Recapitulate and Move Forward hank you to everyone who joined us this year at our Annual General Meeting (AGM), which was held virtually on Friday, November 5, 2021. Many points were discussed by the ADTO Board of Directors, which were supported by the membership. The ADTO continues to be in a strong financial position, and the feedback we got was positive. The major topics discussed were the Suggested Fee Guide, public awareness, membership benefits, and education. We also had two guest speakers from Lawrie Insurance Group and the College of Dental Technologists of Ontario (CDTO). One elaborated on membership benefits and the other on future regulatory outcomes for the membership. As we close off the year, we have witnessed many changes which impacted us greatly. Many challenges were brought up to the Board, and we took massive action to try and create a process that became not only the solution but also the way for the future. Many members seem to find it difficult to have access to or be able to find trained dental laboratory associates. This has been difficult for dental laboratories for many years, especially in the regions outside of the GTA. Connecting dental labs with potential employees has now become easier than ever. As an ADTO member, you can utilize www.adto.ca and use the classified section to promote your dental lab and what you are looking for. We will provide George Brown students and other trained

This process of ‘employee finding the employer’ can allow RDTs to focus on important business matters. In-person trade shows were something of the past; having hundreds of people in one venue seemed impossible. We are proud of the Palmeri Media Group for successfully putting together Spectrum Day Toronto. They went against the tide with daily changes on COVID-19 protocols and rules that made it difficult to have individuals sit next to one another. This shows that we are closer to having more in-person trade shows, and venues can be filled up once again to promote dentistry. The ADTO is looking forward to the new year; as COVID-19 measures lessen, we will be in the process of working towards our ADTO Gala. Membership feedback has given us direction to provide you with the necessary benefits for professional success; your voice matters. So please continue to email us and communicate your needs. The End of Another Year ecember is here, and we are at the end of another year. It’s a time, as always, to reflect on the past year and look forward to an exciting new year ahead. The Board of Directors and staff continually strive to work in your best interest and provide a professional association that offers career-building opportunities and helps strengthen our profession. We will continue to: • Strengthen our partnership with the Pacific Dental Conference to expand professional and networking experiences for you throughout the three-day conference. This year, the PDC will be live and in-person at the Vancouver Convention Centre from March 10-12, 2022. The Lab Expo is being incorporated into the main Exhibit Hall on Thursday and Friday. Once again, registration is free for DTABC members during the early bird registration, which runs from November 15, 2021, to January 14, 2022. See page 8 for more details. • Work is still ongoing with the other dental associations and regulatory colleges to help establish the new One Oral Health College that is part of the government’s modernization of regulation agenda. Unfortunately, as I write this, government is still totally focused on COVID-19, so activities around

the modernization agenda have been extremely limited. Hopefully, in 2022, COVID-19 will be on the decline, and we will be more active in the design and implementation of the new One Oral Health College, which will take some time to evolve. • We have been collaborating with the College of Dental Surgeons (CDSBC) and the BC Dental Association to inform dentists why they should always work with an RDT and to start a conversation on the future of dental technology within the context of the new One Oral Health College. This will include discussions around the Scope of Practice for RDTs. On behalf of the entire Board of Directors and staff of the DTABC, we wish you a happy holiday season and a successful 2022, both professionally and personally. accination can be a controversial and sensitive topic. In recent news, we’ve heard about businesses protesting vaccine certificates and how waning immunity from vaccination is becoming a concern as those at risk of COVID-19 are starting to get their third doses. Concerns about vaccination could affect people’s mental health, their decisions, and how they feel about coming into work. This could also affect the way people work together on both sides of the debate on vaccination.

