Letter from the Editor Welcome to our second issue of Rinklefree Living. With each issue, I am striving to create a credible patient resource geared at aesthetic improvements along with healthy lifestyle activities. In this issue, I am focusing on facial rejuvenation and body contouring, two of my most requested cosmetic procedures. In addition, our injection team will be introduced. Our team includes plastic surgeons, dermatologists, physician assistants, and nurses, all of whom are highly trained in the latest techniques of facial analysis and rejuvenation performed through both minimally and non-invasive procedures and techniques. With the holiday season upon us, looking and feeling good is even more important than ever as we cross paths with friends and relatives at countless social celebrations. It is a time when feeling good also goes hand in hand with looking good. Whether that equates to a new holiday outfit, a new hairstyle or a few wrinkle removing procedures, looking good on the outside helps us all feel more confident on the inside. I hope this holiday season is happy, healthy, and beautiful for all of us.

Season’s Greetings From Our Family to Yours Dr. Robert Skalicky is one of the most highly respected plastic surgeons in the area having been in practice in the Bucks County area for over 25 years. Board certified in plastic and reconstructive surgery, Dr. Skalicky completed his undergraduate training at Franklin and Marshall College and LaSalle University prior to receiving his medical training and degree from Philadelphia College of Osteopathic Medicine. He then completed his plastic surgery training at multiple institutions including Mercy Catholic Medical Center, Fox Chase Cancer Center, Westchester County Medical Center, and Philadelphia College of Osteopathic Medicine where he served as Chief Resident in plastic surgery. He later completed an MBA degree at Drexel University graduating summa cum laude. He is the President and founder of Rinklefree Aesthetic Centers and President of Bucks County Plastic Surgery. Dr. Skalicky’s practice encompasses all areas of plastic surgery, both cosmetic and reconstructive including breast surgery, facial rejuvenation, body contouring, cancer reconstruction, rhinoplasty, abdominoplasty, brazilian butt lift, and hair transplantation. He has received multiple “best plastic surgeon” awards throughout his career and, in 1999, performed the world’s 1st live internet breast reconstruction to over 500,000 viewers. Dr. Skalicky is noted not only for his artistry and skill, but also for his compassion and honesty with patients. Dr. Michael Shafran is a New York native, who graduated magna cum laude from the prestigious Hofstra University. He earned a Bachelor of Science in Biology, and was elected into the Alpha Epsilon Delta pre-medical honor society. He continued his medical education and graduated with distinction from NYCOM in Old Westbury, New York, where he refined his surgical studies while performing his clerkships within the North Shore – LIJ health system. He then went on to complete a General Surgery residency in New York City where he had the privilege of rotating through Memorial Sloan Kettering Cancer Center as well as NY Presbyterian/Weill Cornell Medical Center. He served as Chief Resident in his final year, responsible for the administrative duties and education of 15 junior surgeons as well as 100 medical students. Dr. Shafran is also a graduate of the Philadelphia College of Osteopathic Medicine plastic and reconstructive surgery fellowship. During his years of specialized training he was mentored by some of the finest plastic surgeons in Pennsylvania, and was affiliated with such teaching institutions as: Hahnemann University Hospital, St. Christopher’s Hospital for Children, St Mary’s Medical Center, Forrest Health Medical Center and the Reading Health System. Dr. Shafran’s areas of focus during his training included: breast reconstruction and oncoplastic surgery, trauma and maxillofacial surgery, hand and microvascular surgery, as well as facial, breast, and body aesthetic and cosmetic surgery. Dr. Shafran stands out as a young, energetic plastic surgeon, characterized by his warmth and compassion for his patient’s needs. Dr Shafran’s attention to detail, precision and technical skill, combined with his desire to help people reach their desired aesthetic goals has earned him the respect and admiration of both his peers and patients.


Season’s Greetings From Our Family to Yours Dr. Ivy DeRosa recently established her dermatology practice with Rinklefree Aesthetic Centers after completing the Beaumont Health Dermatology residency program of Michigan State University College of Osteopathic Medicine (MSUCOM), where she proudly served as Chief Resident. Dr. DeRosa was born and raised on the South Shore of Long Island, New York and graduated in the top 10% of her class at Hofstra University with a B.A. in Biology. She then attended and graduated with honors from the New York College of Osteopathic Medicine where she discovered her passion for dermatology. While in residency Dr. DeRosa gained extensive experience with Mohs and cutaneous surgery, skin cancer detection, dermatopathology, laser therapy for vascular and pigmented lesions, laser skin resurfacing, photo-aging, cosmetic fillers, skin biopsy and excisions, full-spectrum treatment for acne, psoriasis, eczema, disorders of the hair and nails, and pediatric skin conditions. She is a member of the American Academy of Dermatology, American Medical Association, American Osteopathic College of Dermatology, American Society for Dermatologic Surgery and is on the clinical faculty for Michigan State University. She has lectured and presented her research on Follicular Mucinosis and Cutaneous Blastomycosis and is published in the medical journal Cutis for Autoimmune Progesterone Dermatitis with various articles and case studies on the history, diagnosis and treatment of non-melanoma skin cancers. Dr. DeRosa believes in educating her patients to ensure they feel knowledgeable and confident in the treatment plan they build together. Her outgoing personality and caring nature have quickly built a loyal patient following that she brings to the Rinklefree Aesthetic Centers’ dermatology division.

Kaitlyn Nelson, MS, PA-C Physician Assistant Kaitlyn is a board certified, state licensed physician assistant specializing in plastic surgery and aesthetics. She graduated magma cum laude from DeSales University with her undergraduate degree in Biology. She then obtained her Master’s degree with honors from Cornell University Graduate School of Medical Sciences. Kaitlyn trained at prestigious institutions such as Memorial SloanKettering Cancer Center and New York PresbyterianCornell Hospital, focusing on the surgical specialties. After several years living and working in Manhattan, Kaitlyn is thrilled to be back in her hometown of Philadelphia. She is now the sole physician assistant at Rinklefree Aesthetic Centers. Kaitlyn acts as an extension of the doctors to fortify patient care. She is involved in all aspects of a patient’s surgical journey; from the time of their initial consultation, through assisting in the technical aspects of each procedure, to post-operative treatment and follow up care. Kaitlyn is also an experienced and accomplished cosmetic injector, fully accredited by the Master’s training programs of every major pharmaceutical distributor of dermal products. Her skilled hands and keen eye for aesthetics and symmetry allow Kaitlyn to create individualized treatment plans with the most beautiful, natural results.

