and leverage metrics have been somewhat volatile in recent years. At March 31, 2016, BIC had $174 million of surplus and its annualized premiums-to-capital and surplus ratio was 1.4x. From 2013 through 2015, the company’s premiumto-capital and surplus ratio averaged 4.8x and its NAIC risk-based capital ratio (on a company action level basis) averaged 311%. At year-end 2015, the company’s RBC ratio was 428%. BIC does not have any debt in its capital structure. RATING SENSITIVITIES Given the referral of financial and operational strength to BIC from Bupa, BIC’s rating is sensitive to changes in the relationships between (1) Fitch’s stand-alone assessment of BIC, (2) BIL’s rating and (3) BIC’s strategic classification within the Bupa organization (‘core’, ‘very important’, ‘important’, or ‘limited importance’). Fitch believes that BIC’s standalone assessment is the most likely of the three variables to change over the longer term. If Fitch raised BIC’s stand-alone assessment to ‘BBB-’ and maintained the company’s categorization as a ‘very important’ subsidiary, BIC’s rating could be upgraded one notch. Fitch believes that BIC needs to demonstrate consistent profitability as the company implements a strategy under which it focuses on assuming IPMI business written primarily by subsidiaries or affiliates domiciled in Latin America, for its stand-alone assessment to be revised to ‘BBB’. Conversely, if BIC’s stand-alone assessment were revised downward BIC’s rating could be downgraded. eimbursement Insurance Will Be the Fastest-Growing Area in Chinese Health Insurance, Outpacing CriticalIllness Policies, According to a Study by BCG and Munich Re. The market for private health insurance in China will surge fivefold to RMB 1.1 trillion by 2020 ($167 billion), from RMB 241 billion ($36.7 billion) in 2015, as middleclass and wealthy Chinese look for an alternative to the public insurance system. A new report by The Boston Consulting Group (BCG) and Munich Re, Opportunities Open Up in Chinese Private Health Insurance, reviews the changing consumer expectations and government efforts to encourage more private health care options that are driving this substantial growth. The fastest growth is expected in reimbursement policies, which are more expensive but more flexible than the critical-illness policies that many Chinese have today. (Critical-illness insurance pays a lump sum if an insured person is diagnosed with a covered medical condition; reimbursement insurance pays on an ongoing basis if a health problem requires multiple medical consultations or hospital visits.) “Private reimbursement insurance makes tremendous sense in China. There isn’t yet a mass market for it because of the cost. THE CUSTOMERS A survey of Chinese consumers reveals the opportunity for carriers: the most likely purchaser of reimbursement insurance today is 35 to 55 years of age, married with children, with a minimum annual household income of RMB 200,000 ($30,333). This group is expected to grow to over 40 million Chinese by 2020. The wealthiest consumers in this group would be willing to pay between RMB 30,000 and 60,000 ($4,550 to $9,100) for a reimbursement policy covering a family of three. “Today, private reimbursement policies are generally sold through group insurance. Purchases by individuals

are still rare, and pure reimbursement players in China simply haven’t been able to make money due to the small size of the market. This is set to change,” says John Wong, head of BCG’s Greater China Health Care practice. Government Policies The authors of the report expect increasing profitability, partly because of government policies that support the privatization of insurance and health care, like tax breaks for private health insurance. Rising demand and encouraging government policies have already led to a quadrupling of the number of private hospitals, to more than 12,500 today from about 3,200 in 2005. SIX KEY REQUIREMENTS “Insurers have enormous opportunities if they start formulating the appropriate strategies now,” predicts Doris Hoepke, member of Munich Re’s board of management, responsible for Munich Health. “To participate in the reimbursement market, organizations need to have a clear understanding of target customers and their needs, which should be reflected in innovative products tailored for these segments. Other requirements are an efficient claims and operations management, which guarantees customer satisfaction while keeping internal costs low, attractive and user-friendly sales channels, and an approved network of hospitals and health service providers.” The report outlines the six key requirements for insurers looking to compete in reimbursement insurance in China. A pure increase in the size of the market will not guarantee profitability. The reimbursement market will represent 36% (RMB 400 billion) of the RMB 1.1 trillion private insurance market in 2020. The other 64% will consist of critical-illness policies (dominated by life insurers), which are expected to grow at a slower rate than reimbursement insurance. “Insurance companies need to understand the characteristics and preferences of different customer segments: the amount of money they are willing to spend on reimbursement insurance and what they expect to receive in return,” says Bill Bossany, deputy chief executive of Munich Re, Beijing (Health branch), and a report coauthor. Expertise in analytics and strong data infrastructure are essential in gaining valuable insights and making customer interactions more straightforward.

he Health Insurance Group has partnered with Voyager Insurance Services Ltd to offer a comprehensive suite of specialist international insurance products that provide individuals and groups with specialist kidnap and ransom [K&R] cover. This is an ideal solution for those operating in high risk or remote regions of the world. “Every year there are more than 40,000 kidnappings globally, and over 40% of these are professionals and business personnel or their dependents. Kidnappings can occur in any region of the world, but particularly in hot-spots such as war zones and crisis zones such as Iraq, Afghanistan and Syria, and sub-Saharan Africa, South America and areas of political or social unrest,” said Sarah Dennis, Head of International for The Health Insurance Group. “In any K&R situation the issues and risks are both complex and interwoven and few details are ever made public. Our K&R capability is designed to reassure businesses and help them fulfil their duties towards protecting staff overseas, who may have been asked to work in some potentially remote or hostile environments.” The K&R products are provided by a leading insurance company rated A+ (‘Superior’) by A. M. Best. Cover is available to UK, EU, US, Australian
and Canadian citizens in any country worldwide, or any company or organisation in those countries who are sending staff overseas. “According to security experts, as foreign oil, mining and security firms have expanded into new territories the risks to westerners working abroad has increased in recent years,” continued Dennis. “This has fuelled a new breed of terrorist that use kidnapping foreigners as a chief means of fundraising. It is, however, sobering to know that a hostage’s chances of freedom also comes down to just two things: their nationality and whether or not they work for an employer with ‘kidnap and ransom’ or K&R insurance.” The first kidnap-for-ransom insurance policy, offered by insurance giant Lloyd’s of London, dates back to 1932, shortly after the highly publicised kidnapping and murder of US aviator Charles Lindbergh’s 20-month-old son by a German ex-convict. l

The plans offer the first health insurance solution for employers who require dedicated plans for their Equatorial Guinean workers and their international staff. The local plan, Salud GE, offers full domestic coverage within Equatorial Guinea. The international plan, Salud Global GE, offers coverage worldwide, outside of the United States, and is aimed at both Equatorial Guinean employees who may work abroad and expatriate staff living in Equatorial Guinea. rom 1st April 2016 AXA PPP International is adding new options to its health plan designed specifically for residents in the Channel Islands and Isle of Man. The refreshed health plan continues to give customers the choice of cover levels Core, Classic, Premier and Ultimate, and from April customers will also be able to choose an extended hospital network for treatment in the UK, including London hospitals, as well as the option to include everyday necessities such as dental and optical cover. And for eligible cancer treatment on the Premier and Ultimate plan level, transportation from the Islands to the UK (for treatment not available locally) is also included. “We understand the unique needs of our customers and we are dedicated to offering support on all levels” says Tom Wilkinson, Managing Director of AXA PPP International. “We have created the Islands Health Plan to offer customers’ the flexibility of healthcare whenever they might need it.” Wilkinson continues, “As well as treatment on the Islands themselves, we understand that there will be times when members may need access to a medical facility on the mainland, and whether that’s in the UK or Europe, the choice