How Might Misinformation Affect Working Relationships? Rumours and conspiracies surrounding vaccination and the pandemic can cause tension in the workplace. Unvaccinated employees in the workplace may feel stigmatized for their decision or feel pressured to get vaccinated. Those that have strong feelings on the subject might even judge vaccinated workers. Meanwhile, vaccinated workers might be unhappy about working with unvaccinated colleagues and want to avoid this for the time being. Fears of waning immunity from the vaccines might also cause anxiety about being at work if it’s been a while since they got their last dose. Why It’s Important for Employers to Handle Misinformation in Their Workplace? With so much misinformation online, it’s easy for people to lose track of the facts and get carried away by a strong emotional response. Therefore, it is in the interest of employers to address and manage misinformation in their workplace in order to protect their staff, customers, and business as a whole. Providing ongoing updates and education to staff is important for helping workers make the right decision when it comes to vaccination. “One of the best ways to avoid vaccinerelated issues in your workplace is providing staff with credible information on the subject. The more employees are educated on the

benefits of vaccination, the more likely they are to get their dose,” says Olivia Cicchini, Legal Content Specialist at Peninsula Canada. More information is becoming available as more studies are completed, and it’s vital that the sources being cited are credible. Provide Workers with the Right Information and Training Providing training on how vaccines work can help resolve vaccine hesitancy. Giving updates on how long vaccines last and whether more doses of the vaccine will be necessary can also help clear up confusion in the workplace. Employers can train staff in person with a presentation or virtually by providing online resources. What matters most is that the information is taken from reliable sources. Communication on vaccines should be clear and issued as often as there are news updates or changes to legislation. Staff should feel that their employer is transparent about how vaccination will be managed in the workplace and should feel comfortable asking questions or raising concerns with management. “While employees should not know the vaccination status of their coworkers, some employees may share their stance on vaccination with their friends at work. If members of your staff feel anxious about working alongside their unvaccinated colleagues, employers should

assure them that the business is observing all public health guidelines to ensure a safe and healthy workplace,” says Olivia. Use Policies to Handle Misinformation and Stop Inappropriate Behaviour To curb the spread of misinformation, rumours, and judgement among coworkers, employers can set expectations for staff behaviour relating to vaccination at work. Workers should be discouraged from discussing their vaccination status or inquiring about others’ vaccination status, as this can be a very sensitive subject. Instead, workers should be instructed to raise any concerns with their employers directly. This can help prevent the spread of misinformation and keep the workplace a professional and inclusive space for all. This applies whether you are a lab technician, a denturist, or a dentist.

The Journey to Perfection Honestly, it has often been said that when you finish your technical/professional training, you are just embarking on a learning journey into the real world. Person by person and case by case, each one is so unique and dependent on your intimate and extensive knowledge of the technology that it is a miracle that you create the perfection you do. This is because there are so many magazines, articles, blogs, conferences, and courses to stay current with to maintain your position of ‘state-of-the-art’ and stay at the top of the technical mountain. Getting Lost on the Way There are so many distractions and unfortunate incidents that can discourage you or your staff while trying to embrace the perfection you seek. For example, your premises can be too small for you to expand in, your equipment breaks at the worst moments, new technology makes what you already have become obsolete, great staff seem harder to find than ever, accidents and illnesses throw off your committed delivery dates, pandemics shut you down, and so on. With all these factors, it is very easy to feel overwhelmed and defeated. Keep Your Eye on the Mountain If you own a lab, it is vital to be the best possible leader you can be and keep your eye on your mountain. You have the duty and the need to be inspirational to your clients and your team. You must have a strategy of how you will move forward with your business step-by-step into the future 10

WINTER 2021

toward your goals. Then you break that strategy down into small, doable action plans and set deadlines for accomplishing each forward step. If you don’t have goals to work towards, you are just wandering in the dark and will eventually feel tired and burnt out and perhaps even give up. For an optimum growth trajectory, you need a strong strategy that is very real and doable, and then you must follow up weekly and monthly on the progress you are making. Creating a Great Team to Help You You will need to add technicians one by one as you grow. And you must mentor them and set high bars of excellence and perfection for them. Furthermore, when they turn out perfect products, you must celebrate their achievements and then challenge them to learn more skills and keep improving. You want to turn every one of them into true masters of their profession and encourage them to have great pride in their accomplishments.

