ABOUTME

After graduating with a Bachelor of Pharmacy (Honors) from International Medical University in 2010, I completed my Provisional Registered Pharmacist (PRP) training at Hospital Queen Elizabeth, Sabah, and have since dedicated my career to serving as a ward pharmacist in various hospitals across Sabah and Kedah. In 2016, I was honored to receive the Excellence in Service Award from the Kedah State Health Office In 2019, I earned the Credentialing for Registered Pharmacists in Ward Pharmacy (Medicine) from the Ministry of Health Malaysia My professional focus includes active participation in Antimicrobial Stewardship Programs, medication safety, and the Medication Therapy Adherence Clinic (chronic kidney disease) at the hospital level

WHYMPHARM?

I chose to enroll in the MPharm program at USM because of its esteemed reputation in clinical pharmacy education and its commitment to advancing the practice of Clinical Pharmacy The program's structured coursework provides specialized knowledge and skills essential for delivering highquality pharmaceutical care Additionally, USM's commitment to fostering innovation in healthcare and emphasis on clinical practice and the integration of evidence-based medicine This program focuses on identifying and managing drug-related problems, aligns with the core principles of Clinical Pharmacy a discipline where pharmacists provide patient care that optimizes medication therapy, promotes health, and supports disease prevention Moreover, the field of pharmacy is constantly evolving with new medications, therapies, and guidelines

ABSTRACT

THEEVALUATIONOFTHECHOICEOFEMPIRICANTIBIOTICTHERAPY, FACTORSANDCLINICALOUTCOMESOFPERITONEALDIALYSISASSOCIATEDPERITONITIS:ARETROSPECTIVESTUDYINHOSPITAL SULTANABDULHALIMMALAYSIA

Introduction: Peritonitis is one of the main complications among peritoneal dialysis (PD) patients Treatment of PD peritonitis is a challenging area due to the diversity of factors involved in its development There is growing evidence that the microbiological spectrum of PD peritonitis differs according to the geographic location with the patterns of antibiotic resistance changing over time However, only a handful of studies assess the factors that influence the clinical outcome of peritonitis in Malaysia There are scarce studies on selecting appropriate empiric antibiotics for treating PD-associated peritonitis The local microbiological profile for each PD center must be monitored to ensure appropriate choice of empiric antibiotic regimen

Objective: This study aims to evaluate the choice of empiric antibiotic therapy, factors, and clinical outcomes of PD-associated peritonitis in Sultan Abdul Halim Hospital

Methods: This study is a single-center, retrospective observational study Retrospective data of research subjects were extracted from the Electronic Hospital Information System and the peritonitis record book of the Peritoneal Dialysis Unit at Sultan Abdul Halim Hospital Descriptive data was reported for the patient’s demographic characteristics, causative microorganisms and empiric antibiotic sensitivity, peritonitis episode characteristics, and clinical outcomes Simple and multiple logistic regression analyses using Generalized Estimating Equations were performed to determine factors that predict the complicated episode of PD-peritonitis, and the results were expressed as an odds ratio (OR) with a 95% confidence interval (CI)

Results: Overall, the study included 130 patients with a median age of 56 years and 270 episodes of PD-associated peritonitis This study observed that 80% of the patients had complicated PD-peritonitis outcomes. Of the 270 peritonitis cases, 41 11% (n=111) were attributed to Gram-positive microorganisms, while 40 74% (n=110) were attributed to Gram-negative microorganisms Methicillin-sensitive Staphylococcus aureus and Pseudomonas aeruginosa were the most often isolated Gram-positive and Gram-negative microorganisms, respectively The peritoneal dialysis duration (months) (Adjusted odd ratio, AOR 0 87, 95% confidence interval, 95% CI: 0 79, 0 95), number of PD-associated peritonitis > 2 episodes (AOR 137 43, 95% CI: 19 75, 956 24), catheter removal (AOR 45 61, 95% CI: 2 16, 962 61), and duration of first hospital admission (days) for PD-associated peritonitis (AOR 1 37, 95% CI: 1 12, 1 69), were factors significantly associated with complicated PD-peritonitis

Conclusion:This study revealed that the PD duration (months), > 2 episodes of PD-associated peritonitis, catheter removal, and each additional day of hospital length of stay per first admission for PD-associated peritonitis were found as significant factors in predicting complicated outcomes of PDassociated peritonitis

I chose to enroll the MPharm program at Universiti Sains Malaysia (USM) due to its strong focus on clinical pharmacy education and research, which aligns with my goal of improving patient care and clinical decisionmaking USM’s reputation as a leading healthcare institution, coupled with its comprehensive curriculum and experienced faculty, offers the ideal environment to deepen my expertise in areas like pharmacotherapy, patient safety, and evidence-based practice This program will equip me with the skills needed to address healthcare challenges, particularly in rural and underserved areas where resources are limited. Additionally, the emphasis on research and innovation will enable me to contribute to developing new strategies for optimizing patient outcomes, aligning with my long-term vision of making a meaningful impact in clinical pharmacy

