Denton businesses register with E-Verify Nearly 50 Denton-area employers were among the 6,047 Texas businesses registered with E-Verify as of May, according to data obtained by the Denton Record-Chronicle. Many local companies have signed up to use a government program to verify whether their new employees are eligible to work in the United States. E-Verify, a voluntary program, compares an employee’s Form I9 information with more than 455 million Social Security Administration records and with

more than 80 million records in the Department of Homeland Security immigration database. Employers including Jostens, Sally Beauty Holdings Inc., Morrison Milling Co., Miller of Denton and McNatt Toyota Dodge and DATCU are among the registered participants. 1-7

NCTC holds open house at new campus North Central College held its dedication and open house in Flower Mound today. “It’s exciting to move into something new,” said Roy Culberson, dean of the Corinth and Flower Mound campuses. The public will have a chance to tour the two-story, 32,000square-foot Parker Square building at the open house. The campus, located at 1200 Parker Square, houses 10 classrooms, a science lab, two computer labs, a lecture hall, a bookstore and a library. 1-9

Texas Health Resources gains MedicalEdge Texas Health Resources, which operates Texas Health Presbyterian Hospital Denton,

announced last week that it acquired MedicalEdge Healthcare Group, and along with it, 420 physicians, physician assistants and nurse practitioners. With more than 250 practices in the Denton, Collin, Dallas, Grayson, Johnson, Parker and Tarrant counties, the MedicalEdge group gives Texas Health a slightly larger physician base than its main competitor, Baylor Health Care System. Texas Health now has about 680 physicians and midlevel providers including physician assistants and nurse practitioners. The Baylor system has a little more than 500. Neither Texas Health nor MedicalEdge disclosed the acquisition’s price, but both entities said it was the country’s second-largest acquisition of an independent physician practice. The largest acquisition was the Blackstone private equity firm’s $1 billion purchase of Team Health Inc. in 2005.

Grizzaffi Darby acquired by Dallas-based firm Lockton Dunning Benefits, an employee benefit consulting firm based in Dallas, announced this week it has acquired Denton’s Grizzaffi Darby. According to a news release, Grizzaffi Darby will take the Lockton Dunning Benefits name and will continue to serve clients at its Denton location, 319 W. Oak St. The transaction was completed Monday. Terms of the acquisition were not disclosed. The operation’s combined workforce will be 150 employees. Since its founding in 1966, Lockton Companies LLC has provided more than 15,000 clients with insurance, benefits and risk management services, the release stated.

Boutique announced Tuesday on Facebook that they closed their Denton storefront and moved all sales online. Co-owners Crissy Camp and her mother, Teri, have run their store, located at 1800 Brinker Road, for about two years. “We will focus a lot more on the online store and build it from there,” Crissy Camp said. Camp, who has accepted a full-time job at a Denton company, said she and her mother will run Emma’s Boutique via their Facebook posts. 1-12

County hires engineer for I-35W widening Denton County commissioners approved an engineering contract with HNTB Corp on Tuesday. Under the $1.8 million contract, the company will be widening Interstate 35W from State Highway 170 in Fort Worth to its junction with Interstates 35 and 35E in Denton. Career Advancement DATCU employee Lee Albright recently was promoted to human resources generalist. Albright has been part of the DATCU family since June 2008 and worked as human resources assistant until the recent promotion. Albright has Albright more than 20 years’ experience in office administration, with the last six years in the human resources field, said a news release announcing the promotion. She earned a certificate from the Bish Mathis Institute School

of Business in 1984 and in 2009, she was recognized as DATCU’s Support Staff Employee of the Year. Lee is a native Texan and an active volunteer at DATCU community events, said the release. Turmoil for ‘muni’ bonds “There will be a terrible problem, and then the question becomes: Will the federal government help?” — Warren Buffett in June 2010, on municipal bonds