Mentorship You would do well to find a mentor to push YOU and encourage YOU. A consultant can also make all the difference in the world (we know because we have been consulting healthcare professionals for 32 years, and the difference is night and day for the business owner to have a great consultant). Having someone to talk to, discuss your fears and hopes, and inspire YOU to be the best you can be is super important. You deserve the help because you have the weight of a business on your shoulders! Be innovative! Go for excellence!

The Team Approach to FULL-ARCH REHABILITATION Eric Kukucka, DD

“Collaboration has no hierarchy. The sun collaborates with soil to bring flowers on the earth.” – Amit Ray, the contemporary Indian author, scientist, and spiritualist. In dentistry, as in nature, collaboration is elemental. When arranging patient care for any implant treatment modality, an interdisciplinary approach is imperative. In my practice, for both digital and traditional or ‘analog’ dentures, working as a team with appropriate professional specialists is a non-negotiable constant. A collaborative approach affords the patient a full team of professionals, each with their own area of complementary expertise. By setting egos aside, beginning with the end in mind, a team can evaluate and determine a comprehensive, customized care plan including the necessary procedure(s), appropriate post-operative care, and an ongoing lifetime maintenance program. Throughout, whether they initially present at a dentist’s practice or at our clinic for an initial consultation, patients will enjoy the best prosthetic treatments available on the market today. And these treatments, when appropriately administered in the appropriate context, are truly special. The most exciting emerging technologies in our industry enable denturists and their teams to resolve ‘impossible’ cases every day. Whether I am preparing for a routine procedure or a particularly challenging, complicated project, my immediate initial approach is to methodically break down the various elements involved in the particular patient’s case: Does the patient present with natural dentition, 12

and if so, does it appear to be healthy? Is restorative work needed? What is the patient’s goal or desire? I use these questions to help assemble our collaborative team utilizing their expertise and ability to diagnose. Often, patients come to our office with a referral from their dentist, oral surgeon, or periodontist. We work with patients’ current care providers and assist with connecting them with other disciplines as necessary. Each specialist is an expert in their field; in each unique case, the team works together for a common goal in the patient’s best interest. Rehabilitating a patient’s smile is a genuine challenge that tests even the most experienced professionals on a daily basis. While several viable treatment modalities are available, the sheer number of options can overwhelm and confuse patients and oral health professionals alike. In my experience, modern terminal dentition or edentulous patients demand two key factors: aesthetically appealing results and function. This pair of requirements often presents an opportunity to educate the patient on a variety of options and expectations. As a team, we discuss all possible treatment options. Following this conversation, I present the appropriate options to the patient, educating them about the differences across full dentures, an implant-supported overdenture, fixed removable and full-fixed options. I explain the patient’s responsibilities, the longevity of each prosthetic, required maintenance, and possible complications.

Naturally, I consider the patient’s current condition, the patient’s goals, and the relative affordability of the various options. We allow each patient to make an educated decision regarding the treatment they want and to which they can commit.

OUR TEAM DYNAMIC In order to administer this treatment concept successfully, I believe it is imperative to exercise what I call ‘The Team Approach.’ In our practice, we conduct our treatments as a collaborative team – this is our mandate. Our collaborative team approach has broad applications, but it is ideal for implant treatment modalities. As Dr. John A. Hodges notes in DentistryIQ, dental implant treatment has guidelines rather than rules. Because every person’s dentition and oral health situation is different, every dental implant case is unique. Successful clinicians must begin a case with a broad understanding of the patient’s relevant particularities in order to devise a best-case treatment plan. Individual patient budgets clearly help to shape the course of treatment. Still, implant cases are also affected by underlying factors like bone quality and quantity, parafunction, occlusion, periodontal disease, and the size of the space to be restored. Part of our office culture at The Denture Center involves recognizing each patient who passes through our doors as an authentic individual. Every patient has a unique story and a

