On a Fall morning in the Southcentral US, a loader operator left the logging deck to make an emergency trip to the auto parts store to purchase a replacement part after experiencing a ruptured hydraulic hose on the loader. Conditions were clear and not a contributing factor in the accident.
UNSAFE ACTS AND CONDITIONS
The driver was exceeding the posted speed limit which caused him to lose control of the company service truck. Additionally, he was not wearing a seatbelt and was driving on a suspended license.

The driver was traveling on a straight 2-lane roadway and began to exceed the posted speed limit of 45 mph as he approached an overpass. He continued to accelerate, reaching an estimated speed of 100 mph. Once he topped the overpass, the driver lost control of his vehicle and crossed the center line. The truck continued down an embankment and struck the guide wires for a utility pole, causing the truck to flip and travel across a street that ran parallel to the road on which he was initially traveling. The truck continued to flip numerous times while striking several trees, and eventually struck the backside of a natural gas building.

The driver was completely ejected from the truck and seriously injured. He was found lying unresponsive on the ground directly in front of the crashed vehicle. Local authorities and paramedics arrived on scene shortly after the accident and stabilized the injured driver for emergency transport. The driver was initially transported by ground ambulance to a local hospital for further evaluation and treatment. Upon arrival, he was immediately airlifted to a regional trauma center and admitted to the intensive Care Unit (ICU) with numerous life-threatening injuries, including a traumatic brain injury, numerous spinal fractures (lumbar, thoracic, and cervical), and a collapsed lung.