FACTORSINFLUENCINGSPUTUMSMEARCONVERSIONIN SMEAR-POSITIVEPULMONARYTUBERCULOSIS:ANANALYSIS OFSYMPTOMPRESENTATIONATADMISSION, SOCIODEMOGRAPHICCHARACTERISTICS,COMORBIDITIESAND BASELINELABORATORYFINDING

Introduction: Tuberculosis (TB) remains a significant public health issue in Malaysia, with 23,644 cases reported in 2020, 91 7% of which were new, indicating ongoing transmission Among these, 58% were smear-positive pulmonary tuberculosis (PTB), which poses a high risk for disease spread Delayed sputum conversion in PTB patients increases the risk of treatment failure and relapse, with contributing factors varying across regions Objective: This study aimed to identify factors associated with sputum conversion among smear-positive PTB patients, determine the proportion of patients achieving sputum conversion within two months, and assess the relationship between symptom presentations, sociodemographic factors, comorbidities, and baseline laboratory findings with sputum conversion.

Methods: A retrospective observational study was conducted using secondary data from 224 patients treated between January 2013 and December 2021 Various statistical tests (chi-square, Fisher’s exact test, ttests, and Mann-Whitney tests) and logistic regression analyses were used to identify significant predictors of sputum conversion

Results: Results showed that 44 6% of patients achieved sputum conversion within two months, a proportion lower than other studies in Malaysia and abroad, possibly due to differences in patient demographics, treatment adherence, or drug resistance Seven risk factors were significantly associated with sputum conversion included low of appetite (AOR: 2 11, 95% Confidence Interval [CI]: 1 058–4 195, p=0 034), presence of Hepatitis C (AOR: 0.11, 95% CI: 0.023–0.570, p=0.008), higher baseline globulin levels (AOR: 1 06, 95% CI:, 1 012–1 109, p=0 014), higher baseline ALP levels (AOR: 1 01, 95% CI: 1 003–1 016, p=0 005), lower baseline eosinophil percentages (AOR: 0 75, 95% CI: 0 609–0 931, p=0 009), lower neutrophil-to-lymphocyte-ratio (AOR: 0 89, 95% CI: 0 796–0 988, p=0 030) and higher monocyte-to-neutrophil ratio (AOR: 5 24, 95% CI: 2 064–13 290, p<0 001)

Conclusion: These findings suggest that nutritional, immunological, and inflammatory factors play a critical role in sputum conversion Addressing these factors could improve TB treatment outcomes and reduce the TB burden in Malaysia, warranting further research to refine TB management strategies

Sains Malaysia (USM) in 2023 During my undergraduate studies, I actively participated in various societies, including the USM Green Lung society, which fostered my passion for community health and environmental sustainability I gained practical experience through a one-week attachment at Sunway Multicare Pharmacy, where I developed essential skills in pharmaceutical care and patient interaction. Currently, I am pursuing a Master of Clinical Pharmacy at USM to further enhance my clinical knowledge and expertise in providing optimal patient care

I chose to enroll in the MPharm program at USM for several reasons Firstly, I am eager to deepen my knowledge and skills in clinical pharmacy practice and research, which this program is renowned for I believe that gaining a comprehensive education in this field will fully equip me with the expertise and confidence needed before I step into the professional world. My goal is to make meaningful contributions to clinical pharmacy services, improving patient care and outcomes Additionally, I am keen on enhancing my research capabilities, as I understand the importance of evidence-based practice in advancing the field of pharmacy In the future, I hope to further my studies and ultimately become a pharmacy lecturer. ACross-SectionalStudyonThePrevalenceofMenstrualProblems andTheirAssociationwithPsychologicalStressinUndergraduate FemaleStudentsStudyingatUniversitiSainsMalaysia.

Introduction: This study aims to determine the prevalence of menstrual problems and their association with psychological stress among undergraduate female students studying at Universiti Sains Malaysia (USM)

The study employed an anonymous, self-completed online questionnaire distributed through USM’s official social media platforms, including Facebook and Telegram, with the assistance of student representatives across all three campuses A total of 377 female students participated, meeting the sample size determined using the Raosoft® Sample Size

Calculator Objective: The research objectives included assessing participants’ knowledge of menstruation, identifying the prevalence of menstrual problems, and exploring the association between menstrual problems and psychological stress.

