OK, we’re not quite on the level of the Proclaimers or Forest Gump (yet), but the Kent Express team racked up some serious miles last month! Starting in Teddington Lock and finishing at Dentsply Sirona’s offices in Weybridge, the team walked an impressive 13 miles along the picturesque River Thames in support of Dentaid, The Dental Charity.
On the ‘Miles for Smiles’ walk was Paula, Karen P, Harry, Carl, Jade, Emily and Larry from our Sales and Marketing Team. All of you did an incredible job! As our steps hit 30k at the finish line, the team was able to enjoy a thoroughly well-earned cold drink and a slice of pizza or two (or six if your name ends in arry).
In total, the walk raised a mightily impressive £15,251 for Dentaid. Every single penny of this will go directly to supporting their fantastic work. A huge shout out to Dentsply for organising the event with such success.

What is Dentaid?
Dentaid the Dental Charity provides crucial care for people who struggle to access dentistry. The charity mostly carries out emergency dental treatment to the most vulnerable people in our communities, including people experiencing homelessness, abuse, poverty and harm.
In the UK, the charity has two mobile dental units that travel up and down the country. The vehicles park up at shelters, hostels and centres to treat those in urgent need of dental care. Delivering the treatment are volunteer dental care professionals who often spend one day a month working on Dentaid’s mobile units.
The charity is always looking for new volunteers to help, along with fundraising contributions to broaden its reach and enable it to carry on providing the invaluable services that it does.

How to minimise endo file separation
Endodontist Dr. Rico Short DMD gives his eight rules for reducing the risk of file separation during root canals and picks out one of his favourite endo systems for the job.
“The 8 Golden Keys” To Minimise File Separation
1 Assess Case Complexity
If a case has severe root curvatures or if you see a canal then it disappears in the middle or apical 1/3 then it will be a challenge. There is probably a bifurcation. Pick the best rotary file for the job after using hand files to the working length.
2 Provide Adequate Access
Straight line access and coronal flaring is very important. It takes unnecessary stress off the file. In addition, the “crown down” method is more efficient than the “step back”. In other words, the rotaries are used from larger to smaller sizes in the crown down method. This allows for better irrigation and less binding of the files.
3 Glide Path
Adequate glide path to the apex is critical before even picking up a nickel titanium rotary file. Recommend a hand file at the apex to at least a size #15 prior to rotary use.
4 Light Pressure
Use light touch and torque control motor. Various files cause for different speeds and torque settings. Refer to the manufacturer’s instructions. The pressure I recommend is to hold the rotary hand piece like you would hold a very sharp pencil trying not to break the lead tip while writing.
Choosing the Proper Rotary File
5 Don’t Start Then Stop
Sudden changes in the motion of the file while inside the canal can create unwanted forces on the file.
A smooth gentle motion should be used while inserting and withdrawing the file inside the canals. I like to use the 5 second rule. I work a canal with a NiTi file for five seconds or strokes then change to a different file.
6 Lubricate Well
The canals should be always lubricated while cleaning and shaping. Recommend use of sodium hypochlorite. These agents will create less friction on the files while cleaning and shaping the canals.
7 Check Rotary Files
It is important to evaluate each rotary file before placing it into the canals. Look for shiny spots or flattened areas. If you see any of these replace immediately. Thermal sterilisation does not affect fatigue life of the files.
8 Replace Rotary Files
Most manufacturers recommend rotary files to be single use. However, there are some cases in which the file may not contact any dentine or very little. In these cases, it should be ok to re-use the file.

I’ve used many types of nickel-titanium rotary files in my over 17 years of practice. Some were very good, and some were not. However, now EdgeEndo has developed some very unique files. One of my favourite files from Edge Endo is the Edge File X7 Universal .04 taper. It’s strong, flexible, and economical. These files can be pre-curved to get into difficult and limited access areas. Once they are inside the canal, they follow the canal in a very smooth and silky way. They are also very durable due to their specialised heat treatment process of the nickel titanium.