She brings over 35 years of nursing experience and expertise to his surgical practice. Michele has worked with injectors from around the world in an effort to “learn from the best” and has developed her own unique injection techniques. Her efforts have cultivated an active injectable practice earning her the respect of physicians and colleagues in her field. Michele received her Bachelor of Science degree in Nursing from Villanova University. While she has practiced in a variety of clinical settings, prior to injecting, she has found Facial Aesthetics to be her “niche” and finds it both challenging and rewarding. Michele frequently teaches beginner, mid-level and advanced injection techniques to nurses, physicians and dentists. Her passion for injecting is reflected in her commitment to ongoing medical education for the purpose of remaining abreast of new and current procedures related to the Art and Science of Injectables. Nicole Ciasullo BSN, RN Aesthetic Nurse Injector Nicole has over 20 years of nursing experience. She graduated Summa Cum Laude from Holy Family University and was a member of Sigma Theta Tau, the Honor Society of Nursing. She has an extensive background in the field of Nursing and spent most of her career at the Hospital of the University of Pennsylvania where she was recognized with multiple awards for clinical leadership, innovation, clinical judgment and decision making as well as several patient recognition awards. Nicole’s extensive plastic surgery experience provides her with the foundation to become a superb nurse injector. She is a perfectionist with a passion for injecting and she has an artistic eye and a gentle touch. Winter Fitness: Why Staying Healthy Is a Must By Julie Davis

W

inter weather conditions may make you reevaluate your outdoor fitness routine, but hibernation is not the answer, even if you live in the chilly northeast. To be most effective, fitness needs to be consistent. Staying active in the winter boosts immunity by keeping the lymphatic system active — as long as you don’t overtrain, which has the opposite effect. Winter fitness also

prevents weight gain that can occur from inactivity and holiday eating and drinking, as well as the aches and pains that arise from sedentary behavior. Get Walking If you’re new to outdoor activity in winter, walking is a good way to begin. It’s an effective cardio workout that suits a range of fitness levels. Try to choose activities that are appropriate for your abilities, and


those you truly enjoy. If you’re going out walking, you can warm up by doing some simple stretches and by walking at a slower pace for the first 5 to 10 minutes of your walking routine. Additionally, be sure to dress for the weather. Choose performance exercise clothes and outerwear that allow you to layer on a thin, thermal first layer — a pullover and leggings, for instance — and then a vest, pants, and an outer shell that’ll give you warmth without bulkiness and enable you to peel off the layers as you heat up. Head for the Hills Winter is the perfect time for outdoor sports such as downhill and cross-country skiing, snowboarding, and other snow-based activities. If it’s nice out, even walking in the snow is great cardio because it takes more effort than walking on a clear sidewalk or track. Also, this is fun to do with friends.. Snowshoeing is another enjoyable seasonal activities that’s good for winter sports novices. It requires minimal equipment — just the snowshoes. Go Out and Play! Let your kids inspire you with winter activities that don’t feel like exercise. After a fresh snowfall, go out and build a snowman with them. Go sledding, snowshoeing, or take a nature walk through the snowy woods. If you’re tech-savvy, you can try geocaching — a treasure-hunting game in which you use GPS devices to find hidden geocache treasures. If your kids are a little older, consider taking them cross-country skiing, snowboarding, or out to the local pond to ice skate or play hockey. Playing outside can be fun at any age. Indoor Activities at the Gym Use the season as an excuse to expand your exercise repertoire and check out new fitness classes offered in your area. If you’ve wanted to try a new fitness activity at your gym, stop hesitating and sign up. Whether you’re interested in

spinning, kickboxing, or salsa dancing or are looking forward to swimming in the heated pool or walking on the treadmill, now is the perfect opportunity to challenge yourself. Your new activity may even turn into your favorite winter exercise. Indoor Workouts at Home Be prepared with an indoor workout when the weather conditions keep you stuck at home. If you’re snowbound, put on a DVD that’s geared to your fitness level. The great thing about DVDs is that you can pause them if you need more time to get the position right. Don’t have a fitness DVD? Look for a fitness channel on your local cable or satellite television, or find a fitness Web site you like and get a workout online. Restorative Activities Don’t overlook gentle or restorative activities like yoga, tai chi, and qigong that can build strength, increase flexibility, and help you relax. They can also refresh your energy and improve your stamina throughout the winter. Your energy can become de-


pleted from the stresses of too much you did during the holidays.. This can lead to diminished immunity and colds & flu. Household Chores Unless you’re tuning in and working out with a fitness show, limit your downtime on the computer or in front of the TV. If you’re stuck indoors, get a jump on spring cleaning. (Remember, when the warm weather returns, you’ll want to be back outside, not cleaning the house.) Collect and recycle old clothes and used appliances that have been taking up space in your closets, and take care of the various household chores you’ve been putting off since the summer. An hour of housework burns about 200 calories; plus, this winter activity will give you a great sense of accomplishment. Turn Shopping Into Exercise Mall walking is a popular winter exercise, especially if you get to the mall before the stores open

and focus on a brisk walking routine. Walking and window-shopping at a casual pace burns about 68 calories in 30 minutes; that number jumps to 112 calories if you hurry along at a brisker, three-milean-hour pace. Keep Eating From Becoming a Winter Activity Remember that eating isn’t a winter sport, so while you may indulge in some tasty treats, follow the same healthy eating guidelines that you normally would. Build meals with lean protein, whole grains, vegetables, and fruits. Practice moderation with alcoholic beverages, too — moderate drinking means one alcoholic beverage per day for women and two for men. At cocktail parties, exercise self-control and limit the number of appetizers and desserts you consume. All of these efforts will help you have a happy and healthy winter season.

