A YEAR OF GROWTH At John C. Lincoln Health Network, we pride ourselves on our ability to adapt and grow to meet the changing needs of our community with the award-winning care it deserves. Looking back, our accomplishments over the past year reflect that value, which inspires everything we do. In fact, the hallmark of 2013 was expanding the reach of our top-quality care so we can help create healthy communities across the Valley. With the finalization of our affiliation with Scottsdale Healthcare last October, we began a new chapter in our history. The resulting non-profit corporate entity, Scottsdale Lincoln Health Network, is poised to thrive in today’s rapidly evolving healthcare environment while providing highquality care and innovation to patients in the metropolitan Phoenix area. The shared vision for our new system is to become a health system that is fully integrated and locally controlled. We want to improve the health and well-being of those we serve, and we want to transform healthcare for our communities. Together, we are leveraging our strengths to improve our community’s health, which is something of which we all can be proud. At John C. Lincoln, we also were honored to receive many accolades last year. North Mountain Hospital became the only hospital in Arizona to be designated a third time as Magnet, something achieved by only 1 percent of America’s hospitals. Deer Valley Hospital was designated a Truven Top 50 Cardiovascular Hospital in the United States for 2014 and a Healthgrades Distinguished Hospital. In addition, MGMA awarded its Fred Graham Award to John C. Lincoln Physician Network for its Accountable Care Organization Transition Specialist program, and the National Committee on Quality Assurance designated the majority of Physician Network practices as Patient-Centered Medical Homes, making our primary care group one of the largest NCQA-accredited practice networks in the country. We concluded the year on a high note, too, by celebrating the grand opening of our new John C. Lincoln Sonoran Health and Emergency Center. This center offers around-the-clock emergency care in North Phoenix while offering state-of-the-art medical imaging tools and cutting-edge breast healthcare. Though we are excited about what we have accomplished in 2013, there’s more to come as we continue to enhance the quality, technology and scope of our services and facilities. Accomplishments and Highlights ABOUT THE NETWORK Since 1927, the John C. Lincoln Heath Network has been providing health and human services to meet the needs of the community. Now, as part of Scottsdale Lincoln Health Network, we’re part of an affiliation that provides quality care to even more Valley residents. With approximately 10,500 employees, 3,700 affiliated physicians and 3,100 volunteers, this network encompasses five hospitals, an emergency center, an extensive primary care physician network, urgent care centers, cancer center, clinical research, medical education, an Accountable Care Organization, two foundations and extensive community services.
As the philanthropic arm of the John C. Lincoln Health Network, the John C. Lincoln Health Foundation is working to continue the legacy of caring for our community that began nearly nine decades ago. Through our healthcare facilities and the programs offered by Desert Mission, we’re helping ensure that some of the most vulnerable members of our community receive the support they need to reach their full potential for health and self-sufficiency.

Hospitals and Medical Services JOHN C. LINCOLN DEER VALLEY HOSPITAL The 204-bed hospital with all private rooms excels in emergency heart care, pediatric emergency and inpatient care, orthopedics, spine, and many other specialties. The hospital has a provisional Level III Trauma designation and a pediatric emergency certification and ranks among the top hospitals in patient care according to publicly reported data from the Centers for Medicare & Medicaid Services. 50,546 inpatient and outpatient days. 24,434 patient visits to the 24/7 Children’s Emergency Center, Mendy’s Place, part of the Virginia G. JOHN C. LINCOLN NORTH MOUNTAIN HOSPITAL The 262-bed hospital serves as the Valley’s northernmost Level I Trauma Center, taking care of the most severely injured patients. It’s a three-time recipient of the prestigious Magnet designation for excellence in nursing care as well as the home of two nursing units honored with the Beacon Award for Critical Care Excellence. North Mountain is accredited as a Primary Stroke Center. 76,140 inpatient days and outpatient days. 49,631 patient visits to the Emergency Department. 3,171 individual patients who needed Level I Trauma care. »» JCL.com/northmountain

