Congrats! Your child or children have been enrolled into Camp Kaleidoscope!! We welcome all parent(s), guardian(s), and camper(s) to Camp K!! This summer we are looking forward to new staff, new camper(s), and tons of fun! Camp Kaleidoscope is up and in season for six weeks starting Monday, June 26th to Friday, August 4th. Please read the information outlined in this handbook carefully, as many of your questions about Camp Kaleidoscope operations will be answered for you. We thank you in advance for your flexibility, cooperation, and feedback. Our accreditation by the American Camping Association (ACA) helps us to ensure a fun focus on program quality, health, and safety issues. We are proud to be a member of this nationally recognized camping organization. CAMP MISSION: It is the mission of Camp Kaleidoscope to provide an opportunity for children to participate in healthy, educational, and fun activities during their summer vacation. No child will be denied participation based on gender, color, race, religion, or inability to pay. 2
CAMP GOALS: 1. Safety - To provide a safe, nurturing day camp experience for children. 2. Fun - To provide an opportunity for children to participate in healthy, educational and fun activities during their summer vacation. 3. Skill Development - To give campers the opportunity to develop new skills in a variety of areas. 4. Citizenship - To help campers make positive contributions to the camp community through individual responsibility and leadership. PROGRAM OBJECTIVES: 1. Safety - Campers will learn safety rules regarding travel, field trips, swimming activities, use of equipment and supplies, and personal behavior. 2. Fun - All of our activities are designed with fun in mind, and camper(s) will be able to make individual choices for various activity periods each week to give them choices on what to do. 3. Skill Development - Campers will improve their skills in swimming, arts and crafts, positive decision making, and personal communication. 4. Citizenship - Campers will participate in activities designed to promote teamwork and cooperation. Age group break downs feature: 5-6 Camp, 7-8 Camp, and 9-12 Camp. Camp K features arts & crafts, games/sports, exciting electives, trips to state parks, attractions, and swim lessons. Guaranteed fun with a thoroughly trained and enthusiastic staff!

PAYMENT OF FEES: A non-refundable $20 deposit for each additional week must be paid at the time of registration. Checks should be made out to the City of Lebanon. All remaining camp fees must be paid in full by Wednesday, June 1, 2016. Fees may be paid only at the Lebanon Recreation & Parks Department. All finances are managed by the City of Lebanon Recreation & Parks department staff including the Camp Director. ATTENDANCE: a. Cancellations: If your camper will not be able to attend a week they are registered for please let the Recreation & Parks Department know as soon as possible so that we can notify our waiting list. If cancellation is made prior to 10 business days up to the Thursday before camp, a full refund less the $20 nonrefundable deposit will be issued. Cancellations made after the previous Thursday will not receive a refund. b. Illness or absenteeism: If your camper(s) is ill or unable to attend camp, please notify the Camp staff. If a camper(s) is absent after morning check-in the camp staff will contact the parent(s)/guardian(s) to verify the absence, and note the absence on the check-in sheet. If parent(s)/guardian(s) notify the camp of their camper(s) absence in advance it will be noted on the check-in sheet. STAFF/CAMPER(S) RATIO: Staff members must take extra caution when supervising campers in public situations. Head counselors will assign supervision areas to each counselor, ensuring that approved camper to counselor ratios are maintained. The staff/camper ratios at Camp Kaleidoscope are as follows: Ages 5 - 6 1:7 (3 Staff) Ages 7 - 8 1:8 (4 Staff) Ages 9 – 12 1:10 (3 Staff) There will be a minimum of two staff members required for all activities with the exception of on-site group games where all staff is on site and split into groups on the same field, on the playground, within the facility, or during arts and crafts and the art director is present. The camp director will assist with supervision when available, and this includes the site assistant stepping in if needed to ensure proper ratios are being followed. It is policy that a staff member is never left alone with one camper and that staff members have communicated plans to their head counselor or the director. It is expected that campers are in a group of 2 or more when with one staff member. In an emergency situation off site where