How to step up your tooth whitening
The latest predications for the global tooth whitening market are for it to grow by 13.4% between 2022 and 2029 and reach £4.6 billion of sales.
Should this happen, the market will have more than doubled in the space of eight years. As the only UK professionals legally able to carry out tooth whitening treatment containing more than 0.1% hydrogen peroxide, dentists are in prime position to spearhead this growth. Here are our four keys to providing an attractive and successful tooth whitening offering that can increase your practice earnings.
Price
With many teeth whitening products now available on the high street at a low price, it is important that your treatment fees do not scare away potential patients. It’s also important to stress the differences between an over-the-counter High Street product and a regulated professional dental one, and how this leads to the price difference.

Tactfulness
It is one thing having a team clued-up on your tooth whitening options, but another thing choosing the right time to open this dialogue. This can be a sensitive skill to master. It is important to be tactful and avoid coming across as rude or insulting when starting conversation with a patient.
One approach to increasing the likelihood of the patient starting the conversation is to promote your whitening options around the practice. This could be through posters, commercials played on the waiting room screen or with a menu of whitening services.
Putting together a menu of services that spell out the tooth whitening services you offer, what they get and how much it costs can make it easy for them to digest and understand exactly what their options are. To gauge the interest of new patients from the get-go, including a question on their medical history form related to whitening treatment is a recommended.

Many patients want to be able to whiten their teeth from home with fast results, so it is important the whitening products you offer can accommodate this. An unsung hero that plays a key role in preventing the transmission of harmful bacteria and viruses in dentistry is the humble headrest cover. Providing a protective barrier between chair and patient, headrest covers are commonplace in practices thanks to their effectiveness and simplicity.
How important are headrest covers?
During treatment, all surfaces in a dental setting are at risk of becoming contaminated.
In being so close to the site of treatment, the dental chair headrest is a particularly high-risk surface. Covering the head of the chair can help capture any harmful contaminants which can then be safely disposed of in the appropriate waste bin.
Are headrests covers mandatory?
Like many dental infection control protocols (ICPs) and cleaning techniques, the use of headrest covers is not mandatory for dental practices but is advised in HTM 01-05 (Health Technical Memorandum 01-05) guidelines.
HTM 01-05 encourages the use of headrest covers but adds that they should not be taken as a substitute for regular cleaning. Covers should be removed, and surfaces should be cleaned after each patient contact. To maximise cleaning efficiency, the use of headrest covers should be combined with regular decontamination of the dental chair with alcohol free wipes, such as Cyber Ultraclean Wipes, so not to damage the chair material.

Disposable or reusable covers?
Headrest covers are available as either single-use disposable barriers or reusable covers that can be washed and reused across patients. Though reusable dental items can be convenient, they do come with the additional hassle of constant cleaning and wear and tear.

Why Helix Tests are so important

Sterilisation is critical within dentistry. From direct patient care to lab work, large numbers of instruments and handpieces are used each day.
Key to ensuring these items are fit to be reused and working to full capacity is the autoclave, able to sterilise large quantities through the combination of heat and time. As vital as the autoclave is to the repurposing process, the Helix test is arguably even more so. Because if you don’t have concrete proof that your autoclave is working efficiently, there’s no guarantee that your instruments and equipment are safe enough to use. In which case, you’re putting patients at risk of infection as the items may still be harbouring bacteria and viruses.
How often should you be testing your
autoclave?
All autoclaves require routine testing and maintenance daily, weekly and annually.
Class B autoclaves require a steam penetration test once a day before the autoclave has been used by using either a Helix test device or Bowie Dick. Helix tests measure the strength of steam penetration within the chamber. This demonstrates how effectively the steam in the autoclave penetrates the items inside. The Helix test is aimed more at testing hollow instruments while the Bowie Dick test is for testing steam effectiveness on large porous loads.

Timeless Technique Why retraction cord remains so popular with dentists
While the use of retraction paste and lasers has grown in recent years, in many practices cord remains king.
Gingival retraction cord is primarily used to push the gum tissue away from the prepared margins of the tooth to create an accurate impression. The oldest technique of the three, retraction cord continues to have universal appeal due its unique advantages and the advancement of tissue management systems in recent years.
About those advantages…
Cord is often preferred over alternative retraction methods because of how effective it is at moving and retracting gum tissue to provide a clear working view of the tooth. This is especially true where thick periodontium is present.
Retraction is also an ideal impression technique for digital scanners, which are becoming increasingly prevalent in practices. And one of the biggest benefits of retraction cord is its cost-effectiveness. It is a less expensive option versus paste and lasers.
What about bleeding?
The challenges of bleeding soft tissue during retraction have been well documented over the years. This is true of all techniques.

