Senior Choir performing at Queenwood Foundation Day Service St Clement’s Anglican Church, Mosman Wednesday 18 October Information for Parents and Students October 11, 2017
Dear Parents/Guardians,
Performance Girls in Senior Choir will be singing at the Foundation Day Service, at St Clement’s Anglican Church, Mosman on Wednesday 18th October commencing at 8:30am. There will be a warm up and sound-check on the day of the service. Girls will need to make their own travel arrangements to the church and need to arrive no later than 7:45am. Full school uniform is to be worn, including blazer, hats and red ribbon. At the conclusion of the service all girls will return to Senior School together. A truck will be provided on the day to transport school bags to Senior School.
Rehearsal There will be a rehearsal in the church on Tuesday 17th October between 2:00-3:15pm. Girls are to meet the bus at Lawrance at 1:45pm. Please ensure all lunch is consumed prior to boarding the bus. It is suggested that all girls bring lunch from home on this day as there can be unexpected delays when collecting lunch from the canteen. At the conclusion of the rehearsal girls may choose to either return to school on the bus, or may be released from the church if parental consent is given. Please select the appropriate option on the Consent Form to indicate if your daughter will require a bus back to Senior School, or if she is permitted to leave from the church at 3:15pm. The attached consent form is to be returned to the Music Department no later than 8:30am on Monday 16th October.
Risk assessment A risk assessment has been completed for this excursion and it is considered that attending the Foundation Day Service is of a LOW RISK nature. Please note the risk warning provided at the end of this letter. Consent Form This form seeks your consent for your daughter to attend and for the school to arrange medical treatment for your daughter while on an excursion should it become necessary. You will be advised as soon as practicable of action taken in relation to medical treatment for your daughter. Insurance Queenwood School has taken out student accident insurance for all girls enrolled at the school. For further information please refer to the Queenwood website or contact the Business Office. Non attendance at an excursion If your daughter cannot attend due to illness or some other reason please notify the school immediately.
Risk Warning Under Section 5M of the Civil Liability Act 2002 issued on behalf of Queenwood School for Girls Ltd: While Queenwood School for Girls takes measures to make the activities as safe as possible for participants, there is a risk that students can be injured and suffer loss (including financial loss) and damage as a result of their participation in these activities, whether at practice or in actual events. Injury can occur while the student is engaging in or watching an activity, or travelling to and from the event. The injury may result from a student’s actions, the actions of others or equipment failure. In very rare cases the injury can be life threatening or result in permanent disability.