a medical transport is needed a staff member will accompany a camper in the ambulance and remain present in the hospital. It is this exception only when staff will be with outside medical staff and the ratio of staff to campers would be adjusted for the remaining campers and staff. When the camp is back on site the site assistant, camp director, or art director would assist to ensure the proper ratios are being followed. RULES AND REGULATIONS: It is expected that camper(s) will: a. Respect the property of others b. Respect themselves and others c. Conduct themselves in a safe and responsible manner at all times d. Follow all rules as presented by the Camp Staff CHILD ABUSE: Child abuse is a serious criminal offense. Staff who may suspect a camper is being abused, or to whom a camper discloses abuse (physical, sexual, emotional, or neglect), will notify the Camp Director of any concerns. The Camp Director will notify the Recreation Coordinator who will contact the Lebanon Police Department for assistance in notifying DCYF. 4
BEHAVIOR MANAGEMENT POLICY: Every attempt will be made to clearly and respectfully communicate behavior expectations to campers, as well as the consequences of misbehaviors. Each situation will be judged individually, and parent(s)/guardian(s) will be notified of major rule infractions, among them: - Violence - Injury to other campers - Injury to staff - Destruction of property - Stealing - Verbal abuse of others Socially sensitive issues such as smoking, drugs, tattoos, body piercing, sexuality, dating, cults, religion, ghosts or horror stories, divorce, and personal lives of staff will not be discussed with campers. TIME TO THINK: When a camper chooses to continue a negative behavior after being given a rule reminder from a counselor and an opportunity to change the behavior she/he will be given a “Time to Think.� This time away from the group should provide the camper with time to cool down and reflect on the effect their behavior has had on themselves and/or the rest of the group. RULES AND REGULATIONS OF CAMPERS: It is expected that campers will: a. Respect the property of others b. Respect themselves and others c. Conduct themselves in a safe and responsible manner at all times d. Follow all rules as presented by the Camp Staff, and created by the campers at the first morning meeting CHECK-IN/CHECK-OUT PROCEDURE: Check-in and check-out will take place at the Rear Entrance of Hanover Street School off of Evans Drive. Parent(s)/Guardian(s) must accompany their child to check-in and check-out. The check-in/check-out counselor must note in writing the time of their arrival and departure. The counselor will provide the parent(s)/guardian(s) a Camp Kaleidoscope Newsletter informing them of the plans for the week, field trip information, and contact information for the camp staff. Campers will not be released to parent(s)/guardian(s) waiting in cars. If camper(s) will be late in returning from a field trip the Camp Director will contact parent(s)/guardian(s) by phone, and or setup check-out and inform parent(s)/guardian(s) as they arrive the expected return time. CHECK-IN/CHECK-OUT TIME: Check in time is from 7:30 - 8:30am for ALL CAMPERS. If unforeseen circumstances do not allow check in by 8:30am, parent(s)/guardian(s) are asked to please notify the Camp Kaleidoscope staff, or staff will contact you in the AM to ensure attendance or not. Activities are in progress by 8:30am and the check-in table is not staffed beyond this time. For the safety of your camper(s) please call ahead if you will be late in the morning. On field trip days, we cannot guarantee that the camp activities will be delayed for late camper(s). Should your camper(s) be late for a field trip departure please be aware that she/he cannot remain at Camp Kaleidoscope as there is no staff to watch your camper(s). Check out time is from 4:00 - 5:00pm for ALL CAMPER(S), unless noted otherwise on weekly newsletter for field trips. EARLY CHECK OUT: Please notify a counselor in the morning if you will be picking your camper(s) up early (prior to 4:00pm) for any reason. Camper(s) participates in clean-up activities and a closing meeting during which important information is given out for the next day, and campers are acknowledged for achievements during the day. We feel this is an important aspect of our program, so please allow your camper(s) to stay at camp until 4:00pm whenever possible! 5
