Vertigo or Dizziness?

Vertigo, dizziness and balance problems can be debilitating and life altering. Unfortunately, these common symptoms can steal a person’s confidence and independence. According to the Vestibular Disorder Association, dizziness is the most common complaint in primary care over age 70. These issues, which can affect people of any age, are often under-diagnosed or misdiagnosed. The average dizziness patient sees approximately 4.5 physicians before getting a diagnosis, let alone proper treatment (Epley & Weinberg, 1999). There so many causes of dizziness, vertigo and imbalance. However, the most common cause of vertigo is a condition called benign paroxysmal positional vertigo (BPPV). BPPV may occur at any age, impact a patient for years and will affect approximately 50 percent of individuals at some point.

BPPV is characterized by sudden episodes of vertigo (room-spinning dizziness) when an individual lies down in bed, rolls over, rises, bends at the waist, and/ or looks up or down. Attacks are intense and typically short (less than one minute). However, people with this condition may also experience a more general sensation of lightheadedness and imbalance. According to current research, this condition can double the rate of depression and cause a twofold increase in falls.

BPPV is caused by microscopic calcium particles that occur naturally in the inner ear. These particles can migrate into one of the ear’s semi-circular canals (another inner ear structure), where they do no belong. When this happens, the canals become sensitive to gravity, which results in a sensation of dizziness with positional changes.

The gold standard treatment for BPPV is both simple and effective. It involves a noninvasive repositioning procedure to clear the particles out of the ear’s semi-circular canals to deposit them back into the part of the inner ear where they belong. Research has shown a high success rate for treatment. In fact, the author has treated nearly 5,000 patients with this condition with a success rate approaching 99.6 perfect.

In addition to BPPV, there are a multitude of conditions that cause dizziness and balance difficulties. Fortunately, a specialist can complete comprehensive and advanced testing, leading to an accurate diagnosis and treatment. The key to getting help is an accurate diagnosis.

In summary, BPPV is a very common and often under-diagnosed condition.

BPPV has a number of very troubling symptoms, which deserve attention.

Unfortunately, the dizzy patient has traditionally been evaluated by a number of doctors without receiving an accurate diagnosis. The high incidence of BPPV and great success of treatment support the need for referral to a specialist in dizziness/vertigo. A specialist can help find an accurate diagnosis and recommend treatment, which will help get patients the help they need sooner, without unneeded referrals and testing.

Where Should I Go For Medical

Care?

Whether it’s a bump on the head or a really bad cold, when your body suffers a blow, all you want to do is feel better. So where do you go to get the care you need? Next time you experience a nasty fall or you think you’re suffering from a particularly serious case of the flu, use these guidelines to help you decide which healthcare provider makes the most sense for your medical needs.

Emergency Room (ER) The ER should be used for serious, life-threatening issues, such as: • Chest pain or pressure • Uncontrolled bleeding • Sudden or severe pain • Difficulty breathing • Sudden dizziness, weakness or changes in vision • Changes in mental status, such as confusion • Broken bones or complex fractures • A high fever that won’t go down Urgent Care If your physician is not available, an urgent care clinic can treat a more serious illness or non-life threatening injury that needs more immediate attention. Make a trip to urgent care for conditions such as: • Sprains or strains • Minor broken bones or simple fractures

• Minor infections • Cuts that may need stitches

Smart Patient Tip: Not every urgent care facility is the same – some can take X-rays, while others cannot. Check out the urgent care centers in your area to determine which one will work best for you and your family now, so you’ll know where to go the next time you need help.

Primary Care Physician If you are not dealing with a life-threatening situation, and you believe you can wait to receive care until the next day or after the weekend, your best bet will always be your doctor’s office. They know your medical history, and have records of your medications and past treatments. It’s usually safe to wait for your physician when you have or need: • Routine tests/checkups • Common colds or illnesses

• Shots and vaccinations • Health exams

When it comes to medical emergencies, each case is different, but it’s always better to be safe than sorry. If you ever have any doubts, don’t wait! Head to the hospital immediately.

Get comfortable. If you're tense on the exam table, tell the doctor or nurse what would help you relax. If you'd like a nurse, friend, or family member there, let the doctor know.

•

Be honest. Topics like sex or illegal drug use may be tough to discuss, but your doctor needs to know about them to give you the right care. Keep in mind, your doctor cannot discuss anything you confide without your permission. Having a friend or family member there may make it easier.

•

Take time. The better you're able to explain your concerns, the better your doctor can advise you about them. Stay focused, but don't rush through a symptom if giving more information could be more helpful for your doctor.

•

Ask follow-up questions. If you don't understand your diagnosis or treatment, speak up. If you don't feel that you can follow through with a treatment, find out if there are other options. If you need someone who speaks your preferred language, let your doctor know.

•

Take notes. Bring along a friend or family member to take notes or ask questions.

•

Find out how to keep in touch. Can you email your doctor? Or is it better to leave a phone message?

Special Needs: Transitioning From Childhood to Adult Services The transition from childhood to adult services can be a difficult time for young adults with special needs. Many families of young adults often say, “If I only had the resources back then, the transition would have been much smoother.” There are many factors to consider as young people with special needs approach this transition time. We have outlined a few tips to help you make this a time of discovery and adventure, rather than one of fear and trepidation.