Holiday Decorating

Splurge or Save? By Laura Williams

T

here’s nothing quite like a home full of holiday decorations to put you in a festive mood. However, not all decorations come cheap. Don’t throw away your entire holiday budget on decor that may not suit your style down the line. Instead, invest your money wisely by identifying and even splurging on the decorations that will define your holiday spirit for years to come. Then, add special “saves” that round out your decor without breaking the bank. Saving money this holiday season will help you with holiday stress management. Before you purchase new decorations, think carefully about what you already have on hand, what you’re likely to want to change from year to year, and what truly defines the holiday for you. For instance, if you look back on your childhood with fond memories of personalized Christmas stockings and homemade Christmas decorations, you may want to continue the tradition with the next generation by investing in beautiful stockings and creating handmade tree ornaments. After you assess your personal needs, you’ll be able to make sound financial choices without getting caught up in the hoopla of the season. As a general rule, reuse or re-purpose items you already have on hand, invest in heirloom pieces or decorations that will last for years to come,

and save on “trendier” decorations that you may not want to display every year. Even if you take advantage of holiday decoration “saves,” make sure you set a budget before hitting the stores. While small items like greenery, pine cones, and spray paint don’t cost much by themselves, they can add up quickly if you’re not keeping track. To ensure that you don’t overspend, put cash in a holiday decoration envelope to use specifically for decorations and supplies. Once the money is gone, you’re done spending for the season – simple as that. Not only will this envelope budgeting system force you to be more conscientious financially, it may also force you to be more creative with the supplies you already have on hand. Getting the best holiday decorations has very little to do with how much you spend. It all comes down to what’s most important to you. Do you want to continue family traditions, start new ones, or simply express yourself? In addition to your priorities, put some thought into your surroundings. Look at your indoor and outdoor spaces to see how and where to place your creations, finds, “saves,” and investments. And don’t forget, holiday decorating is fun! Grab your family and friends, put on some music, and make memories. After all, that’s what the season is all about!

In The Winter Months By Holli Harms

L

et’s face it, the winter months and the layers of hats, scarves, gloves, coats and bulky sweaters can make you feel nothing like a sexy woman. I don’t know about you, but fighting the cold literally takes the sexy right out of me. I’m no longer walking casually in fun little dresses, or shorts. I don’t have tan lines to show off or the sun kissed streaks in my hair. I’m not sitting at some cute little outdoor cafe sipping a glass of chardonnay and wearing my fabulous sunglasses. I’m rushing as fast as I can from car to house to store… to any place indoors! I wear boots and heavy sweaters, and some days my long johns. Not exactly sexy.

I was a nervous wreck. To try to gain composure and confidence I decided to give myself a secret. I wrote on my stomach: I AM A WARRIOR PRINCESS. I went into the audition with my little secret and the secret gave me confidence, and the confidence – well, I got the job. Another way of feeling sexy is by feeling physically in charge. The winter months can take that in charge feeling right out of you. I am a runner and the summer is great because I can hit the roads and trails around my house. As winter sets in, I stay inside on the treadmill. Running in place while staring at a wall is mind numbing and easy to avoid. When I don’t run, I start to feel sluggish and thick like my heavy winter clothing, so I force myself to go to the gym. The more I run, the better I feel. The feeling of mind and body connecting is sexy. So this winter, try wearing some sexy underwear, exercising more, and feel free to write, “I AM A WARRIOR PRINCESS” somewhere where no one can see. There are ways to make you feel sexy and confident under all those layers! Holli is a writer and playwright and lives in New York and Pennsylvania with her husband and daughter. Last year a friend confided in me her secret to keeping the sexy on through all the cold: lace underwear. (The comfortable ones not the itchy lace of yesteryear.) She said it’s her little secret that keeps her spirits up and the fire burning inside to get her through the long winter days. So I tried it last winter and it worked. My little secret gave me an inner confidence, and that is after all what sexy is, confidence. A confident woman that is comfortable in her own skin, even if her skin is hidden under mountains of clothing, is a sexy woman. If you happen to attract a man with all the beaming confidence shining through that bulky mountain, you’ll be prepared. The “Precision Lift”…. The Neck’s Best Thing! By Dr. Robert Skalicky

W

ith so many options that exist today for rejuvenating the face and neck, how does one know what is the right procedure to select to turn back the clock and give a person a more youthful, refreshed look? Injectables, botox, dysport, Kybella, ultrasound, radiofrequency, lasers, and, of course, surgery are all options heavily advertised as being the

latest and greatest fountain of youth. There are many choices with too little education for the patient. While each procedure or product can give rejuvenation benefits, patient education is critical to align the appropriate procedure with patient expectaions and avoid costly errors that sometimes occur when patients select easier procedures that fail to accomplish their desired aesthetic outcome.


The Precision Lift offers patients a single procedure that naturally rejuvenates the face and neck, giving real results that dramatically refresh facial appearance.

After years of treating patients and seeing their satisfaction with different procedures, I decided to develop a technique for facial rejuvenation that gives the best aesthetic rejuvenation result with the greatest return on investment. The procedure, termed “The Precision Lift”, is a technique that combines traditional surgical techniques with laser technology. This procedure utilizes a two layer surgical tightening and volumizing technique in the cheek and mouth area along with with laser skin tightening on the neck area utilizing Cynosure’s Precision TX technology. The laser treatment of fullness in the neck area works by “melting away” the excess fatty tissue that is present and also “shrinking” the skin of the neck through laser heating of the dermal skin layer. Recovery is approximately 10 days for most patients.