Growth and Innovation Scottsdale Lincoln Health Network: John C. Lincoln’s affiliation with Scottsdale Healthcare was finalized in October, and Scottsdale Lincoln Health Network’s Valleywide presence offers patients more convenient access to acute and preventive care, increased coordination of medical care, an expanded network of high-quality primary care and specialty physicians, a single electronic health record available throughout the affiliated health network, and an environment where best practices are leveraged to enhance overall quality.
Specialists provide designated Medicare patients with personalized support and guidance to help them navigate follow-up medical instructions, prescription drugs, doctor appointments, nutrition and costs of care. As a result, John C. Lincoln Hospitals’ Medicare patient readmission rates have dropped to 6 percent, well below the national average of 20 percent. The expansion was funded in part by a $500,000 grant from the Del. E. Webb Foundation.
John C. Lincoln Sonoran Health and Emergency Center: The new center opened in December and brings residents of North Phoenix communities quality healthcare right in their own neighborhood. This freestanding, emergency center offers care 24 hours a day, seven days a week to people of all ages. The center serves as a destination for emergency care, breast health and medical imaging in this growing area of the Valley.
Provisional Level III Trauma Center: Patient care services in the Deer Valley Emergency Department have been upgraded with additional resources and staff education for its designation as a provisional Level III Trauma Center to better serve traumatically injured patients in the North Valley.
»» JCL.com/sonoran Pediatric Emergency Certification: The 24/7 Children’s Emergency Center, Mendy’s Place, at John C. Lincoln Deer Valley Hospital in Phoenix, has been granted a PediatricPrepared Emergency Care (PPEC) certification by the Arizona Chapter of the American Academy of Pediatrics (AzAAP) and Arizona Department of Health Services (ADHS). The recognition is part of an effort to improve outcomes for ill and injured children visiting emergency rooms, and Arizona is among only five states to implement such a specialized pediatric emergency care system. »» JCL.com/pediatrics Residency Program: Midwestern University in Glendale and John C. Lincoln Deer Valley and North Mountain hospitals are working together to augment local medical resources by creating a general surgery training program for osteopathic medical school graduates. The new osteopathic general surgery residency program, accredited by the American Osteopathic Association, was jointly developed by Midwestern and John C. Lincoln. Low Medicare Patient Readmission Rates: John C. Lincoln expanded its program to hire retired military combat medics and corpsmen as transition specialists to care for discharged elderly patients. »» JCL.com/deervalley Arizona’s First Low-Dose 3D Breast Cancer Screening: The John C. Lincoln Breast Health and Research Center offered the first low-dose, 3D mammography, the latest innovation in breast cancer screening, in the state. The new, low-dose 3D system from Hologic requires less compression time and reduces radiation exposure. It does this by creating 2D images from the 3D data set, thus eliminating the separate digital X-ray that was part of the original 3D imaging process. While 3D mammograms met FDA safety standards, reduced exposure can alleviate concerns about lifetime radiation exposure and result in a more positive patient experience. »» JCL.com/breasthealth Patient-Centered Medical Homes: Nearly all of the medical practices within John C. Lincoln’s Physician Network have been recognized as Patient-Centered Medical Homes by the National Committee for Quality Assurance, a private, not-for-profit organization that for almost a quarter century has been dedicated to continually improving health care quality. Patient-centered medical homes are models of care delivery in which patient treatment is coordinated through a primary care physician to ensure patients receive necessary care when and where they need it, in a manner they can understand. Community Benefit In addition to providing top-quality care to our patients, the John C. Lincoln Health Network also works to address the social needs of our most vulnerable neighbors through Desert Mission. We were able to help improve the lives of those in our community through our award-winning healthcare offerings at our Valley-wide locations, emergency food programs at the Food Bank, early childhood education at Lincoln Learning Center and much more. Our free health screenings, community events and support groups offered hundreds of community members tools to assess and care for their physical and emotional well-being while our physician education seminars offered opportunities for our medical providers to stay up-to-date on the latest research and innovations. On top of these contributions, the Network also provided $43 million in uncompensated healthcare to patients who could not afford it otherwise.
DESERT MISSION Desert Mission strives to help individuals and families reach their full potential for health and self-sufficiency using an integrated approach. Its programs, which include the Food Bank, Community Health Center, Children’s Dental Clinic, Adult Day Health Care, Lincoln Learning Center, Marley House Behavioral Health Clinic and Neighborhood Renewal, are designed to meet the needs of the whole family, continuing Desert Mission’s legacy of caring for the community that began in 1927. 45,369 emergency food boxes distributed at Desert Mission Food Bank. 45,974 Snack Pacs (emergency weekend food for children) distributed to local schools. 8,541 student dental screenings at school and 3,611 visits to the Children’s Dental Clinic. 3,476 patient visits to the Community Health Center. 8,871 participant visits to Adult Day Health Care, with an average daily census of 35. 675 school-based counseling sessions for at-risk youth facilitated by Marley House Behavioral Health Clinic. 459 children enrolled at Lincoln Learning Center. 24 new homeowners created through Neighborhood Renewal. »» JCL.com/desertmission

