AN UPDATE ON UNDERGRADUATE PUBLIC HEALTH EDUCATION: PUBLIC HEALTH & HEALTH NAVIGATION ASSOCIATE DEGREES By Richard Riegelman, MD, MPH, PhD

P

ublic health education enters its second century in 2016. In this “second century,” undergraduate public health education has moved from behind the scenes to center stage. In the last year, data from the Department of Education published in the prestigious Journal Public Health Reports documents the exponential growth of undergraduate public health education. During the decade after the Institute of Medicine recommended in 2003 that “…all undergraduates have access to education in public health” there has been more than a 5-fold increase in the number of undergraduate majors in public health with more than 8,200 majors graduating in 2013. This does not include the large number of colleges and universities that offer minors, many of which are developing majors as well. The Community Colleges and Public Health project is encouraging associate degrees and academic certificate programs using two prototype curricular models. The Public Health: Generalist and Specializations model is designed for transfer to bachelor’s degree programs in general public health, health education, health administration, and environmental health. The growth in undergraduate public health education includes generalist education using the Educated Citizen and Public Health model as well as Health Education, Environmental Health, and Health Administration specialty degrees. The number of undergraduate public health majors is growing so rapidly that it is possible to envision the day in the not-too-distant future when the number of undergraduate majors will approximate the number of MPH graduates each year. This growth in public health education, however, has not yet reached associate degree programs. The next phase of growth in undergraduate public health education is now set to occur in associate degree programs in community colleges as well as career schools. To encourage and help structure this process, the Association of Schools and Programs of Public Health (ASPPH) and the League for Innovation in the Community College, which represents more than 800 of the over 1100 + community colleges, developed the Community College and Public Health project.

This should make articulation between bachelor’s degrees and MPH degrees easier to achieve. It should also encourage an integrative approach to the MPH core. It lays the groundwork for BS-MPH and specialized MPH degrees in which the recommended core is accomplished in whole or in part by undergraduate course work. Understanding the Health Navigator by Kay Perrin, PhD, MPH

“There is now a growing commitment to provide [Health Navigator] job positions and an emerging strategy for integrating these positions into the health care and public health systems.”

What Is a Health Navigator? As the U.S. health system becomes more and more complex, the term “Health Navigator” is starting to appear in the national media as the newest emerging career. For example, Health Insurance Navigators are employed to assist individuals find their way through the Affordable Care Marketplace. In other venues, Health Navigators are called Community Health Workers or Patient Navigators. However, the general term “Health Navigator” is increasingly used to describe a variety of job descriptions.

Who Is Going to Train the Health Navigators? The answer to this question is being addressed by two prominent national organizations. First, the League for Innovation in the Community Colleges, which represents over 800 of the 1,100 community colleges, issued the Community College and Public Health Report and the Recommendations (http://www. league.org/ccph). This report details the two prototype curricular models that have been developed with support from the U.S. Department of Health and Human Services and include the Health Navigator and the Public Health Generalist and Specialization with specializations in health education, health administration, and environmental health designed for transfer to bachelor’s degree programs. Second, the Association of Schools and Programs of Public Health (ASPPH) has made recommendations for several academic degrees and certificate programs to be offered by community colleges. In their November 2014 Community Colleges and Public Health Report, they describe how the new degrees are designed to prepare students to work as health navigators.

What Employment Opportunities Are Available for Health Navigators? While this need has existed for a long time, until recently there have not been paid positions with well-defined roles. This is rapidly changing. There is now a growing commitment to provide job positions and an emerging strategy for integrating these positions into the health care and public health systems. New funding mechanisms as part of the Medicaid/Medicare 30-day hospital re-admission policy, and the Affordable Care Act have dramatically increased interest in developing the types of paid positions requiring academic Health Navigator education. The salaries for these positions range from $30,000 to $55,000 per year. The Labor Department estimates that such positions will increase at least 25 percent by 2022 due to the expanding elderly population. It is important to recognize that entry level and supervisory positions are being defined, providing the potential for career advancement. In addition, the two reports recommend that associate degree health navigator programs be designed to allow students to transfer to bachelor’s degree programs in Health Education.

What Curriculum and Training Is Involved for Health Navigators? The two reports recommend that all health navigators at the associate degree level complete the following seven courses: • • • • • • •

Population and Personal Health Overview of Public Health Health Communications Prevention and Community Health Healthcare Delivery Health Insurance Health Information

In addition, the two reports recommend up to nine semester hours of electives with specialized courses focused on specific categories, such as: • • • •

HIV Navigators Cancer Navigators Pediatric Navigators Medicare Navigators

Health Navigators will be trained to assist individuals with limited health literacy as well as the elderly with accessing the maximum array of benefits from community services, clinical care, and health insurance. They may facilitate access to care and followup for sick and complicated patients with cancer, HIV, and a range of other complex health problems. In addition, these individuals can assist with identifying and enrolling patients in various health insurance plans including ACA Exchanges, Medicaid, Medicare, and disability services.

n June 26th, 2015, the United States Supreme Court released its opinion in the case of King v. Burwell—the Court’s third pronouncement concerning the legality, meaning, and/or operation of the Affordable Care Act (ACA) since 2012—which concerned “whether the Internal Revenue Service [IRS] may permissibly promulgate regulations to extend tax-credit subsidies to coverage purchased through exchanges established by the federal government under Section 1321” of the ACA. (See my blog #1 on the background of the case.)