Dysport is a prescription injection for temporary improvement in the look of moderate to severe frown lines between the eyebrows (glabellar lines) in adults less than 65 years of age. Important Safety Information What is the most important information you should know about Dysport? Spread of Toxin Effects: In some cases, the effects of Dysport and all botulinum toxin products may affect areas of the body away from the injection site. These effects can cause symptoms of a serious condition called botulism. Symptoms of botulism can happen hours to weeks after injection and may include swallowing and breathing problems, loss of strength and muscle weakness all over the body, double vision, blurred vision and drooping eyelids, hoarseness or change or loss of voice, trouble saying words clearly, or loss of bladder control. Swallowing and breathing problems can be life threatening and there have been reports of death. The risk of symptoms is probably greatest in children and adults treated for muscle spasms, particularly in those patients who have underlying medical conditions that could make these symptoms more likely. The toxic effects have been reported at doses similar to those used to treat muscle spasms in the neck. Lower doses, in both approved and unapproved uses, have also caused toxic effects. This includes treatment of children and adults for muscle spasms. These effects could make it unsafe for you to drive a car, operate machinery, or do other dangerous activities. Do not have Dysport treatment if you: are allergic to Dysport or any of its ingredients (see the end of the Medication Guide for a list of ingredients), are allergic to cow’s milk protein, had an allergic reaction to any other botulinum toxin product, such as Myobloc® or Botox®, or have a skin infection at the planned injection site. The dose of Dysport is not the same as the dose of any other botulinum toxin product. The dose of Dysport cannot be compared to the dose of any other botulinum toxin product you may have used. Dysport may not be right for you if: you have surgical changes to your face, very weak muscles in the treatment area, your face looks very different from side to side, the injection site is inflamed, you have droopy eyelids or sagging eyelid folds, deep facial scars, thick oily skin, or if your wrinkles can’t be smoothed by spreading them apart. Tell your doctor about all your medical conditions, including if you have: a disease that affects your muscles and nerves (such as amyotrophic lateral sclerosis [ALS or Lou Gehrig’s disease], myasthenia gravis, or Lambert-Eaton syndrome), allergies to any botulinum toxin product or had any side effect from any botulinum toxin product in the past, a breathing problem (such as asthma or emphysema), swallowing problems, bleeding problems, diabetes, or a slow heart beat or other problem with your heart rate or rhythm, plans to have surgery, had surgery on your face, weakness of your forehead muscles (such as trouble raising your eyebrows), drooping eyelids, or any other change in the way your face normally looks. Patients with a disease that affects muscles and nerves who are treated with typical doses of Dysport may have a higher risk of serious side effects, including severe swallowing and breathing problems. Human Albumin This product contains albumin taken from human plasma. Steps taken during donor screening and product manufacturing processes make the risk of spreading viral diseases extremely rare. In theory, there is also an extremely rare risk of contracting Creutzfeldt-Jakob disease (CJD). No cases of spread of viral diseases or CJD have ever been reported for albumin.

Allergic Reaction to Injecting in the Skin It is not known if an allergic reaction can be caused by injecting Dysport into the skin. The safety of treating excessive sweating with Dysport is not known. Minus the Holiday Guilt By Michelle Welliver

I

have lived with a sweet tooth all of my life and grew up thinking dessert was a food group. As an adult, I have learned that I might not be able to squeeze into that cute little party dress, if I indulge in every sugary treat I can get my hands on this time of year. So, to satisfy all of these sweet cravings and still feel slim and sexy at the next holiday function, I devised some recipes that are “secretly skinny” but delicious enough to share at your next gathering. Feel free to enjoy some of your favorite holiday flavors, without all the guilt, and don’t be surprised when your girlfriends are begging to know what your secret is!

Simply Skinny Eggnog Fudge 1 c. light eggnog 2 tbsp. butter ½ c. white chocolate chips 2 c. granulated sugar

Line an 8” square baking pan with foil spray with non-stick cooking spray. Set aside. In a large pot, combine the eggnog, white chocolate chips, butter, and sugar. Cook over medium-low heat, stirring constantly, until the white chocolate and butter has melted. Bring to a boil, cook for another 10-15 minutes, stirring frequently, until a small amount becomes moldable when dropped in a glass of cold water. Remove from the heat and the burner, allowing the hot mixture to sit undisturbed for 6 minutes. After 6 minutes, stir vigorously until the mixture

becomes thick and creamy and lightens in color. Spread into prepared pan, and allow the fudge to cool to room temperature before slicing into 1” squares.

Guilt-Free Glazed Pumpkin Cake Pumpkin Cake 1 -15 ounce can 100% pumpkin- puree 1 box of spice cake mix

Preheat oven to 350°. In a large bowl, mix cake mix and pumpkin, beating on medium speed for 2 minutes. Pour batter into a greased 7 x 11 inch baking dish. Bake for about 28 minutes or until toothpick


comes out clean. Cool completely before topping with glaze. Serves 8.

Combine diced apples, lemon juice, brown sugar, and apple pie spice in a medium pot.

Glaze

Cook over low-medium heat until the juice from the apples begins to boil.

1 cup powdered sugar 2 T. apple cider 1/2 t. pumpkin pie spice

Dissolve cornstarch in water in a separate bowl, and stir into apple mixture.

In a small bowl blend all ingredients until smooth. Pour over the cake, letting drizzle down the sides. Let set before covering.

Bring to a boil again and allow the mixture to thicken.

Cure That Craving Apple Pie Dip

Finish by adding caramel and pour into a serving dish. Serve with cinnamon flavored pita chips or low fat graham crackers.

Good Grief ! Holiday Wrapping Time

o to enough holiday parties and you’ll meet all kinds of people. A few nights ago, I met my polar opposite. “There’s nothing I like to do more,” she told me, “than wrap presents.”

