SAFETY AND HEALTH INDUCTION Page 1 of 8 Tenant : _____________________________ Lot No. : _____________________________ Date : _____________________________ Time : _____________________________ INTRODUCTION: The purposed of this safety induction meeting is to ensure that all tenants, contractors or any other personnel intend to carry out work within Paradigm Mall Johor Bahru (PMJB) area are aware with Paradigm Mall Johor Bahru (PMJB) safety rules and regulation, Factory and Machinery Act 1967, Occupational Safety and Health Act 1994 and any other relevant Malaysian government regulation to be applied on work site. 1. WORK PASS When a contractor arrives at PMJB to carry out works, they are required to report at security post located at Ground Floor Fire Control Room (FCR) to obtain working pass for each working personnel. They shall produce worker name list and valid work permit approved by the management. 2. PERSONAL PROTECTIVE EQUIPMENT (PPE) All workers must be properly attired that is they must: a) Wear long pants - No short pants. b) Wear long sleeve/short sleeve shirts/T-shirts - No singlet. c) Wear safety shoes/boots - No slippers. d) Wear safety helmet (work above 2meters). e) Safety harness (if working at height) f) Do not wear torn clothing. 3. SMOKING Strictly no smoking activity at the work area or any places of PMJB except at designated smoking area. Anyone caught smoking should be removed from the work site IMMEDIATELY 4. ALCOHOL AND DRUGS Alcohol, intoxicating drinks and drugs taking are strictly forbidden at work area. Disciplinary action shall be taken against any workers under the influence of alcohol or drugs. 5. PERSONAL BEHAVIOUR AT WORKPLACE Fighting, horseplay or practical joking is not permitted at the workplace. Appropriate disciplinary action will be taken against any workers who indulge in such practice. Outsiders who are not related to the work are not allowed to be taken into the work area by any personnel.

ELECTRICAL COMPLIANCE a) All electrical tools used must be in good working condition. b) Any defective tools, exposed wire, damaged conduits, plugs or switched must not be used until fully rectified. c) Do not attempt to repair defective equipment which should only be repaired by an authorized electrician. d) A 3-pin plug must be used for all electrical connection. e) Inserting of wire into the plug points is not allowed. f) Switch off the main power supply whenever equipment is not being used. g) Any offence or non-compliance on the above may result stop work order by PMJB management. 7. LOITERING Workers are not allowed to loitering far from their work area. Anyone caught loitering far away from his or her work area must be requested to return to his respective area. 8. HOUSEKEEPING Good housekeeping is the basis for a safe work environment. It prevents accidents and creates a business-like workplace. In order to achieve this, the following rules shall be adhered into: a) Pile or stack of materials in a stable manner and line them up with the job. – pile or stack material at corridor especially fire escape route are strictly prohibited. b) Remove rubbish, scrap and debris from work areas as soon as practicable. Do not allow the unsafe accumulation of waste material as it will create health hazards and a source for vector breeding, e.g. rats, aedes mosquitos, etc. c) Combustible materials such as oil-soaked or paint-covered rags, waste, rubbish etc. should not be allowed to accumulate except in areas specially provided for this purpose. d) Fuel storage area must be isolated and properly secured. A ‘No Smoking’ rule will be enforced. e) Welding cable, hose, electric leads etc. must not be in walkway where they create tripping hazard. 9. PRESENCE OF SUPERVISOR Ensure at least senior personnel at work area to supervise the workers and monitor closely all works. They are to ensure there are safety compliances. 10. ACCIDENT / INCIDENT REPORTING All accidents, incidents, near-misses and any other form of activity, which may have endangered life or cause injury to worker, must be reported to the Control Room immediately. 11. LADDER Ladders are only intended as a means of access and not a working platform. Where it is not practical to erect a scaffolding to carry out the work, a ladder shall be used provided: -


SAFETY AND HEALTH INDUCTION Page 3 of 8 a. The ladder is a well-constructed type. b. The top rung is lashed to a strong point. c. Do not use the top rung as a working platform. d. No timber ladders and self-fabricated ladders are allowed in the premises. Only manufactured ladders are allowed. Ladder used shall be of appropriate height of the works 12. TROLLEY All contractors must bring along their own trolley. 13. WORKING AT HEIGHT 13.1 SCAFFOLDING 13.1.1 Any scaffolding erection shall comply with Factory and Machinery Act 1967 (FMA 1967) under (BOWEC) Regulation Part X and any requirement implement by PMJB. 13.1.2 Any scaffolding exceeding 3 meters should be erected by competent erector registered with Department of Occupational Safety and Health (DOSH) depending to their level of competency. 13.1.3 Copies of valid certificate of the competent erector, plan and design load calculation shall be attached together with PTW and submit to Safety Department for records and approval purposes before commencement of work. 13.1.4 Any erection 15 meters (frame type), 40 meters (tubular type) required a Professional Engineer (P.E) design and submits to Security Department for comment and approval. 13.1.5 One (1) competent person can lead maximum of four (4) workers only under his supervision during the erection, alteration and dismantling of the scaffolding. 13.1.6 All workers involved in the erecting, altering and dismantling of the scaffolding structure are required to wear proper Personal Protective Equipment (PPE) such as safety helmet, safety shoe/boot, safety harness (safety belt is prohibited) and others relevant safety equipment at all times. 13.1.7 The surrounding of the working/erection area need to be barricaded/cordoned off in proper manner and with warning sign at all times to avoid any incident/accident to any person passing through the area. 13.1.8 All scaffolding shall be properly supported only by an approved adjustable base plate, fix base plate or castor wheel. Direct support/placement to the floor is prohibited. 13.1.9 After the erection has been completed, the competent person shall inspect the scaffold and submit the inspection form to security department for records/approval. 13.1.10 For any erection below than three (3) meter, the submission of the inspection from and competent erector shall be exempted. However, supervision by the supervisor and visual inspection is a must prior to use the scaffolding. 13.1.11 Please fill the Risk Assessment Form SAFETY NOTES a) Never attempt to move the tower with worker or material on it or lift up the stabilizer from the ground surface in order to push the tower. Do not remove stabilizer while pushing the tower. b) Always be aware of any overhead hazards and make sure that the route to be taken is level and there is no hole or other obstruction. c) Do not lean ladder against the tower or climb the outside of the tower. Whatever your intended access system, it should be used inside the tower. d) Do not work from ladders or stairway, they are means of access only.