Methodology: The study employed an anonymous, self-completed online questionnaire distributed through USM’s official social media platforms, including Facebook and Telegram, with the assistance of student representatives across all three campuses A total of 377 female students participated, meeting the sample size determined using the Raosoft® Sample Size Calculator

Results: Results indicate that most participants had a comprehensive understanding of menstruation, with mothers being the primary source of information The study found a high prevalence of menstrual problems, with premenstrual symptoms being the most common issue A significant positive correlation was observed between menstrual problems and high levels of perceived stress

Conclusion: These findings highlight the need for targeted support services and interventions at USM to promote menstrual health and stress management

I recently graduated with a Bachelor of Pharmaceutical Sciences (HONS ) from the Universiti Sains Malaysia (USM) in 2023 During my final year in bachelor, I was elected as the President (YDP) of the pharmacy local society (Persatuan Sains Farmasi) and actively participated in many events and activities and this society was awarded as the Top 7 society in that tenure 2022/2023 Upon graduation, I was attached to Hospital Kulim and Siang Pharmacy for 3 weeks in Aug 2023 for more working experience Soon after, I joined the MPharm programme at USM in Oct 2023

WHYMPHARM?

One of my goals is that I would like to acquire as much knowledge as possible as a clinical pharmacist as I will be able to work more efficiently in the ward. Additionally, I would also like to see myself as a future lecturer/educator in a university With this knowledge, I will be able to educate students as well as gain more experience in the lecturing field

Introduction: The improper disposal of medications poses significant risks to human health and the environment, including water and soil contamination, antibiotic resistance, and adverse effects on wildlife This study investigates medication disposal practices, knowledge, and affecting factors in Penang,

Malaysia

Objective: The study aims to assess medication disposal practices, evaluate their knowledge and awareness, determine factors affecting disposal practices, and analyze the relationships between knowledge, practices, and affecting factors

Method: A cross-sectional survey was conducted with 273 participants in Penang Data were collected using validated questionnaire and was spread throughout online platforms Chi-squared test, was utilized to identify associations between demographic factors, knowledge, and practices

Result: In this study, results indicated that 59 0% of participants (n=161) demonstrated high knowledge scores (8-10 points), 35 2% (n=96) had moderate knowledge scores (4-7 points), and 5 9% (n=16) exhibited low knowledge scores (0-3 points) On the other hand, disposal practices, the most common methods were returning medications to a pharmacy or clinic (34 0%, n=145) and throwing them away in the trash/garbage (33 6%, n=143) The study also highlighted key factors affecting disposal practices, including age, educational level, and awareness of proper disposal methods Chisquare tests revealed significant associations between age and disposal practices (p < 0 05), highlighting the significance of medication disposal practices.

Conclusion: This study highlights a significant gap between knowledge and practices among Penang residents Despite a majority of participants having high knowledge levels, proper disposal practices were not consistently followed, especially among older adults Thus, the need for enhanced educational efforts and targeted interventions to bridge this gap and promote safer medication disposal practices across all demographic groups

Introduction: Hypertension, a significant public health concern worldwide, is no longer confined to the elderly but also affects youth populations

Understanding the knowledge, attitudes, and practices (KAP) regarding hypertension among youths is crucial for effective prevention and management strategies

Objective: This cross-sectional study aims to investigate KAP regarding hypertension among youths in Penang, Malaysia

Method: A sample of 384 youths aged 18 to 24 years old from Penang participated in this study A structured and validated questionnaire was used to assess their knowledge of hypertension risk factors, signs, and complications, as well as their attitudes towards adopting healthier lifestyles and seeking medical attention Practices related to hypertension prevention were also evaluated

WHYMPHARM?

I chose to continue my studies in a Master of Clinical Pharmacy because I am passionate about deepening my knowledge and skills in clinical practice My Bachelor’s degree provided a strong foundation in pharmacy, but I want to specialize further to make a more significant impact on patient care The Master’s program allows me to stay updated with the latest advancements in clinical pharmacy, improve my ability to make evidence-based decisions, and better collaborate with healthcare teams. Ultimately, this advanced training will enable me to contribute more effectively to patient outcomes and the healthcare system.

Results: Majority of respondents (63 8%) have good knowledge about hypertension, indicating a solid understanding of the condition A large percentage of respondents (70.1%) have a positive attitude towards hypertension, showing receptiveness to managing the condition effectively While a substantial portion of respondents (47 7%) demonstrate good practices related to hypertension, there is also a notable percentage (41 7%) with inadequate practices Majority of respondents (57 0%) have a good overall KAP score, indicating a combination of good knowledge, positive attitude, and adequate practices regarding hypertension

Conclusion: The findings emphasize the importance of targeted education and awareness campaigns aimed at improving knowledge, attitudes, and practices regarding hypertension among youths in Penang These campaigns can focus on dispelling myths, promoting healthy behaviors, and encouraging regular screenings Interventions like peer education, health coaching, and lifestyle modification programs can bridge the identified gaps. The significant associations between KAP regarding hypertension also underscore the need for policy initiatives and programs that promote holistic approaches to hypertension management, encompassing not just medical interventions but also educational and behavioral components

ABOUTME WHYMPHARM?