specific set of needs and expectations. It logically follows that each patient needs an individualized program of care and treatment. Our denturists evaluate patients on a case-by-case basis and work closely with specialists and general dentists to determine and deliver specialized care. Within our collaborative treatment team, a pervasive culture of mutual respect and a collective commitment to ‘check egos at the door’ enable us to create the most predictable, reliable, and efficacious treatment outcomes possible. Our team members rely on each other’s specific strengths, offering suggestions and observations as necessary. This collective approach is reassuring for patients; they know that all productive lines of communication are open, and the individual professionals responsible for their care are committed to working together toward a common objective. During collaborative work, specialized dental professionals inevitably – and reasonably – focus on their respective areas of expertise. As we do this work, there is never pressure to stray outside our scope of practice; there is accountability amongst us, and support is always available. Specific responsibilities are fluid and situation-specific, but it might be productive to outline some typical assignments: • Oral surgeon’s role: Rehabilitating any damage caused by trauma; determining necessary corrective bone or tissue measures; placing implants. • Dentist’s role: Performing restorative work in appropriate individual teeth, as required; determining which teeth are terminal; placing abutments; monitoring the patient’s care annually for oral health management; cleaning existing dentition or implants; providing overall care. • Denturist’s role: Fabricating the prostheses; performing prosthetic maintenance; communicating with patients. • Periodontist: Cleaning implants, preventative maintenance. • Lab Technician: Ceramics, additional technical support. Of course, there is room for potential ‘crossover’ or overlap between these defined roles, and all members of the team have important contributions to make to ongoing care and maintenance. Oral surgeons, dentists, periodontists, and denturists must all watch for evidence of changes in the oral cavity that might signal cause for concern.

Unlike conventional implant therapies, which typically require repeated visits across multiple office locations, protracted healing periods, and invasive surgical procedures like bone grafting and sinus augmentation, the All-on-4 dental implant solution permits brandnew, high-quality replacement teeth in as little as one day, using as few as four dental implants. This patient committed to new complete dentures as their treatment modality. Each year, the patient visits the clinic for an annual examination and cleaning of their denture. We also advise the patient to see their dentist annually for an oral health screening. The patient is also made aware of any warranties associated with their dentures and the estimated longevity of the prostheses. It is important the patient knows that our practice

The Terminal Dentition Patient Patients that enter our clinic (or are referred by their dentist) with terminal dentition are often the most vulnerable and anxious about treatment. They often have a limited understanding about dentures and don’t know what to expect. We typically begin conversations with the patient, trying to understand their fears while explaining the treatment. We don’t try to ‘sugar coat.’ Instead, we deliver a realistic perspective on ‘short-term pain for long-term gain.’ We communicate with the patient’s dentist and the oral surgeon coordinating the denture to be inserted on the day of the surgery. We often discuss details such as bone reduction, opening the patient’s vertical dimension, and anything else that may be of concern with the goal to reduce the number of variables and surprises. We emphasize the importance of interim transitional prosthesis for the patient. Communicating as a team enables us to plan with the end result in mind.

