As the Regional Accountable Entity (RAE), Rocky Mountain Health Plans (RMHP) is responsible for connecting Health First Colorado members with both primary care and behavioral health services for Region 1, Larimer County. The RAE seeks to improve each individual’s experience of, and access to, health care. At the same time, it establishes accountability for cost management and improved health using a patient-centered medical home model.

Box 3616, Estes Park, CO 80517 Provides food, vouchers for laundry, showers, clothing, limited housing, utility shut-off, and assistance with medical bills and prescription medications. Also provides training in money management and energy efficiency for low-income households. Services limited to residents of Estes Valley.

Helps to stabilize and strengthen the at-risk families, children, youth, and seniors of the Loveland and Berthoud communities.

PAFC’s mission is to foster leadership and strong community collaboration that will plan, design, and implement sustainable strategies to enhance the quality of life for older adults in Larimer County. With the use of grassroots teams of volunteers called Self Directed Volunteer Teams (SDVT), PAFC has a focus on four priority groups to help promote a livable community: Housing, Health and Wellness, Mobility and Access, and Culture of Aging.