***Please be aware that the camp day ends at 4:00pm, unless noted otherwise on weekly newsletter for field trips. PICK-UP PERSONS: On the camper(s) registration form, parent(s)/guardian(s) must provide the names of any and all persons that may be picking their camper(s) up from camp at the end of the day. These names will be kept on file at the check-in table. Camp staff is not authorized to release your camper(s) to anyone except those persons on the list. If a pick-up person arrives and they are not on the list please have the pickup person wait while a phone call is made to approve the pick-up. Please be advised that pick up person(s) will be asked for identification (License) until the staff becomes familiar with parent(s)/guardian(s) and friend(s) of the camper. We appreciate your cooperation in our efforts to keep your camper(s) safe while at camp. WHAT YOUR CAMPER(S) SHOULD BRING TO CAMP: a. Daily lunch & 2 snacks - Should include a full lunch, a morning, and afternoon snack and beverages for the day. No beverages or snacks will be provided by the camp, although water is available at all times. There is no refrigeration available, or microwave to re-heat, or cook food. Please pack drinks and food in insulated thermoses and coolers to prevent spoilage. Please put only water in drink bottles to avoid attracting insects, and prevent dehydration with other beverages such as soda, coffee, or milk. If camper(s) forgets their lunch or snack, the camp staff can provide a meal option with these supplies available; cheese, ham, turkey, animal crackers, gold fish, granola bars, and wheat bread. ** Please note that we do have camper(s) allergic to peanuts and tree nuts (almonds, Brazil nuts, cashews, chestnuts, filberts/hazelnuts, macadamia nuts, pecans, pistachios, and walnuts) and we ask that if your camper(s) are having any nut related products prior to camp to please wash their hands, and please consider other lunch alternatives, and or please let the staff know at check in that your camper has peanut butter sandwich, Nutella sandwich, PB crackers, etc‌ Thank you for your understanding and consideration in assisting us to have a safe environment for all camper(s). b. Rain gear - Camp goes on field trips/activities/swim lessons rain, or shine. Please provide adequate shoes and rain gear if the weather looks at all questionable. c. Extra clothing - Please provide a sweater, or sweatshirt and a change of clothes, labeled clearly with your campers name on a tag, sleeve, or with tape. d. Bathing suit and towel - Please bring a swim suit and towel every day. e. Sneakers - Your camper(s) feet will be safer if sneakers are worn at all times. Please do not allow your child to wear jelly shoes, sandals, flip flops, flats, crocs, and other unsupportive shoes that leave the feet exposed and will not allow your child to participate fully and safely!!! We ask that the entire foot is covered during the camp day and that camper(s) have adequate support in activities. This is especially important on field trip days!! If attending a water park, state park, or facility that has rough surfaces and your camper(s) would like to bring water shoes that is acceptable. f. Backpacks - These will be especially helpful on field trip days when the camper(s) may be hiking, or traveling to field trip locations. The Recreation & Parks Department has a supply of packs to give any camper(s) who may need one. It is important that camper(s) are equipped with a bag/pack of some kind to carry a lunch, wet clothes, and a water bottle. Much of the mentioned above will need to fit into a back pack. g. Sunscreen Lotion & bug spray - We recommend that camper(s) bring their own packed in their own bag. However, the Camp staff will provide sunscreen (lotion only) and aid camper(s) as needed to apply sunscreen only. Sunscreen is provided as a courtesy, and we ask that parent(s)/guardian(s) apply sunscreen to camper(s) prior to starting the camp day. The Camp staff is NOT allowed to administer bug spray, and if camper(s) would like their own they must bring their own. 6
WHAT TO NOT BRING TO CAMP: To ensure the enjoyment, safety, and health of all camper(s) attending camp the following items should be left at home: a. Money – Camper(s) will not need money at camp or during field trips. b. Personal items - Candy, gum, weapons (knives, water guns, etc…), electronics (walkman, discman, radio, walkie talkie, iPod, iTouch, iPad, Nintendo DS, PSP, MP3 Players, cell phones, Game Boy Advance), board games, books, fishing equipment, Legos and sporting equipment. We ask that any of the above items are left at home in order to not be broken, damaged, or lost at camp as the Camp staff is not responsible for any of the above items. If a camper(s) is to ask Camp staff if they could bring in some of the above items for a specific activity this could be arranged, and those items will be stored in the Staff Office until ready for use. ACTIVITY