1. Begin the Transition Process Early Starting early helps you see the journey ahead and makes the process less overwhelming.

Transition planning should start by age 16. The Individualized Education Program (IEP) team should come together and assist with goals and outlining the necessary skills for living, socializing, working and identifying adult agencies. Many schools have transition coordinators to help with this process; check with your school to see who that person may be and give them a call.

2. Support Coordination Support coordinators play a vital role in the lives of people with special needs and their families. They help to identify the support the person needs, plan for the life the person desires, and connect the person to services in order to achieve a quality life. If you are not already connected with a support coordinator, contact the Department of Mental Health

if you live in Missouri DMH.mo.gov or the Department for Aging and Disability Services if you live in Kansas KDADS.ks.gov.

3. Transition Fairs Many school districts offer transition fairs which give you the opportunity to talk with agencies who provide a variety of services to adults with special needs. It’s important to attend these fairs and learn as much as possible about your options. This can be both an exciting and anxious process as your child takes the next step toward independence, but the more information you have, the less stressful it will be. You will want to look into as many resources as possible to learn how to successfully set up transition services for your child.

4. Adult Agency Services Adult services are very different from childhood services. Unlike childhood services, a disability alone will not guarantee individuals will receive funding or be served continued on page 14

through adult services. Certain services, such as physical therapy, occupational therapy, speech therapy, and a full school day are often mandated by law for students.

These are not mandated in adult services. A person must qualify for services as an adult (see ‘Benefits’ section of this article), and it is the person’s responsibility to identify and secure these services. That’s where you as a parent become the best advocate for your child.

You will want to meet with agency representatives and tour programs before making a decision of where to send your adult child, and it’s

important to start early as there are often wait lists for these programs.

Your support coordinator should be able to give you a toolkit that includes a list of agencies to use as a guide in determining services.

5. Benefits Medicaid, Social Security Disability Insurance (SSDI), and Supplemental Security Income (SSI) are important gateways to accessing services.

Such services include Medicaid waiver slots, health care, assistive technology and therapeutic supports. Your state will determine if you qualify for these services once the application is submitted.

6. Guardianship At age 18, individuals become their own guardians, regardless of their mental or physical ability. If the disability interferes with the ability to make decisions, guardianship may need to be obtained. Parents are responsible for contacting a lawyer to obtain guardianship, which is another financial responsibility of the parent.

Although transitioning from child to adult services can be challenging for you and your family, having the information and starting early will be key in helping you navigate the process. Be reassured there are wonderful adult agencies who will

love and care for your adult child— and there are people to help you through the process. Check with your school’s transition coordinator, and don’t let road blocks stop you. Keep asking questions and checking resources until you find the appropriate services for your child.

Five Steps to Improve Your Health What Are The Facts?

Chronic disease (cancer, stroke, heart disease, autoimmune disease, obesity, type 2 diabetes) accounts for 86 percent of our country’s health care costs.

Chronic disease is responsible for 7 out of 10 deaths of our friends, family, co-workers and neighbors. About half of all adults have at least one chronic disease. And this number is climbing in adults and even in young children.

What Can You Do About It? The good news is you can take steps to stay healthy. Here are some tips on how to achieve your greatest wealth…your health! You can reduce your risk of developing a chronic disease by 80 percent by implementing these five lifestyle behaviors: 1. Consume a whole-food, mostly plant-based diet While there are many types of diets on the market, one thing will always hold true: the diet should have an emphasis on plant-based foods. A diet rich in whole foods and one that is plant based offers the most benefits. It can lower cholesterol, blood pressure and blood sugar; prevent and reverse heart disease and obesity; lower your risk of cancer and diabetes; slow the progression of certain types of cancer; improve symptoms of rheumatoid arthritis; and improve overall quality of life. Try to make 80 percent of your intake from vegetables, fruits, seeds, nuts, legumes and whole grains. Science has proven the average American diet of an abundance of meat, sugar, and processed food is making us sick. Instead of completely eliminating these foods, cut your intake and minimize your consumption of meat (red meat, pork, chicken, fish), dairy products, and eggs, as well as sugar and highly refined processed foods.

2. Effectively manage stress You cannot always avoid stress, but you can manage it before it damages your health, relationships or quality of your life. Stress can make it difficult to adopt the other lifestyle behaviors on this list. When stress hits, sleep is sacrificed, convenient unhealthy food/drink choices are the go-to, and exercise is the last on your to-do list. However, a healthy diet, exercise and adequate sleep will help you better manage stress and increase your ability to cope with challenges.

3. Get adequate sleep Ideally adults need 7 to 8 hours of good quality sleep. This means you should feel energized upon wakening. Poor sleep habits can interrupt a delicate cycle and make you more moody and can make it difficult to focus during the day. It can also make you more prone to illness and shorten your life expectancy.

4. Avoid toxins Smoking, drinking too much alcohol and consuming heavily processed foods full of chemicals puts a toxic load on the body and contributes to disease.

5. Exercise If you had to pick a place to start out of these five lifestyle factors, choose exercise. This lifestyle behavior has the greatest impact on the others as it can help you sleep, detoxify and manage stress. Always start with low impact exercise such as walking when you are just starting an exercise program.
