From the President & CEO

At John C. Lincoln Health Network, our mission serves as a beacon to guide and inspire us as we work to meet the needs of our community head on. I am extremely proud of what the John C. Lincoln team has accomplished over the past year. Our dedicated team has worked together in our ongoing quest for continuous improvement, and we have achieved great things. First, we have expanded and enhanced our physical locations to ensure that they continue to meet and exceed the needs of those who require our services. In 2012, we broke ground on our new state-of-the-art John C. Lincoln Sonoran Health and Emergency Center in far North Phoenix. We also created a new “Pathway to Healing” at John C. Lincoln North Mountain Hospital, a lobby and communal area that welcomes our patients and their families in the spirit of hope as they work toward achieving wellness. We are evolving our services, too. In 2012, we added six new practices to our Physician Network. We also launched G-60, our new geriatric trauma services program at John C. Lincoln North Mountain Hospital, to provide effective and expedited care for our senior trauma patients. In our effort to deliver more integrated and efficient care, we developed our Accountable Care Organization (ACO). This new entity consists of groups of doctors and other providers who work together to carefully coordinate wellness-focused care for Medicare patients. Our ACO employs retired military medical personnel as specially trained transition coaches who provide our patients with resources and assistance as they move from the hospital to their homes, rehab centers or skilled nursing facilities. To streamline our services even more, we launched our new electronic health record system, JCL Connect, which allows our physicians, hospitals and even patients to confidentially and seamlessly share health information to enhance the quality of care. As we prepare to meet the future health care needs of our neighbors, we will continue to look to our mission for guidance and inspiration. After all, we are here to help build healthy and strong communities — and together we’re up to the challenge. With gratitude,

Rhonda Forsyth, president & CEO John C. Lincoln Health Network