INFORMATION: PLEASE NOTE THAT SCHEDULES AND ACTIVITIES MAY CHANGE DUE TO UNFORESEEN CIRCUMSTANCES, CAMPER(S) BEHAVIOR, & WEATHER CONDITIONS! Activities include arts & crafts, sports and games, outdoor environmental education, reading, swimming lessons twice a week, free swim one to two times a week, and field trips once a week. Camper(s) ages 5 to 8 will also participate in the “Thursdays in The Park” program at Colburn Park (just across from City Hall) and will be visiting local parks with larger playgrounds, and a skatepark. CAMPER FIELD TRIP ORIENTATION: Before each field trip in an all-camp meeting, campers will receive a brief orientation about the week’s field trip. This is the responsibility of the head counselor. Safety precautions will be covered, including bus safety rules, location of restrooms, trail etiquette, and trash disposal. Counselors will use group time prior to the field trip to review the information, answer any questions and set up a buddy system. Counselors will review safety rules specific to the facility with campers before entering the facility. LOST AND FOUND: Every day any items lost will be placed at the check-in/out table. If the item is unclaimed at the end of the day that item will be placed in lost and found. On Friday the Camp staff will empty the lost and found and display all items to be picked up. During this time camper(s) and parent(s)/guardian(s) will have the opportunity to claim any articles that may have been lost during the week. At the end of camp any unclaimed items will be taken to LISTEN CENTER. TRANSPORTATION: The speed limit in school zone is 10 mph. Parent(s)/guardian(s) must provide transportation to and from camp each day, unless camp staff is notified of an alternate pick up or drop off plan. Cars delivering campers must use the designated driveways, parking, side-walks, and street sides. If the camper is riding a bike or using an alternate mode to get to and from camp, a note must be provided at the start of the camp day with parent(s)/guardian(s) permission. This note should include dates this mode of transportation is approved for. Parent(s)/Guardian(s) must accompany their child to check-in and check-out. The check-in/check-out counselor must note in writing the time of their arrival and departure. The counselor will provide the parent(s)/guardian(s) a Camp Kaleidoscope Newsletter informing them of the plans for the week, field trip information, and contact information for the camp staff. Campers will not be released to parent(s)/guardian(s) waiting in cars. If camper(s) will be late in returning from a field trip the Camp Director will contact parent(s)/guardian(s) by phone, and or setup check-out and inform parent(s)/guardian(s) as they arrive at the expected return time.

* Please feel free to use the above form for the first day of camp. (If needed). In case of an emergency and your camper requires medical transportation: 1. An emergency is defined as a situation that requires an immediate response to insure the safety and well-being of campers and staff. 2. The first on scene will assess the situation and administer first aid as required within their ability. They will determine if 911 assistance is necessary. 3. The second on scene will call 911 give their name, location and description of injury when calling. 4. The second on scene will also contact the Recreation & Parks Department and inform them of the emergency, camper’s name, and what hospital they will be transported to. 5. The Recreation Department will contact parent(s)/guardian(s), fax the appropriate health form & insurance information to the hospital. 6. The first on scene will remain on scene until emergency personnel arrive, and will travel with the camper during transport and remain with the camper at the hospital until he/she is released to the parent(s)/guardian(s). The camp director or recreation coordinator would arrange transportation for the camp staff that has accompanied the camper at the hospital. 7. The staff member must complete the accident report form and will provide the form to parent(s)/guardian(s) upon being released from the hospital and will be required to sign the form. The report is then submitted to the Recreation & Parks Department and kept on file. All inquiries from the media will be referred to the Recreation & Parks Department Director. In the event a camper does not require emergency transportation: 1. The first on scene will treat, or observe the injury/illness of the camper. 2. The first on scene will consult with the Director, or peer staff member to determine if the camper needs to go home. 3. The Director or designated staff member will contact the parent(s)/guardian(s) (if needed) to inform them of the injury/illness and the camp staff member will remain with the camper until he/she is released to the parent(s)/guardian(s). 4. The first on scene/camp staff member must complete the accident report form and place ON THE ACCIDENT CLIP BOARD before check out begins. 5. Parent(s)/guardian(s) will be notified at pick up of any treatment(s) or injury(s) a camper has sustained during the camp day and will be required to sign the form. Once complete the report is then submitted to the Recreation & Parks Department and kept on file. 8
