From the President: Cliff Gronseth, MD Greetings, colleagues! It is my pleasure to welcome you to 2014 as the current BCMS President. The BCMS Board of Directors and I have great aspirations for the year to come, and hope to draw you all ever nearer to the beating heart of the organization. We are all here for each other; to listen and learn, dream and aspire to greater things. The Boulder County Medical Society is an organization that represents physicians (employed and independent) in Boulder County. The winds of change are blowing harder than ever. Physicians everywhere have taken a beating. At the end of the day, unless we can pull together, we will continue to be flogged. Your voice is important, and we want to hear it. My goals for BCMS this year: • BCMS will act as a catalyst for physician-to-physician dialogue. We will host several “pleasant soirees,” or short cocktail hour evenings of discussion, focused around interesting topics. The intent is to create opportunities for physicians to cavort and to opine about important health related issues. • BCMS hopes to collaborate with other health organizations such as the

Your voice is important, and we want to hear it.” Boulder County Health Department and other local medical societies (perhaps Denver) to effect change and to enhance public health and awareness of our presence in the community. • We hope to repeat a strong presence at the Colorado Medical Society meetings and assist with initiatives. (Please review the important dates outlined in this newsletter and on the BCMS website). • The BCMS website will list pertinent news and a calendar of events that hopefully will interest you. A boat moves best when everyone is pulling together…and “organized medicine” via BCMS and CMS may be your best hope. SO...PLAN TO JOIN OUR MEETINGS AND GATHERINGS! Introducing the Boulder Valley Care Network By Laird Cagan, MD, BCMS President-elect The Boulder Valley Care Network (BVCN) is an organization in which most of you are members. I hope that you are aware of our organization. This report is an introduction/reintroduction to BVCN. BVCN has been in existence for the last three years. Our mission is to work with self-insured employers to improve health care quality and reduce medical care costs. We work with self-insured organizations because that is the only way we can get medical care data to analyze (insurance companies will not release proprietary data). Our current partners are Boulder Valley School District and Longmont United Hospital. We are unique because we support physicians in Boulder County. We aim to help physicians in their practices, not take restrictive actions based on incomplete or inaccurate data. We strive to reduce work burdens and not send unproductive paperwork to fill out or other information of little interest. We are local and we are your colleagues. We have a nurse care coordinator for each employer.

The coordinator can help you with patients if, for example, you are having trouble getting a needed procedure or medication authorized or if your patient needs assistance of any kind between office visits. The nurses are also working proactively with diabetic and coronary artery disease patients to improve their health. Patient education is their forte. Our physicians as a group have increased generic prescribing from 73 percent to 85 percent since inception. Our medical costs have increased only by 2 percent per year, well below national medical care cost increases. We are now working on risk analysis with our in-house data engine to help predict which patients are at risk for serious health problems and to act to prevent them. The Conversation Project: Speaking up about end-of-life care By Eben Carsey, MD, FAAP
I have been working with a Boulder County group that has brought Ellen Goodman’s Conversation Project (www.theconversationprojectinboulder.org) to the county. Ellen Goodman’s project is sponsored by the Institute of Healthcare Improvement. The goal of this project is to foster meaningful and effective conversations about endof-life care. The project utilizes a “Starter Kit” that briefly states the facts of how infrequently people actually make their endof-life wishes known even though they think it is important to do so. (I would add that we doctors, unfortunately, are often ones that neglect this conversation.) Consequently, many people spend their last days in a way that they would not prefer to spend them, or their surrogate decisionmakers have great difficulty and discomfort because they are not sure what is wanted.
The kit has “Where I Stand” scales to help a person think about what matters most and plan an advance directive. It can also help a patient plan a conversation with family members about directives that are already in place and has a checklist of topics to address in the conversation. Ms. Goodman’s group has also developed a guide entitled, “How to Talk to Your Doctor.” Finally, the county project has “presenters” who can make effective presentations to groups and “coaches” who are available upon request to help individuals as they consider how to have this conversation with their loved ones. We look forward to serving Boulder County Medical Society’s doctors and patients as best we can.