I graduated with a Bachelor in Pharmacy from the International Medical University (IMU) in 2015 and completed my Provisionally Registered Pharmacist training at Hospital Queen Elizabeth, Sabah In 2017, I took on the role of Head of Logistic Pharmacy at Hospital Papar, where our team was fortunate to receive the Best Performance Pharmacy Logistics award I was also honored to receive the Excellence in Service Award from the Sabah state health office in 2018 In 2019, I had the opportunity to establish the Clinical Pharmacy Service, which supported several important initiatives, including the Thalassemia and Retroviral Disease Adherence Programs and Antimicrobial Stewardship. In 2022, I contributed as a content writer and event moderator for the Malaysia Clinical Pharmacy Symposium. Most recently, I was grateful to receive the Khazanah Watan Postgraduate scholarship in 2023, allowing me to pursue a Master in Pharmacy (Clinical Pharmacy) at USM

Introduction: Thalassemia is an autosomal hereditary disease with high economic burden in Malaysia To date, iron chelating agents such as deferoxamine, deferiprone and deferasirox remained the mainstay of treatment for chronic iron overload due to life-long blood transfusion

Objective: This study aims to describe the incidence rate of severe adverse drug reaction, risk factors and prescribing pattern associated with the use of iron chelation drugs among pediatric thalassemia patients in a tertiary hospital in Malaysia

Methods: A retrospective observational study was conducted in a public tertiary care hospital in Sabah, Malaysa. The study participants (n=181) who meet the inclusion criteria were recruited from a Thalassemia Day Care Clinic listed from the year 2022 to 2023 Medical records were reviewed, and data collection was carried out using a standardized data collection form Incidence rate of adverse drug reactions was expressed in per 100 patient-year, while risk factors were examined using binary univariate regression followed by multivariate regression The prescribing pattern of iron chelation drugs for the year 2022 and 2023 were evaluated using prescribed daily dose (PDD)

My aim is to refine my critical thinking, literature appraisal, research skills, and holistic approach to pharmaceutical care. I aspire to contribute to the advancement of clinical pharmacy services in Malaysia, ultimately enhancing patient outcomes Having previously studied at a private university in Malaysia, I am eager to gain new perspectives by studying at a public university that is ranked among the top 100 globally in the field of Pharmacy and Pharmacology

Results: The incidence of severe adverse drug reaction was reported to be 5 1%, which incidence rate were, deferiprone induced agranulocytosis at 0 55 per 100 patient-year and deferoxamine-induced ototoxicity at 0 55 per 100 patient-year Combination therapy was associated with increased risk of deferasirox-induced elevated alanine transferase while higher dose of deferasirox with higher risk for proteinuria There were no other variables which identified as risk factors for adverse drug reactions associated with the use of deferoxamine and deferiprone. Besides, there was no statistically significant difference in prescribing daily dose for deferoxamine, deferiprone and deferasirox throughout the year 2022 and 2023

Conclusion: This study highlights that incidence of severe adverse drug reactions associated with iron chelator drugs in the thalassemia pediatric population is comparable to other studies reported in adult Malaysian thalassemia patients Also, prescribing pattern of iron chelation drugs have been consistent from year 2022 and 2023 A more routine monitoring of renal and liver profiles should be done for patients with higher dose of deferasirox and in combination with other iron chelators

I have confidence in the quality of education and professional development USM provides. The program’s coursework in Master of Clinical Pharmacy covers diverse areas such as critical care, oncology, and pediatrics, preparing students to handle a broad range of patient scenarios in their future careers

Introduction: Stress ulcers are common and serious complications among patients admitted to the ICU Stress ulcer prophylaxis (SUP) is an important approach to prevent these complications and is a standard practice in international ICU guidelines Proton pump inhibitors (PPIs) are effective in reducing the risk of clinically significant gastrointestinal bleeding. Although guidelines recommend against SUP in patients without risk factors, many practitioners still prescribe SUP for all ICU patients, and the timing for discontinuation of SUP varies widely among practitioners and centres, with some continuing beyond ICU care

Objectives: This study aimed to explore the appropriateness of PPI use as SUP management among ICU patients

Methods: A single-centre retrospective cohort study was conducted at Hospital Sultan Abdul Halim, involving 208 eligible patients who met the study’s criteria Data were collected using data collection form to evaluate the presence of guideline-listed risk factors at the initiation of SUP and upon ICU discharge if SUP was continued