I

n previous columns, I issued warnings about the bond bubble that has built up in U.S. financial markets. Like lemmings, investors have in recent years funneled hundreds of billions of dollars into bonds of all sorts — U.S. Treasury, corporate and municipal bonds — as they scrounge for yields amid the zero-interest-rate environment. As a result, most bonds became quite overpriced. In recent weeks however, a reversal has roiled the U.S. municipal bond — or “muni” — market. Investors have been lulled into complacency by munis’ relatively solid track record. There have never been any defaults of gener-

Jonathon FITE | al obligation municipal bonds since the Great Depression. General obligation municipal bonds are backed by state or local governments through sales, income and property taxes. Other munis backed by revenue from specific projects, such as water treatment plants, also have a history of low default rates. The credit rating agency Moody’s indicates that even in case of default, it anticipates “near 100 percent recovery on any defaulted general obligation bond.” But investing based only on

past performance is like driving while looking only at the rearview mirror. And given the notorious role that ratings agencies played in the subprime housing debacle, their forecasts should be taken with a big grain of salt. America’s state and local governments are in serious financial trouble. The shattered creditworthiness of the most dysfunctional states, Illinois and California, is now in the same neighborhood as Iraq and Greece, based on the prices of their credit default swaps (debt insurance). For years, state and local governments increased spending rapidly, giving unionized public employees generous salaries and promising pensions they could not afford. The recession has further devastated governments, as tax revenues have plummeted while social spending, especially Medicaid, has soared. The health care reform bill will add

to the challenges, as millions of people get added to state Medicaid rolls. As if the fiscal picture is not dismal enough, some states are now being investigated by the SEC for deceptive accounting and poor disclosure of their financial situation. It is also troubling that the stigma of default is fading among public officials. “To disrupt our services because we can’t make a bond payment would just be unconscionable. And as a leader I couldn’t do it.” Those were the words of Linda Thompson, the mayor of Harrisburg, Pa., as she announced that her city had decided not to meet its muni bond payment promises. This trend of elected officials portraying muni bond holders as greedy evildoers whose claims can be ignored to fund a town’s police force and fire department is likely to get worse. In the case of munis that are “insured,” public officials might feel even more comfortable stiffing some faraway, faceless corporations like

MBIA or Assured Guaranty. These companies are on the hook for insured munis but they can’t handle a big wave of defaults, especially after they got clobbered in the subprime housing crisis. States and local governments do not have the same freedoms that the federal government enjoys. While the federal government runs gigantic deficits too, Uncle Sam has the luxury (so far) of a central bank willing to print trillions of dollars out of thin air, and foreign central banks, notably China and Japan, buying up its Treasury bonds to prevent their currencies from rising too much against the dollar. Muni bonds don’t have this backstop from such deep pockets. The U.S. taxpayer has been subsidizing state and local governments over the last few years. The 2009 federal stimulus was in some respects a back-door bailout of unionized state public employees. (That’s why there’s

not that much infrastructure to show for the $800 billion that has been spent.) Once the federal stimulus funds run out midyear, state budget problems may worsen. It is hard to imagine taxpayers in relatively solvent states like Utah or Texas will want to bail out spendthrifts like California and Illinois again. Instead of a federal bailout, legislators from the stronger states are suggesting weaker states should be allowed to declare bankruptcy to allow them to escape their onerous pension commitments and restructure their heavy debts. Of course, mere mention of the “B” word would further agitate the muni markets. What does this all mean for

investors? Well, speculators might try to trade on the shortterm ebb and flow of market sentiment. But true long-term investors should instead ask: Is this a good investment? Would I personally give a 20-year, low interest rate loan to a deficit-ridden state or local government that hasn’t properly funded its public pensions for years, and might use shady accounting tactics? It really doesn’t sound like such a wonderful deal! Also, once high inflation really kicks in, prices of such longdated bonds will plummet as investors demand a high enough yield to compensate for the shrinking purchasing power of cash. This is not merely a distant, abstract concern. Inflation from lax monetary and fiscal policies has already driven dou-

ble-digit price increases for food and oil. This is already fueling political upheaval in many developing countries. Income-oriented investors might be better off with worlddominating companies with pricing power, hard assets and international operations that are throwing off growing dividends. These will stand a better chance of keeping pace with inflation in the future. Of course, we should all stay alert. If there is a muni market panic, and short-term muni bonds for creditworthy governments become dirt-cheap, that would be really interesting. entrance and exit ramps will be, said Commissioner Andy Eads. That will help cities with planning, and developers will have a guide as to how the corridor can be developed by the public and private sectors. Funding for the contract will come from the Precinct 4

Denton looks to absorb 1,600 acres Denton city leaders are targeting more land for annexation after absorbing thousands of acres last year.