Good grief. Someone who likes to wrap presents? Someone who’s actually good at it? I could feel myself getting hot & bothered. Either that, or I’d had too much to drink and had begun to feel dizzy. It was a lot better for my husband and me, a/k/a the wrapping incompetents, when our kids were young. We could drag them to birthday and holiday parties with shoddily wrapped presents — the bow lopsided and clearly ripped off some other package, the paper gaping at one end and folded over several times on the other — and kind of pretend the kids had wrapped it with their own grubby little hands. Which would explain other problems: such as the occasional grease stain when something had sat in our kitchen more than five minutes, or a “creative” combining of two different kinds of wrapping paper when supplies were low, or use of the Sunday funny papers. You get the point. We don’t care a lot about this kind of stuff. Which is why I always get a heavy case of heartburn when I read about somebody like that dreadful, obnoxious Martha Stewart, who makes her own gift-wrapping paper and pulls together packages that look like a work of art. Packages so gorgeous you don’t want to disturb them by unwrapping them. Believe me, nobody ever has that problem with any of our packages. They’re usually halfway unwrapped to begin with; if the tape is hanging on well, the overall effect is usually so pathetic it screams, “Unwrap me! Can’t you see I’m ashamed of myself?” For people like us, the invention of the gift bag was a present from heaven. More recently, I’m pleased to note that the advent of the green environmental movement has made wrapping packages unfashionable and ecologically taboo. O, happy day! What a holiday present for people like us! You’ll see us coming to a gathering with a present that isn’t even wrapped and, instead of being disgusted, you’ll be enchanted and bursting with admiration. You won’t think we’re slobs the way people have for decades. You’ll think we care too much about the environment to try to impress you in a superficial way. To me, there’s a lesson here. Just keep on doing what you’re doing and, eventually, if you wait long enough and have absolutely no pride, you may suddenly turn fashionable. Maybe you like to wrap, but I’m saving the earth.

n today’s world, we are all concerned with improving our quality of life, as well as looking and feeling our best. In order to accomplish this, some of us would like to lose the excess weight we have gained over time through lack of activity, genetics, or possibly the after effects of pregnancy and childbirth. For some patients, this is achieved through diet and exercise programs, lifestyle changes, or quite possibly weight loss surgery. Once patients have lost the weight and reached their goal, they are often left with large amounts of hanging skin from the upper arms, breasts, abdomen and thighs. Unfortunately, further dieting and exercise will not help the skin retract back because the skin and underlying tissues have been permanently stretched from weight fluctuations. It can be difficult to find clothes that fit properly, since the excess folds of skin can be challenging to disguise. Some patients even experience rashes within the folds that can be a nuisance as well. Many patients find these issues very distressing after they have worked so hard to lose the weight. These patients have the tremendous psychological burden of looking into the mirror and seeing all of this extra skin that’s hanging off of them, and no matter how much weight they lose, a lot of them still see themselves as heavy because of it. On the one hand they are thrilled to have lost so much weight, but they are trading one dilemma for another. While the following is an actual testimonial from one of my patients, it is a sentiment that I find is commonly held by many men and women who

want a more complete and aesthetically pleasing transformation: “I was overweight for much of my life until about 8 years ago when I decided to make a change. I lost over 100 pounds, but unfortunately that left my 5’2 frame with lots of excess skin. It didn’t matter how much I worked out or lifted, there was simply nothing I could do about the skin that was left over. It was so depressing because although I had worked so hard to lose all of the weight, now I still looked in the mirror and didn’t like what I saw. I basically traded in one batch of insecurities and collected another. So that is when I decided to do something about it.” These issues can be addressed through surgical body contouring. Liposuction can improve the contour of your flanks, hips, chest, thighs, neck, upper arms and knees, and can be laser assisted for the added effect of skin shrinking and tightening. The “tummy tuck” or abdominoplasty, removes stretch marks as well as loose and excess skin in the lower abdomen below the belly button, while strengthening and tightening the core abdominal “six pack” muscles. able when you need them. Here are a few tips to make sure that you enter every party with an LBD that’s perfectly suited for the occasion. First, determine what your budget is. Depending on your lifestyle, and how often you will be wearing the LBD this year, go for classic since it will remain in style for awhile. If you are on a budget, look for Jersey dresses which are less expensive, usually under $300, but have a season-less and classic styling. They are easier to dress up or down, layer and accessorize, based on where you are wearing it! A great Jersey dress can be worn with a pair of pumps, pretty shawl, and great earrings. Or, worn with a leather jacket, black tights and boots can make it a funkier, sexy night-time look. Sleeveless dresses are also more versatile than long sleeves because you can wear them all year round. Stick with lengths that are just a little below or above the knee. If you go too short, the dress is not as adaptable to the event. You can always add a higher heel pump or boot, but you cannot add length to an already shorter dress. Go for styles that can be worn with either a strapless or regular bra, so that you can feel comfortable where you are going and never worry about having a wardrobe malfunction. Try on a variety of styles to determine what you feel most attractive in. Find a designer that works and stick with them, because they usually have a consistent fit. 10 Minute Therapy How to Argue & Feel Closer By Sara Cohen

U

nderstanding how to fight fair in a relationship is crucial for the success of any relationship, be it a long one or a new one. When two individuals pick an argument, even if they’re in love with each other, both of them instinctively want to win the argument and prove that they’re right. But in love, depending on how you argue, it can either make both of you the winners or make both of you suffer too. The next time you find yourself in a relationship fight, use these ten steps to fight fair, and come out of an argument as winners who have made their relationship stronger, instead of weaker.

#1 Stay focused on the conflict

It’s easy to stray from the real conflict when you fight in a relationship, especially when you’re on the losing side. You may start arguing about how your partner leaves you alone at a party and drag the fight all the way to how your mate doesn’t help with the chores. Always stick to what upset you. It’s easy to focus on one aspect and solve the issue instead of going all over the place.

#2 Don’t bring up past issues

If you want to know how to fight fair in a relationship, never bring up unnecessary details of the past just to make your partner feel miserable. If your partner almost cheated on you three years

ago, there’s really no need to bring that up just to win an argument or make your partner shut up.

