2011 4-H WISCONSIN WINTER ADVENTURE CAMP REQUIREMENTS About Wisconsin Winter Adventure Camp Wisconsin Winter Adventure Camp offers a variety of fun and educational activities for campers. This CNIC all-expense paid winter camp is coordinated and supervised by local Wisconsin 4-H professionals held at the Upham Woods Learning Center from 26-31 December 2011. Camp programming includes skiing, snowboarding, snowshoeing, broomball, dog sledding, and Cross Country skiing. Various leadership, teamwork, and technology activities, stress management, and yoga will be offered. Off-site field trips include visiting the Wisconsin Maritime Museum and Chula Vista Indoor Water Park. Dorm style housing and bedding will be provided. The Learning Center dining hall will provide all meals including a sleep-in brunch bonanza, pizza night, sub-stravaganza, and dinner at the ski hill. All ski, snowboard, snowshoe rentals, and admission to field trips are paid and offered on-site. CNIC Requirements The following will be provided by CNIC as part of this Winter Camp opportunity: Six professional 4-H camp staff to support winter camp instruction, outdoor, recreational and other environmental experiences. Airline travel to/from camp and local airport (Dane County Regional Airport, Madison, WI). Outgoing and Incoming Baggage Fees (Provide receipt to CNIC POC for reimbursement). On-site nurse/medic to dispense meds and support first-aid and other health related needs of campers during the week. Provide lodging, meals, snacks for the week, according to attached schedule. Orientation of Navy Adult Chaperone(s) upon arrival at camp. Camp registration, code of conduct and insurance information to be collected from individuals attending. Evaluation of campers and adult chaperones’ experiences. Installation Requirements Each installation selected to attend the Wisconsin Winter Adventure Camp must be able to fulfill the following requirements: Assign one Navy CYP supervisory staff member with a complete, favorable National Agency Check and Inquiries (NACI) to chaperone youth to/from local installation to Wisconsin Winter Adventure Camp. Chaperones provide supervision of youth for any disciplinary or behavior issues in collaboration with the 4-H Camp Director throughout the entire camp week. Provide ground transportation for the chaperone and youth to/from local airport in relation to selected Command Installations. Enclosure (1)


Select youth campers to attend Wisconsin Winter Adventure Camp. Ensure chaperones bring an updated and complete CNICCYP 1700/04 for each individual youth while in transit and at Wisconsin Winter Adventure Camp. Chaperones will possess a cell phone for emergency purposes only. Conduct a parent/participant briefing to discuss camp/airport expectations, emergency action plans, local transportation plans, and airline regulations no later than 2 December 2011. CNIC personnel and Wisconsin Winter Adventure Camp staff will attend this briefing via conference call. All sponsors and youth are required to sign Wisconsin Winter Adventure Camp Expectations Letter and additional Wisconsin Winter Adventure Camp waivers at this time. Eligibility Eligible youth must: o o o o o

Reside in the 48 Contiguous United States (CONUS), Be 14-18 years-old by 26 December 2011, Register with Navy CYP, Preference will be extended to youth that support the creation of their respective Youth Program’s application, Possess an updated and complete registration form (CNICCYP 1700/04). How to Apply

Limit one (1) application per Youth Program. Youth Programs will provide information and application requirements to eligible youth about the Wisconsin Winter Adventure Scholarship Program. CYP professionals are expected to advise and assist youth in acquiring video production equipment to complete their group application. However, CYP professionals are not allowed to create the written document or edit the video production of this youth-created application. Cover Letter: A cover letter with the following information must be submitted: Youth Program Name Installation CYP Point of Contact (i.e. Youth Director) Contact Email and Phone Number Content: All applications must answer, “What has Navy Youth Programs meant to me?” Specifically, please discuss at least four (4) of the following Youth Program attributes: Youth Sponsorship Programs Youth Councils and Clubs Teen Employment Programs

Selections All selections will be announced by CNIC by 28 October 2011. Two Youth Programs will be selected for the following scholarship opportunities: 1st Place – 15 teens and 1 Navy CYP professional scholarship award 2nd Place – 10 teens and 1 Navy CYP professional scholarship award Family Financial Responsibilities (No Reimbursements) 1. Spending/Souvenir/Emergency money (Recommended $100). 2. Airport Parking Fees. 3. Cold weather gear for several days - Warm gloves, hats, boots, insulated undergarments, warm socks, snow pants/jacket. (Approximately $250.00).