The City Council met Tuesday to discuss annexing three areas totaling nearly 1,600 acres, including a 1,154-acre area south of U.S. Highway 380 near Lewisville Lake. Public hearings are scheduled for Feb. 1 and Feb. 15, but a final vote wouldn’t come until 2013. Council members met behind closed doors with attorneys to

discuss legal issues involved with the annexations. Afterward, they heard a brief presentation from city planning director Mark Cunningham, who said the proposal was part of an annexation plan adopted last year. The plan targeted 18 areas totaling about 9,035 acres — land that was home to at least 2,100 people last year, according to city esti-

UNT football coach to speak at luncheon

D

an McCarney, the new head football coach at the University of North Texas, will be the featured speaker at the Denton Chamber of Commerce’s next membership luncheon. A traditional highlight of the February membership luncheon is the presentation of the Small Business Person of the Year Award. Scheduled for Friday, Feb. 18, the luncheon will be held at the UNT Gateway Center. Coach McCarney took over the Mean Green football program in early December after serving as assistant head coach at the University of Florida since 2008. Before that, McCarney spent 12 years as head coach at Iowa State University. Small Business Person nominees were considered primarily based on business-related accomplishments as of Dec. 31. Criteria include a combination of comparable growth indices, innovation and the ability to overcome adverse circumstances. The differentiating components typically include facilities expansion or improvements, management techniques, innovative merchandising and/or marketing practices that help set the nominee apart from his or her competition. The award for 2009 was presented to Lynn and Joni Chatwin, co-owners of

Chuck CARPENTER | Alphgraphics. This will be the chamber’s 25th presentation of the Small Business Person of the Year Award. Plan to dissolve airport board draws scrutiny Denton Airport tenants and city residents added their voices to the debate over the future of the airport’s citizen advisory board Wednesday night. During a special meeting of the Denton Airport Advisory Board, several tenants and residents said they worried that a city-backed plan to dissolve the board and break up its duties would deprive the airport of board members’ aviation knowledge. At the same time, most speakers at the meeting voiced general support for boosting the airport’s marketing and economic development efforts — a key element of the city’s plan. The board called the meeting to offer the public a chance to weigh in on the city’s airport governance plan ahead of an expected City Council vote Feb. 1. Board members, who are council appointees, also agreed to form a three-member subcommittee to help draft a final position paper to offer to the council. Under a plan endorsed by the city manager’s office, the board’s duties would be shifted to the city’s Economic Development Partnership board and a newly created airport committee made up of three council members.

28 because some national corporations are slow in reporting results and the campaign has only raised a little more than 73 percent of its $2.4 million goal. Final reports for about 20 national corporations in the area are still out, said Jodi Ramon, the organization’s resource development director. For example, General Electric just filed its results Wednesday, she said. The 2010 campaign so far has raised $1,769,973.

Treating a broken system

As health care reform takes shape, local clinics and hospitals are focusing on preventive care and moving patient information over to electronic records — hoping that, in time, taking these steps will lower costs and improve efficiency. “Obviously, the big concern in the American health care system these days is the cost of care,” said Richard Kurz, dean of the School of Public Health at the

University of North Texas Health Science Center. Kurz said the high cost of health care is due in part to physicians driving demand

through prescriptions. “We pay the physicians on a piecework system,” Kurz said. “The more they do, the more they get paid.”