Results: The study found that 6 7% of patients were initiated on PPIs without valid indications Additionally, 81 7% continued PPIs beyond their ICU stay, with 14 1% of these cases deemed inappropriate Factors associated with the inappropriate continuation of PPIs included intubation during ICU admission and prolonged ICU stay

Conclusion: The findings indicate a significant rate of inappropriate PPI use for SUP among ICU patients, highlighting the need for stricter adherence to guidelines and the development of targeted interventions to optimize SUP management in critical care settings

Objective: The purpose of this retrospective cross-sectional study was to explore allele frequency and associations between the presence of single nucleotide polymorphisms (SNPs) in the ABCB1 gene (rs1045642) and CYP7A1 gene (rs3808607) on lipid-lowering efficacy of atorvastatin among Malaysian male patients

Methods: The study used a total of subjects 64 from a previous genetic variation study of 136 patients who were on atorvastatin (n=97) and simvastatin (n=39) therapy The patients using atorvastatin in this study were divided into two groups i e controlled (LDL level <3 mmol/L) and uncontrolled groups (LDL level ≥ 3 mmol/L) with an equal number of patients i.e. 32 patients in each group. All the male patients aged between 18 to 59 years were genotyped to know the allele frequency of both SNPs i e gene ABCB1 (rs1045642) and CYP7A1 (rs3808607) and its effect of polymorphism on lipid profile after the atorvastatin therapy

Results: A total of 64 patients were genotyped to study genetic determinants of atorvastatin response The genotype distribution for all polymorphisms investigated was in Hardy–Weinberg equilibrium The genotype frequency of gene ABCB1 (rs1045642) was higher in the variant allele GG 37 5% as compared with the reference allele AA 10 9% The genotype frequency of the gene CYP7A1 (rs3808607) was also higher in the variant allele TT 36% as compared with the reference-type allele GG 15 6%

Patients with the variant carrying allele of ABCB1 and reference allele of CYP7A1 gene will be more likely in the controlled group in response to atorvastatin. A significant association was observed between the genotype ABCB1 (rs1045642) and HDL level (p=0 045)

Conclusion: We concluded the variable response to atorvastatin therapy in terms of lowering of LDL- and TG and increase in HDL levels due to genetic variations in ABCB1and CYP7A1 is a promising finding The variant allele of gene ABCB1 and the reference allele of CYP7A1 showed a more favorable lipid profile The allele frequency of variant allele of both studied SNPs was higher as compare to the reference allele

Introduction: Over the years, the pharmacy profession has evolved from a traditional product-oriented approach to an advanced patient-centred model Consequently, advanced training and specialisation in clinical pharmacy practice have become integral components of the profession

Objective: This study aimed to analyse the employment patterns among M ClinPharm graduates from USM, spanning from 2013 to 2023 The primary objectives were to evaluate the overall employment rate, determine the primary sectors or areas of practice, assess salary trends and examine the factors affecting postgraduate placement decisions among M.ClinPharm graduates.

Methods: A cross-sectional study was conducted using an anonymous, selfadministered online survey questionnaire The research instrument was developed based on literature and validated through face and content validation as well as test-retest reliability The data collected were used to generate a detailed overview of employment trends among M.ClinPharm graduates from USM who graduated between 2013 and 2023

I chose to pursue the Master of Pharmacy (Clinical Pharmacy) programme at Universiti Sains Malaysia (USM) in Penang to advance my expertise and deepen my knowledge in clinical pharmacy practice My decision was driven by a desire to become an academician and marked a crucial first step in my transition into academia I embraced this new challenge to broaden my professional horizons and push myself beyond my comfort zone USM stood out to me because of its excellent reputation and long history in clinical pharmacy education. As the pioneer in formal pharmacy education in Malaysia, USM was an ideal choice for my academic and professional development

Results: The overall response rate was 56 8%, with 75 out of 132 graduates participating Most respondents secured employment within a month of graduation, resulting an overall employment rate of 93 3% The majority were employed in the government sector, primarily in clinical or ward pharmacy roles Salary trends revealed that most graduates fell within the middle-income group However, a significant moderate negative correlation was observed between the graduation year and income, indicating that recent graduates generally reported lower incomes compared to earlier cohorts

Conclusion: This study provided valuable insights into the employment outcomes of M ClinPharm graduates from USM The study demonstrated that the programme effectively enhanced employability of graduates The findings can guide curriculum adjustments, career guidance for graduates and policy decisions within the pharmacy field in Malaysia Future research is recommended to include comparative studies with other institutions to provide more comprehensive understanding of employment trends and to further enhance the overall employability of M ClinPharm graduates

Introduction: Pharmacogenomics (PGx) is an emerging field that tailors drug therapies based on individual genetic profiles, offering significant potential for personalized medicine

