Choose a retirement community Serving senior citizens is a rewarding and fun career, according to those in the profession, and one that can be done for generations of men and women who have contributed so much for local communities. When making a decision for senior living and long term care, making the right decision is a crucial step. Some things to consider are: • Finding the right fit. • Safety. An effective, attentive and considerate team of professionals will ensure quality caregiving and meeting needs for a client and their families. • Comfort. A resident will want a place to call home for years to come. Be sure the choice you make for senior and retirement living is one that is comfortable, warm and welcoming. Be sure that any long term living facility encompasses the community outside its walls as well. Residents will enjoy being involved in

local activities on college campuses and with county senior centers for example. Highlights of a “good” facility include 24/7 services, easy accessibility to staff and security and an overall sense of compassion and relationship building on behalf of staff members and residents alike. One mistake many people make in choosing the move to a retirement/senior living community is waiting too late. Pick a primary care physician Primary care physicians often regard themselves as part of a patient’s family, as when committed to caring for members of a community, often providing quality healthcare at discounted rates to an underserved population. It is important for patients to consider the following when choosing the right provider for primary care, according to industry professionals: • Can they provide you and your family with a Patient Centered Medical Home? At the heart of the Patient Centered Medical Home is the relationship between the patient and his or her health care team led by your doctor or provider of choice. If you are currently not enrolled in an insurance plan, ask your health care provider if they can help. Some providers employ uniquely qualified case managers for the Medicaid Outstation worker program, which was established in 1999 in Arkansas. Case managers can help if you have questions with the following: a. Applying for Medicaid b. Applying for Social Security c. Medicare Advantage Plans d. Medicare Part D e. Low Income Subsidy f. Community Resources • And last but not least, do you genuinely like and trust this provider? After selecting a provider that suits your requirements, schedule an appointment to meet them. During your visit,

ask yourself a few questions: Does the doctor listen to you without interrupting? Does she fully answer your questions? Does he explain your diagnosis and treatment, and specify a date for a follow-up visit? You are more likely to seek treatment when you like and trust your primary care provider. Primary care practices these days often will offer a variety of assistance programs to help patients and their families with even more services, including transportation to medical appointments, pediatric care, extended night and weekend hours, electronic patient portals and more. It is always a good idea to ask questions and learn as much as possible about a primary care practice and its physicians and policies before making decisions for your family.

Find the right architect Designing a building is a complicated process, and finding the right architect is the key to satisfaction with the final project. Be sure to research local architects who are educated and trained and know to ask the right questions of a client to determine their needs, wants and vision. The architect and team will take that program information and design a facility that meets those requirements in a functional and attractive way that includes comfort, technology and productivity. This should be as specific as selecting the right heating and cooling system that satisfies the budget and the desire for comfort. Options should be presented, and do not rule out the need for compromise in many cases, which requires a good relationship between the client and architect from the beginning. The architect should act as a representative and advocate in the construction process, leading the client through the processes. services. Hiring a local architectural firm is something every client should consider. A local firm is on many occasions able to devote more time to local projects, even driving by projects to and from work. This offers credibility and responsibility on behalf of a local architectural team as well. Too many people select an architect based on the fee quote alone. A low fee usually means you will get services that reflect that low fee. It’s an old saying but “you get what you pay for”. When selecting an architect, people should do a little research. Find out exactly what services are included in that fee. Determine if the architect has a good reputation. Find out if the firm designs buildings that reflect the client’s needs rather than the architect’s wants. Ask around to determine if the architect has a history of bringing projects in budget. Ask questions such as: Is the architect easy to work with? Is he responsive? Is he available when you need him? Paying your architect an extra percentage point in his fee could bring

big benefits to your budget and schedule. I always recommend that people look for an architect with a good track record. Does he have a string of happy clients behind him? Architectural software continues to evolve as it does in all fields. Design plans are now easily entered into a computer and the software sees it in three dimensions. It keeps up with size, finishes and shapes so architects can access all kinds of information that used to require a lot of effort. For example, if a designer enters a brick wall on the side of a building, the software can provide the number and texture of the bricks as well give the client an image of the way the wall looks. Realistic looking, three-dimensional, computer generated images have become the standard in the industry. The days are gone when a client did not know what he was getting until the building was completed. Local firms are keeping abreast of the latest technology, and as Faulkner County continues to grow, the architectural industry will keep up.

Urgent care should not be considered a replacement for ongoing primary care, nor a substitute for emergency care, which remains the best option for emergency situations. With no appointment needed and convenient hours, urgent care centers offer peace of mind for patients that comes with knowing they have access to quality medical care when their doctor’s office may not be open.