He said such a system needs to be controlled. Margaret Jarvis, spokeswoman for Blue Cross Blue Shield of Texas, says health care costs weren’t suddenly going to drop with the passage of last year’s health care reform law. It will take time to see any change in health care costs, she says, since the law is more focused on the long term. “Addressing the cost of health care will take a continued,

focused effort on the part of every stakeholder,” she said. AN EMPHASIS ON PREVENTION Many health care providers are counting on patient education as a means to lower illness rates and, as a result, lower their associated costs. And at the top of the list of behaviors causing preventable illness? Smoking. A third of all cancers can be


linked to cigarette smoking and other tobacco use, Kurz said. “We know if you don’t smoke, you are less likely to get disease,” he said. By educating tobacco users and teaching them how to control their habit, one-third of all cancers can be controlled, he said. As it is, about 1 in 5 deaths each year in the U.S. are the result of tobacco use, according to Cancer Facts and Figures 2010, and about $193 billion was spent between 2000 and 2004 to treat tobacco-related illnesses. “It’s not true everywhere, but in many places, the number of people who are smoking don’t want to smoke,” Kurz said. Poor eating habits are also contributing to the country’s rising health care costs, he said, and are responsible for illnesses such as heart disease and diabetes. And as with tobacco users, education is key to improving Americans’ poor eating habits, Kurz said. “We are trying to make the system more efficient” through preventive care, he said — an opinion growing in popularity throughout the health care system. “A lot more people are getting their preventive services,” said Dr. Adam Wayne McDowell with Corinth Family Medicine. “That is the most prominent effect we have seen in our clinic since the new health care changes took place.” The services he refers to — including scheduled immunizations, recommended screenings and counseling on healthy living — were mandated by last year’s health care reform law as free services for Medicare patients and those with new health policies. Under the provision, which took effect in September, health plans are required to cover preventive care services for infants, children, adolescents and women. McDowell said pediatricians in particular are happy about the new provision. One of the best changes, he said, is that insurance companies are required to pay for children’s sports screenings. Many parents wrongly assumed that a sports screening

Not anymore. “Insurance companies can now provide those screenings for free in addition to a wellness exam,” he said. COST OF PREVENTION Stan Morton, president of Texas Health Presbyterian Hospital Denton, says offering wellness care is important, since many diseases can be prevented with regular screenings and education. But, he said, an important question looms over hospitals: How many preventive services can they afford to offer? Texas Health Denton currently funds its own community education services. “What gets reimbursed is caring for the sick patient,” Morton said. This is a weakness in the health care system, he said — insurance companies need to start reimbursing hospitals to keep people from getting sick, not just reimburse them to treat the ill.

over their health care decisions.” McDowell said he fears the cost of preventive services could make it more difficult to implement some of the law’s provisions in the coming years. “I hope … preventative services [can be] cost-effective and can be maintained,” he said. ELECTRONIC RECORDS James Purgason, a physician with Family Medicine of North Texas, said preventive care has

always been one of his health care philosophies. A bigger adjustment for him has been switching over to electronic medical records, a “more efficient” method of collecting patient data than the current system, which seems disconnected and often results in duplicate testing, he said. Hospitals and clinics are being offered an incentive for making the switch to electronic records over the next five years. In that time, health care providers will be required each year to meet certain criteria in order to qualify for the incentive payment. This year — the first year of the incentive program — providers are required to get the electronic records to the point of “meaningful use.” If they meet this criteria, clinics and hospitals must maintain the change for 90 days before they can receive an incentive payment. Purgason hopes to have his practice to that point within the next month. | CONTINUED ON PAGE 16

In the coming years, health care providers must make their patient records available to other labs, hospitals and doctors over a connected network — a big process that will cost a lot of money, Purgason said. The government has mandated the network, but hasn’t clarified who will pay for it or how they’ll afford it, he said. Despite what happens with the health care reform law — which a federal judge in Florida voided last month after ruling the health insurance mandate was unconstitutional — “health care is going to move forward in some form or fashion,” Morton said. A big part of moving forward, at least for Texas Health Denton, will be the switchover to electronic records. Morton said the change — which he anticipates will launch in December — will

