THE PATIENT IS A 21 YEAR OLD FEMALE PRESENTING TO THE OFFICE TO FOLLOW UP ON A RECENT CONCUSION THAT TOOK PLACE IN EARLY DECEMBER 2019. SHE HAD FALLEN HITTING THE BACK OF HER HEAD ON THE GROUND. AT THAT TIME SHE SAW TORI HOGAN NP AND WAS ADVISED NOT TO GO TO WORK AND HAVE COMPLETE BRAIN REST THAT INCLUDED TV, PHONE, AND READING. SHE HAS BEEN ABLE TO DO LIMITED THINGS BUT HAS NOT RESUMED TV OR READING. SHE WORKS THREE PART TIME JOBS AND FEELS THAT HER SYMPTOMS ARE STILL TOO FREQUENT TO GO BACK. SHE IS PHONOSENSITIVE, PHOTOSENSITIVE, AND MOTION SENSITIVE. SHE REPORTS NOT BEING ABLE TO DRIVE MORE THAN TEN MINUTES WITHOUT A MIGRAINE STARTING. SHE CANNOT WTOLERATE BEING IN A ROOM WITH ANY LIGHT OR ANY NOISE. SHE REPORTS THAT HER OVERALL INTENSITE OF THE HEADACHE HAS REDUCED FROM A 10/10 TO A 6/10 DAILY. ANXIETY AND DEPRESSION - SHE HAD STARTED PROZAC SEVERAL WEEKS AGO HOWEVER AFTER ONE WEEK SHE LOST HER BOTTLE AND STOPPED TAKING IT. SHE SCORED SIXTEEN ON THE DEPRESSION SCALE TODAY.

EXAMS: 001162427 MR HEAD-BRAIN WO CON TECHNIQUE: Multiplanar multisequence bran MRI performed without contrast. INDICATION: Concussion COMPARISON: None. FINDINGS: There is a subtle 8 mm T2 hyperintense lesion in the left aspect of the pons, demonstrating mild blooming artifact on the gradient echo sequence. No additional lesions are identified. There are no sites resricted diffusion to suggest an acute infarction. No extra-axial collection. The grey-white matter differentiation is normal. The cortical sulci, basal cisterns, and ventricles are unremarkable. The corpus callosum is normal and the cerebellar tonsils normally positioned. Grossly preserved principal intracranial arterial T2 flow-voids. No sellar or suprasella mass is identified. IMPRESSION: 1. No acute intracranial process. 2. Nonspecific 8 mm T2 hyperintense lesion in the left aspect of the pons demonstrating mild blooming artifact which could indicaqte the presence of old blood products. It is uncetain if this reflects an old petechial hemorrhage (in the setting of prior trauma) or other lesion such as a lowgrade glioma. Further evaluation with postcontrast MRI is recommended.