Maximize your hearing Hearing loss is one of the most common chronic conditions in The United States and affects more than 10 million Americans over the age of 65. According to the National Center For Health Statistics it is more common than cataracts, diabetes and orthopedic problems. Hearing loss may be acquired by heredity, the normal aging process and exposure to loud sounds. All of these conditions create cochlear nerve damage and are treated through the use of hearing aids. Selecting collision repair Unfortunately, accidents do happen, and sometimes they involve motor vehicles. When choosing the professional you would like to use, there are many things to consider. You have invested a lot of money and time into your vehicle, so it is imperative to do your homework and find the shop that will best fit your needs. Also remember that your vehicle carries the most important people in your life — your family — throughout the world each day. A decision on who will fix your vehicle is not to be taken lightly. First, get a police report, this is the only way to “prove” damage has occurred. Then you can decide if you need to involve the insurance company. Second, once the initial police reports and insurance claims are filed, it’s time to choose a collision repair shop. We suggest stopping by and meeting the staffsee who you will be dealing with. It is always easier if the repair shop and the customer have the same in mind. Some insurance companies have lists of shops they recommend to do collision repair on their insured vehicles. determined which shops to approve and if they guarantee their work. Also ask if the guarantees will not exist if you use a shop that is NOT one they recommend. By obtaining a list, you may have fewer shops to investigate, hopefully meaning you’ll be on the road more quickly. More importantly, it is against the law in Arkansas for an insurance company to tell a customer they have to use a repair facility just because that company considers them a preferred shop. If the insured says they want ABC Collision Center to fix their vehicle, the insurance company has no choice but to work with that person and their shop of choice. Third, be sure to ask any questions that you may have: • Do you offer a warranty? • I will need a rental vehicle to drive, what do you suggest? • How Long will repairs take? • I am uncomfortable driving, or my car is not drivable, can you pick it up or have it towed? • I have an estimate from my insurance company, why is it different from yours? Don’t hesitate to ask for a tour of the facility! Get estimates from each shop, and get the estimate in writing to avoid

misunderstandings later. This is important for you, the customer, so you aren’t charged for unexpected work, and also for the auto shop, so they aren’t bombarded with unexpected complaints about the outcome of the repairs. Three or four estimates should be sufficient to find a reputable repair shop. Always ask if there are repairs that need to be done to the car that will not be covered by your insurance agency. If so, get a separate estimate for those so you know what you may need to spend out of pocket. Never make your decision based on price — just because a shop gives you the lowest estimate doesn’t necessarily mean it is the best choice for you. Make your choice based on your visit to the shop, recommendations from friends and even the advice of your insurance agent. Ask co-workers and friends what shop they use; why they use that particular shop; and will they use it again if needed. But how do you find an orthopaedic surgeon when there isn’t an emergency and you need a specialist to check out that sore knee or chronic shoulder pain? Here’s a look at what orthopaedists do and how to choose the one that’s right for you. According to the American Association of Orthopaedic Surgeons, “an orthopaedic surgeon is a physician devoted to the diagnosis, treatment, prevention and rehabilitation of injuries, disorders and diseases of the body’s musculoskeletal system. This system includes bones, joints, ligaments, muscles, nerves and tendons. While orthopaedic surgeons are familiar with all aspects of the musculoskeletal system, many orthopaedists specialize in certain areas, such as the foot and ankle, hand, shoulder and elbow, spine, hip or knee. Orthopaedic surgeons may also choose to focus on specific fields like pediatrics, trauma, reconstructive surgery, oncology (bone tumors) or sports medicine.” Some orthopaedists may specialize in several areas and may collaborate with other specialists, such as neurosurgeons or rheumatologists, in caring for patients. There are many musculoskeletal conditions that can be treated without surgery through the use of medication, exercise and other rehabilitative or alternative therapies. Things to Consider when Choosing an Orthopaedic Surgeon One of the first things to consider is the doctor’s level of education. An orthopedist may have completed up to 14 years of formal education, including four years of undergraduate education, four years in medical school, five years of concentrated study in an orthopaedic residency at a major medical center, and an additional year of specialty training. After becoming licensed to practice medicine, an orthopaedic surgeon completes board certification. To become board certified, an orthopaedic surgeon must undergo a peer-review process, and then demonstrate his/her expertise in orthopaedics by passing both oral and written examinations given by the American Board of Orthopaedic Surgery. It is important to visit orthopaedic surgeons who are either board certified or in the process of becoming certified. Because orthopaedic surgeons complete a rigorous re-certification process every 10 years, they spend many hours studying and attending continuing medical education courses to maintain current orthopaedic knowledge and skills. Next, make sure the orthopaedist you choose has experience in your injury or condition. Chances are your primary care physician has already diagnosed your problem and has made the referral, but when you contact the orthopaedic clinic make sure to ask for the orthopaedist with experience for your problem. If your primary care physician hasn’t made a referral for you, ask for a recommendation of the best local orthopaedists. Talk to your friends. For common conditions, such as arthritis, ACL surgery, carpal tunnel, etc., you are likely to have co-workers, friends or family who know a good orthopaedist and will have firsthand experience. In addition, an excellent source of information is the AAOS website: www.aaos.org. Once you have made a selection, think about what you want to ask the doctor. During the course of an appointment, an orthopaedist will obtain a complete medical history, prescription drug/medication inventory, and description of the problem. Orthopaedists are skilled in the diagnosis of an inju-