VOLUNTEERS Our caring and dedicated volunteers generously give their time and talent to help us make a positive impact on people’s lives. Whether they’re providing comfort to patients, sorting through food donations or raising money to help educate the next generation of healthcare providers, our volunteers are an essential part of our success. In 2013, they added $4.4 million of value to the Network, an amount equal to 96.6 full-time positions. 1,890 individuals and groups of volunteers served with 200,940 hours. 40 pet therapy teams visited 32,422 people. $221,978 raised for the Network, including $40,000 for scholarships for students pursuing degrees in healthcare and $20,000 to fund recreational activities for Adult Day Health Care clients.

Awards and Recognitions Third Magnet Designation: John C. Lincoln North Mountain Hospital became the first medical facility in Arizona to be designated a Magnet facility for the third consecutive time. This designation is granted by the American Nurses Association (ANA), the nation’s foremost authority on the quality of patient care. Magnet status is based on documented exemplary practice and outcomes for patient care and is considered the nation’s gold standard for nursing quality. Only about 6 percent of the nation’s 6,000plus hospitals have earned Magnet status. Less than 3 percent of that group has earned redesignation and less than one percent has been designated a third time. Top 50 Cardiovascular Hospitals: John C. Lincoln Deer Valley Hospital was named one of the nation’s 50 Top Cardiovascular Hospitals by Truven Health Analytics, an international consulting organization that provides information and solutions to improve the cost and quality of health care. These hospitals achieve superior clinical outcomes, provide better care and are more efficient than their peers. Donor Network of Arizona Champions: North Mountain Hospital’s Level I Trauma Center and three hospital staff members were honored by the Donor Network of Arizona as community champions for their outstanding work promoting and facilitating organ and tissue donation. Primary Stroke Center: John C. Lincoln Deer Valley Hospital in North Phoenix became the Valley’s newest hospital certified as a Primary Stroke Center by DNV Healthcare, an international certifying agency approved

by the US Centers for Medicare & Medicaid Services. Deer Valley Hospital demonstrated that its stroke care program follows national standards and guidelines that can significantly improve outcomes for stroke patients. Fred Graham Award for Improving Community Health: MGMA-ACMPE, the premier membership association for professional administrators and leaders of medical group practices, presented its Fred Graham Award to the John C. Lincoln Physician Network for activities and solutions to advance the effectiveness of healthcare delivery and improve community health. Nathan Anspach, FACMPE, FACHE, Senior VP and CEO, John C. Lincoln Physician Network and Accountable Care, and his administrative team created a program that recruits combat medics and corpsmen returning from Iraq and Afghanistan and trains them as transition specialists to serve the patients of the John C. Lincoln Accountable Care Organization. Gold Plus Award for Stroke Care: North Mountain Hospital received the Gold Plus Award from the American Heart Association and American Stroke Association Get With the Guidelines – Stroke program. This recognizes hospital teams who use the most up-to-date, researchbased guidelines with the goal of speeding recovery and reducing death and disability for stroke patients. Vista Award for Outstanding Renovation Project: In recognition of the John C. Lincoln North Mountain Hospital’s completely reimagined and rebuilt Specialty Surgical Care third floor, the American Society of Hospital Engineering (ASHE) selected the hospital as recipient of its annual Vista Award for the year’s most outstanding renovation project. The $8.2 million, 21,350 square-foot North Mountain hospital floor was completely transformed into a new facility that is now the standard for future construction design throughout the John C. Lincoln Health Network.
American Hospital Association’s Hospital Awards for Volunteer Excellence (HAVE): In recognition of the significant difference they make in the community, the Desert Mission volunteers of the John C. Lincoln Health Network received this national award. The HAVE Award recognizes volunteers who provide leadership in community outreach, particularly with innovative and measurable programs with external partners to address challenges in the community. About 500 of the Network’s volunteers regularly contribute time to Desert Mission’s community service programs, and provide at least the equivalent of 18 FTEs each day. 2013 Distinguished Hospitals Award for Clinical Excellence: John C. Lincoln Deer Valley Hospital has been recognized with a 2013 Distinguished Hospitals Award for Clinical Excellence from HealthGrades, as reported by fiercehealthcare.com, a website of healthcare management news.
