AN UPDATE ON UNDERGRADUATE PUBLIC HEALTH by Richard K. Riegelman, MD, MPH, PhD, Essential Public Health Series Editor
U
ndergraduate public health education is booming. According to the College Board, there are now approximately 500 public health and related undergraduate programs. There were only a handful of programs 10 years ago when the undergraduate public health movement was launched by the Institute of Medicine’s recommendation that “…all undergraduates should have access to education in public health.” The growth of undergraduate programs continues not only in institutions with graduate public health education but in colleges and universities of every size and type. The Framing the Future of Public Health Education Task Force sees undergraduate public health as key to the continuum of public health education from community colleges to doctoral education. In fact, community colleges are emerging as fundamental to the development of undergraduate public health education. Community colleges are now working closely with public health organizations to examine recommendations for Associate degree and Certificate programs that meet workforce needs and can articulate with bachelor’s degrees. Encouragement for continued growth in undergraduate education at 2-year and 4-year institutions has been provided by an American Public Health Association (APHA) resolution. APHA is “the oldest, largest and most diverse organization of public health professionals in the world” and a co-publisher of the Essential Public Health Series. In addition, Healthy People 2020 features new objectives to greatly increase the number of undergraduate public health programs at 2-year and 4-year colleges. Undergraduate public health is now on the national agenda, and according to the Chronicle of Higher Education and the Washington Post, it has become a hot major. The Educated Citizen and Public Health movement began in 2006 with a Consensus Conference of public health and undergrad-
uate educators. In follow-up to the conference, undergraduate education organizations including the Association of American Colleges and Universities (AAC&U) and public health education organization including the Association of Schools of Public Health (ASPH) developed efforts to promote undergraduate public health including Public Health 101, Global Health 101, and Epidemiology 101. Today both AAC&U and ASPH maintain excellent undergraduate websites with a wide range of resources and list serves for faculty and institutions developing undergraduate public health

The Council on Education for Public Health (CEPH) is now planning to voluntarily accredit colleges that offer bachelor’s degree programs but do not have a graduate public health program. CEPH is planning to utilize the Critical Component Elements as the basis for their curricular review. An annual Undergraduate Public Health “summit” now brings together hundreds of undergraduate public educators at a yearly meeting held the Saturday before the APHA meeting. The presentations and posters highlight the range of exciting innovation that is occurring in undergraduate public health education. New opportunities for undergraduate public health now exist, as the revised MCAT examination will include health behavior and research methods and statistics beginning in the spring of 2015.

DIFFICULTY ACHIEVING HEALTH REFORM IN THE UNITED STATES An Excerpt from Teitelbaum & Riegelman’s 2013 Annual Health Reform Update
T
he array of problems facing the healthcare system has led to numerous health reform proposals and implemented policies. The concept of health reform can have several different meanings. Given the patchwork health insurance system, health reform often refers to changes that seek to reduce the number of uninsured. Due to the high and increasing cost of healthcare services, health reform might also include changes that seek to contain costs and control utilization. The notion of health reform could also address other shortcomings, such as trying to reduce medical errors, strengthening patient rights, building the public health infrastructure, and confronting the rising cost of medical malpractice insurance. The Patient Protection and Affordable Care Act (ACA), the health reform law passed in 2010, touches on many of these issues. We begin with a discussion about why health reform is difficult to achieve in the United States and then introduce some of the reforms that have been attempted on a national level with varying degrees of success. Numerous authors have addressed the main factors that deter significant social reform in this country, including health reform. Factors that are prominently discussed include the country’s culture, the nature of U.S. political institutions, the power of interest groups, and path dependency (i.e., the notion that people are generally opposed to change).

Many advisory services, however, are staffed with people who do not know Global Health well and who lack good connections with organizations that can provide internships and jobs to their students and graduates. As a Global Health practitioner who turned to teaching after retiring from 25 years at the World Bank, I have always enjoyed advising students about their future. This is so much fun for me, that I spend considerably more time at it than I have! EMPLOYABILITY: IF YOU DON’T A.S.K., YOU DON’T G.E.T. by Sharon B. Buchbinder, RN, PhD
W
hen we speak to students about careers in health care management, we often talk about the perfect storm we now have of demand for services and the retirement of baby boomers, leaving our healthcare system with a void of skilled workers. In many of these conversations, we are so enthusiastic about the market and availability of jobs, we have a tendency to overlook the obvious: the field needs well-prepared graduates who are employable. The March 2013 issue of Industrial and Organizational Psychology: Perspectives on Science and Practice focused on employability and career success. The articles addressed such topics as transitioning from school to workplace, need for definitions of career success, developing employability, stigma and discrimination, focusing on the employer, and the role of “social/interpersonal compatibility.”
Although fifty-eight years apart in time, these two models are not that far apart in terms of what it means to obtain and retain employment. There is, however, a bit of a disconnect between health administration competency models and these models.

These researchers asked employers what they wanted. Using this as an example, the next step would be is skill acquisition. In addition to the internship, practicum, or capstone experience, who should teach and assess these skills? Can we instill this in our students by providing scaffolded assignments with increasing levels of difficulty?
According to Hogan, Chammoro-Premuzic, and Kaiser (2013), the definition of employability is “the capacity to gain and retain formal employment.” The authors created the RAW model (rewarding/ ability/willingness) of determinants of employability from the candidate’s profile and the employer’s perspective: “Social/interpersonal compatibility so they can be rewarding to deal with. Abilities, expertise, know-how mean they are able to do the job. Ambition, work ethic, drive mean they are willing to work hard.” (Hogan, Chammoro-Premuzic, and Kaiser, 2013, p.12) On first blush, this model resonated with me. For over two decades, I have had the pleasant challenge of educating students to become competent, confident, and reflective practitioners. Years ago, I relied on the classic article by Katz (1955) on the skills of effective administrator, to inform and advise students about what they needed to be employable. Katz wrote that three skills were needed:

Conceptual skills (big picture and systems thinking); Technical skills (knowledge and expertise in a specific area); and Human skills (what we now call interpersonal skills). (Katz, 1955, p. 34).
While healthcare management speaks at great length about interpersonal skills, I fear these critical skills continue to receive short shrift in the curriculum at both the undergraduate and graduate levels. The emphasis for many programs continues to be on coverage. Often, we are so frantic to make sure we check off all the boxes for the subject matter for professional development, we forget to reflect on the process of professional development. Moreover, while this process is frequently referred to as soft skills, I like to remind my students that interpersonal and process skills are darn hard to acquire and practice each day of our lives. We also need to ask our employers what they want and expect in terms of interpersonal skills for new graduates. A sampling of the research I have found on these “soft