#3 Don’t use a low blow on your partner

Sometimes, when all you see is red because you’re on the losing side, the easiest way to hurt your partner is to hit them where it hurts. By picking apart parts of your lovers character, perhaps telling them how lazy you think they’ve become, you’re definitely tweaking a sensitive nerve. If you want to fight fair, always avoid a low blow that can traumatize your partner for a long time.

#4 Don’t be sarcastic

Sarcasm can hurt, a lot. And it can hurt a lot more when you use it in a lovers’ tiff. And it could also piss your mate off and force them to start low blowing you or bringing up past issues just to get even. And what started off as a misunderstanding can lead to an all out war, and guess what, with no solution.

#5 Listen to each other

Contrary to what most people think, an argument will always help a relationship get better and help both of you understand each other. But that’s only as long as both of you respect each other’s opinions and are willing to listen to each other without cutting across.


#8 Don’t blame your failures on your partner

Were you late for work because your partner didn’t wake you up on time? It’s really your own fault. If you wanted to wake up earlier, try and make an effort yourself. Are you constantly frustrated because you’re a stay-at-home mom or dad and not a restaurateur? Do something about it or make the best of the circumstances you have. Don’t blame someone else for your misfortunes or your shortcomings. If you feel strongly about something, speak about it without losing your cool. It may or may not be either of your faults. Sometimes, it could all just be a big misunderstanding.

#9 Remember why you’re fighting

Most couples always forget the real reason for the conflict. Always remember that both of you are fighting, not to score points or prove dominance, but only because one of you is hurt and wants to be heard. Never forget that.

#6 Don’t raise your voices

Raised voices may have worked in the roman senate, but it’ll never help in a relationship tussle. When one partner raises their voice, it’ll infuriate the other partner who may start yelling too, or perhaps even throw a few things around in frustration. Never make a mountain out of a molehill, and don’t raise your voice unless you have no choice. When you try your best to stay in control and speak with a calm voice, it’ll also help your partner calm down and help both of you have a discussion instead of a fight.

#7 Don’t gloat over your wins

As furious as both of you may be, always hug, kiss and make up after a fight even if you couldn’t come to a conclusion. Don’t lock yourself in a room or storm out of the house. It makes things worse for both of you, and you’re only prolonging something that can end in minutes. Odds are, you may understand each other better and have a happier relationship once the dust settles.

When you want to fight fair in a relationship, you should learn to accept an apology with humility. Tis The Season for the “Winter Itch”

‘T

is the season for falling leaves, the holidays, and family. But also itchy dry skin? Unfortunately, that’s right. The cooler, dryer months leave us susceptible for a condition called asteatotic dermatitis, also known as ‘winter itch’. Have you ever looked down at your lower legs and noticed white flakes on your skin or on the inside of your pant leg? Do you wake up scratching your skin or know which of your kitchen utensils makes the best back scratcher? If so, you may be suffering from this exceedingly common winter dermatitis. It manifests as persistently itchy, dry skin and results in an uncomfortable and aggravating skin disorder. The etiology of asteatotic dermatitis is multifactorial, with numerous identifiable triggers. The underlying issue is a lack of moisture and insult to the integrity in the most superficial layer of the epidermis in our skin. Normally, this layer, called the stratum corneum, works to provide cohesiveness and helps maintain the barrier function of our skin.

When it is compromised, as in winter itch, it leaves the skin susceptible to dryness and infection. As we age our skin loses its ability to hold onto moisture, compromising this protective barrier function. Add that to the extremely low levels of humidity and dry indoor heat experienced during the colder months, and the skin easily becomes dehydrated. Bathing habits are a huge contributor to this condition. A common culprit is the use of harsh soaps that can strip your body of its natural oils and lipids. An easy fix for this is to look for ones with ‘hydrating’ and ‘fragrance free’ on the label, or ones made for sensitive skin. Also you know those long, hot showers that feel so good on a chilly morning? Ditch them! They are a major contributor to dry skin and although it feels better in the shower it will make the associated itch worse. Ideally, showers should be a luke-warm to cool temperature setting and should be less than ten minutes long. Another effective way to help prevent as well as treat exceedingly dry itchy skin is to apply a moisturizing lotion or cream. The best time to apply moisturizer is after showering, when your skin is still slightly


damp. The best lotions for reparative purposes are those that contain ceramides, these help replenish the lipid holding components of your skin and fortify the stratum corneum. Our skin can be a reflection of what is going on internally. Make sure you are hydrating with plenty of water and eating healthy, antioxidant rich foods on a daily basis. Asteatotic dermatitis, is not simply ‘just dry skin’, it’s important to take this condition seriously. Think of skin as your armor, when extreme dryness occurs it is like having chinks in your shield. Your protective barrier is compromised and leaves you open to obtaining infections; both bacterial and viral. The best offense is a good defense. Making small

changes today, like altering your bathing habits, having a humidifier on at night, and adding in an extra glass or two of water a day, are effective ways to prevent the symptoms of asteatotic dermatitis in the future. It is always important to be evaluated by your dermatologist if you think you are suffering from winters’ itch. At times, prescription strength medications may be needed to help with symptom treatment or the symptoms may be associated with a different condition. I certainly didn’t know any of that when I purchased it from the ABC Carpet and Home store but the intonation of the bells has become part of my daily ritual as I attempt to restore balance and centeredness to my busy existence. Early morning in a dark room, it takes me awhile to wake. Bare feet on a wooden floor, I lean forward to tie back the curtains, and peer into the world outside. I crack the window and feel a flood of cool air brush against my face, and it is here in this moment that I make my intention for the day, sealing the unspoken words with the chime of a Tingsha Cymbal. All before my morning coffee. With so many distractions vying for our attention, I think we can all agree that it’s a bit of a challenge to create a calming center anywhere, especially in our own mind. There are some days that I feel achieving this balance has become the holy grail of our era, and yet, I know that with a little personal effort it’s not an impossibility either. The road to enlightenment begins simply. One must literally turn off the world, and then tune back into self. There are three things to keep in mind when integrating (or for some, bringing back) the Zen into life: Mindfully commit to your intention. Whether for ten minutes or 90 prepare your heart and mind to be present in this moment, leaving everything nonessential behind. Things like the laundry, your list of

