Background

Out of the Closets and into the Streets The LGBTQ, Lesbian, Gay, Bisexual, Transgender, and Queer community is an underprivileged group. They are connected by a common set of challenges in accessing health services. Despite increasing social acceptance, there is a long history of anti-LGBTQ bias in healthcare. As recently as 1973, homosexuality was listed as a disorder in medical text, and transgender identity still is. Attitudes have improved, but there is still a long way to go. Right now, individuals in the LGBTQ community are significantly more likely to report unmet health needs due to inadequate healthcare providers. Reports of physical and verbal abuse, refusal to touch patients, and a general lack of knowledge have all been recorded. Healthcare providers could better serve their patients if they knew more about risks associated with the LGBTQ community.

Research

Forget everything you’ve seen on television and in the movies. While the LGBTQ acronym may just look like a string of letters, it represents a diverse community of vivacious individuals. These individuals live very different lives and encounter a variety of different challenges everyday. What binds them together as social and gender minorities are common experiences regarding discrimination. There is very little conclusive evidence about LGBTQ patients, as the first longitudinal study has recently begun under the care of the UC San Francisco Medical Center. Most of the known health risks affecting the community are founded on correlations and independent surveys. The following is a list of resources my data has been compiled from, including community advocacy groups, and books on the subject written for healthcare providers.

GLMA: Health Professionals Advancing LGBT Equality (previously known as the Gay & Lesbian Medical Association) is the world’s largest and oldest association of lesbian, gay, bisexual and transgender healthcare professionals. GLMA was founded in 1981, as the American Association of Physicians for Human Rights, with the mission of ensuring equality in healthcare for LGBT individuals and healthcare professionals. Today, there is still considerable ignorance about LGBT health issues, with many assuming that LGBT health involves only HIV/AIDS. In fact, the full scope of the LGBT health agenda includes breast and cervical cancer, hepatitis, mental health, substance abuse, tobacco use, depression, access to care for transgender persons and other concerns.

Gay & Lesbian Alliance Against Defamation rewrites the script for LGBT acceptance. As a dynamic media force, GLAAD tackles tough issues to shape the narrative and provoke dialogue that leads to cultural change. GLAAD protects all that has been accomplished and creates a world where everyone can live the life they love. The organization amplifies the voice of the LGBT community by empowering real people to share their stories, holding the media accountable for the words and images they present, and helping grassroots organizations communicate.

In 2014, the organization officially changed its name from “Parents, Families, and Friends of Lesbians and Gays” to, simply, PFLAG. The mission and vision of the organization were also updated to reflect PFLAG’s decades of inclusive work. PFLAG is the nation’s largest family and ally organization. Uniting lesbian, gay, bisexual, transgender, and queer (LGBTQ) with families, friends, and allies, PFLAG is committed to advancing equality and full societal affirmation of LGBTQ people through its threefold mission of support, education, and advocacy.

A small, but dedicated group of teachers in Massachusetts came together in 1990 to improve an education system that frequently allowed its lesbian, gay, bisexual and transgender (LGBT) students to be bullied, discriminated against and/or fall through the cracks. Almost 25 years later, that small group has grown into the leading national education organization focused on ensuring safe schools for all students. Today, they’re known as GLSEN, the Gay, Lesbian & Straight Education Network.

Simone de Beauvoir rejects the theory of biology-as-destiny in favor of the view that gender is social construct. She is famously quoted for the phrase, “one is not born, but rather becomes, a woman.�

An ally is a person who advocates and supports members of a community other than their own. LGBTQ allies are integral to community support and growth by promoting and demonstrating social acceptance.

Institutional oppression affects the LGBTQ community in a variety of arenas including marriage, tax law, hospital visitation, and sometimes even sodomy laws. Without fair treatment by the government, social equality will not exist.

Outing is a hurtful tactic used by people in and out of the LGBTQ community. It is the act or practice of revealing the gender or sexuality of a person. This often results in mental and/or physical harm including bullying, displacement, and expulsion from school.

Passing refers to a person’s ability to be regarded as to be either a biological man or a woman. This distinction can literally mean life or death in the trans community. In some cases, mental or physical harm occurs to the individual who failed to pass for their intended gender.

Specific risk factors affecting LGBTQ health are largely based on correlations rather than causation. Research in this field remains largely unfunded. Research grants could go a long way in closing this disparity of information and would help to better serve the community.

Audience Profiles

By coming together, we make history. Identifying and understanding who you’re talking to is one of the most important initial steps in starting a conversation. Exploring the range of the potential audience that exists on each end of the spectrum leads to more rounded and informed conversations. By identifying these archetypes, we can use our resources appropriately to find citizens with the potential to achieve great things and affect great change. While this thesis project is mainly for doctors, especially primary care physicians, there are a host of other people in the medical community that could benefit from a clear and concise resource. Office and hospital personnel should be knowledgeable about all their patients to avoid disruption and humilitation. The following profiles represent a broad spectrum of people related to the topic. The final two community members represent the apathetic minority.

Insights

Information should not be hard to find. Through the course of my research some key insights came to light. These are the main problems that lead to the formation of my thesis, as well as what helped determine the content healthcare providers need to better serve LGBTQ patients. Early on I began to realize information on this topic was incredibly hard to find. Naming

What’s in a name? One big step towards making a project a real entity is coming up with a name. It was very important to me to involve the word queer. As a queer identified person, I am a part of the movement to reclaim the word. Many waves of feminism before me have reclaimed defamatory words in the past for a variety of reasons. Stereotypes and generalizations have marred so many labels, but the reclamation of the word queer presents the opportunity to create a new set of ideas.

Queer [noun]:

An umbrella term for sexual and gender minorities that are not heterosexual or cisgender. Originally meaning strange or peculiar, queer developed a usage as a pejorative term for homosexual in the late 19th century. Queer may be used by those who reject traditional gender identities as a broader, less conformist, and deliberately ambiguous alternative to LGBT.

Logo

Accessibility is key. A very large portion of my time developing an identity was spent on the creation of a logo. This process was long and arduous, but the final result is clean, professional, and easily recognizable. I wanted the Q to be able to stand alone to be used for App icons and as stickers in hospitals and offices to visibly signif y that a provider is sensitive to the LGBTQ community and is making an effort to provide better services. Visibility of the logo is essential to its effectiveness. The more providers that promote the visual identity, the more community members will look for its presence in their providers office. Starting the conversation is a big step in providing culturally competent care and the logo can ignite a discussion. The following chronicles my journey of logo ideation and creation.

This is how we do it. A name that is easy to remember and a mark that can be seen across a room are crucial to the brand’s identity. Visibility is the top priority. While all print materials are available according to brand standards, accessibility is key. Materials will be available to download and readily in the hands of users. It is important to make this information as easy to access as possible.

App Organization

Science is organized knowledge. During the course of my research I found that doctors and other healthcare providers have begun to use smart phones and tablets to check medical records and look up medical definitions and journals between seeing patients. The medical industry, like the rest of the world is going digital. An iPad mini is the perfect size for providers who wear white coats. It fits easily into the front pockets and doesn’t encumber movement. Due to this information, I’ve optimized the App for an iPad mini horizontal orientation. The App has three points of entry: top health risks of each community, a glossary of terms, and best practices that give providers insights into patient’s triggers. The app also serves to make available more comprehensive intake forms while providing a community for sharing ideas and experiences.