BUS SAFETY RULES: 1. Loading and Unloading of Buses: One counselor will be the first person to board the bus and sit in the back, one counselor will sit towards the center of the bus, and one counselor will be the last towards the front of the bus. This procedure will be followed for disembarking also. At no time will campers be on a bus without at least one staff member on board. Campers will board promptly find a seat and stow their gear under the seat in front of them or on their laps. Seat belts must be worn at all times by both campers and staff. 2. All passengers will be expected to wear their seatbelts at all times. 3. All passengers will remain seated while the bus/van is in motion. 4. Aisles and exits must be clear of backpacks and other supplies at all times. 5. All passengers are prohibited from extending their head, arms, or objects out the bus windows. 6. All passengers will enter and exit from the appropriate door, unless an evacuation of the bus is needed and evacuation drills would be followed. BUS EVACUATION DRILLS: The Camp Director and head counselor will conduct a bus evacuation drill for each camp at least once every two weeks. When conducting evacuation drills on site campers will make their way to the aisle and evacuate at the rear of the bus. As campers approach to exit the rear of the bus campers must sit down and with the assistance of a counselor jump down and are escorted 20 feet away from the bus, away from traffic, and remain in a safe area with staff. The safe area will be designated by the first counselor that has exited the bus, and marked that area with cones. PERSONAL CAMP STAFF VEHICLES: Camp staff vehicles are used for personal transportation of camp staff to work, and from work and will remain on site during the camp hours. At no time during the camp day will campers be transported in any vehicle not intended for passenger use (ie. back of pickup trucks, or wagons where seats are not attached), or in personal vehicles owned by the staff. Campers will be transported only in buses, or vans contracted by the camp, in the Lebanon Recreation Department van or by emergency use vehicles (ambulance). SUNSCREEN/HYDRATION POLICY: Parent(s)/guardian(s) are asked to apply sunscreen to campers before the day begins. Camp staff will remind campers to re-apply sunscreen at mid-morning and mid-afternoon and after swimming. Camp staff will apply sunscreen for campers ages 5-8, and supervise and assist if needed in application as necessary for campers ages 9-12. Camp staff will have campers check their water bottles and refill if necessary at the same time sunscreen is applied. All campers will have their water bottle with them throughout the entire camp day. Campers will refill in the morning, during lunch, and snack breaks, but will always have a water break before, during, and after an activity. On more humid, and hot days water breaks will happen multiple times during an activity, and shade/indoor use will be used to best keep campers cool and hydrated. HEALTH-CARE POLICIES/PROCEDURES ON & OFF SITE: Each camper(s) must have a medical form with a signed waiver on file before the first day of camp or he/she will not be allowed to attend. Camp staff is trained in CPR and First Aid, but do not administer injections. In emergency situations, staff will assist camper(s) when needed. Camp staff will review with campers expectations of field trip at morning meeting, if there is an emergency on who to contact. Camp staff is provided with a list of all campers with known allergies/health concerns/medications. Camp staff will administer standard first aid and Adult/Child CPR only to the extent of their knowledge and training provided by American Red Cross/American Heart Association. Camp Kaleidoscope Staff will provide this service when needed during any and all activities throughout the camp day. If a medical emergency arises that requires treatment beyond their knowledge, professional treatment should be sought. The Emergency Action Plan should be put into action if 911 is needed. An on-call nurse is available during the camp day hours and days of operation.