save the hospital both time and money. “It will really change a lot of how we will do our work,” Morton said. BEYOND THE HOSPITALS Outside hospital exam rooms and beyond doctors’ offices, the new law can be seen affecting more than just health care providers. For one thing, young adults now will be able to remain on their parents’ insurance until they turn 26. This provision pleases Purgason, because otherwise “they would be uninsured,” he said. Young adults — particularly those right out of college, trying to find a job — are often unable to afford individual coverage, he said, and this provision gives them more time to find a job with full health benefits. Small businesses also are see-

ing changes ahead — one of which is coming in the form of a tax credit. Under the health care law, if a business has fewer than 25 employees and wants to provide them with health insurance, that business can qualify for a tax credit of up to 35 percent to offset the cost, according to information posted on www.health care.gov. A 25 percent tax credit is available to nonprofit organizations. Besides expanding coverage to more young people and providing tax credits, the new law also makes insurance coverage available to more low-income individuals, families and children. Jarvis, the Blue Cross spokeswoman, said the law has prioritized access to health care services over lowering costs, at least in the short term. “It’s been widely recognized

that the Affordable Care Act is focused primarily on increasing access to health care services, particularly for lower-income individuals,” Jarvis said. No matter what the changes in store following the bill’s passage or whether it’s eventually overturned by Congress or in the courtroom, the current system is unsustainable, said Kurz, the School of Public Health dean. If changes aren’t made to the current health care system, he

said, neither insurers nor patients will be able to keep up with rising costs. “We want people treated in the best possible way while containing costs,” he said. Customer service meets patient care By Lucinda Breeding

Caleb O’Rear, CEO of Denton Regional Medical Center, says some health care reform comes from the marketplace, not the ballot box. In November, Denton Regional launched radio spots and billboards along Interstate 35E that announce the average emergency room wait time at the local, for-profit hospital. Denton Regional also makes its ER wait times available to iPhone users through fastERTX, an application that will download the locations of nearby ERs, along with current wait times. The hospital also lists its current wait time on a website, www.fastertx.com, which shows a map with ERs affiliated with the Hospital Corp. of America, one of the largest private operators of hospitals and clinics in the world. “This is really about transparency,” O’Rear said. “We’ve moved toward a more consumerist approach to health care, which means that mom and dad — and you and I — are in the driver’s seat when it comes to making decisions about health care for themselves and their families. We think in being transparent about what’s happening in the ER, we’re helping people make those decisions.” O’Rear said the hospital started to shave minutes from the wait time two years ago. Hospital staff started paying attention to the bottlenecks that patients encounter in the ER

and throughout the hospital. “The project made a lot of sense to me as we made changes to our trauma program,” said Scott Domingue, a nurse and director of Denton Regional’s ER. “Getting the patients from the waiting room into the emergency room bed where they can be seen and treated by a provider is always the goal. Getting the patient in front of the provider as quickly as possible is better for everyone — the patient and the staff. Time is of the essence in the emergency room.” What changed? Patients are greeted immediately at the triage window, where admission starts. The patients give their information once, and are brought in as soon as a bed is available. Custodians clean and prepare rooms and beds as patients move into the hospital or go home. More tests are administered at patient bedsides. “We really looked at what we call the ‘through-put,’ which is getting a patient from one point of their evaluation and treatment to the next. Throwing more staff at a busy ER isn’t necessarily the best use of your resources, because you can never truly predict when activity in the ER is going to spike,” O’Rear said. “We found that we made more of an impact in small but important areas, like having a lab test done at the bedside and getting the results right there on the spot. Having the ultrasound of a mom’s belly right there in

faster, you’re ready for those emergencies. It’s much more satisfying for the physicians. No one wants to linger in the ER.” But make no mistake — Denton Regional’s faster care isn’t reckless, Beard said. “We aren’t sacrificing the quality of care,” Beard said. “We’re not rushing patients through. We’re just not parking them some place until a bed is ready.” Jose Pagan, professor and chairman of health management and policy at University of North Texas Health Science Center, said Denton Regional’s focus on wait times could translate to more access to health care. If you can get a CT scan in one hour instead of two, if you get a room cleaned out and a bed ready

