Non-Emergency Medical Transportation People who have Medicaid can access local non-emergency ground transportation to get to medical appointments. To request transportation assistance through Medicaid, ask your doctor to request prior authorization for Non-Emergency Medical Transportation (NEMT) when you make your next appointment. Physicians and Care Coordinators may request prior authorization for travel a Medicaid members to get to medical appointments by contacting Conduent at 6446800, option 5. Local non-emergency ground transportation will only be approved when the member has exhausted all other means of travel, including public transportation. The following information will be needed when requesting a prior authorization for nonemergency transportation: * Member’s Medicaid ID * Member’s date of birth * Type of transportation needed (taxi/wheelchair accessible van) * Date transportation is needed * Name of clinic or medical provider the member is traveling to * Purpose of the appointment