Objective: This descriptive cross-sectional study aimed to assess the knowledge, attitudes, and perceptions of senior pharmacy students in the United Arab Emirates (UAE) regarding PGx

Methods: A total of 380 final-year pharmacy students from various UAE universities participated in the study Data were collected via a structured online survey that covered socio-demographic information, knowledge, attitudes, and perceptions toward PGx Data analysis was performed using SPSS version 28 0 Mann-Whitney U and Kruskal-Wallis tests were used to determine differences in total scores of KAP while Chi-square test assessed association between factors contributing to knowledge and attitude

Results: The findings revealed that 56 8% of respondents had poor knowledge of PGx, while only 21 3% demonstrated a good understanding Knowledge levels varied significantly based on gender, university, and educational program The mean PGx score was highest for Gulf Medical University (GMU) 5 86±2 116, followed by Ajman University with a mean score of 4 23 + 2 894 [χ2(7)] = 60 676, p = 0 00015 Male students and those in Doctor of Pharmacy (Pharm D) programs scored higher in pharmacogenomics (PGx) knowledge compared to females in Bachelor of Pharmacy (B Pharm) programs The mean PGx score for males was 4 43 ± 2 77, while for females it was 0 358 ± 2 845 Pharm D students had a mean score of 5 79 ± 2 115, compared to 3 56 ± 2 836 for B Pharm students This difference was statistically significant: t(378) = -4 463, p = 0 0001 Despite the knowledge gaps, students displayed overwhelmingly positive attitudes, with 84 5% of total participants in this study expressing interest in additional training or workshops on PGx.

Conclusion: Investments in research, infrastructure, and interdisciplinary collaboration are necessary to facilitate the implementation of PGx in the UAE healthcare system By addressing these educational gaps and promoting PGx, UAE pharmacy students and professionals can be better prepared to embrace personalized medicine and improve patient care outcomes

Background: Ceftriaxone, a third-generation cephalosporin, is widely used for treating infection during childhood Due to its broad spectrum against bacteria, long half-life and single daily dosing, physicians prefer to use it more frequently Ceftriaxone is mostly eliminated through the kidney and the remainder is eliminated via the biliary system It binds with calcium ions producing reversible precipitations that form biliary sludge and/or lithiasis, called cholelithiasis in children Identification of the predisposing factor is essential which can lead to ceftriaxone induced cholelithiasis and affect the quality of life of pediatric patient

Objective: This study aimed to evaluate the association of cholelithiasis among pediatric patients treated with ceftriaxone through a meta-analysis Additionally, it sought to identify the predisposing factors affecting ceftriaxone induced cholelithiasis and assess its impact on the quality of life (QoL) of these patients through a systematic review.

Methods: We conducted a thorough search of multiple databases, including Web of Science, PubMed, Google Scholar, and Scopus, up to March 2024 Our search was focused on finding studies that examined the link between ceftriaxone and cholelithiasis in children aged 0-18 years

Results: We selected 11 studies that met our inclusion and exclusion criteria, which consisted of 1 randomized controlled trial (RCT) and 10 cohort studies (9 prospective and 1 retrospective) Our meta-analysis revealed a pooled frequency of approximately 15% for cholelithiasis among pediatric patients on ceftriaxone, with a 95% confidence interval (CI) ranging from 9% to 23% This association is further supported by the statistical significance of the overall effect (p = 0.00, z = 4 73), indicating that the observed increase in cholelithiasis cases is unlikely to be due to random chance The statistical significance of this association was confirmed by sensitivity testing, which consistently showed significant p-values (p < 0 05) across various scenarios Predisposing factors identified included high doses of ceftriaxone (>2g per day), reduced oral intake, short bolus injections, long-term use of the drug (>5 days), and patient-specific factors such as prior surgery, older age, or a family history of cholelithiasis

Conclusions: There is notable association between ceftriaxone and cholelithiasis regarding its use in pediatric patients especially in those with predisposing factors. Clinicians should carefully weigh these risks and monitor for potential complications to minimize the impact on the patient's quality of life

Introduction: The number of resistant organisms is correlated with the overall hospital's use of antibiotics However, the data of the relationship between individual specialties within hospital is scarce

Objective: The aim of this study was to determine the trend of antibiotic usages, resistance and correlations according to the individual specialties in a tertiary hospital in Penang, Malaysia

Methods: This retrospective study included data from hospital wide inpatients collected between Jan 2018 to Dec 2023 Data on antibiotic consumption were expressed as Define Daily Dose (DDD)/1000 patient days The resistance rates of each of antibiotic-resistant organism (CRAB, ESBL-E, CRE, CRPAE, MRSA, VRE) was reported as the percentage of resistant isolates among all tested isolates. All data was segregated in ICU, Surgical and Medical areas The Jonckheere-Terpstra test was utilized to analyze trends in antibiotic usage and resistance while Spearman’s correlation was used to assessed correlations

