Questionnaire III Effective CAF User Label - The TQM Maturity of the Organisation The questionnaire for TQM Assessment Maturity is based on the eight EFQMŽ Fundamental Concepts of Excellence as formulated in the Excellence model version 2013. In the context of the CAF External Feedback Procedure, these principles of excellence were translated into four maturity levels adapted to the public sector environment All eight principles are to be checked by the organisation as a first step, in order to gain an understanding of the TQM maturity within the organisation. During the site visit, two principles – as chosen by the applicant – will be assessed in depth (one of which must either be Result orientation or Customer focus). The evaluation relates to the maturity level achieved by the organisation as a result of the self-assessment and the action plan. In order to obtain the Effective CAF User Label, all eight principles must at least reach the maturity level "Initiation level".

Rating Scale 0 = Initiation level has not been achieved E = Initiation level R = Realisation level M = Maturity level For each principle (all 8) a rating is to be given in the appropriate light blue field (0 to M) being marked with an "X". If the organization rates E, R or M, please include the instruments used per principle (all 8). Please tick appropriate instruments and / or add instruments in the field provided "{input possible here}. The External CAF feedback Actors review only the two principles selected by the applicant organization.