Your essential guide to fluoride

Fluoride varnish is one of the most popular treatments used to protect patients from cavities and tooth sensitivity. Used predominantly on children, some practices are now choosing to apply fluoride varnish to all children attending appointments.
Advice from Public Health England recommends that all children aged 3 years and over have fluoride varnish applied at least twice a year. Informed consent must always be given by the patient or parent/guardian.
What is fluoride varnish?
Fluoride varnish is a form of topical fluoride that is brushed directly onto the tooth’s surface during dental appointments.
Once applied, the sticky-like substance hardens and begins slowly releasing fluoride into the enamel. It typically stays on the teeth for 12 hours or more.
Fluoride has been found to help prevent cavities by making enamel more resistant to the actions of acid and sugars (demineralisation) and repairing existing damage (remineralisation).
What is it made of?
Fluoride varnish is composed of a high concentration of fluoride in a fast drying, alcohol and resin-based solution. The fluoride content of a varnish is commonly referred to as parts per million (ppm) or as a percentage.
The active ingredient of most fluoride varnishes is usually 5% sodium fluoride, (22,600 ppm fluoride), though some variants contain only 2.5%.
Fluoride has been found to help prevent cavities by making enamel more resistant to demineralisation. ”
Why is it used a lot on children?
Fluoride varnish treatment is especially recommended to children because of its ability to protect the teeth from cavities and tooth decay.
Tooth decay is the most common reason for children aged five to nine to be admitted to hospital in the UK and is one of the most prevalent chronic diseases among children worldwide.
In 2019/20 alone there were approximately 37,000 hospital admissions of children to extract decayed teeth in 2019-20. Fluoride varnish treatment is quick, non-invasive, painless and therefore an easy application on most children.
Can adults get fluoride varnish treatment?
Absolutely. It is not only children who can benefit from fluoride varnish. While it may not be important for everyone to continue fluoride treatment into adulthood, patients who are more at risk of cavities and tooth sensitivity can benefit from regular in-office fluoride application.
Fluoride varnish application best practice
Fluoride varnish is applied directly onto the tooth surface with use of a small soft brush. It is recommended that fluoride varnish be applied to as many tooth surfaces as possible for the enamel to fully benefit. A moist tooth surface is ideal for application and the treatment should only take a few minutes when done correctly.
NHS guidance recommends that patients avoid eating or drinking for approximately 30 minutes after treatment. For best results, they should avoid hot and sticky foods for 4 hours and stick to a soft diet for the rest of the day. Toothbrushing should be restricted to the morning after treatment. At this point the varnish should come off easily when brushed with toothpaste and any discolouration should also disappear.

Is it safe?
Despite the high fluoride concentration, the amount of varnish applied during treatment is very small. Because of this and how well the varnish adheres to the enamel, almost no fluoride is swallowed and therefore the risk of dental fluorosis is minimalised. An appropriate medical history should be taken, and a risk assessment should be carried out.
It is recommended that children showing obvious signs of systemic illness (e.g. colds, flu) or any abnormality of the face, lips or soft tissues of the mouth should be excluded on the day from fluoride varnish application.
Who can administer treatment?
Fluoride varnish treatment should only be administered professionally in a dental practice. Without this, patients are at risk of unsafe levels of fluoride and experiencing side effects.
Treatment can be carried out by a dentist or under the prescription of a dentist by a dental therapist or hygienist. According to the General Dental Council (GDC), dental nurses can also carry out fluoride varnish application on prescription from a dentist or via a dental health programme.
Characteristics to look out for
Colour: White varnish is often preferred by children and adults over brown/yellow varnish as it offers natural tooth-like aesthetics.
Handling: While tooth surfaces should ideally be kept dry during application, this isn’t always fully possible. Look for a varnish that claims to be easy to apply on wet tooth surfaces.
Flavour: Children can be particularly fussy when it comes to taste and smell. Stocking a range of different flavours can help with compliance in young patients.
Gluten-free: Opting for a paste that doesn’t contain gluten can help accommodate for the 8.5 million or so people in the UK following a gluten-free diet.