Morton said American forprofit hospitals measure efficiency using the same yardstick: how easily patients are admitted and treated by hospital departments. For Texas Health Denton — and for the network of health care companies across the nation — the transition from paper to electronic patient records is one of the “first big steps” in streamlining health care, from treatment to bill collecting. Texas Health Resources, a private health care operator that owns Denton’s Presbyterian hospital, has spent about $250 million to get patient files out of manila folders and into a huge, secure computer network available to affiliated clinics and hospitals and also to unaffiliated hospitals. “Electronic health records are really a big deal, and it’s been in the works for a long time,” Morton said. “One of the biggest

ing the infrastructure for this transition, and now we’re building the systems. We think it will be a huge next step,” Morton said. “We think we will be able to mine data for quality outcomes, like better care for diabetic patients. “Part of this is about evidencebased care. Yes, there’s the advent of all that’s been proposed in the law. But if everything [in the law] were unwound, we think the groundwork is there to make evidencebased care from the transition.” Patients in the Texas Health Resources network also will be able to get their records, using a

“My Care” secure account. Patients will be able to read information from their records in plain language, Morton said. Providers expect that when patients have access to their own records, they’ll become more involved in their own care and comply more readily with physician orders for treatment. “There are other ways to use these accounts, too,” he said. “If congestive heart failure patients are able to have a better longterm dialog with their doctor, their treatment can get better. We’ve found that if providers can have a longer dialog about a congestive heart failure patient’s weight gain and loss, we can predict when they will be readmitted to the hospital. That kind of information is in patient records.” There might be a lot of anxiety around shared electronic records, but Morton said they can actually keep patients safer. “Whenever you have multiple pieces of paper in a bunch of different hands, the greater risk there is of errors,” he said. “You

can make the money argument, but I think you can make the safety argument, too.” O’Rear said Denton Regional is already looking at ways to translate the efficiencies in the ER to the rest of the hospital. “We’re looking now at bringing this idea to the whole patient-management process,” he said. Pagan, the UNT Health Science Center professor, said applying the same efficiency practices beyond the ER is definitely possible. “Yes, in the sense that a more efficient ER means less overcrowding and the adoption of better patient-management strategies, which should impact other hospital services as well,” he said. “Lessons learned in one unit of a hospital — the ER, for example — can spill over to other areas of a hospital as well.” LUCINDA BREEDING can be reached at 940-5666877. Driving down Hickory Street, one can see a lot of changes in the works. Here, a barbershop is relocating after many years operating in a different area of Denton. There, an Italian restaurant springs up where a pizza place shut down last month. Hot Box Pizza closed its location near downtown Denton after the franchise owner sold the space to the owner of Giovanni’s Pizza and Pasta, said Greg Johnson, CEO and managing partner of Verus Real Estate Advisors. The new restaurant at 214 E. Hickory St. is owned by Argyle resident John Parks. “I think it is going to be good for my restaurant and for the other tenants,” Parks said about the changes taking place on East Hickory Street. “It is going to draw people there — and with Mellow Mushroom coming and the [A-train passenger] rail, I think the area will not look the same.” Parks owns another Giovanni’s — or Gio’s, as it’s also known —

in Argyle, a restaurant he has run for five years. He said Gio’s is his own concept, and serves pizza, pasta and sandwiches. Parks said he did not make any structural changes to the restaurant other than changing the name. He said he kept as many Hot Box employees as he could. “Joseph Smitter — ‘Smitty,’ as he is known — was the manager with Hot Box, and he will be continuing with us,” Parks said. Other changes coming to the area just east of downtown include a new restaurant at 212 E. Hickory St., once the home of Cold Fusion Lounge. “It will be a great concept — a combination of a tavern, restaurant and sports bar,” Johnson said. Right across the street from Gio’s, Campus Barber Shop is slated to relocate this year. Johnson said Gene Hartman will still continue to do hair at the Denton mainstay, but will let someone else run the shop; he said the new home will let Hartman focus on what he does best.
