Exceptional Family Member Forum at Little Creek NAVAL MEDICAL CENTER PORTSMOUTH, Va. – Naval Medical Center Portsmouth’s Exceptional Family Member Program will hold a forum Tuesday, Sept. 18, 9 am to noon, at Joint Expeditionary Base Little Creek Chapel Annex. The forum is for EFM families (adults and children) to share their stories, their ideas and build a network of knowledge, resources and friendship. Local, military and Fleet and Family resources will be available during the “meet and greet” portion and after the forum. Program assistance and information will also be available. A child qualifies for the EFM Program when he or she lives with the active duty sponsor and: •
Takes medication regularly
•
Is receiving care in a specialty clinic
•
Has been evaluated or treated for asthma
•
Has a current or past mental health diagnosis
•
Receives special education supplementary aids or services
•
Receives special education services or accommodations under an IEP, IFSP or 504 plan
•
Has been evaluated of received treatment for Autism Spectrum Disorder or other developmental disorder
If the patient has answered “yes” to any of these trigger questions, the sponsor should contact an EFMP Coordinator or Case Liaison about enrolling the child in the program.