When off-site on a field trip, Camp Kaleidoscope Staff will have the contact information for the closest hospital and will still have access to the on call nurse name. An accident report will be created of the emergency and shared with Parent(s) or guardian(s). Parent(s) or guardian(s) will be notified immediately of any illness or injury that requires medical transport. A “communication log” will be kept in the check-out binder to keep a record of communication with parent(s), or guardian(s). All media inquiries will be directed to the Recreation and Parks Director. **A note about medications: If your child receives medication throughout the school year, Camp Kaleidoscope highly recommends that he/she remain on that medication throughout his/her experience at camp to avoid difficulty in adjusting to new faces, places, and activities. Please feel free to contact the Lebanon Recreation & Parks Department with questions pertaining to this issue. We do understand that it is great for children to get a break, but please inform the staff that you are choosing to do so, so the staff can plan and train staff accordingly. MEDICATIONS POLICY/MANAGEMENT: It is policy that all medications provided for a camper(s) to be taken during the camp day must have written instructions from a physician and, or permission from the camper(s) parent(s)/guardian(s) to be provided to the camper(s). If neither written permission from the parent(s)/guardian(s) and, or a doctor’s order has not been obtained then the camp director, or head counselor will follow up with the parent(s)/guardian(s) that the medication will not be provided for the camper(s) to take until proper permission has been given. At check-in parent(s)/guardian(s) are to give medication to check-in camp staff. The camp-issued “Permission to Provide Medication to a camper” form must be completed. At the check-in table the parent will complete a medication sticker with the camper(s) name, parent(s)/guardian(s) signature and what to be given. The sticker will then be attached to the medication provided. 1. The check-in counselor will ensure medication is given to the correct counselor that is assigned to that camper(s). 2. Camp staff is responsible for ensuring medications provided by parent(s)/guardian(s) are provided to the assigned camper(s) in order to be taken by the camper(s) in their group (excluding injections). 3. Camper(s) is responsible for taking medication as camp staff will remind camper to administer medication. 4. Camp staff will record the name of the camper(s), date, time, dosage, medication, and initial the form. This will be done anytime a camper(s) is taking medication during camp day hours. 5. Medications will be kept on the counselor’s person in the camp-issued fanny pack. At the end of the camp day the parent(s)/guardian(s) will take medication home. 6. No medications will be provided without written instructions from a physician and, or permission from the camper’s parent(s)/guardian(s). If both written permission from the parent(s)/guardian(s) and a doctor’s order has not been obtained then the camp director, or head counselor will call/notify the parent(s)/guardian(s) that the medication will not be provided for the camper(s) to take until proper permission has been given.

LEBANON RECREATION & PARKS DEPARTMENT CAMP KALEIDOSCOPE PERMISSION TO PROVIDE MEDICATION TO CAMPER I give my permission for the Camp Kaleidoscope staff to provide the medication that I ________________________ (Parent(s)/Guardian(s) Name) have provided to my child as per the physician’s instructions on the medication, or by my request if needed. Any non-prescription medication will be given according to the instructions on the bottle and will be provided by the parent(s)/guardian(s). CERTIFICATIONS: The camp will only use aquatic facilities and natural bodies of water which have on duty staff that are qualified to administer CPR and one of the following certifications: ARC Lifeguarding, YMCA Lifeguarding, Lifeguard BSA, Royal Lifesaving Bronze Medallion or equivalent certification. In addition camp will only use aquatic facilities and natural bodies of water which have the site and equipment in good repair, and rescue equipment and staff that are readily available and or in good repair. All camp staff is required to have First Aid, CPR, and AED training. ACCIDENT REPORTS: City insurance policies require that an accident report must be completed for injuries requiring treatment of any kind. The attending camp staff member must complete the accident report form and put ON THE ACCIDENT CLIP BOARD before check out begins. Parent(s)/guardian(s) will be notified upon check-out of any treatment(s) or injury(s) a camper has sustained during the camp day and will be required to sign the form. The report is then submitted to the Recreation & Parks Department and kept on file. Accident reports are assessed weekly at camp staff meetings, summarized at the end of each season, and reviewed annually to evaluate and improve camp safety procedures and practices. A running spreadsheet is updated weekly of all accidents/illnesses in regards to the camper, date, time, location, injury, treatment, camp staff that treated, and camp age group. Camp staff should complete accident reports as thoroughly and accurately as possible. At the end of each season accident reports are reviewed by the Recreation Coordinator, Camp Director, and Art Director and taken into consideration for programming and training for the upcoming camp season. COMMUNICATION: Please communicate any concerns you may have regarding your camper(s) camp experience with the Camp Director or camp staff. We welcome positive and constructive feedback and are always looking for ways to improve Camp Kaleidoscope.