Terminal Dentition Prior to Treatment

A second example: When assessing patients in need of additional stability, we often propose an overdenture with two Locators. This is a popular treatment modality for patients who are faced with anatomical limitations, are unwilling or unable to endure an invasive surgery, decline bone grafting procedures, or have financial limitations. Here is where the team approach and commitment are key. Collectively, as a first step, we invest significant time in planning. As a denturist, I focus on the prosthetic, as outlined above. At our clinic, we fabricate high-quality interim immediate provisional dentures, then evaluate for the patient’s final definitive denture. This ultimately produces the best possible outcomes for soft tissues and the patient’s ability to function. It also vastly reduces prosthetic fractures and failures. The Ailing and Failing Dentition Patient Patients often come to our clinic in pain due to years of failing dentition, taking a toll on the individual's oral, physical, and mental health. This patient was faced with multiple personal traumas combined with a fear of dentistry. She was considering leaving the country for cut-price denture work when she came to our clinic. Listening to her story, we helped navigate her treatment. The first step was convincing her to see a dentist to diagnose and treat her pain. In this case, the dentist extracted her remaining teeth, and a new immediate digital denture was inserted. The Terminal Cosmetic Driven Patient The patient was referred to our clinic by a local dentist who entrusted our team with restoring her smile and, indirectly, her self-confidence. Her concerns and goals were cosmetically driven, but the dentist confirmed the longevity of the patient’s existing dentition was compromised. Collaboratively, our team, including her dentist and an oral surgeon, performed formal consultations, including x-rays, CBCT, and assessments. With the end result in mind, the team planned to discuss all the possible complications. Her treatment included two temporary phases to evaluate and manage soft tissue changes and the final zirconia prosthesis. This treatment involved many x-rays performed by the dentist and oral surgeon monitoring the implants at each stage, as well as evaluating the soft tissue and the final insertion of the definitive zirconia restoration. In this case, the goal was a complete arch rehabilitation producing optimal patient satisfaction and aesthetic with long-term results. Coordinating photos continued on page 17.

A second example: A patient of ours who, at 91 years of age, requested “straight, white teeth!” A dentist referred this patient to both us and an oral surgeon with the expectation we would work collaboratively. We offered all options to her;

the patient did not want anything removable and wanted quick results. Her health and age were major concerns, but she was ultimately very healthy and committed to a fixed hybrid acrylic prosthesis with a titanium bar (Montreal bar). Here are some images of the treatment process.

“Collaboration has no hierarchy. The sun collaborates with soil to bring flowers on the earth.” – Amit Ray, the contemporary Indian author, scientist, and spiritualist CONCLUSION The end result with every patient is a life-changing transformation. We’ve all heard the eyes are the window to the soul. We can admit this truism while still maintaining the smile is our best indication of a person’s self-confidence. Naturally, a smile is an external, public-facing indicator of a person’s happiness; in a very real sense, one’s oral situation affects how they, as a whole person, are perceived by friends, family members, and strangers alike. But just as – if not more – importantly, the health and quality of our smiles affect how we perceive ourselves. Here, as a result of our collaborative, team-based approach, in each case, the patient has left the clinic with an enduring perception of having received truly specialized treatment. When trust is earned, a patient understands that a complete team will provide them with long-term care. As a professional team that has invested time and labour in their specific case, the implications for post-treatment satisfaction are profound.

Eric D. Kukucka is president and owner of The Denture Center, a network of clinics currently comprising three facilities in Southwestern Ontario. Eric is simultaneously a prominent sales strategist and advocate for marketing best practices, as well as a skillful oral health professional dedicated to creating high-quality digital removable, fixed, and fixed-detachable implant prosthetic restorations. In addition to his clinical practice, Eric regularly contributes to collaborative research projects and product development and testing initiatives at a global level. Most significantly, he is recognized as an important consultant and key opinion leader by Ivoclar Vivadent, 3Shape, and Nobel Biocare, all of which are top-ofmind multinational companies in the global dental materials manufacturing, implant, and imaging software industries. Eric lectures extensively in North America and internationally on digital denture technology. He has given 60 keynote presentations over the past five years, including at the International Dental Show in Cologne, Germany. Eric is positioned to work with world-renowned dental products in a clinical environment, forming the connective tissue between research/development and the clinical and commercial aspects of denturism. Always endeavouring to knowledge-share and build meaningful relationships within the dental community, Eric has authored over 30 published articles and delivered dozens of podium lectures on behalf of Ivoclar, 3Shape, and Biocare, in five countries and two languages. Eric graduated from George Brown College in 2010, where he was named valedictorian of the Centre for Health Sciences.