ry or disorder, prescribing treatment, recommending rehabilitation, and offering information on prevention. But, it’s still important to ask questions. Consider bringing a friend or family member with you to help you remember questions. Don’t be afraid to ask about your doctor’s credentials and experience. Here are some questions recommended by the American Association of Hip and Knee Surgeons: • Do you have written materials or videotapes about this surgery that I can review? • How much improvement can I expect from this surgery? • What is your experience doing this type of surgery, and how many have you done? • Are you board certified? • What are the risks involved? How likely are they? • What type of anesthesia will be used? What are the risks? • What type of implant will be used? What is the track record for this type of prosthesis? • Will I have to stop taking any of my medications before surgery? • What options are available to avoid a blood transfusion? • How long will I have to stay in the hospital? • How much pain is normal to expect and how long will it last? Will I receive medication for the pain? • When will I start physical therapy? Will I need home or outpatient therapy? • Will I need to arrange for some assistance at home? If so, for how long? • What limits will there be on activities, such as driving, bathing, climbing stairs, eating, etc.? • How long will I need to be off work? • How often will I need to return for follow-up visits? • What complications, if any, can arise after surgery? What are the signs to look for? Look for an experienced, well-regarded surgeon. Meet your orthopaedist to make sure you can work well with him/her. Trust your instincts. If a meeting with a doctor does not go well, it doesn’t mean he/she is a bad doctor or you are a bad patient. It simply may mean the two of you are not a good fit.

Cost is based on the needs of the individual but, because home care requires a physician’s order, services are usually covered by private health insurance, Medicare or Medicaid. Once you have determined whether anyone in your family needs specialized care, you can begin to look at what each facility you are considering offers and whether it meets or exceeds your short and long-term needs. Do you have a PCP? If you have a Primary Care Physician (PCP), ask for their input on which hospitals offer the best care, especially if you have certain health care needs that may require a multi-disciplinary approach. Also, learn where your PCP has admitting privileges. Your doctor can help recommend specialists or surgeons at the hospital with which they are most familiar and comfortable. It’s best to establish this information now, and avoid having to make a hasty decision later or in the event of an emergency. Regardless of whether you have a PCP, learn whether the hospital you are researching employs hospitalists. Hospitalists are physicians who are responsible

for the overall care of patients while they are hospitalized. If you have a PCP, the hospitalist will communicate your condition and follow-up treatment plan with your primary care doctor. Hospitalists are available to patients 24/7 while the patient is hospitalized, a practice that is especially beneficial since primary care physicians must divide their time between private practice and the hospital. The hospitalist serves as a main point of contact for patients and works with every member of an individual’s medical care team, from surgeons, to nurses, to therapists and more. Comprehensive care In keeping with the “one place does it all” concept, explore everything the hospitals on your short list provide. When surgery, therapy and follow-up care can all take place within the same medical center, and with health care professionals who collaborate as a medical team, it is often much more convenient for the patient, and outcomes are better, when there is continuity of care. Treatment after surgery, after an injury, or for a chronic disease doesn’t always end when a patient is discharged. Follow-up care such as rehabilitation in the form of physical therapy, pediatric care after labor and delivery, or behavioral issues for example, require longerterm outpatient or sometimes inpatient care. When that care is offered at the same medical facility, not only is it more convenient, it benefits the patient in the long run. Convenience Convenience is a consideration that is two-fold. First, does the hospital you are considering offer care in enough disciplines to provide your family with all of the health care options you need within

that particular hospital? Secondly, how important to you is it that your hospital is close to home and family? It’s not uncommon for local hospitals to offer all of the services found in major metropolitan medical centers, but don’t assume that so-called satellite hospitals in your community offer comprehensive care. Satellite hospitals may offer care in a particular specialty, but they may not offer comprehensive care in that area. Be sure to inquire. The convenience of having access to comprehensive health care that is just miles from home is appealing. Plus, local hospitals employ members of the community so it’s sometimes easier to form beneficial relationships with the physicians, nurses and staff who will be treating you and your family. Insurance Healthcare is expensive, so it’s important that you know how your health insurance factors into your hospital of choice. How much does your plan cover at the different hospitals and, if there is a wide margin of how much your insurance will cover between hospitals you are considering, does the quality of health care they offer make up for any additional cost you would be paying? Quality and reputation Patient outcomes and safety are always the priority at hospitals. There are a number of websites that list hospitals’ ratings on safety, quality, patient outcomes, patient feedback and other key metrics. It’s always a good idea to ask friends and colleagues who have been treated at a particular hospital for their feedback on their personal experience.