to do items, the pile of papers that need to be filed, all those unanswered emails —they’ll all keep. Put the computer to sleep, ask your partner to mind the kids and the dog, turn off your phone and step into it. Now that’s done, it’s time to clear the air. I like to use leaves from the sage, rosemary or lavender bushes in my backyard but if you don’t have dried herbs handy you can use a favorite scented candle. Light the leaves (or candle) in a flame-proof dish and walk through each room, allowing the scent to permeate the space. Here’s where you recite your intention to yourself, and then make it known in your meditative space, repeating it to yourself until you’ve made a complete circuit of the room. Quiet your mind. Choose an activity that has a calming effect on your mind. For some this could be yoga, for others a few laps in the pool. It could be soaking in a tub, taking a bubble bath or simply nestling into a comfy chair with a favorite book. Whatever it is, find the rhythm that brings you two steps slower than your norm and allow yourself to steep in it, and relax. arole Algier and her two sisters work together, baking and selling specialty cakes. Their days, she says, are just filled with joy. Their business, Full Spirited FlavoursTM, is a second career for each of them and something the closeknit trio believes was destined to be. But that’s not all these women do together. In the past two years, all three have had double knee replacements, Carole this past June. All had surgery at Pottstown Memorial Medical Center, by the same orthopedic surgeon, Nikos Pavlides, MD, of Brandywine Institute of Orthopaedics. And they all stayed in the same room at PMMC’s Center for Orthopedics and Spine. “Dr. Pavlides is so tuned in to you as a patient, and his team at Pottstown is the same way,” says Algier, a Glenmoore resident. “Everyone is on the same page. You feel safe and secure in their hands and so well cared for.” Sister Sue was the first to have surgery, two years ago. Eating for Beauty By Erika Ringwald

W

e do many things to take care of ourselves. Healthy habits can include: working out, getting plenty of sleep, drinking water, abstaining from alcohol, meditating, and eating right. If I were to generalize eating right, it would be eating to maintain a healthy weight and eating a variety of foods to get all the nutrients your body needs. The healthy weight is fairly objective and easy to monitor. Getting all the nutrients we need, well that is a bit harder to monitor and doesn’t provide an aesthetic reward, or so you may think. Having a balanced intake keeps your body healthy and it shows from the inside out.

When a client sits across the desk from me and tells me that they generally eat well, I take a look at their skin. I don’t do this to judge them, but because our skin is actually a good indicator of our diets. Many people think health is merely the number on the scale, but it goes much deeper than that. I am using skin as an example of evaluating how healthy your diet is because, quite frankly, who doesn’t want their skin to glow? The skin you see in the mirror today will be gone and replaced with new skin cells in about a month. In order for the new cells to provide us firmness, elasticity and luminosity, we need some key nutrients; those being vitamins and minerals, essential fatty acids and proteins. There is a saying that if a food has the capability to rot, we should be eating it. Our body is organic and the food we put into it should be “alive” too. I am finding that more and more people are making a concerted effort to take in healthy fats and to eat protein. This is great, but do you know exactly how these things turn into compounds our bodies need? It happens with the help from vitamins and minerals. We get a plethora of nutrients from whole and minimally processed foods. If the majority of your food comes from a bag or a box, most of the nutrients have been destroyed or removed. Enriching our food doesn’t do us any justice. We need vitamins and minerals from their natural form. All the compounds of our food work together, just like a team. Don’t evaluate how balanced and healthy your diet is by the number on the scale or the calories tracked with your cell phone app. Start by taking a look in the mirror. Holiday Travel Tips

oliday travel can be stressful, and like so many, not something I look forward to. But traveling 45 weeks a year for work, has made me a self-proclaimed pro. Below are some of the things I have learned over the years to ease the travel stress, keep your holiday cheer, and maybe even enjoy the travel during the holidays.

when flying, there are often satellite airports close to your destination airport. If you can’t get to your destination airport, ask the airline customer service agent if they can get you into a satellite airport. It may not be Option #1, but it’s much closer than you would have been.

Plan Ahead

Most of those that cross your path this holiday season are embarking on the same type of journey. They are trying to get somewhere that is as important to them as your destination is to you. Remember that no one is out to make your trip harder than it has to be… except Mother Nature, she might be an exception. There are hundreds of people working together to get you and other travelers alike to the final destination safely. Most of the time what is happening is out of their control.

Don’t wait until the last minute. Pick your dates, and are they flexible? If they are, you may have the opportunity to get a better deal, so explore your options and book early! When flying, book your flight five weeks before your trip on Tuesday. That is when fares are the cheapest. Don’t pack in a rush; things will be forgotten. Take the time to determine what you are going to wear and when, to avoid over-packing. Baggage fees are no joke, so the less you pack means more money staying in your pocket. Lastly, don’t forget to manage other areas of your life before leaving. Do you have a pet? If so, pet hotels and kennels fill up quickly over the holidays. Make arrangements for Fido early, and take care of bills that are due while you are away.

Have a Plan B.

