The use of tobacco in any form is not allowed on USD 368 property, in USD 368 owned/leased/rented vehicles, in contracted school buses or at USD 368 activities at any time, by anyone. (See JCDAA). USD 368 respectfully informs parents and students that the Family Privacy Act of 1974 applies to our student body. Student Directory information will be considered public and may be released by USD 368 including, but not limited to, colleges and the military. A written notice should be filed within 14 days at the Superintendent of Schools’ office, 1115 E. 303rd St., if the parent or student wishes to withhold the following Student Directory information: (1) Student’s name, address, phone number, date of birth; (2) The names of the student’s parents; (3) The student’s designation/grade level; (4) The student’s participation in officially recognized sports and activities; (5) The student’s weight & height if a member of an athletic team; (6) The student’s work, degrees, honors and awards received; (7) The student’s photograph; (8) Schools/dates of attendance; (9) High School student district email address. Students in Kindergarten through 6th grade must have 5 DTaP (diphtheria, pertussis and tetanus). 4 Polio, 2 MMR (measles, mumps, and rubella), 3 Hepatitis B Vaccines and 2 Varicella Vaccines. The first MMR/Varicella must be on or after the child’s first birthday. One dose of polio must be after the child’s fourth birthday. All 7th-12th grade students additionally are required to have one dose of DTaP regardless of the interval since the last dose of DTaP or TD. All kindergarten students must present a signed physical exam Paola USD 368 will present a Golden Ager Pass to residents who live in USD form at the time of enrollment. Physicals are also required for students 368 boundary area and are 65 years of age or older. SCHOOL LUNCH/NUTRITION PROGRAM A breakfast and lunch program is available for all students, grades K-12. All schools operate on a closed lunch system (meaning all students will remain in the school during the lunch period). Breakfast prices are FREE for grades K-8, $2.00 for grades 9-12 and $2.15 for adults. Lunch prices are $2.90 for grades K-5, $2.90 for grades 6-12 and $3.65 for adults. Students who participate in the breakfast and lunch program will be issued a card with an account number. This number should be used on all transactions with food service. IMMUNIZATIONS REQUIRED
monthly, bimonthly or weekly. Free and reduced-price lunch applications are available. Forms may be picked up at enrollment or from the manager/ clerk at the school where your student attends. You also may retrieve a free and reduced-price lunch application from our web site at www.usd367. org. Any student preferring to bring a sack lunch may do so. Students have the opportunity to purchase a second meal for breakfast or lunch or an extra entrée. These items are priced each year. Immunization records will be checked at enrollment to see if your child meets minimum immunization requirements. All students must have the minimum immunizations for enrollment. Kansas State Immunization requirements can be viewed at : http://www.kdheks.gov/immunize/schoolInfo
All students who are attending school for the first time in Kansas are required by state law to present immunization records (available at your doctor’s office) and a health assessment form when enrolling. Students new to Unified School District 367 are also required to show a birth certificate. Kindergarten students must be 5 years old on or before Aug. 31, 2018. First-grade students who have not attended kindergarten must be 6 years old on or before Aug. 31, 2018.
TRANSPORTATION
Bus transportation is provided for students living outside the city limits of Osawatomie and within the boundaries of USD 367. Students living within the city limits of Osawatomie may walk to the attendance center nearest their home or a designated shuttle stop to board a bus to ride to their assigned school. Shuttle service is provided in the morning and afternoon. There is no charge for this service. More information may be obtained at enrollment.