After the ACA was passed and federal agencies began the task of passing thousands of rules implementing the law, the IRS issued a regulation indicating that tax credits were available for purchases under both state-formed and federally-facilitated exchanges. The plaintiffs who initiated King v. Burwell contended that the IRS regulation was unlawful. They argued that the statutory language “estab-

In a straightforward but monumentally important 6–3 ruling authored by Chief Justice Roberts, the King Court upheld the ACA’s statutory and regulatory scheme, permitting federal subsidies to flow through both state-run and federally-facilitated insurance exchanges. In short, the set-up of the case is as follows. The ACA directs states to create an insurance exchange, which amounts to an online marketplace where individuals can compare and shop for health insurance policies. In the 34 states that thus far have elected not to create a state-run exchange, the federal government has filled the void, also per the ACA, by creating a “federally facilitated marketplace” in those states. Furthermore, the ACA offers federal tax credits (i.e., a subsidy) to individuals who need financial assistance in order to purchase products through an exchange. In establishing the formula used to determine the awarding of the tax credits, Congress wrote in the ACA that the credits apply to insurance purchased through an exchange “established by the State.” Put another way, the ACA’s language about the flow of tax credits to those who purchase insurance through an ex-

lished by the State” literally means that ACA tax credits are allowed only in the event that the purchase of insurance occurred through an exchange established by a state. In contrast, the Obama Administration contended that when read as a whole, the ACA makes it clear that both state- and federally-run exchanges are meant to be subject to the law’s subsidy language. The Supreme Court majority sided with the Obama Administration, opting for a commonsensical, contextual reading of the ACA’s subsidy language, tying the subsidies to the overall purpose of the law. Doing otherwise, according to the Chief Justice, would bring about “the type of calamitous result [insurance market failure] that Congress plainly meant to avoid” in crafting the ACA to begin with. In upholding the subsidy scheme, the Court majority relied on an approach that differed somewhat from lower courts that had also ruled that subsidies were available in all exchanges, and with important implications. In an approach that is typical in cases of statutory interpretation, some lower courts ruled that the subsidy language was, at best, ambiguous as to whether all exchanges could provide tax credits, and thus the courts deferred to the IRS (i.e., the federal agency charged with implementing the language) interpretation of the statute. In a critical move, the Su-

preme Court in contrast ruled that because the availability of tax credits was an issue with “deep economic and political significance” to the country, the meaning of the subsidy language should be interpreted by the Court itself, rather than left to agency discretion. This decision means that the only way the subsidy language can be altered now is through congressional action, rather than by a future president whose administration would re-interpret the language more narrowly. This makes it far more likely that the subsidies will remain available in all states going forward. With that settled, the Court proceeded to interpret both the purpose of the ACA and the IRS regulation extending the insurance subsidies to all exchanges, noting that the Court’s duty is to construe statutes as a whole, not “isolated provisions.” Chief Justice Roberts wrote: “Congress passed the Affordable Care Act to improve health insurance markets, not to destroy them. If at all possible, we must interpret the [ACA] in a way that is consistent with the former, and avoids the latter. [The IRS regulation] can fairly be read consistent with what we see as Congress’s plan, and that is the reading we adopt.” In addition to saving insurance subsidies for millions of Americans, the decision in King v. Burwell could have other ramifications, as well. Taken in conjunction with NFIB v. Sebelius (the 2012 Supreme Court decision upholding the constitutionality of the ACA), lower courts may read King’s direction to interpret the ACA as a congressional effort to improve insurance markets as a signal to forestall future litigation against the law. Furthermore, in states that have had difficulty setting up or operating their own exchange, the decision may encourage them to rely on the federal exchange apparatus; since there is no longer the threat that insurance subsidies could easily be untethered from federally-facilitated exchanges, the use of such an exchange could become relatively more attractive. Let me start by saying we should discourage students from trying to plan every minute of their life or believing that they lack worth if they are not sure exactly what they want to do professionally. As my friend Robert Hecht, of the Results for Development Institute, says so well, most of what we worked on professionally in our long careers in Global Health did not exist when we were at Yale. There was no AIDS. There was no cry for Universal Health Coverage. There was no GAVI Alliance, Global Fund, or Global Program for the Eradication of Polio. Thus, it is important to have a strong understanding of Global Health issues, the kinds of problems that might arise in the future, and the questions one needs to raise to help understand them. However, it is not possible to “know all the answers now.”

A similar point holds true in terms of your knowledge of both health and development. Some students may wish to specialize in particular topics and on particular regions of the world. However, their career will be best served if they get a broad understanding now of critical issues in health and development, as they play out across a range of countries, and couple that with the beginning of technical specialization. First, this will allow them to put their specialized interests in context. Second, it will help ensure they are learning the lessons of experience from different places and are able to apply them in a range of settings. Third, it will prepare them to work on different parts of the world as needs change. I “lived and died”

for Southeast Asia when I was at Yale and the needs there were vast at the time. Yet, I worked largely on Africa and South Asia at the World Bank, at a time when much of Southeast Asia saw very rapid economic growth and a widespread reduction of poverty.

Yet, I had no idea what people did in this field, who the key players were, or how they got where they were professionally. Students can learn an enormous amount from looking for such people, learning about their lives, and seeing if they can attach themselves to them or their work. In fact, I cannot stress enough the importance of mentors. I realized a little too late in my career how exceptionally important it is to have