Inevitably, things are going to happen. If you are prepared with a “Plan B”, you will be one step ahead of all the other travelers out there. Weather, traffic, and other unforeseen factors can affect when and if we get to our destination. Before you leave, pick a hotel you would prefer to stay at along your route if something should go awry. One thing to remember

Keep it in Perspective

Instead of getting upset, keep the holiday spirit in perspective. Often being friendly and flashing a smile will ease the stress. I have found that the nicer I am to those working to get me where I need to go, the more willing they are to go out of their way to help me. And don’t forget, delays and detours are the perfect opportunity to see and experience new places and new things, or to even make a few friends. Deck the Halls - Simply By Pattie Krukowski

W

hen I think back about celebrating the Holidays in 1970’s South Florida, a myriad of warm memories flood my mind. Canned snow, liberally sprayed, covered all the windows. A faux brick, cardboard fireplace stood proudly in the corner. It came to life when plugged in to allow a round colored slide of red and yellow to spin atop a bulb while fueling a fan that blew tissue flames. My father demanded that individual strands of tinsel be painstakingly hung piece by piece on heavy

boughs weighted by hand-blown glass ornaments inherited from generations past. Needless to say, the first Christmas I spent in New England with all its genuine and simple elegance was charming. Over the years I have made a point of remembering clever ways to create the feel of a New England Christmas in my own home during the Holidays. To achieve this, I believe the key is “less is more”. Cover the mantle with groups of creamy white pottery stuffed with evergreens and eucalyptus on one side with a handful of whitewashed wooden


candlesticks on the other. Clip evergreen sprigs in symmetric pieces and attach five straight ends in the middle with floral wire to create snowflakes and hang from the ceiling in varied heights. Every year we buy a dozen orchids, all of one type, potted rosemary or braided stem poinsettia topiary to give as last minute gifts. We wrap the pots with fabulous ribbon and display the mass together on a table by the front door until they are picked up or delivered. Garland and lights are both such versatile decorating tools. Use garland to dress up a chandelier, drape from curtain rods or wrap a stove hood. Deck the halls; literally. Run garland along a hallway chair rail inviting overnight guests to their rooms upstairs. Solar, pre-lit faux garland makes exterior decorating a breeze. Surround an entrance and cloak the stair risers to a front door. Be the envy of your neighborhood and hire a landscaper to wrap the biggest tree in your yard with exterior lights or simply wrap just the trunks yourself. A house around the corner from mine wraps just the trunks of about ten trees in velvety red LED lights. The effect is stunning. Feast on the soft glow of candlelight during Hanukah. Design your own Menorah from a glass trifle bowl scattered with blue marbles and candles or gather together a collection of mercury glass candlesticks in a variety of shapes and sizes. Clear jars and vases full of silver and blue wrapped treats encourage guests to celebrate. Cobalt blue martini glasses replace wine stems. A number of sparkling and spring waters such as Ty Nant and Saratoga Spring water sport blue glass bottles and are the perfect accent at the bar. Choose a color scheme and stick with it. Last year all of our packages were wrapped in black and silver. Packages from our family were wrapped with black and silver paisley while those from Santa were wrapped in black and silver stripes. The same paper covered the buffet table and was strewn with every crystal votive I could find. Silver ornaments and fresh white lilies in floral water tubes nestled among evergreen trimmings laid down the center of the white clothed dining room table. A slew of offwhite velvet ribbons were hand tied on our fresh, too-big-for-our-home tree. The tree stand fit into a large shiny black lacquered plantation planter that we topped with floral sheet moss. Even my eight-

year-old son’s Betta fish tank was dressed appropriately. Black and white marbles rested on the bottom of a two foot tall, clear glass vase placed atop an antique silver tray. This year’s tree design has been chosen. I was cruising through the website www.amazing-christmas-ideas.com and fell in love with the tree that is being trimmed by doves. Doves suspended from the ceiling with fishing line hold ornaments and strands of beads and appear to be the ones actually doing the decorating. These whimsical little birds could be used on a mantle to decorate a Menorah with strands of pearls or perhaps look as though delivering a special gift from Tiffany’s. ‘Tis the season to be jolly and the choice of possibilities to create your own special holiday environment are endless. Remember to keep it simple yet focus on the small details. Strike the harp and join the chorus by choosing family favorite holiday songs to get you in the mood. I grew up listening to Barbara Streisand: A Christmas Album from 1967 and I play it over and over again from Thanksgiving until New Year’s. Cinnamon scented pinecones are piled high in buckets by the fireplace while cookie scented candles infuse the house with cheer. Enjoy every minute and always focus on the real meaning of the holidays while you relax in the atmosphere you created. he winter can be brutal on your skin. Dry heat from radiators zaps moisture, and strong winter winds can lead to increased irritation and redness. Tight dry skin and chapped lips don’t have to be a way of life during the colder months though less is more. Don’t overcomplicate your skin care routine. And just because it’s cold outside, doesn’t mean you shouldn’t wear sunscreen. Say Goodbye To The Winter Skin Blues Women should use a mild face cleanser to wash their face and avoid taking very hot showers. Harsh cleansers and hot water will dry out the skin.” She adds, “Women should also look for oil-based moisturizers with vitamins C and E. Oil based moisturizers rich in vitamins are better at sealing in moisture and keeping skin hydrated than water based products. However, if you tend to have oily or problem skin, use a lighter lotion.” The Sun Still Shines In Winter Always use a moisturizer with SPF, and the winter is no exception. It’s important to protect your skin from the sun. Believe it or not, the sun can be even stronger and harsher on the skin in the winter than it is during the summer. Three Things Everyone Should Have In The Medicine Cabinet Aloe vera pure gel for any burns, cuts or rashes. Vitamin E capsules. Break open a capsule and

apply the vitamin E to any scar or cut to help it heal quickly. And coconut oil. It’s very underrated and it’s perfect for winter because it’s so moisturizing. You can massage it onto your hands and scalp. Keep Hydrated It’s most important to keep your skin hydrated… Drink a lot of water and avoid soda and alcohol. Also, always keep great hand creams and my lip balm close by. It’s important to keep in mind that overall, people drink less water in the winter, and therefore are less hydrated, so drink up, even if you aren’t thirsty. Different Season, Different Routine Your skin changes each season with the weather, so you need to switch your skin care products accordingly. Most people use the same type of lotion all year, but women should choose a light lotion in the summer and combat dry skin in the winter with a heavier lotion. What To Watch For People are starting to develop products with stem cells from plants, flowers, fruits and vegetables. These ingredients will be extremely effective in skin care products. Who knows, maybe dry winter skin will be a thing of the past in the near future.