Results: All three areas have increased antibiotic consumption from 2018-2023: ICU area 36.70% (z=4.453, p<0.001), Surgical Areas 42.48% (z=8 488, p<0 001), Medical Areas 18 43% (z=3 753, p<0 001) All these areas also have increased usage trend of BLBI, meropenem, colistin and levofloxacin The antibiotic most consumed in ICU, Surgical and Medical areas are meropenem, cefuroxime and ceftriaxone respectively CRE rates were increased in all three areas However, for ESBL-E and CRPAE, their rate of occurrence has decreased in ICU and medical areas The resistance rate of CRAB was the highest for all areas There was strong positive correlation between ceftazidime and rates of CRE in ICU area (R=0 725, p<0 001), moderately positive correlation between cefuroxime and MRSA in Surgical area (R=0 4, p<0 001), and between BLBI and CRAB in medical areas (R=0 331, p=0 002) All three areas have moderately positive correlation between CRE and BLBI, 3rd/4th generation cephalosporin, carbapenem and polymyxin There was moderately negative correlation between ESBL-E with BLBI and meropenem in both ICU and medical areas

Conclusion: Study findings reveal the difference of antibiotic consumption, resistance and correlation between these three areas Different targeted AMS interventions can be implemented based on specific wards, antibiotics or resistance

Introduction: Peritoneal dialysis (PD) is one of the renal replacement therapies available for patients with end-stage renal disease PD associated peritonitis is among the critical causes of PD failure and mortality among PD patients In Malaysia, little is known about the prevalence and outcome of patients with multiple episodes of peritonitis

Objective: This study aimed to identify the prevalence, patient characteristics, microbiological causes, treatment patterns and clinical outcomes of PD associated peritonitis focusing on relapse and repeat episodes

Methods: A retrospective cohort study conducted in tertiary hospital, Malaysia Medical records of 168 PD patients who developed peritonitis from 1 January 2021 to 31 December 2023 were reviewed through the electronic Hospital Information System and screened for inclusion and exclusion criteria

Results: A total of 46 patients were included in this study, comprising 21 patients with repeat peritonitis and 25 patients with relapse peritonitis We reported a relapse and repeat peritonitis rate of 15% and 13%, respectively Gram-positive organisms were notably higher in the repeat group as compared to the relapse group (71% vs 24%; χ2 (1) = 10 35, p=0 001) while the difference in the prevalence of gram-negative organisms did not achieve statistical significance Nevertheless, the higher use of cephalosporin-based antibiotics (p=0 015) and combination antibiotics (χ2 (1) = 5 74, p=0 017) were reflective of the greater proportion of gramnegative infection in the relapse group The overall clinical outcomes were comparable between groups. Patient with a history of peritonitis were more prevalent in those who did not develop subsequent peritonitis episode (5 9% vs 37 9%, χ2 (1) = 5 709, p=0 017) and in those who succumbed (57 1% vs 20 5%, χ2 (1) = 4 130, p=0 042) A higher rate of mortality was also observed in patients with gram-negative infections (85 7% vs 33 3%, p=0 015)

Conclusion: The relapse and repeat peritonitis episodes are 2 independent clinical conditions with respective microbiological patterns and clinical outcomes

Introduction : Mental health literacy (MHL) is crucial for the early recognition and effective management of mental health issues, particularly among university students who are at a vulnerable stage of life Despite the importance of MHL, there is limited knowledge among university students, contributing to delayed help-seeking behaviors and increased stigma

Objective: This study aimed to evaluate a newly developed mental health education module for improving health literacy among university students by highlighting the importance of enhancing MHL to address the growing mental health challenges faced by young people, emphasizing the need for targeted interventions to improve awareness and understanding

Methods: A quasi-experimental pre and post-test study was conducted without a control group to evaluate a newly developed mental health education module on improving MHL among university students The study involved 46 undergraduate students from Universiti Sains Malaysia, who completed pre-test and post-test 1 (immediate), and 23 undergraduate students from Universiti Sains Malaysia, who completed post-test 2 (two weeks later) assessments The intervention was delivered via a workshop conducted on the Webex platform, and the assessments were administered through a Google Form.