SAFETY AND HEALTH INDUCTION Page 4 of 8 e) Guardrails and toe boards must be fitted to the working platforms. f) Never jump on to or off platforms. g) Do not exceed the safe working load on the platform or structure by accumulating debris, material or tools on platforms as theses can be a significant additional load. h) If you must move the tower, remove all materials and personnel. When moving a scaffold tower, force must always be moved from the base. The tower should only be moved manually on firm, level ground which is free from obstacles. Normal walking speed should not be exceeded during relocation. The ground over which a tower is moved should be capable of supporting the weight of the structure. PMJB management has the authority to stop, penalize and reject any scaffolding work that does not meet/comply with all the legal and requirement as stated. 13.2 HOISTING MACHINE 13.2.1 Any passenger hoist using within PMJB shall comply with Factory and Machinery Act 1967 (FMA 1967) under Notification, Certification of Fitness and Inspection, Occupational Safety and Health Act 1994 and any requirement implemented by PMJB. 13.2.2 A copy of valid PMA and license as an operator/assembler registered with DOSH and list of competent/trained operator, load calculation, updated wire rope certificate and declared maximum person/load can be lifted shall be submitted together with permit to work (PTW) form to operation1001 department for approval at least 1 working day before work commencement. 13.2.3 The operators shall inspect the lifting equipment/machine before use (on the day of using) and submit the equipment/machine inspection form to safety department for review and approval. Inspection form shall be duly completed by the competent person/operator. 13.2.4 In event of use of any passenger hoist (genie lift, gondola and other passenger hoist) belonging to PMJB (that is under the custody of M&E department), only upon approval by the management then can the machine be released to the contractors. Only competent and trained operator can operate the machine, all other person is strictly prohibited from doing so. 13.2.5 Weekly inspection shall be conducted for any use of lifting equipment/machine for duration that is more than a week and such inspection record shall be submitted to safety department. 13.2.6 During the lifting process, the area of work shall be properly barricaded / cordoned off (e.g. with hazard tapes, cones, warning sign, etc.) at all the time until the work has been completed so to prevent any accident/incident that may result due to public passing through the area of work. 13.2.7 Contractors to provide sufficient floor protection to avoid any damages to the floor finishes and adequate drip tray to avoid any staining caused by chemical spillage/leakage from the crane. Any repairs to the damages caused and clean up shall be carried out by the contractor upon completion of work. Failing which penalty and cost of rectification and cleaning up shall be charged to the contractor. 13.2.8 A full time supervisor shall be stationed during the lifting work to supervise and coordinate the work. 13.2.9 During the lifting work, all workers working within the lifting radius/area are required to wear Personal Protective Equipment (PPE) such as safety helmet, safety shoes, safety harness – safety belt are strictly prohibited (at any working height) and other PPE if required by the security department and also based on current working condition.

Page 5 of 8 13.2.10 Contractor shall ensure sufficient lighting is being provided for any lifting work during night time for clearer view to the workers. 14. GENERATORS AND COMPRESSOR 14.1 Generators 14.1.1 Only competent personnel are allowed to operate or maintain a generator. 14.1.2 Lubricant, water level and fuel should be checked before operating. Do not attempt to refuel generators in running condition. 14.1.3 Regular inspection and maintenance to be carried out. Generators should be properly earthed and the termination point checked. 14.1.4 Do not attempt to repair defective equipment which should only be repaired by an authorized technician. 14.1.5 Extra care to be taken when operating any equipment in wet conditions. 14.1.6 All exposed electrical wire, damaged conduits, plugs or switched must not be used until fully rectified. 14.1.7 Switch off the main power supply whenever equipment is not being used. 14.2 Air Compressor 14.2.1 All compressor brought in to this premise shall have a valid PMT certificate and submitted to safety department for review/records. 14.2.2 Compressor shall only be operated by competent persons. 14.2.3 Regular inspection and maintenance is required to ensure unit is safe for use. 14.2.4 Do not use compressed air to dust clothing or to cool the body. 15 HOT WORK Contractor must obtain a hot work permit for works that produced sparks, fire flares and etc (i.e. welding, cutting using acetylene gas and oxygen works and etc.) from the Management 9.00 am 6.00 pm. Please contact Operation Team(07-2313888) at least 2 days prior to commence the hot works for appointment. Failure to comply or present the approved hot work permit upon request by the security will result in work stoppage. 16 CIDB CARD All workers must have valid CIDB card(green card). When apply work pass must handover to security personal (Please not use IC or license or passport) 17 HOARDING All hoarding must install properly if have any defect contractor must rectified between 24HRS PMJB reserves the right to issue a stop work order, penalty, reject and ban the contractors from site if the contractor fails to comply with any of PMJB rules and regulation and /or instruction.
