Experiencing Vertigo or Dizziness?

Vertigo, dizziness and balance problems can be debilitating and life altering.

Unfortunately, these common symptoms can steal a person’s confidence and independence. According to the Vestibular Disorders Association, dizziness is the most common complaint in the primary care setting for patients over age 70. These issues, which can affect people of any age, are often under-diagnosed or misdiagnosed. The average dizzy patient sees approximately 4.5 physicians before getting a diagnosis, let alone proper treatment (Epley & Weinberg, 1999).

There are many causes of dizziness, vertigo and imbalance. However, the most common cause of vertigo is a condition called benign paroxysmal positional vertigo (BPPV). BPPV may occur at any age, impact a patient for years, and will affect approximately 50 percent of individuals at some point.

BPPV is characterized by sudden episodes of vertigo (room-spinning dizziness) when an individual lies down in bed, rolls over, rises, bends at the waist, and/or looks up or down. Attacks are intense and typically short (less than one minute).

However, people with this condition may also experience a more general sensation of lightheadedness and imbalance. According to current research, this condition can double the rate of depression and cause a two-fold increase in falls.

BPPV is caused by microscopic calcium particles that occur naturally in the inner ear. These particles can migrate into one of the ear’s semi-circular canals (another inner ear structure), where they do not belong. When this happens, continued on page 7

the canals become sensitive to gravity, which results in a sensation of dizziness with positional changes.

The gold standard treatment for BPPV is both simple and effective. It involves a noninvasive repositioning procedure to clear the particles out of the ear’s semi-circular canals to deposit them back into the part of the inner ear where they belong. Research has shown a high success rate for treatment. In fact, the author has treated nearly 5,000 patients with this condition with a success rate approaching 99.6 percent.

In addition to BPPV, there are a multitude of conditions that cause dizziness and balance difficulties. Fortunately, a specialist can complete comprehensive and advanced testing, leading to an accurate diagnosis and treatment. The key to getting help is an accurate diagnosis.

In summary, BPPV is a very common and often under-diagnosed condition.

BPPV has a number of very troubling symptoms, which deserve attention.

Unfortunately, the dizzy patient has traditionally been evaluated by a number of doctors without receiving an accurate diagnosis. The high incidence of BPPV and great success of treatment support the need for referral to a specialist in dizziness/vertigo. A specialist can help find an accurate diagnosis and recommend treatment, which will help get patients the help they need sooner, without unneeded referrals and testing.

What Can You Do About It? The good news is you can take steps to stay healthy. Here are some tips on how to achieve your greatest wealth…your health! You can reduce your risk of developing a chronic disease by 80 percent by implementing these five lifestyle behaviors: 1. Consume a whole-food, mostly plant-based diet While there are many types of diets on the market, one thing will always hold true: the diet should have an emphasis on plant-based foods. It can lower cholesterol, blood pressure and blood sugar; prevent and reverse heart disease and obesity; lower your risk of cancer and diabetes; slow the progression of certain types of cancer; improve symptoms of rheumatoid arthritis; and improve overall quality of life. Try to make 80 percent of your intake from vegetables, fruits, seeds, nuts, legumes and whole grains. Cut your intake and minimize your consumption of meat (red meat, pork, chicken, fish), dairy products, and eggs, as well as sugar and highly refined processed foods.

2. Effectively manage stress You cannot always avoid stress, but you can manage it before it damages your health, relationships or quality of your life. When stress hits, sleep is sacrificed, convenient unhealthy food/drink choices are the go-to, and exercise is the last on your to-do list. However, a healthy diet, exercise and adequate sleep will help you better manage stress and increase your ability to cope with challenges.

3. Get adequate sleep Ideally adults need 7 to 8 hours of good quality sleep. This means you should feel energized upon wakening. Poor sleep habits can interrupt a delicate cycle and make you more moody and can make it difficult to focus during the day.

It can also make you more prone to illness and shorten your life expectancy.

4. Avoid toxins Smoking, drinking too much alcohol and consuming heavily processed foods full of chemicals puts a toxic load on the body and contributes to disease.

5. Exercise This lifestyle behavior has the greatest impact on the others as it can help you sleep, detoxify and manage stress. Always start with low impact exercise such as walking when you are just starting an exercise program.

Do I Really Need a Primary Care Doctor?

If you’re healthy overall, or if you already visit specialists for different medical conditions, you might not have a primary care provider. But while skipping primary care might seem like it saves time, it can actually lead to stress, confusion and complications in the long run.

What is a primary care provider (PCP)?

A primary care provider may be a doctor, nurse practitioner or physician’s assistant who acts as your go-to person for general health needs. You can call on your PCP for help with most non-emergency medical issues.

At the same time, your PCP may act as your own personal health coach.

communicate with specialists to make sure each member of your team is on the same page. Doing so can help prevent: • Medication errors: Keeping track of your medication list and communicating with your other doctors lets your PCP recognize and fix or prevent drug interactions.

• Unnecessary testing: Since your PCP probably knows you, he or she can notify other providers if you’ve already had a test that they’ve prescribed.

• Miscommunications between facilities: If you’re being transferred between medical facilities, your PCP can notify your new doctors about your background and any recent changes to your care plan.

Why not just go to an urgent care center?

Many providers offer same day appointments just for those urgent needs of their patients. If it’s after office hours, an initial call to your PCP’s office is a good place to start – your health care provider will let you know if a trip to an urgent care is the best course of action.

By understanding the full content of any new issues, your PCP can make smart, individualized decisions about your care. “Your PCP is responsible for multiple facets of your care over your lifetime. They ultimately put all of these different pieces together to guide you towards better health,” says Patel.

Get comfortable. If you're tense on the exam table, tell the doctor or nurse what would help you relax. If you'd like a nurse, friend, or family member there, let the doctor know.

Be honest. Topics like sex or illegal drug use may be tough to discuss, but your doctor needs to know about them to give you the right care. Keep in mind, your doctor cannot discuss anything you confide without your permission. Having a friend or family member there may make it easier.

Take time. The better you're able to explain your concerns, the better your doctor can advise you about them. Stay focused, but don't rush through a symptom if giving more information could be more helpful for your doctor.

Ask follow-up questions. If you don't understand your diagnosis or treatment, speak up. If you don't feel that you can follow through with a treatment, find out if there are other options. If you need someone who speaks your preferred language, let your doctor know.

Take notes. Or bring along a friend or family member to take notes or ask questions.

Find out how to keep in touch. Can you email your doctor? Or is it better to leave a phone message? Before you leave, ask for the best way to reach the doctor if you have questions about what you've discussed.

Hospitals can choose to be surveyed by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) to make sure they meet certain quality standards. The standards address the quality of staff and equipment, and—most recently—the hospital’s success in treating and curing patients. She had previously been an extern at the Lee’s Summit office while completing her training at the Michigan College of Optometry. She continued her training with Indian Health Services to help treat and prevent diabetic eye diseases and then became the principle doctor behind the development of a Scleral Lens and Dry Eye Clinic in Almagordo, NM.