Results: The study found significant improvements in students' mental health literacy and their ability to manage mental health challenges The mean total score for MHL increased from 9 9 (pre-test) to 11 7 (post-test 1) and 11 9 (post-test 2) Similarly, the understanding of managing mental health challenges improved, with the mean score increasing from 8 6 (pre-test) to 9 9 (post-test 1 and 2) The intervention led to a statistically significant enhancement in the participants' knowledge, with p-values of <0 001 between pre-test and post-test 1 for both MHL and managing mental health challenges

I graduated with a Bachelor of Pharmacy (Hons) from Universiti Sains Malaysia (USM) in 2023 During my studies, I gained experience in a community setting through a two-week attachment at Juzcare Pharmacy in Kajang I was honored to receive the Student Excellence Award during the White Coat Ceremony for my active participation in various events and societies throughout my undergraduate years Notably, I served as the President of Sekretariat Sukarelawan India, USM for the academic year 2021/2022 and as an emcee for the Health Action International Asia Pacific (HAIAP) Forum 2023 and the International Conference of Pharmacy and Health Sciences (ICPHS) 2022 I am currently pursuing a Master of Clinical Pharmacy at USM

WHYMPHARM?

I chose to pursue Master of Clinical Pharmacy because I am passionate about advancing my knowledge and skills in clinical practice Given the constantly evolving nature of the pharmacy field, it is crucial to stay updated with the latest guidelines and clinical trials, which are emphasized throughout various courses of this programme Additionally, I believe the hands-on clerkship experience in various wards such as intensive care unit (ICU), pediatrics, nephrology, cardiology, infectious disease, and oncology, will be invaluable during my Provisionally Registered Pharmacist (PRP) training As a proud USM alumnus, I decided to continue my postgraduate studies at USM due to the university's strong reputation and global recognition for excellence in pharmacy education

ABSTRACT

AssessmentofKnowledge,AttitudeandPerceptionofMaleInfertility amongPharmacyUndergraduatesinUniversitiSainsMalaysia(USM), Penang

Introduction: Male infertility is an emerging global health concern that remains underreported in many countries including Malaysia due to cultural biases and patriarchal preferences Limited understanding of male infertility especially among healthcare providers hinders proper recognition and management of this issue

Objective: The current study evaluates the knowledge, attitude and perception of male infertility among pharmacy undergraduates at Universiti Sains Malaysia

Methods: A quantitative cross-sectional online survey consisting of 31 items was conducted among USM pharmacy undergraduates enrolled in the 2023/2024 academic year within study period of December 2023 to January 2024 Data analysis was performed using SPSS version 28 0 MannWhitney U and Kruskal-Wallis tests were used to determine differences in total scores of KAP while Chi-square test assessed association between factors contributing to knowledge and attitude

Results: The study included 316 completed questionnaires (response rate 79%). Overall, 55.7% students demonstrated low knowledge on male infertility although 97 2% of students displayed positive attitude towards male infertility Male students demonstrated significantly higher knowledge than females (χ² = 6 734, p = 0 009) Additionally, students with higher knowledge were more likely to have positive attitude (χ² = 4 136, p = 0 042)

Conclusion: The study indicates substantial gaps in knowledge about male infertility among students despite overall positive attitudes These findings underscore the need for targeted educational interventions and more extensive research to ensure that future pharmacists can provide comprehensive care to patients facing male infertility.

Background:Managing diabetes in the elderly population was challenging Objective: The study aimed to identify the prevalence of glycaemic control and the factors linked to poor glycaemic control in elderly diabetic patients at Hospital Sultanah Bahiyah

Method: A retrospective cross-sectional study was performed using data between January 2022 and December 2022 in Hospital Sultanah Bahiyah with a total of 330 patients recruited into the analysis

Results: The mean age of participants in the cross-sectional study was 69 25 ± 6 4 years, with 50 6% being men and having a HbA1c of 7 30 (IQR: 1 83 –9 60) % Multivariate analysis showed that several factors, such as race (p value = 0 014), were associated with poor glycaemic control However, when a sub-analysis was conducted between Malay and Chinese participants, the difference was not significant (Malay, AOR = 2.239, 95% CI 0 864 – 5 806; Chinese, AOR = 0 858, 95% CI 0 305 – 2 415) Other factors independently linked to poor glycaemic control included high fasting blood sugar (> 8 3 mmol/L) (AOR = 6 087, 95% CI 3 302 – 11 223), elevated lowdensity lipoprotein (> 2 6 mmol/L) (AOR = 1 952, 95% CI 1 067 – 3 570), increased triglyceride levels (> 1.7 mmol/L) (AOR = 2.150, 95% CI = 1.189 –3 888), receiving a combination of oral hypoglycaemic agent and insulin therapy (AOR = 4 706, 95% CI 2 457 – 9 013), and receiving only one type of oral hypoglycemic agent (AOR = 0 341, 95% CI 0 141 – 0 826)

Conclusion: The management of diabetes in elderly patients might involve the use of appropriate antidiabetic agents and statins In facilities where HbA1c measurement was not available, FBS could be used as an alternative for monitoring diabetes control
